[Reversible peripheral neuropathy induced by vitamin B12 deficiency].

OBJECTIVE: To describe an electroclinical presentation of a peripheral neuropathy in a patient who suffered from vitamin B12 deficiency. PATIENT AND METHODS: We report the case of a 56-year-old man who, 17 years after a subtotal gastrectomy, presented with a diffuse sensory-motor demyelinating peripheral neuropathy with multifocal alteration of conduction confirmed by electromyography. Its causal relationship with vitamin B12 deficiency was biologically demonstrated. This neuropathy improved within a few days after intramuscular hydroxocobalamin treatment. Both clinical and electrophysiological signs had totally disappeared 3 months later on. CONCLUSION: This observation demonstrates that, in spite of its poor outcome reported by several authors, peripheral neuropathy induced by vitamin B12 deficiency can recover completely with substitutive treatment.

Identification of viral agents causing diarrhea among children in the Eastern Center of Tunisia.

Viral diarrhea remains a major cause of childhood morbidity and mortality worldwide. In Tunisia, no comprehensive studies of all viral agents related to diarrhea in children have yet been conducted. The present study was performed to investigate the role of enteric viruses in acute diarrhea in the country. Six hundred thirty-eight stool samples were collected from children under 5 years of age seeking medical care for acute diarrhea between October 2003 and September 2005 in hospitals from the Eastern-Center Tunisia. All samples were tested for rotavirus, astrovirus, and adenovirus using commercial antigen enzyme immunoassays (EIAs). Positive samples for rotavirus and astrovirus were confirmed by an "in-house" reverse transcriptase-polymerase chain reaction (RT-PCR). Samples positive for adenovirus antigen were subjected to further EIA screening for species F enteric adenovirus types 40 and 41. At least one viral agent was found in 30% of the specimens. The frequency of rotavirus, astrovirus, and adenovirus was 20%, 7%, and 6%, respectively. Of the stool samples containing adenovirus, 57% (20/35) were found to be positive for species F adenovirus types 40/41. Dual infections were found in 9% (17/191) of the positive samples. Enteric viruses appear to play an important role in pediatric diarrhea in Tunisia. The introduction of affordable viral diagnosis in pediatric hospitals will improve patient care by reducing the unnecessary use of antibiotics.

[Salivary duct carcinoma].

INTRODUCTION: Salivary duct carcinoma is a very rare tumor of the salivary glands that has been principally reported in the parotid gland. It displays histologically a striking similarity to ductal carcinoma of the breast. OBSERVATION: We report the case of a salivary duct carcinoma arising in the parotid gland of a 62 year-old man. The fact that the tumor was confined to the parotid gland has allowed a radical treatment. After two year follow-up, there was no evidence of recurrence. DISCUSSION: This tumor is characterized clinically by local aggressiveness, frequent metastatic evolution and poor prognosis. This case report illustrates both the clinicopathological features and the prognostic factors of this entity.

Monte Carlo studies of the Tunisian gamma irradiation facility using GEANT4 code.

This paper describes a complete Monte Carlo study of the Tunisian gamma irradiation facility (CNSTN) using the GEANT4 CERN's code. The work focused on the optimization of the absorbed dose distribution inside the irradiation cell, with and without product. For this purpose, 32 different points at the middle plane of the source rack, 29 positions along Z axis and 7 critical points, were carried out using PMMA dosimeters. Then, to achieve a given bulk density, boxes loaded with "dummy" product were used. Simulated and experimental results are compared and good agreement is observed. It is shown that Monte Carlo simulation improves process understanding, predicts absorbed dose distributions and calculates dose uniformity within product.

Evaluation of a total core antigen assay for the diagnosis of hepatitis C virus infection in hemodialysis patients.

