[Coronary artery bypass utilizing vein grafts: why didn't we use an arterial graft?].

Internal thoracic artery (ITA) grafts for coronary artery bypass grafting (CABG) are superior to SVG in the long-term patency and survival. In spite of our effort to utilize ITA whenever possible, we still have some cases which have to receive only vein grafts. Among the consecutive 201 patients undergoing CABG in the past three years, 32 patients (16%) received only vein grafts. We compared these patients (SVG group) with those who received ITA grafts (ITA group). The SVG group consisted of all emergency cases and many cases with single and double vessel disease. In the SVG group, perioperative use of IABP was more frequent and operative mortality was higher, although the number of grafts was fewer (2.66 vs 3.61) and aortic cross clamping time was shorter in this group. Main reasons for selecting only vein grafts were emergency surgery, inadvertent injury of ITA, inadequate ITA free flow etc. Technical refinement in the preparation of ITA is important to make possible wider application of arterial grafts for CABG.

[A case of extra-adrenal pheochromocytoma with frostbite in extremities].

We describe a patient who presented frostbite in extremities in addition to characteristic symptoms, such as severe hypertension, sweating attacks, palpitations and headache. The patient was eventually diagnosed as having single extra-adrenal pheochromocytoma. The frostbite in extremities rapidly resolved after the removal of the tumor as well as other characteristic clinical symptoms. It is speculated that this frostbite might have been induced by severe continuous constriction of peripheral artery and loss of heat by frequent sweating attacks. Regarding cutaneous symptoms in this disease, pallor, acrocyanosis and cold extremities are commonly found. However, it seems that typical frostbite associated with pheochromocytoma has not been reported so far.

Treatment of Behçet's disease.

Behçet's disease is a systemic inflammatory disorder capable of inducing thrombophlebitis. Clinicopathologically, it is characterized by neutrophil and platelet hyperfunction. Lesions are formed presumably because neutrophils infiltrating the affected tissue release active oxygen and lysosomal enzymes in large amounts. The mechanism of neutrophil hyperfunction was obscure, but in recent years attention has focused on the effects of cytokines released by mononuclear cells. As more has been learned about the etiopathogenesis of Behçet's disease, novel anti-inflammatory drugs, immunosuppressants, anti-thrombotic agents, and anticoagulants have helped to achieve remarkable progress in treatment. Particularly notable is the effect on severe uveitis of cyclosporine, which is more effective than colchicine and conventional immunosuppressants. With the advent of cyclosporine, the number of patients with Behçet's disease who lose their eyesight has clearly decreased.

[A case of thymic carcinoma, sarcomatoid type].

A 53-year-old man was found to have a mediastinal mass. Chest X-ray, CT scan, bronchofiber and superior vena vasography showed a left upper giant mediastinal tumor with invasion to left upper lobe and pericardium. And it was performed a surgical treatment and diagnosed a thymic carcinoma (sarcomatoid type) by histological examination. We reported a rare type of thymic carcinoma and added discussion.

[Clinical report of cimetidine therapy for the prevention of the gastro-duodenal bleeding after the open heart surgery].

The efficacy of cimetidine administration after the open heart surgery was studied using the cimetidine concentration examination in the twenty five consecutive cases. Cimetidine was administered intravenously. The cases were divided into five groups for the infusion period. There was no case which developed the gastro-duodenal bleeding after the surgery. The concentration of cimetidine has showed more than 0.5 microgram/ml in each group. It was concluded that the administration of 800 mg cimetidine infused more than three hours, repeated every twelve hours was the possible treatment for the prevention of the gastro-duodenal bleeding after the open heart surgery.

[The results and problems of reoperation for coronary artery disease].

In six hundred and six consecutive patients undergoing coronary artery bypass grafting (CABG) within the past 17 years (May 1974 to March 1991), repeated CABG were performed on 10 patients (1.65%). The main reasons for repeated CABG were graft failure (GF) in 8, progression of native disease (NP) in 5 and incomplete revascularization (IR) in 3 patients. The incidence of GF was high either within a half year or around 5 years after CABG. Although all patients survived from reoperation, four patients continued to have mild angina pectoris. When the recurrence of angina is noted after CABG, coronary arteriography and if necessary PTCA should be done as soon as possible. If a second surgery is inevitable, maximum utilization of arterial graft and accomplishment of complete revascularization are emphasized.

[Clinical analysis of 6 cases of pericardial cysts].

