Sinusoidal fetal heart rate pattern in severe fetal anemia from feto-maternal transfusion.

A case is reported where a sinusoidal fetal heart rate pattern was found in a fetus at 34 weeks. This pattern, at first isolated and later persistent for several hours, during a normal pregnancy became progressively transformed into a silent pattern. Cesarean delivery then performed resulted in the birth of an infant affected by a severe anemia due to a major feto-maternal transfusion and it died aged 36 h. The authors enquire into the significance of this pattern when found in apparently normal pregnancies, not involving rhesus iso-immunization, and they suggest a possible management when confronted with a sinusoidal fetal heart pattern.

[Hysterectomy to stop bleeding in obstetrics. Are there still indications for performing hysterectomy immediately post-partum? (author's transl)].

13 cases of hysterectomy carried out to stop bleeding immediately after delivery have been studied. Among these there were : 5 cases of placenta accreta which confirmed histologically. 4 cases of uterine inertia. 2 cases of failure of coagulation, one of which occurred after there had been a retro-placental haematoma and 2 ruptured uteri. After analysing the clinical circumstances surrounding these 13 cases and the technical details in carrying out hysterectomy on the gravid uterus, the authors studied conservative surgical measures that could be used to deal with post-partum uterine haemorrhage : the hypogastric arteries on the one hand the efferent arteries supplying the uterus on the other hand can be ligated. The technique for this last procedure is illustrated in one case.

[Dystocia at the onset of labour. An evaluation of the different treatments available (author's transl)].

The authors have carried out a random study on 75 cases in order to evaluate objectively the therapeutic methods usually employed in cases of dystocia in starting labour. These studies have shown the superiority of Syntocinon using an infusion together with epidural analgesia. This attitude goes against the methods of treatment usually used in France. The authors explain the reasons for their choice in the light of factors that they have observed using other therapeutic methods.

[Acute leukaemia during pregnancy: favourable course of pregnancy in two patients treated with cytosine arabinoside and anthracyclines (author's transl)].

A report of two cases of acute leukaemia discovered during pregnancy. The first was treated from the 24th week with cytosine arabinoside, daunorubicine and vincristine and then rubidazone. Only an incomplete remission was obtained. The pregnancy ended with the birth at the 36th week of a normal child, which remains well at the age of 13 months. The second patient received during the 29th week of pregnancy chemotherapy combining cytosine arabinoside and daunorubicine. Signs of foetal distress appeared at 31 weeks. Delivery took place during the 33rd week in a phase of aplasia. The child was born apparently dead and was resuscitated. A complete remission of the leukaemia was obtained. The child was well at the age of 13 months. A transient neutropaenia was noted at the age of 2 months in both children.

[Acute upper genital tract infections. II. A. bacteriological study and the therapeutic consequences].

The authors have studied a prospective series of 62 cases of acute upper genital tract infections of different severities. Aerobic and anaerobic bacteriological examination were carried out from different sites: urine, blood, vagina, endocervix, intra-uterine devices and the pouch of Douglas. It is absolutely necessary to follow a strict technique according to the clinical examination in order to interpret the results according to a methodological plan. The results show that the gonococcus was found in 9 out of the 62 cases, which had a very varied flora. The value of the different smears and swabs is then discussed. In this way it is shown that specimens taken from the endocervix are definitely of great value although the ideal is to culture directly from intra-uterine devices or from specimens taken laparoscopically from the tubes. It is possible to give different courses of antibiotics: these should include however active substances against the group of anaerobes, which justifies adding metronidazole routinely. The use of corticoids which are often prescribed to limit the sequelae has not been proven to be of value.

[Severe acute genital infections (Based on 100 cases). A clinical study and the contribution of laparoscopy (author's transl)].

A continuous prospective series of 100 cases of severe acute genital infections which were collected over a period of 16 months at the Gynaecological and Obstetrical Clinic of the Saint Antoine Hospital has been studies using the same protocol. Analysing the history given by these patients before the onset of the illness shows how young they were and how frequently they were nulliparous, as well as the role of intra-uterine contraceptive devices. The correlation between the clinical features and those found on laparoscopy which was carried out almost routinely shows the great advantages of this examination. The authors in their discussion have tried, by analysing groups of symptoms, to show that to make an early correct diagnosis laparoscopy has to be used in a majority of cases. It allows treatment to be instituted quickly and unhesitatingly. Furthermore, it is justified because: It is necessary to find out all lesions that are present and which are unpredictable clinically. It renders it possible to carry out therapeutic manoeuvres (such as cutting adhesions and giving antibiotics locally) and to take swabs for bacteriological examination (this will be discussed in the second part of this article).

