[Echinococcotomy of the upper lobe of the lung in children].

The authors analysed the immediate and late-term results of echinococcectomy of the upper pulmonary lobe in 71 children 3 to 14 years of age conducted by Delbet's (18), A. A. Vishnevsky's (20) and A. T. Pulatov's method (25), 8 children underwent lob--and segmentectomy. Tense cysts were found in 34 and complicated cysts in 25 children. Most of the cysts (37) were localized in the anterior segment of the superior pulmonary lobe. The superior lobes of both lungs were involved with equal frequency. Deformity of the bronchi and pneumosclerosis were encountered in the late periods after Delbet's operation. Residual cavities were formed in 10 out of 20 patients after A. A. Vishnevsky's operation. In 10 patients these operations were complicated by hydro- and pneumothorax at the site of the removed cyst of the superior pulmonary lobe. These complications were due to inadequate drainage of the pleural cavity in the eighth intercostal space. Additional drainage had to be installed in the second intercostal space. The mentioned operations are indicated only in superficial localization of hydatid cysts of small and moderate size in the superior lobe, when the depth and breadth of the residual cavity resulting from echinococcectomy will be no more than 5-6 cm. Creation of an artificial interlobar or intersegmental fissure proved to be the most optimal and effective method of echinococcectomy in large cysts and centrally situated cysts of any size in the upper pulmonary lobe. Fatal outcomes were not encountered.

[The surgical procedure in testicular torsion in children].

The results of treatment of 37 children with torsion of a testicle, including 18 (48.6%) under 2 years of age, are presented. In doubtful viability, a testicle after untwisting was placed into its bed and observed for 12-24 h. Orchiectomy was performed only in total lack of viability. A testicle was preserved in 24(64.8%) of the 33 patients. Two to five years after the operation, a good result was noted in 11, a satisfactory one--in 8, and unsatisfactory one--in 5 patients.

[Nephroptosis in children].

The author suggests a modification of the Rivoir-Pytel-Lopatkin operation for children with II-III degree nephroptosis. It consists in careful atraumatic separation from the surrounding tissues of the whole kidney, the upper third of the ureter, and the main vessels of the kidney for a distance from its hilus to the site of their origin from the aorta and drainage into the inferior vena cava. A pedicle graft measuring 3-3.5 cm in width and thickness and 13-15 cm in length is formed from the psoas major muscle with maintained circulation. The graft is sutured to the lower pole of the kidney preserving the intactness of the capsule. Care is taken that the main vessels of the raised kidney are situated, where possible, perpendicular to the aorta and inferior vena cava. A band is worn for 12-18 months after the operation. Eight children were operated on. The late-term results in follow-up periods of 6 months to 3 years are favorable.

[Diagnosis and surgical treatment of solitary large and giant hydatid cysts of the liver in children].

The article analyses the results of surgical treatment of 33 children with solitary large (16) and giant (17) hepatic cysts in the stage of preperforation. Most of the cysts (18) were located in the subdiaphragmatic region. If the method of ultrasonic examination (USE) is available and bearing in mind the grave general condition, extended radiological methods of examination are not advisable for establishing the diagnosis. An emergency operative intervention is undertaken by wide laparotomy with transverse section of the anterior abdominal wall. The residual cavity is liquidated by turning inside a part of the fibrous capsule (7), filling it with the omentum (10). In 4 patients the residual cavity was drained with prolonged active aspiration. Large dense areas of fusion of the fibrous capsule with other organs should be left as islets. There were no fatal outcomes.

[Perforation of hepatic hydatid cysts into the abdominal cavity in children].

Perforation of echinococcal cysts of the liver into the free abdominal cavity was observed in 10 patients. The operative intervention was performed after overcoming the shock in patients, correction of homeostasis. The performance of wide laparotomy, through revision of the abdominal cavity and its sanitation by means of the abundant washing by the furacin solution are important. No lethal outcomes, disease recurrences were noted.

[Echinococcosis of the spleen in children].

Splenectomy is considered generally accepted in hydatid disease of the spleen. The authors adhere to organ-preserving tactics--closed echinococcectomy. They treated only one of their 10 patients by splenectomy. They draw attention to the sequence in which the method of echinococcectomy is applied and to the specific features and the choice of the therapeutic tactics in perforation of a hydatic cyst of the spleen into the free abdominal cavity.

[Surgical tactics in obstructive-suppurative calculous pyelonephritis in children].

An analysis of results of decompressive nephrostomy for obstructive purulent calculous pyelonephritis in 30 children has detected different complications aggravating the course of the disease and process of treatment. The authors propose a rational variant of operation--primary nephrolithotomia with nephrostomy. Operations were performed on 40 children. Immediate results of treatment were good in 30 children, satisfactory in 9 children. One patient died.

[Pediatric surgical one-day hospital].

670 sick children were operated upon under conditions of a one-day surgical hospital. Selection of the patients and preoperative preparing was fulfilled at the polyclinic. The selected children had the following diseases: inguinal (119) and umbilical (87) hernias, hernias of the linea alba (14), hydrocele (68), cryptorchidism (81), benign tumours of soft tissues (43), phimosis (258). Complications in the nearest postoperative period made up 0.3%. The economic effect of the treatment of 670 children with surgical diseases at the surgical one-day hospital was more than 50,000 rubles.

[Chemical burns of the oral cavity and esophagus in infants].

The author has analyzed 482 sick children younger than 2 years of age. The burn degree was established with the help of narcotized esophagoscopy and by parameters of homeostasis. The washing out of the chemical substance was performed by oral introduction of 6-9 liters of water and the following probe-elimination from the stomach within the first 12-24 hours. Correction of disturbed homeostasis was fulfilled if necessary. The independent nutrition was started on the 2nd-3rd day. The method allowed treatment of the burned esophagus without bougieurage provided the patient was admitted to the hospital during the 1st day. Nineteen patients died (3.9%). Long-term results were favorable.

[Variants of hemodilution and intraoperative blood loss in children].

The authors have made a comparative assessment of variants of the infusion-transfusion aid in operations on kidneys in nephrolithiasis and echinococcosis of the lungs and liver in 286 children. Results of the investigation have shown considerable advantages of hemodilution methods. Of special value is thought to be artificial hemodilution with controlled hypotension which makes the donor blood not necessary in 90% of children during operation.
