[Double kidney and ureterocele associated with kidney calculi].

The simultaneous occurrence of infrequent alterations--double kidney, ureterocele and calculi--is reported. Following the operation performed in 2 steps the patient became free of complaints and according to the repeated controls the dilatation of the efferent system regressed, no reflux was observed.

Pre- and postoperative examinations in intestinal tumours of the pelvis.

Pre- and postoperative urological and lower urinary tract urodynamic studies were made in patients operated for intestinal tumours of the pelvis. It was established that urological complications often arise and they often escape detection during the examination or under the conventional surgical care. An examination and a control scheme is recommended for the early detection of the possible complications and for starting an adequate treatment in due time.

[Our experiences with uroflowmetry].

The uroflowmetry is an essential part of the complex urodynamic examinations. One of its advantages is the non-invasive behaviour; the patient does not suffer any inconveniences. It is suitable for the evaluation of the success in surgical interventions due to obstructions in the lower urinary tracts, for control of results of the medicamentous therapy or after operative procedures, which have as their purpose the improvement of the function of the detrusor and for the general differentiation of diseases which are accompanied by dysuric symptoms. The technique renders possible the registration, the preservation of date and means an unlimited repeatable methody.

Horseshoe kidney and its therapeutic problems. (A review of seventy-one clinical cases).

A review of 71 cases of horseshoe kidney accounting for 0.12% of the total number of patients seen at the Department of Urology, Semmelweis University Medical School, Budapest, over 25 years is presented. The incidence of the abnormality has been found twice as high in males as in females. The proportion of surgical cases has been 57.8%, in agreement with published data. The radiodiagnostic investigations, particularly the functional diagnostic studies, are dealt with in detail. Their decisive importance in the considerations for or against surgery is emphasized.

A simple uroflowmeter for routine use.

A simple uroflowmeter has been constructed which, according to the experience gathered so far, can be used satisfactorily for the urodynamic examination of the lower urinary tract.

[Clinical aspects of urethral diverticula].

Experience with the treatment of 39 cases of urethral diverticula between the years 1954 and 1974 is reported. The patients were 32 women and 7 men. In 7 cases (3 women and 4 men) the condition was congenital in origin. In 30 patients the first symptoms appeared between the 31st and 60th year. In a 12 months old infant a hazelnut sized diverticulum in the posterior urethra caused complete ischuria. In two-third of the patients symptoms of cystitis dominated, dyspareunia in women (8%) and prostatitis in men (16%) had a lower incidence. 80% of the diverticula were infected; they contained stones in 3 cases. In one case the diverticulum recurred and histology revealed in its wall a planocellular carcinoma. Thirty-five patients were subjected to operation. Postoperative fistula developed in 3 cases which had to be re-operated. In cases of recurring urethritis, cystitis and prostatitis, urography is recommended, since the symptoms might be due to the presence of urethral diverticula.

[The double-pyelon kidney with special regard to the transport function (author's transl)].

The double-pyelon kidney, i.e. the total or partial duplication of the urinary transport system may involve both diagnostic and therapeutic difficulties. Observations are presented on the strength of 398 cases with total or partial duplication of the upper urinary tract. The incidence rate was 0.7 percent. The various types of duplication are presented with the diagnostic and therapeutic features involved. Transport function is discussed in detail as investigated under ideal physiologic conditions: during excretory urography under the amplifier screen. Aiming at early detection of the congenital anomaly and specification of the appropriate management, a urologic check-up is suggested even with mild presenting symptoms. Special emphasis is given to the consistent day-time enuresis in children over 3 years old, as a possible sign of ectopic ureter. Both children and parents are saved much unpleasantness by early detection, proper management and considerate surgery.

Metabolism of a prednisolone compound in human under normal and pathological conditions.

A method has been presented for the quantitative determination of 21-deoxy-N-(N'-methyl-piperazinyl)-prednisolone (Depersolone), a water soluble corticosteroid, and one of its metabolites, prednisolone, in human urine. Following administration of 30 or 90 mg Depersolone i.v., the dynamic appearance of this hormone and the prednisolone metabolite, their biological half-lives and the effect of the synthetic corticosteroid on the excretion of grouped and individual 17-ketosteroids were determined. Results of experiments revealed a dose dependent excretion of the unchanged hormone and a biological half-life 12 to 23 h in control subjects. Prednisolone as a metabolite seems to play a minor role in Depersolone action. The excretion rate of Depersolone was greater and its half-life less than normal in hepatopathies. The dynamic appearance of the hormone was found slow in the urine of patients with prolonged corticosteroid therapy.