Hemodialysis patients are recognized as a group at high risk of infection with hepatitis C virus (HCV). Therefore, such a population should be screened routinely for the presence of HCV viremia. Since nucleic acid techniques remain expensive and largely unavailable in many laboratories in the developing world, the present study assesses the clinical usefulness of the HCV core antigen enzyme immunoassay for the diagnosis of HCV infection in dialysis patients. One hundred seventy-five dialysis patients were screened for the presence of anti-HCV antibodies and HCV RNA in the serum. One hundred twenty-eight serum samples were collected from the 76 patients who were anti-HCV antibody- and/or HCV RNA-positive. These were evaluated for total HCV core antigen. Of these samples, 55 had sufficient volume to be further tested to quantify HCV RNA by reverse transcription polymerase chain reaction (RT-PCR). Genotyping of the HCV strains showed that the majority belonged to genotype 1b (77%). The HCV core antigen assay showed a sensitivity and specificity of 84% and 89%, respectively. The use of core antigen assay has enabled the early detection of three patients who developed an acute hepatitis C infection during the period of study. A correlation study was undertaken between the quantitative values of viral load, expressed as pg/ml of HCV core antigen in serum, and viral RNA in UI/ml. A significant correlation was observed (Pearson's correlation coefficient: 0.552; P<0.001). In conclusion, detection of HCV core antigen in serum is an inexpensive, reliable, and highly specific assay that can be useful in most laboratory settings to diagnose HCV infection, and especially in laboratories where nucleic acid technologies are not yet available.

[DNA content analysis in thyroid neoplasms: diagnostic and prognostic interest].

Thyroid nodules are frequent and sometimes they pose a diagnostic and prognostic problem. DNA ploidy study and cell cycle analysis could be of value in the distinction between benign tumors and malignant tumors. Formalin-fixed and paraffin-embedded tissues from 69 patients with different benign and neoplastic lesions were investigated. Nuclear DNA content in thyroid cells was measured after Feulgen staining using SAMBA 200 image analysis system. A diploid DNA stemline was revealed in 75% of histologically proven benign thyroid tumors (15/20) and aneuploidy was found in 57.2% of malignant tumors (28/49). There is a significant correlation between aneuploidy and extra-thyroid extension (p=0.007) and bilateral and/or mediastinal lymph node metastasis (p=0.02). In the majority of benign tumors (19/20), the proliferation index was lower than 3% (< or =3%) however, this index value was higher than 3% (>3%) in more than 83% of malignant tumors (41/49) (p<0.001). The S phase fraction analysis revealed that the threshold of 14% divide the near whole of benign and malignant tumors (p<0.001). Our findings show that in follicular lesions, proliferation index and S phase fraction study appears interesting and helpful in the distinction between benign and malignant tumors, and aneuploidy seems more interesting in prognosis evaluation of these tumors.

[Human subconjunctival dirofilariasis: two Tunisian case studies].

Two new cases of human subconjunctival dirofilariasis are reported in Tunisia. Dirofilaria immitis was identified in the first case and Dirofilaria repens in the second. Diagnosis was made by histological examination of excised nodules. We note that the patients are young (8 and 10 years, respectively) and that cases of human dirofilariasis caused by Dirofilaria immitis are present in Tunisia.

[Uterine smooth muscle tumors: retrospective epidemiological and pathological study of 2760 cases].

INTRODUCTION: Smooth muscle tumors of the uterus are frequent. Most of them are benign. Some leiomyomas may have unusual morphologic features difficult to distinguish from leiomyosarcoma. These tumors are: cellular leiomyoma, atypical leiomyoma and mitotically active leiomyoma. OBJECTIVES: The purpose of our work is to study cases of leiomyosarcomas, cellular leiomyoma, atypical leiomyoma and mitotically active leiomyoma among a large series of uterine smooth muscle tumors. MATERIALS AND METHODS: We reviewed retrospectively 2760 uterine smooth muscle tumors. The slides were reviewed and the tumors reclassified according to the criteria of the WHO 2003 classification. Chi-square test or Fisher's exact were used for statistical analyses as appropriate. RESULTS: Review of the slides demonstrated: 12 mitotically active leiomyomas, 18 cellular leiomyomas, 20 atypical leiomyomas, 16 leiomysarcomas, only one case of smooth muscle tumor of uncertain malignant potential. The 2709 remaining tumors were all common leiomyomas. So mitotically active, cellular and atypical leiomyomas were as rare as the leiomyosarcomas. The average age of patients with leiomyomas was 39 years. That of patients with leiomyosarcomas was 54 years (p=0,000002). Average size of leiomyomas was 7.4cm. That of leiomyosarcomas was 10.2cm. The average age and size of the 3 studied variants of leiomyomas were identical to those of the common leiomyomas. Leiomyomas were unique in 60.9% of cases. On the other hand 87.5% of leiomyosarcomas had unique nodules (p=0,04).

[Large bilateral lung opacities].

