Duplications of the bulbous urethra: clinicoradiologic findings and therapeutic options.

OBJECTIVES: We evaluated the clinical and radiological aspects of isolated duplications of the bulbous urethra and assessed the results of surgical treatment. METHODS: The experience with 11 male patients presenting with duplications of the bulbous urethra is reported. The duplications were incomplete in 7 cases and complete in 4, the accessory channel was ventral in 8 patients and dorsal in 3. The presenting symptoms were seldom indicative of the condition. Surgical treatment was performed in 10 cases and consisted in transurethral resection of the septum between the two channels in 2 patients, one-stage urethroplasty in 6 and two-stage urethroplasty in 2. RESULTS: With a follow-up ranging between 1 and 4 years the results were considered satisfactory in 4 cases and excellent in 6. CONCLUSIONS: Duplications of the bulbous urethra deserve careful evaluation of the cost-benefit ratio when surgical treatment is considered. The goal of reconstructive surgery, which can be open or transurethral, should be a functional urethral channel.

Dorsal free graft urethroplasty.

PURPOSE: Dorsal free graft urethroplasty was performed to reduce the incidence of urethrocele. MATERIALS AND METHODS: We treated 12 patients with penile and 13 with bulbous strictures. Of the 13 patients with a bulbous stricture 6 received a dorsally placed tube graft and 7 received a patch graft. RESULTS: Temporary fistulas were seen on postoperative urethrography in 5 cases but they all resolved spontaneously. At a mean followup of 35.8 months clinical and radiological findings were excellent in 23 cases and good in 2. No signs of graft weakening, such as post-void dribbling or diminished ejaculation, were apparent. CONCLUSIONS: The use of free skin grafts for urethral reconstruction is anatomically healthier in the dorsal than in the ventral position.

Reoperative surgery for recurrent strictures of the penile and bulbous urethra.

PURPOSE: We evaluated the outcome of reoperation for anterior urethral strictures. MATERIALS AND METHODS: Strictures that recurred after urethroplasty were repaired by various procedures in 20 patients (penile urethra in 8 and bulbous urethra in 12, mean followup 57 months). RESULTS: Of the recurrent penile urethral strictures 3 showed excellent and 5 satisfactory results, compared to 9 and 3, respectively, for bulbous urethral strictures. Overall 60% of the cases had an excellent outcome and no failures were documented. CONCLUSIONS: Any type of urethroplasty may fail even after a considerable period but reoperation with proper indications can offer a lasting solution.

[Non surgical treatment of benign prostatic hypertrophy: phytotherapy].

Pharmacological treatment of voiding disorders due to benign prostatic hypertrophy offers a wide variety of pharmacological options. Phytotherapy represents one of the oldest drug used in the treatment of BPH. In the last decade, many studies have been performed to demonstrate efficacy and tolerability of this drug. We report a short literature review on mechanism of action and objective and subjective results of treatments with plant extracts. We report on a comparation between literature data and data of our trials. Safety, tolerability and subjective results seems to indicate that plant extract are still a good pharmacological option in the treatment of BPH micturitional disorders.

[Evaluation of acute cardiotoxicity from the combination cyclophosphamide-mitoxantrone-5-fluorouracil (CMF) with Holter ECG].

By making use of a twenty-four hour Holter monitoring, it as been possible to compute the acute cardiotoxicity of the cyclophosphamide + mitoxantrone + 5-fluorouracil (CNF) association in twenty oncologic patients (pts) each of whom being immune from organic cardiopathy emerging clinically and at their first cycle of chemotherapy. The following parameters have been computed: meaningful changes in the heart frequency; premature atrial and ventricular depolarizations, both as a first appearance and as a clear growth in the number; the ST dislocation entity; malignant ventricular arrhythmias. The administration of CNF at the doses of: 600 mg/m2 of cyclophosphamide, 12 mg/m2 of mitoxantrone and 600 mg/m2 of 5-fluorouracil , has caused a meaningful increase in the heart frequency on 6 pts (30%), an increase of premature atrial depolarization on 4 pts (20%) with an appearance ex novo on 2 pts (10%), an increase of premature ventricular depolarization, without any passing to superior Lown classes, on 2 pts (10%) with an appearance ex novo on 3 pts (15%). Although the results in the study point out a frequency percentage of simple hyperkinetic arrhythmias equal to the 55%, the lack of more serious hyperkinetic arrhythmias and of intense disorders of ventricular repolarization testified to a synergic effect as a determining factor on the acute cardiotoxicity of the previously discussed association, in our opinion.