[Management of ovarian cysts].

Ovarian cysts occur frequently in women of reproductive age. These are usually functional cysts which resolve spontaneously and whose evolution can be followed with ultrasound. Non-functional cysts have diverse histologic origins. The most common are serous and mucinous cystadenomas which arise from the epithelial wall of the ovary, endometriomas which arise in the setting of pelvic endometriosis, and dermoid cysts which arise from the germinal cells of the ovary. Endovaginal ultrasound with Doppler enhancement is the best imaging technique to establish the nature of cysts and to distinguish cysts suspicious for malignancy which require more invasive investigation. Pelvic laparoscopy is the surgical approach of choice for the treatment of non-functional benign ovarian cysts. Conservative treatment to shell out the cyst and preserve functional ovarian tissue should be reserved for women desirous of future pregnancies. The risk of ovarian cancer remains a major preoccupation of the surgeon. Where malignancy is suspected, laparoscopy is contraindicated and a median laparotomy is appropriate for radical extirpative surgery. This article describes the diagnostic techniques which allow a laparoscopic approach to presumably benign cysts and discusses surgical techniques specifically adapted to their different histologic nature of ovarian cysts.

Fetal shoulder measurements with MRI.

OBJECTIVE: We studied the ability of MRI to predict fetal shoulder width (FSW). MATERIALS AND METHODS: In 30 patients referred for MR pelvimetry, measurement of FSW was performed and compared with caliper measurements at term. We report here the feasibility of a method using axial and coronal MR images oriented to the fetal body axes. RESULTS: Shoulder width by MRI (mean = 12.76 +/- 1.42 cm) correlated significantly with postnatal orthopedic caliper measurements (mean = 12.99 +/- 1.37 cm; r = 0.955, SEE = 4.29 mm, p = 0.00001) and with birth weight (r = 0.63, p = 0.0005). Mean paired differences showed a statistically significant 2.3 +/- 4.2 mm underestimation of FSW by MRI (p = 0.01). This reasonably quick nonionizing technique seems to have the potential for evaluating shoulder dystocia and deserves further evaluation.

[Arterio-venous fistulae of interest in the uterine vessels (author's transl)].

The authors report an interesting case of arterio-venous fistula in the uterine vessels in a young two-para of 23 years of age. A pulsatile and throbbing mass was found in the right parametrium at her post-natal visit after the second delivery, although no abnormal sign had been noted during the pregnancy. The diagnosis was confirmed by arteriography. Two surgical procedures have not been able to cure this patient entirely of these multiple fistulae which were congenital in origin. The production of an arterial embolus finally got over the difficulty of removing the residual troubles at the same time allowing the patient to keep her uterus. Following this case, the authors discuss the aetiology of acquired or congenital fistulae and the means by which this rare vascular pathological condition can be diagnosed and treated in the pelvis.

[Causes, diagnosis and treatment of uterine perforations by intrauterine devices].

Perforations of the uterus by intra-uterine devices (I.U.D.) are still at present serious complications. Their incidence is very varied and diverse because they are often asymptomatic. Pregnancy of a frequent circumstance under which they are discovered, as well as the more general way, which is the disappearance of the strings, which should always give rise to the suspicion that perforation has happened. There are two types to be distinguished from one another: Immediate perforation due to a faulty technique in insertion, secondary perforation due to gradual wearing down of the uterine wall. The factors predisposing to the occurrence of these accidents are numerous but notable among them are the date in the cycle when the insertion was done and above all the time in relationship to the last pregnancy, parity, the position of the uterus and the experience of the operator. Diagnosis of perforation is not always easy and may require not only radiological investigation without preliminary preparation but also hysterography, ultrasound examination and hysteroscopy. Withdrawal of a device that has perforated the uterus is advisable even if its migration has not given rise to any clinical symptoms. The technique for removing it varies often from case to cause between laparoscopy, laparotomy and culdotomy. Prevention of perforations should not only take into account the contra-indications to insertion but also knowledge of favourable factors, of a rigorous technique for insertion as well as clinical supervision in order to screen for secondary perforations.
