Haemodialysis and hypothalamo-pituitary-testicular axis.

Disturbances in testicular function develop in uraemic patients, but the effect of haemodialysis on them has not been determined definitively. Because of this, 22 patients with chronic renal failure who were receiving regular dialysis treatment underwent studies of the hypothalamo-pituitary-testicular axis before, during and after a dialysis. On all these occasions, serum testosterone concentrations were lower than the values found in 26 normal men. In contrast, serum concentrations of FSH and LH were higher than normal. 13 of these patients showed a normal response of the hypothalamo-pituitary system to the clomiphene stimulation test and a diminished response of Leydig's cells to the human chorionic gonadotropin stimulation test. It is concluded that haemodialysis does not affect the impaired gonadal function of male uraemic patients.

Early events in various forms of experimental acute tubular necrosis in rats.

Metabolic and morphologic changes occurred in the kidneys of rats within 3 hours after inciting acute tubular necrosis by completely clamping the renal blood supply, by intramuscular injections of glycerol, and by subcutaneous injections of HgC12. Although the initial trend was for p-aminohippurate and tetraethylammonium transport to decrease and for oxygen consumption, ammonia production, and gluconeogenesis to increase after glycerol, all of these parameters changed in opposite directions after renal pedicle clamping and after subcutaneous HgC12 (4.7 mg. per kg;). In addition, early morphologic changes in glycerol-injected rats differed from those seen with pedicle clamping and low dose HgC12. With high dose HgC12 (25 mg. per kg.), the metabolic and morphologic changes were somewhere in between those seen with the other insults. Coinciding with early metabolic and morphologic changes, cardiac output and renal blood flow decreased soon after the glycerol was given. On the basis of our findings, we cannot ascribe all of the early metabolic and morphologic changes in the glycerol model to ischemia, and we postulate that the circulating heme proteins may be nephrotoxic to ischemic renal tissue.

Interleukin-1 beta and tumor necrosis factor-alpha in normal/infertile men.

Cytokines play an important role in intercellular communications. Human sperm contains a wide spectrum of cytokines. such as interleukin-1 beta (IL-1beta) and tumor necrosis factor alpha (TNF-alpha). Their effects on semen quality are subject to debate. The aim of this study was to determine concentrations of IL-1beta and TNF-alpha in normal fertile men and in different groups of male infertility in an attempt to clarify the physiology and suggest possible clinical uses. Sixty-six subfertile male patients with varicocele (n = 22). infection of accessory genital glands (n = 14), varicocele plus infection (n = 4), chronic epididymitis (n = 8). post-renal transplantation status (n = 5), idiopathic oligoasthenoteratospermia (n = 9), cryptorchidism (n = 1), and homozygous beta-thalassemia (n = 3) as well as 5 male controls were studied through history, physical examination, spermiograms, plasma basal hormonal levels, and IL-1beta and TNF-alpha levels in seminal fluid. There was no significant statistical difference regarding IL-1beta and TNF-alpha among fertile men and subfertile patients of any cause. 1L-1beta and TNF-alpha were in tight positive correlation (p<.001). Determination of IL-1beta and TNF-alpha does not provide useful information in male routine infertility workup. Nevertheless, a better understanding of these mediators in semen of normal men and infertile patients may contribute to a new approach to the management of male infertility.

Haplotypes of human leukocyte antigens among patients with nasopharyngeal cancer in Greece.

The concept that the major histocompatibility complex (MHC) plays an important role in the pathogenesis of nasopharyngeal cancer (NPC) in several ethnic groups has gained increased attention during the last 15 years. Earlier studies have suggested that an increased risk of NPC is associated with specific phenotypes of human leukocyte antigens (HLA). The present study was performed to examine the association of HLA specificities and haplotypes with NPC in a Greek population. In a genotypical study of 53 patients, a significant association between the haplotype HLA-B5DR11 and NPC was found, mainly in patients > 45 years. Also, the above haplotype was significantly associated with longer disease-free interval. HLA-B5 and HLA-B5DR11 were more often seen among patients with squamous cell histology than among those with the undifferentiated type. These results suggest that MHC loci are probably implicated in the pathogenesis and outcome of NPC in Greek patients.

A comparative clinical trial of duodenal ulcer healing with two regimens of cimetidine: 800 mg once nightly and 400 mg twice daily.

The efficacy and safety of two dosage regimens of cimetidine were compared in a single-blind study of 50 adults with endoscopically proven duodenal ulcers. Patients were randomly allocated to receive 800 mg cimetidine taken once nightly or 400 mg cimetidine taken twice daily for 4 weeks. Following 4 weeks' treatment patients again underwent endoscopy and, if healing was incomplete, they received a further 4 weeks' treatment. At 4 weeks, healing occurred in 21/25 (84%) patients on the once-daily regimen and in 15/25 (60%) patients treated twice daily (P less than 0.05). Cumulative healing rates after 8 weeks' treatment were 92% and 96%, respectively. Symptomatic improvement was obtained to the same degree with both regimens. No adverse effects were recorded and laboratory values remained normal during both treatments. The results indicate that cimetidine given as one dose each night was superior to the twice-daily regimen.

Cardiotoxicity evaluation in patients treated with a mitoxantrone combination as adjuvant chemotherapy for breast cancer.

Indices of cardiac function were measured in 49 women who received adjuvant treatment for stage II breast cancer. The combination chemotherapy consisted of six monthly courses of cyclophosphamide, 500 mg/m2, mitoxantrone, 10 mg/m2 and fluorouracil, 500 mg/m2 (CNF). Left ventricular function was assessed by echocardiography, systolic time intervals and nuclear angiography. The values of the echocardiographic left ventricular end diastolic diameter (Dd) and end systolic diameter (Ds), and those of the index of preejection period (PEPI) and the ratio of the preejection period to left ventricular ejection time (PEP/LVET), determined in 41 patients before chemotherapy, at midcourse and after chemotherapy by echocardiography and systolic time intervals (STI), showed a slight but significant increase. All these values remained within normal limits. Resting nuclear angiography, performed before and after treatment, showed a decrease in LVEF by 10% or more in four patients; the postchemotherapy values remained within the normal range in all cases. In conclusion, adjuvant treatment of breast cancer patients with 6 cycles of the CNF combination, as judged by its effect on the measured indices, does not appear to be cardiotoxic.

High-dose leucovorin, fluorouracil and oral dipyridamole in the treatment of advanced colorectal cancer.

Fifty-five patients with advanced colorectal cancer were treated with the combination of leucovorin (LV), fluorouracil (FU) and dipyridamole (DP). Two (4%) patients achieved a clinical complete remission, 4 (7%) a partial remission, 24 (44%) had stable disease while 25 (45%) patients progressed during the chemotherapy period. Median survival was 47 weeks and median time to progression 19.5 weeks. Major toxicities included diarrhea (66%), leukopenia (45%), anemia (50%) and nausea/vomiting (44%). In conclusion, the addition of oral DP does not appear to improve the efficacy of the standard LV/FU regimen in patients with advanced colorectal cancer.
