[The function of corneal endothelium after penetrating keratoplasty as measured with fluorophotometry].

The aim of the study was to calculate the corneal endothelial permeability (Pac) 12 and 18 months after penetrating keratoplasty in patients (aged 42-60 years) with very good prognosis for graft clarity (10 eyes with pseudophakic and 4 eyes with aphakic bullous keratopathy; 6 eyes with keratoconus; 2 eyes with granular dystrophy; 6 eyes with central inactive scars; 1 eye with early central Fuchs' dystrophy). The normal eyes (10 eyes) served as control group in persons aged 40-65 years. Each operated eye was submitted to fluorophotometry of the anterior segment with measurement of corneal endothelial permeability (Fluorotron Master, Coherent) 12 and 18 months after the surgery. The cornea thickness measurement and endothelial cell counting were performed by specular microscopy with pachymeter 12 and 18 months after penetrating keratoplasty in cases with very good prognosis for graft clarity. The mean values of Pac: 4.48 x 10(-4) +/- 1.24 cm/min after 12 months were significantly higher (p < 0.05) than in the control group (3.61 x 10(-4) + 0.51 cm/min). Neither significant changes in corneal thickness nor endothelial cell density were noted as a result of surgery. The calculation of Pac in a late period after penetrating keratoplasty revealed a stable partial exhaustion of the corneal endothelium function in cases with good prognosis for graft clarity.

[Comparison of the mean deviation of retinal sensitivity in the static perimetry threshold test with biomorphometric parameters of the optic disc in eyes with glaucoma].

The aim of the study is to evaluate results of mean deviation of retinal sensitivity obtained in static perimetry threshold test in glaucoma simplex eyes and to compare them with some biomorphometric parameters of the optic disc measured with scanning laser ophthalmoscope. The analysis was performed with the results obtained in 50 glaucomatous eyes with the disease lasting 4 to 15 years. The mean values of mean deviation, cup to disc ratio, mean cup depth and neuroretinal rim area were 6.71, 0.459, 0.031 and 1.464, respectively. The best correlation coefficient value (0.847) was found between mean deviation and neuroretinal rim area. The results confirm the importance of neuroretinal rim area value in evaluation of optic disc in glaucoma cases.

[Disseminated intravascular coagulation syndrome with secondary fibrinolysis activation in prostatic carcinoma].

We reported on a 74 year old patients with local advanced prostatic carcinoma. Following prostatic surgery an increased bleeding tendency was observed. The patients showed clinical and laboratory evidence for consumption coagulopathy with hyperfibrinolysis. The laboratory data were: marked decrease of AT III, Protein C, increase of thrombin/AT III complex level, fibrin degradation products (FDP) and antigen of t-PA. The treatment was ended successful.

[Axial and para-axial fluorophotometry in optic neuropathies].

PURPOSE: To investigate optic neuropathies, with normal results in fluorescein fundus angiography, by axial and paraaxial fluorophotometry (a type of topographic vitreous fluorophotometry) in order to determine differences examined in the amount of fluorescein leakage in macular and optic disc. MATERIAL AND METHODS: We studied 12 eyes with unilateral idiopathic retrobulbar inflammatory optic neuropathy and 4 eyes with unilateral posterior ischaemic optic neuropathy during the acute phase: control group consisted of 5 healthy eyes. After 3-4 months, in final stage of optic neuropathies, in which primary atrophy of optic disc developed (6 eyes), fluorophotometric examinations were repeated. All cases of control group underwent axial and paraaxial fluorophotometry. Complete ophthalmic, physical and neurological examinations were performed. RESULTS: The mean fluorophotometric values of PVPR (posterior vitreous penetratio-ratio) for f-scans (foveal) were lower than for t-scans (PVPRf < PVPRt) in all neuropathies during the acute phase but all values of PVPR were higher than in control eyes. In late phase of the neuropathies in which primary atrophy of optic disc developed we found a trend: PVPRt < PVPRf, but all values of PVPR were lower than in control eyes. We noticed increased permeability of the blood-ocular barrier and difference between fluorophotometric readings for optic disc directions (t-paraaxial scans) and for foveal directions (f-axial scans) in optic neuropathies during the acute phase, when the findings on ophthalmoscopic and fluorescein fundus angiography were normal.

[Three dimensional analysis of the optic disc with scanning laser tomography].