We report the case of a 42-year-old woman operated for neurofibroma of the scalp on two occasions who consulted for bilateral chest pain which reveated two voluminous bilateral pulmonary masses. Biopsy of one of these masses was in favor of secondary or primary leiomyosarcoma. The association of Von Recklinghausen's disease and soft tissue sarcoma is rare: most often neurofibroma which degenerates is observed. Other types of sarcoma, like leiomyosarcoma, have been described.

Detection of group a rotavirus strains circulating in calves in Tunisia.

Faecal samples were collected from 89 dairy calves to determine the prevalence of rotavirus infection in Tunisia and the genomic diversity of bovine rotavirus strains. After screening of all faecal samples by enzyme-linked immunosorbent assay, rotavirus strains were analysed by RNA polyacrylamide gel electrophoresis and characterized antigenically by monoclonal antibodies to the VP6 subgroup. The VP7 genotype was determined by nested RT-PCR. Of the 89 calves tested, 27 (30%) were positive for rotavirus antigen. Four different long electrophoretypes were identified. All VP6 typeable strains carried the subgroup I specificity. G8 genotype was the most prevalent, but G6 and mixed strains G(6 + 8) were also detected.

[Well differentiated neuroendocrine carcinoma of the stomach: a case report].

Gastric well differentiated endocrine carcinomas were previously believed to be rare lesions, representing less than 1% of gastric neoplasms. These tumours are often associated with chronic atrophic gastritis, rarely with Zolling-Ellison syndrome. Some sporadic lesions are rarely reported. The prognosis is relatively good, but sporadic forms are more aggressive. We report a case of gastric endocrine carcinoma developed by a 52 year-old man classified as sporadic form after many investigations. Through this observation and after a review of literature, the clinicopathologic features of these tumours, their histogenesis and clinical behaviour are reviewed and discussed.

Epidemiological and antigenic analysis of respiratory syncytial virus in hospitalised Tunisian children, from 2000 to 2002.

Human respiratory syncytial virus (RSV) is the major viral cause of lower respiratory tract disease in children. Infections with the virus occur as annual winter epidemics in temperate climates, placing considerable pressure on the provision of hospital beds. Most molecular epidemiological studies have, until now, focused on isolates from infants in industrialised countries. No data have been available with regard to RSV strains from northern Africa. In this report, a recent RSV outbreak in Tunisia was studied and results showed that 176 of 815 (21.6%) nasopharyngeal aspirates collected from hospitalised children were RSV-positive by immunofluorescence assay. This RSV outbreak showed a temperature-dependent pattern (P=0.026) but no significant association with rainfall. A total of 73 RSV-positive samples were tested by two reverse-transcription-polymerase chain reaction assays (RT-PCR): RT-PCR-1, which amplifies the RNA of all RSV strains, and RT-PCR-2, which allows subgroup classification of RSV. Analysis by hybridisation assay of RT-PCR-2-amplified 1B protein gene products showed a higher prevalence of group B RSV than that of group A (82.5% vs. 17.5% among the typed strains). Knowledge of the variants is important in terms of both diagnosis and definition of a vaccine composition.

Evaluation of a prototype HCV NS5b assay for typing strains of hepatitis C virus isolated from Tunisian haemodialysis patients.

Hepatitis C virus (HCV) strains isolated from 68 haemodialysis Tunisian patients exhibiting chronic infection were genotyped targeting the NS5b region of the HCV genome using a prototype assay developed by Bayer HealthCare-Diagnostics (TRUGENE NS5b HCV). The overall results were compared to those obtained with another assay of the same company based on sequencing of the 5' non-coding region (TRUGENE HCV 5'NC genotyping kit). All strains could be typed by the 5'NC typing kit, but only 62 (91; 2%) by the NS5b prototype assay. All the 62 strains typed by both methods exhibited the same pattern at the type level: 57 were type 1, 3 were type 2, and 2 were type 4. At the subtype level, eight strains that gave undetermined results by the 5'NC kit were successfully typed by the NS5b kit; eight additional strains exhibited discrepant results. The overall agreement between the two assays was 74.2% at the subtype level. In conclusion, the NS5b region appears to be much more accurate than the 5'NC region to subtype HCV strains, especially in those isolated from patients attending haemodialysis centres where the subtype distribution suggests frequent nosocomial transmissions.

[High prevalence and incidence of hepatitis C virus infections among dialysis patients in the East-Centre of Tunisia].

