Extra-abdominal desmoid mimicking malignant male breast tumor.

A rare case of extra-abdominal desmoid tumor is reported. A palpable mass was detected in the right breast of a 47-year-old man. Mammography showed a stellate mass without calcification, and breast ultrasound examination revealed a solid, inhomogeneous, non-calcified lesion. The result of cytological examination of the fine-needle aspiration biopsy specimen was equivocal. Histology of the surgical specimen showed extra-abdominal desmoid tumor. Extra-abdominal presentation of this semimalignant tumor is rare and may mimic malignant breast tumor. Differential diagnosis is difficult and usually based on the result of the histological examination.

[The role of 99mTc-MIBI mammoscintigraphy in the diagnosis of breast cancer].

Among several investigative methods currently undergoing evaluation for the differentiation of biological features of breast mass lesions, mammoscintigraphy with different radiopharmaceuticals appears promising. The reported study evaluated the efficacy of 99mTc-MIBI mammoscintigraphy in detection of the malignancy of focal breast lesions. Mammography, 99mTc-MIBI mammoscintigraphy were performed in 51 women with palpable breast mass lesions. Following surgical removal of the abnormalities, histological examination revealed 40 malignant and 11 benign breast mass lesions. In the mammoscintigraphy, early (5 min p. i. of MIBI) and late (2 h p. i. of MIBI) planar images of the breast and the axillary regions were evaluated visually and semiquantitatively. The sensitivity and specificity values of MIBI mammoscintigraphy were in the detection malignant breast lesions evaluated visually according to the early images 95% and 73%, according to the late images 98% and 82%, respectively. Revealed to the quantitative results the corresponding results were according to the early images 90% and 64% according to the late images 95% an 64%. The visual scores and the quantitative T/NT values with MIBI demonstrated a significant difference between malignant and benign breast mass lesions. A significant difference was also found as concerns the grade of malignancy from the MIBI accumulation. The late MIBI images seemed optimal. In the detection of metastatic lymph node involvement, the sensitivity and specificity with MIBI were 53% and 81%. It was concluded that MIBI (2 h p. i.) mammoscintigraphy is a useful and simple method for differentiation of malignant breast abnormalities from benign lesions and for determination of the grade of malignancy.

[Intracystic tumors of the breast].

The authors report their experience obtained on the diagnostics of intracystic breast tumours in the last five years. It can be stated that ultrasound is currently the most valuable supplementary imaging method for the diagnosis of this rarely observed tumour. With the aid of ultrasonography, guided sampling can easily be carried out, and hence the preoperative diagnostic accuracy is increased. Pneumocystography is needed only in special cases.

Modified Blalock-Taussig shunt using allograft saphenous vein: six years' experience.

BACKGROUND: The occurrence of life-threatening late infectious complications after the use of expanded polytetrafluoroethylene conduits as modified Blalock-Taussig shunts prompted us to apply allograft saphenous veins instead. METHODS: In 23 cyanotic patients (age, 1 week to 18 years) allograft saphenous veins were used for performing Blalock-Taussig shunts from July 1989 onward. Veins stored in Hank's solution were implanted in 8 patients and cryopreserved ones in 15. All patients were followed up regularly up to 15 months. RESULTS: There were two early and two late deaths: none were related to shunt occlusion. Clinical, angiographic, and echocardiographic studies proved that, except for one early occlusion, all shunts were patent and functioning well after an average of 41 months. Donor cells disappeared 1 to 3 days after implantation, and several months after the operation both the wall and the luminal surface of the grafts were repopulated with cells possibly of recipient origin. No difference was found between veins stored in Hank's solution only and cryo-preserved grafts, concerning clinical outcome and histology. CONCLUSIONS: Allograft saphenous veins function well as modified Blalock-Taussig shunts at least up to 6 years. Owing to the good results and lack of complications their clinical use is recommended.

[Modified Blalock-Taussig shunt using a saphenous vein allograft. Five-year experience].

From July 1989 to June 1994 23 modified Blalock-Taussig shunts were performed using allograft saphenous veins. Veins stored in Hank's solution were implanted in 8, and cryopreserved ones were used in 15 cases. No operative death, bleeding, or infectious complication has occurred. There were 2 early and 2 late deaths, none was related to shunt occlusion. Clinical, angiographic and echocardiographic investigations proved that the rest of the shunts are still patent and functioning well after an average of 30 months follow up. Histological studies showed that donor cells have been replaced after several months with recipient cells both in the wall and at the luminal surface of the grafts. No difference was found between veins stored in Hank's solution and cryopreserved grafts concerning clinical outcome and histology.

[Pheochromocytoma in Recklinghausen neurofibromatosis].

The authors describe the case of a 38-year-old hypertensive woman who suffered from neurofibromatosis, pheochromocytoma, scoliosis and diabetes mellitus. Because of the residual pheochromocytoma surgical intervention was repeated. According to the available literature this is the first case in Hungary where pheochromocytoma was associated with neurofibromatosis.