The aim the study was to evaluate the optic disc's topography, to compare it with conventional photographs and with defect of the visual field in glaucomatous patients. We performed our study in 18 glaucomatous and 19 healthy control subjects with laser topographic scanning ophthalmoscope (TopSS, Laser Diagnostic Technologies Inc.) and compared the results with those in the photographic method. Defect of the visual field was assessed with "Octopus 1-2-3" using the GL programme GL. A topographic image of the cup area in control subjects and the neuroretinal rim area in glaucomatous subjects obtained with TopSS was smaller than in the photographic method (p < 0.05 and p < 0.01 respectively). Mean values of cup volume in control and glaucomatous subjects were 0.222 mm3 (SD = 0.011) and 0.619 mm3 (SD = 0.021) respectively. A correlation between the optic disc parameters obtained in TopSS examinations and the mean defect of the visual field was statistically significant in glaucomatous patients. Topographic results of optic disc obtained by TopSS allow for an accurate assessment of optic nerve damage.

[Transthoracic approach to the thoracic spine via the rib cage].

Transthoracal approach to the thoracic spine is presented. It has been used in 19 patients with trauma or tumor within thoracic spine. This approach allowed for direct access to the vertebral body where the pathology was located. Transthoracal approach, however, should be performed by a surgeon with sufficient knowledge of thoracic surgery.

[Blood-retinal barrier and visual field in retinitis pigmentosa].

The aim of the research was to determine, with new-generation fluorometer (Fluorotron Master), the factor of fluorescein leakage into the vitreous and to reveal, with the static perimeter (Humphrey's), the quantitative parameter of the visual field in retinal pigmentary dystrophy. The factor of fluorescein leakage into the vitreous was almost ten times higher in retinal pigmentary dystrophy and it indicated the damage of blood-retina barrier. The correlation between the factor of fluorescein leakage into the vitreous and the degree of visual field defect has not been found.

[Various clonidine drops in treatment of glaucoma].

Seventeen patients with glaucoma were given bilaterally a single drop of Isoglaucon (Clonidine hydrochloride) of various volume (15 microliters and 40 microliters) and various concentrations of the drug (0.125%, 0.25%, 0.50%) according to the principle of a double blind trial. In comparison with placebo all sorts of Isoglaucon drops cause a statistically important reduction of intraocular pressure; an essential lowering of the general systolic and diastolic pressure (a side effect of the treatment) appeared after application of a larger drop (40 microliters) of a 0.5% concentration. By reducing the content of the drug in the drop or reducing its volume one can avoid the fall in the general blood pressure.

[Naclof and dexamethasone in topical application after trabeculectomy].

Evaluated was the topically applied solution of natrium salt of diclofenac--Naclof (22 eyes) and a 0.1% solution of dexamethasone phosphate (23 eyes) on the healing of postoperative wounds after trabeculectomy in a clinical examination in patients with simple glaucoma. After the filtering operation the authors observed in the group of patients treated by dexamethasone a longer and more pronounced flattening of the anterior chamber (up to 14 days) a greater hypotensive effect and a more increased facility of the outflow (in an observation period of 6 months). In the case of a considerable flattening of the anterior chamber after trabeculectomy application of Naclof gives a better prognosis than of dexamethasone.

[Surgical treatment of low pressure glaucoma].

Eyes with non-controlled--in spite of a conservative treatment--bilateral low-tension glaucoma with a decreased facility of outflow (10 patients, 20 eyes) were subjected to antiglaucoma operations. Trabeculectomy was performed in one eye, a filtering operation in the second one. During the observation period of 1.5 years it showed that in the low-tension glaucoma the visual function undergoes more frequently stabilization after a more radical glaucomatous procedure (of filtration type) when the postoperative fall in the intraocular pressure exceeds 20% and its daily oscillations are considerably reduced.

[Measurement of lens transparency in workers at risk of developing of glassblower's cataract].

The authors evaluated the transparency of lenses in persons on a stand predisposing to the formation of a glassblower cataract as well as in office clerks. They used for their measurements a fluorophotometer (Fluorotron Master) and an opacity lensmeter. In a group of 40 ironmasters they found a considerable fall in light transparency through the lens and a significant increase of the lens autofluorescence in comparison with a group of office clerks. The quantitative measurement of lens transparency by means of a fluorotron and a opacity lensmeter creates the possibility of an earlier detection of a ++pre-cataract condition.