Undiversion with ileocecal segment following high transureteroureterostomy.

A boy presenting with a small bladder and refluxing blind-ending ureteral stumps following multiple attempts at ureteral reimplantation and supravesical diversion with a high left-to-right transureteroureterostomy and right cutaneous ureterostomy underwent successful undiversion. Surgery consisted of bilateral ureterectomy and ileocecocystoplasty, with intussusception of the ileocecal valve and anastomosis of the ileal tail to the right renal pelvis.

[The diagnostic value of delta-R and delta-Q as positive criteria of the ergometry test in a group of patients with angina pectoris of effort].

ECG-exercise tests were performed in 29 patients affected by effort chest pain. All patients underwent coronary angiography and/or revealed positive results in the cycloergometer effort test. The aim of the study was to evaluate ultrasonic variations in the R and Q waves due to maximal effort, so as to increase the information provided by the effort ECG. In the coronary angiographic test, 21 out of 29 patients revealed significant hemodynamic coronary stenosis; 10 patients were affected by previous myocardial necrosis; 8 patients showed no hemodynamic coronary lesions (OV). In the OV patients a statistically significant increase was observed in the Q wave at maximal effort in comparison with average basal values; on the other hand, there was a decrease, although not significant, in voltage at peak effort in patients with coronary stenosis. R wave amplitude was smaller in comparison to basal values in OV patients, while a significant increase was observed in the 21 patients with coronary lesions.

Can body surface mapping improve the diagnostic power of standard electrocardiography in effort myocardial ischemia?

Limitations of the standard 12-lead electrocardiogram in sensitivity, specificity and information content on the effort induced ischemic process might be partly due to an inadequate sampling of cardiac electrical events. An extensive array of electrodes is an effective way to verify this hypothesis. Actually body surface mapping provides: a 10-15% increase in diagnostic yield mainly in patients with mild coronary artery disease and elusive signs of ischemia in standard leads a very approximate indication of the extent of coronary artery disease a substantial contribution to the identification of the site of ischemia. Regarding the last point, preliminary results of an ongoing study are reported. One hundred and three patients with recent myocardial infarction have been studied by exercise test with simultaneous recording of surface map and Thallium 201 scan. So far in the subset with inferior myocardial infarction (76 patients) three different map patterns corresponding to different ischemic regions (anterior, inferior and posterior) have been identified. In anterior myocardial infarction (27 patients) a characteristic map pattern for exercise-induced myocardial ischemia has been observed, apparently able to discriminate between ischemia and the changes of early repolarization induced by the dyskinesia of the infarcted ventricular wall. In conclusion, electrocardiographic mapping makes a practical contribution to the understanding of ischemia-induced cardiac electrical events.

[Early antibiotic therapy following urodynamic examination. Results of a randomized, controlled study].

Forty-three young men from the Italian army underwent urodynamic tests following the diagnosis of enuresis. Of these, 37 were included in an assessment trial to define the rationale for early anti-bacterial therapy following the test. The subjects were subdivided into two groups: one group received 500 mg Cinoxacin b.i.d. for 5 days, and the other group was not treated. The comparison of results revealed a high incidence of irritative disorders in both groups (78.9% of treated subjects and 88.9% of untreated subjects) but the most significant complications were observed in the untreated group (feveret in 27.7% and one case of septic fever). Early anti-bacterial therapy following standard urodynamic tests therefore seems to be a ration tool in urological practice.

Our experience with sacral evoked potentials in impotence.

Pudendal-pudendal and pelvic-pudendal sacral evoked potentials (ESP) were studied in 40 patients complaining of erectile impotence. Electromyographic results allowed us to recognize the organic cause of impotence in 4 men with visceral neuropathies. Sacral evoked responses extend our diagnostic possibilities in evaluating erectile impairment secondary to subclinical neurological abnormalities.