We present a clinical analysis of 6 cases of pericardial cysts. There were two males and four females. Only one case of them had complaint of persistent cough. Four of 6 were observed in right costo-phrenic angle, and 2 were in left costo-phrenic angle. Surgical excision was performed in 6 cases, and all patients recovered good health. We suggest that in cases of pericardial cysts, surgical treatment should be performed, even if patients would not have remarkable complaint.

[A case of coronary artery bypass for bilateral coronary ostial stenosis in aortitis syndrome with occlusion of bilateral subclavian arteries].

A 52-year-old female with bilateral coronary ostial stenosis in aortitis syndrome underwent CABG. Vein grafts were used instead of arterial grafts, because of the occlusion of bilateral subclavian arteries. Proximal anastomosis of the grafts was performed after oval stomas larger than usual were created on the aortic wall to prevent late graft occlusion due to intimal proliferation of the aorta. On postoperative CAG, all grafts to RCA, LAD and Cx were patent. The patient left the hospital 3 weeks after surgery. We believe that CABG is preferable to the transaortic endarterectomy in the surgical treatment for coronary ostial stenosis associated with aortitis syndrome, because of the prevalence of technical difficulty and postoperative morbidity in the latter.

[Effects of endothelin on retinal blood vessels].

Endothelins (ETs), which are one of the most potent vasoconstrictors, were discovered in 1988. ETs are involved in the regulation of vascular smooth muscle and are thought to function like a local hormone. In this study, we measured changes in the caliber of retinal blood vessels caused by the intravitreal injection of ET 1 in rabbits, using fundus photography. The retinal artery showed an immediate vasoconstriction after the injection (0.1 ml) of ET 1 at 10(-6)M. However, ET 1 in lower concentrations (10(-7)M and 10(-8)M) caused vasodilations initially and subsequently vasoconstrictions. A dose-response relation was found. The blood vessel caliber reduced maximumly to 53% the value before injection. The vasoconstriction due to ET 1 may induce retinal ischemia and hypoxia. We speculate that ET 1 may be involved in the regulation of retinal blood vessels.

Basic fibroblast growth factor promotes proliferation of rat glomerular visceral epithelial cells in vitro.

Glomerular visceral epithelial cells (vGEC) play an important role in the synthesis of the glomerular basement membrane (GBM), and together with glomerular endothelial cells and the GBM, in glomerular ultrafiltration. Therefore clarification of the properties of vGEC is essential to investigations of glomerular morphology and function in both physiologic and pathologic conditions. This article demonstrates that basic fibroblast growth factor (bFGF) is mitogenic to vGEC in vitro. Its effect was found at concentrations as low as 1.25 ng/ml, and was synergistic with epidermal growth factor (EGF). In contrast, EGF by itself had no demonstrable mitogenic effect at concentrations of 1.25-100 ng/ml. In addition, mRNA for bFGF was identified in cultured vGEC by the method of reverse transcriptase polymerase chain reaction and the immunoreactivity of bFGF was found in GEC of the Sprague-Dawley rat kidney. These results suggest that bFGF stimulates the proliferation of vGEC in an autocrine manner in vivo. A unique relationship similar to that observed in endothelial cells may also exist among bFGF, vGEC, and the extracellular matrix (ECM). In a word, bFGF may be produced by vGEC and stored in the ECM, that is the GBM, and may be one factor that stimulates vGEC to proliferate when vGEC are injured and lost in vivo.

Quantitative measurement of regional cerebral blood flow with I-123 IMP SPECT: a correction of the microsphere model by global extraction between artery and internal jugular vein.

Quantitative measurements of regional cerebral blood flow with N-isopropyl-(Iodine 123)p-iodoamphetamine (I-123 IMP) as a microsphere model were performed in forty cases. The regional cerebral blood flow values obtained with I-123 IMP were slightly underestimated compared with those of Xe-133 inhalation methods (y = 0.90x-2.1, r = 0.85, p less than 0.01). After correction by global extraction (87%) between the artery and internal jugular vein, which was measured in four patients by means of a catheter technique, the underestimation of the values obtained with I-123 IMP was improved (y = 1.0x-2.4, r = 0.85, p less than 0.01). Several problems in the accurate quantitative measurement of regional cerebral blood flow with I-123 IMP are discussed.

The existence of 25-hydroxyvitamin D3-1 alpha-hydroxylase in the liver of carp and bastard halibut.