The effect of subchronic poisoning with potassium nitrate and sodium nitrite on the processes of intestinal absorption of D-xylose in rats.

The intestinal transport of D-xylose was studied during subchronic poisoning of male Wistar rats with the oral administration of potassium nitrate and sodium nitrite. The metabolic parameters of small intestine mucosa were determined one hour after xylose administration, i.e., Na+/K(+)-ATPase, alkaline phosphatase, oxygen consumption, and lactic acid level. Nitrite reduced the absorption of xylose and decreased the activity of Na+/K(+)-ATPase and alkaline phosphatase. No effect of sodium nitrite was demonstrated on the aerobic metabolism of intestinal mucosa with an increased lactic acid level. Potassium nitrate did not effect the processes of intestinal absorption of xylose nor the metabolic parameters of small intestine mucosa.

The effect of acute poisoning with potassium nitrate and sodium nitrite on the processes of intestinal absorption of D-xylose in rats.

The intestinal transport of D-xylose was studied during the acute poisoning of male Wistar rats with orally administered potassium nitrate and sodium nitrite. At the peak of xylose absorption, the metabolic parameters of Na+/K(+)-ATPase, alkaline phosphatase, oxygen uptake, and lactic acid level were determined in the small intestine mucosa. Nitrite in a dose of 80 mg NaNO2/kg b.w. increased the permeability of gastric mucosa for D-xylose and raised the uptake of oxygen by the small intestine mucosa. No changes were observed in the activity of Na+/K(+)-ATPase and alkaline phosphatase. A dose of 10 mg NaNO2/kg b.w. was not followed by increased absorption of this sugar. It was also demonstrated that potassium nitrate had no effect on the process of intestinal absorption of D-xylose and failed to change the determined metabolic parameters of the small intestine mucosa.

The effect of exercise associated with subchronic poisoning with potassium nitrate and sodium nitrite on the processes of intestinal absorption of D-xylose in rats.

The intestinal absorption of D-xylose was studied during the subchronic poisoning of male Wistar rats with orally administered potassium nitrate and sodium nitrite associated with exercise; running on a moving track during the last two weeks of poisoning. The metabolic parameters of Na+/K(+)-ATPase, alkaline phosphatase, oxygen uptake, and lactic acid level in the small intestine mucosa were determined one hour after D-xylose treatment. Exercise increased the toxicity of potassium nitrate and sodium nitrite. The experiment demonstrated post-exercise reduction of D-xylose absorption and decrease activity of Na+/K(+)-ATPase and alkaline phosphatase. Exercise caused transient hypoxia of the small intestine, which was observed only in the groups subjected to exercise on the day of the determinations.

The effect of exercise associated with acute poisoning with potassium nitrate and sodium nitrite on the processes of intestinal absorption of D-xylose in rats.

The intestinal absorption of D-xylose was studied during acute poisoning of male Wistar rats receiving intragastrically potassium nitrate and sodium nitrite and small intestine perfusion with these compounds. The metabolic parameters, Na+/K(+)-ATPase, alkaline phosphatase, oxygen uptake, and lactic acid level, were assessed in the small intestine mucosa one hour after administration of these compounds. Exercise was demonstrated to reduce the intestinal absorption of D-xylose, to raise the level of lactic acid, and to increase the oxygen uptake by the small intestine mucosa, but caused no changes in the activity of Na+/K(+)-ATPase or alkaline phosphatase. Also, exercise failed to change the direction of the toxic effects of sodium nitrite but increased potassium nitrate toxicity as evidenced by reduced absorption of D-xylose from the intestine despite lack of changes of the enzymes Na+/K(+)-ATPase and alkaline phosphatase in the mucosa.

[Incidence of pulmonary tuberculosis in the Lódź province among persons in contact with tuberculotic patients].

The results of an analysis on new cases of pulmonary tuberculosis from contacts with sputum positive tuberculosis are discussed. In subjects with contacts with sputum positive tuberculosis the incidence rates were 16 fold higher in comparison with the general population. In patients that reported contacts with sputum negative tuberculosis the incidence rates were even higher. This contradiction can be explained by the quicker hospitalization in the first group of patients thus terminating contacts.