[Morphology of cardiac myxomas].

Author describes her experiences gained by light microscopic, immunohistochemical and electronmicroscopic studies of 11 cardiac myxomas. New data on debated histogenesis of myxomas were tried to be gained by her studies. A part of myxoma cells was poor in organelles ultrastructurally, mostly settled by one, in the other part more organelles and intra-cytoplasmatic filaments could be detected, majority of them was of process. Glandular structures, showing mucous production, were in a tumor. Majority of tumor cells showed vimentin immunoreactive by immunohistochemical studies. In addition, lysozyme immunoreactive cells could also be observed; 8th factor associated antigen, however, could be detected in glandular structures. On basis of studies, the assumption seems to be probably, according to which the cardiac myxomas originate from embryonal cells, capable to endodermal and mesodermal cytogenesis.

[Morphologic studies of emdomyocardial biopsy specimens].

Authors have studied 153 heart biopsies of 120 patients and results of 133 endomyocardial biopsies studied by light and electron microscopy are described. In addition to typical morphological picture of myocarditis, in majority of cases non-specific changes appearing also in idiopathic cardiomyopathy were observed and their ultrastructure is fully stated. Results are compared to relevant data of literature. Indications, informative value, practical and scientific research significance of myocardial biopsies are briefly overviewed.

Myocardial lipid peroxidation in rats after chronic alcohol ingestion and the effects of different antioxidants.

The effects of chronic intake of dietary alcohol on myocardial peroxidation (measured as formation of diene conjugates), reduced glutathione content, and morphology and the protective actions of different antioxidant compounds (vitamin E and (+)-cyanidanol-3) were studied in rats. Alcohol, comprising more than 30% of the dietary calorie content, was administered to rats for six weeks. Compared with the controls, the left ventricle of the alcoholic animals had an increased diene conjugate content (5.4(0.5) vs 4.3(0.6) optical density X g wet weight-1) and a slightly, but not significantly, decreased glutathione content (1.62(0.05) vs 1.66(0.07) mumol X g wet weight-1). Simultaneous administration of antioxidants (vitamin E or (+)-cyanidanol-3) prevented the pathological changes in diene conjugates and significantly increased the glutathione content compared with the alcoholic rats. Electron microscopy showed remarkably few ultrastructural abnormalities in the myocardium of alcoholic animals fixed by vascular perfusion. The data are consistent with the hypothesis that reactive oxygen radicals are involved in the ethanol induced biochemical changes and that the antioxidants could prevent the increased formation of peroxides in the myocardium.

Dimethoate-induced toxic cardiac failure in the guinea pig.

In anaesthetized guinea-pigs treated with lethal doses of dimethoate, cardiac failure and serious ECG disturbances developed in the early phase of intoxication. The toxic cardiac phenomena appeared to be unrelated to the degree of cholinesterase inhibition, but showed a close correlation with myocardial dimethoate concentration. Cardiac failure and mortality were first observed at a critical pesticide level of about 110 micrograms/g, while a level of 221 micrograms/g resulted in death in all cases. The present investigation refers to the direct effect of the pesticide on the myocardium, independent of its anticholinesterase action.

[Structural changes in xenografts used for heart valve replacement].

Macroscopic, lights-and electron microscopic examination of 14 xenograft valve prostheses was carried out over a follow-up period of 1-49 months. 8 out of the 14 were acidic formaldehyde pretreated porcine aortic valves, 4 glutaraldehyde pretreated ones (Carpentier-Edwards) and 2 of them were glutaraldehyde calf's pericardium valves (Ionescu-Shiley). Formaldehyde pretreated grafts had to be surgically removed for valve failure. Glutaraldehyde grafts were obtained at autopsy. Until death, their function was excellent. The main pathological finding in both type of grafts was disintegration of collagen, which in glutaraldehyde grafts appeared to be of milder degree. In formaldehyde pretreated grafts cellular reaction indicating chronic rejection has also been observed. It is concluded that formaldehyde pretreated xenografts are not suitable for clinical use. On the other hand glutaraldehyde pretreated ones inspite of structural alterations may be functioning for years.

[Renal siderosis in cardiac hemolysis].

Renal siderosis caused by intravasal haemolysis has been studied in 202 autopsy cases, which based on the origin of the intravasal haemolysis have been divided into three groups: 1. patients deceased months or years after artificial valve implantation; 2. patients with congenital or acquired defective heart valves without surgical treatment; 3. patients deceased several days after open heart surgery. Renal siderosis has been detected by prussian-blue staining method. In 1/3 of the cases quantitative determination of the iron content has also been carried out. In cases of chronic defective heart valves without an operation renal siderosis has not developed. Among patients, who underwent open-heart operation it has been observed only in one case. After valvular replacement-except one case-renal siderosis developed only in patients with valvular failure. These findings seem to indicate that unless rare exceptions renal siderosis develops only in cases of long lasting intravasal haemolysis.
