Rhinocerebral mucormycosis: use of liposomal amphotericin B.

Rhinocerebral mucormycosis is a rare but often fatal condition characterized by an aggressive necrotizing infection spreading from the nose to the paranasal sinuses, orbit and hence to the central nervous system. A case is reported in which a diabetic male with advanced mucormycosis was successfully treated by a combination of surgery, supportive therapy and liposomal amphotericin B. Liposomal delivery allows the drug to be both less toxic and more effective, and this is the first reported case of its use in rhinocerebral mucormycosis.

The issue of substance abuse in the health care industry. A preliminary survey of managerial thinking.

Substance abuse in the workplace has been a growing concern in recent years and ways to detect employees and applicants who use drugs has been the subject of much debate. In this article, the results of a survey on the attitudes of medical practice human resource managers on drug testing of both employees and applicants are given. For this study, 273 human resource managers were sent questionnaires detailing various attitude statements and were asked for their level of agreement. Categories of questions included the administrators' general philosophy of drug testing, their attitudes toward the organizational impact of drug testing, their attitudes toward the implementation of drug testing and their attitudes toward specific issues pertinent to drug testing in medical groups.

Potential usefulness of 99mTc-labeled monoclonal antibody A7 for immunoscintigraphy of human pancreatic carcinoma.

BACKGROUND: To improve survival rates of patients with pancreatic carcinoma, it is critical to develop new technology for early and certain diagnosis. One possibility is the application of radioimmunoscintigraphy using radiolabeled monoclonal antibodies. MATERIALS AND METHODS: Monoclonal antibody A7 (Mab A7) was labeled with technetium-99m (99mTc) and injected into athymic nude mice bearing human pancreatic carcinoma xenografts in order to examine its usefulness for radioimmunoscintigraphy. RESULTS: The binding activity of 99mTc labeled Mab A7 was nearly identical to that of non-labeled Mab A7. When 99mTc-labeled Mab A7 was injected intravenously into tumor-bearing nude mice, tumor accumulation of 99mTc-labeled Mab A7 increased until 24 hours after injection and then decreased slowly. The tumor tissue/blood ratio of radioactivity was significantly greater than that of normal mouse IgG. CONCLUSION: These results suggest that 99mTc-labeled Mab A7 is suitable for radioimmunoscintigraphy of human pancreatic carcinoma.

[Endocrinological disorders in association with alopecia areata-a 27 patients study].

Alopecia areata is a dermatological disease, characterized by the loss of hair, which affect men, women and children and can evaluate alone or in association with a variety of other disorders. Between these endocrinological diseases, especial thyroid disorders, have a high incidence. Twenty-seven patients with alopecia areata (12 women and 15 men) aged between 3 and 46 years were endocrinologically investigated. Eighteen of them (66.6%) had endocrinological disorders. Thyroid diseases were present in 10 cases (37%): 4 cases with endemic goiter, 2 cases with nodular goiter and 4 cases with hypothyroidism (1 case with autoimmune thyroiditis, 1 case with nodular goiter, 1 case with cystic goiter and 1 case with hypothyroidism post thyroidectomy for thyroidal lymphoma). Twelve cases (44.4%) were found with tetania. The incidence of thyroid diseases in alopecia areata is higher then in general population (2%), as well as the incidence of tetania. These evidences suggest that it is necessary to make a screening of endocrinological disorders in patients with alopecia areata.

[Treatment of alopecia areata with diphencyprone].

Topic immunotherapy in alopecia areata consist in a mild contact dermatitis with help of a chemical substances, with a high potency of sensitization, such as dinytroclorbenzenul (DNCB), squaric acid dibutylester (SADBE) and diphencypronil (DPCP). Eight patients with alopecia areata, 3 with mild form and 5 with severe form was treated with DPCP in acetone solution. We have obtained a positive result in 3 cases (37.5%), one with total regrows and 2 with partial regrows and a negative result in 5 cases, 3 with partial regrows and loss of the hair in other areas and 2 cases with no response after 24 weeks. We consider this method like an alternative therapy in severe alopecia areata resistant at other treatments.

Experimental research about the influence of pentazocine and reserpine on morphine addiction.

By means of a conveyer belt two types of conditioned reflexes were developed in rats, at two functional levels. When these had become stable, we observed the animals' behaviour during a morphine treatment, until they developed tolerance. Thereafter the treatment with morphine was suddenly discontinued, or morphine was replaced by pentazocine or reserpine. We found that morphine impaired both types of conditioned reflexes and that ni about two weeks drug tolerance appeared. When treatment was discontinued a syndrome of abstinence developed, which exclusively impaired the conditioned reflex of the second order. Pentazocine and reserpine prevent such manifestation of the abstinence syndrome. Moreover, reserpine suppresses the tolerance to morphine. Concerning these results we suppose that opioid receptors type mu are involved in behavioural trouble produced by morphine, while opioid receptors type k are involved in the abstinence syndrome. Certain adrenergic mechanisms operate both in the development of tolerance and in behavioural modifications caused by the syndrome of abstinence.