Hemodialysed patients are recognised as a group at increased risk of infection with hepatitis C virus (HCV). The aim of this study was to determine the prevalence and incidence of HCV infection among dialysis patients of the east-centre part of Tunisia. Two hundred and seventy-six patients dialysed until 2001 were recruited within seven hemodialysis units located in the cities of Sousse, Monastir and Mahdia. The serum markers of HCV infection were tested over the period of March 2000-December 2002, by a 3rd generation ELISA test for antibodies and by qualitative RT-PCR technique for viral RNA. The prevalence of anti-HCV antibodies and of HCV RNA was 32.6% (90 patients) and 25.7% (71 patients), respectively. Between 1998 and 2002, 20 new infections were documented in five of the seven dialysis units corresponding to an incidence of 2.34% per year, with an average time of contamination after the beginning of dialysis of 4.6 years. If all the infections are assessed to have occurred during dialysis, the density of incidence of HCV contamination was 4.4% per year of dialysis. A high correlation was noticed between the presence of HCV markers in serum and the duration of dialysis (F = 34.15, P < 0.0001). In the absence of other risk factors (transfusion, drug-addiction), these results plead for the nosocomial transmission of the observed HCV infections. A phylogenetic analysis of the E2 hypervariable region of the viral genome is in progress to confirm this assumption.

[Lymphoepithelioma-like carcinoma of the uterine cervix: case report].

We report a case of a 79-year-old Tunisian woman who presented with post-coital bleeding. A polypoïd mass of the cervix was discovered. Histopathological examination of the specimen biopsy showed a squamous-cell carcinoma. The patient underwent radical resection. The definitive diagnosis of lymphoepithelioma-like carcinoma of the cervix was retained after histopathological and immunohistochemical examination. There was no local involvement and no metastasis. The lymphoepitheloma-like carcinoma of the uterine cervix is a rare entity with distinct morphological features and a usually good prognosis.

Epidemiology and clinical presentation of respiratory syncytial virus infection in a Tunisian neonatal unit from 2000 to 2002.

Respiratory syncytial virus (RSV) is an important viral pathogen causing lower respiratory tract infection (LRI) in infants. This study describes the clinical and genetic epidemiology of RSV infection among Tunisian neonates. Nasopharyngeal aspirates collected from 268 newborns with LRI were screened for RSV by immunofluorescence assay. Positive samples were analysed by RT-PCR-hybridisation assay for subgroup classification of RSV genomes. RSV infection was present in 23.1% of neonates, with a predominance in males. Peak incidence occurred in winter. Subgroup classification showed a higher prevalence of group B than group A strains. Nosocomially acquired RSV infection was present in 37% of neonates, 54.3% had an underlying condition predisposing to severe disease and 13% died. The average duration of hospital stay was 10 days and 87% of newborns required supplemental oxygen. As no currently effective treatment is available, preventive measures are a priority in high-risk infants.

[Retinaculum flexors: clinical and experimental anatomic bases in favor of reconstruction in carpal tunnel surgery].

The goal of this anatomical study is to prove that reconstruction of the flexor retinaculum in carpal tunnel syndrome surgery is a reliable procedure and can improve the results of this procedure. In the first static study, we noted that when left to heal spontaneously, after 3 months the gap between the two edges of the cut retinaculum remained equal to or even larger than the gap created at the time of the surgery. A special Canaletto implant was created to achieve this reconstruction. It is technically possible to do this reconstruction by a 2 to 3 cm surgical approach in the palm. In another, dynamic study, we used a transducer to analyse the forces borne by the retinaculum during flexor tendon contraction. We compared these forces in a normal retinaculum, a reconstructed retinaculum with the Canaletto implant and a retinaculum opened endoscopically. This reconstruction conserves the anterior continuity of the retinaculum, allows excellent carpal tunnel decompression and immediately permits near-physiological forces. We discuss the advantages and the limits of this reconstruction. We think that this new surgical technique in the Carpal Tunnel Syndrome (CTS) will be able to improve the results, particularly in recurrences, in wrist malunion with CTS, in cases with severe electromyographic change with a sensory conduction speed of under 15 m.s-1 and in metabolic CTS. As always, his implant needs to be evaluated through a mid and long term clinical follow-up. By recovering better grip strength and reducing postoperative pain, this surgical technique can decrease the time needed to return to work and hence the economic costs of CTS surgery.