We have found that carp and bastard halibut contain 25-hydroxyvitamin D3 (25-D3)-1 alpha-hydroxylase in the liver besides in the kidney by the following in vivo and in vitro experiments. When [3H]-25-D3 was intraperitoneally injected to vitamin D(D)-deficient carp and normal bastard halibut (D-deficient bastard halibut could not be raised because they died during farming), the profiles of high-performance liquid chromatography (HPLC) of the plasma lipid extract showed the formation of a peak corresponding to [3H]-1 alpha,25-dihydroxyvitamin D3 (1,25-D3). When [3H]-25-D3 was incubated with liver homogenates of the fish, a peak corresponding to [3H]-1,25-D3 was also observed in the profile of HPLC. The formation of the metabolite was confirmed by the thermal isomerization into the pre-isomer and mass fragmentography. Although the 1 alpha-hydroxylase was also observed in the kidney, the activity of the enzyme was lower than that in the liver. The results suggest that 25-D3-1 alpha-hydroxylase exists in the liver of carp and bastard halibut and the 25-D3 formed from D3 in the liver is immediately metabolized into 1,25-D3 in the same tissue. The suggestion is supported by the fact that D3 is a major circulating compound with small amounts of 1,25-D3 in the fish while the plasma levels of 25-D3 are under the limit of detection.

Uneven distribution of excitatory amino acid receptors on ventral horn neurones of newborn rat spinal cord.

1. The distribution of excitatory amino acid receptors on ventral horn neurones was investigated using slices of newborn rat spinal cord. 2. The neurone and the tip of the pipette used to inject amino acids were visualized using Lucifer Yellow under a fluorescent microscope. The pipette was precisely located on the soma and dendrite of the neurone under visual control, and L-glutamate (Glu), L-aspartate (Asp), N-methyl-D-aspartate (NMDA), kainate (KA) and quisqualate (Quis) were ionophoretically applied with a short pulse. The potential changes were intracellularly recorded from the soma. 3. Sensitivity to Glu as tested with short pulses (1-2 ms) was almost the same at the soma and along dendrites. 4. The amplitude of the responses to NMDA produced at the soma and the proximal part of the dendrite was about the same as that of Glu, but smaller than that of Glu at the distal part of the dendrite. Suppression of the Glu potential by an NMDA receptor antagonist, 2-amino-5-phosphonovaleric acid (APV), was greater at the soma than at the dendrite, suggesting that the contribution of NMDA receptors to the Glu potential was greater at the soma. 5. Sensitivity to Asp was about one-half that to Glu sensitivity on the soma and even less on the dendrite. Sensitivity to KA was high at the soma and low at the dendrite. However, Quis responses were produced throughout the neurone. 6. The Quis response induced by the application of a short pulse showed two phases: a fast response followed by a very slow depolarization that lasted more than 10 s. 7. The fast Quis response was easily desensitized and insensitive to APV. The time course of the fast Quis potential was shorter than that of Glu. 8. The slow Quis response was more pronounced at the dendrites than at the soma and was reduced by the intracellular injection of EGTA, suggesting the contribution of Ca2+ in the cell, possibly mediated by a second messenger system. 9. Experimental results suggest that the distribution of excitatory amino acid receptors differs between the soma and the dendrites of spinal neurones.

2-Amino-5-phosphonovalerate

A novel synthetic vitamin D3 analogue, 2-beta-(3-hydroxypropoxy)-calcitriol (ED-71): its biological activities and pharmacological effects on calcium metabolism.

A novel vitamin D3 analogue, [2 beta-(3-hydroxypropoxy)-calcitriol: ED-71] showed a similar Ca-regulating activity as calcitriol in the in vivo and in vitro Ca mobilization test and ex vivo intestinal Ca absorption assay using vitamin D-deficient rats. The differentiation-inducing activity of ED-71 in mouse myeloid leukemia cell line (WEHI-3 cell) was slightly less than that of calcitriol. ED-71 distributes predominantly in plasma as an intact form and its half-life plasma was twice as long as that of calcitriol. Further study revealed that the higher binding potency of ED-71 to plasma-specific vitamin D-binding protein (DBP) compared with that of calcitriol accounts for its stability in the blood circulation. The pharmacological effect of ED-71 for the animal models with osteoporosis seemed to be better than that calcitriol. These results suggest that ED-71 should become a valuable therapeutic long-acting drug for patients with osteoporosis.
