Lentoids within sacrococcygeal teratoma: origin by transdifferentiation?

Within a teratoma removed surgically from the sacrococcygeal region of a female newborn, clusters of lens-like cells (lentoids) surrounded by the immature tissue of the neural retina were revealed by routine histologic analysis. Comparison of the cytologic and microtopographic characteristics of lentoids that develop in experimental embryo-derived teratomas suggests that the lentoids within the sacrococcygeal teratoma originate by transdifferentiation (cell-type conversion, metaplasia) of cells of the immature neural retina or the pigmented retinal epithelium. The embryonic origin of sacrococcygeal teratomas is discussed in the context of complex morphogenetic features at the posterior end of the early embryo.

[Granular cell tumor].

Eighteen granular cell tumors (GTC) were detected from January 1976 until September 1988. GTC is found in 0.03% of all the biopsy specimens. Women have been found to be affected more frequently. The most common anatomic sites where it appears are the skin and subcutaneous tissue of the trunk and the vulva. Tumors ranged in sizes from 0.6 to 4 cm (average 1.5 cm). One patient had multiple lesions. The lesions show typical histologic features, usually with ill-defined edges. The preoperative diagnoses were never determined as GTC. Pseudoepitheliomatous hyperplasia of the overlying epithelium led to the wrong intraoperative diagnosis of squamous cell carcinoma in one lingual GTC. On gross inspection, one breast GTC was mistaken for invasive carcinoma with desmoplasia. One vulvar lesion was previously diagnosed as pseudo-decidual reaction. Malignant behaviour could not have been noticed, since it is of importance to pathologists and clinicians not to misinterpret this lesion as carcinoma.

[A neurogenic cyst of the ovary].

A case of a bilateral adult cystic teratoma of the ovary in a 45-year-old woman is reported. The cyst of the right ovary was a typical "ovarian dermoid". The cyst of the left ovary consisted of adult neurogenic tissue, with islands of the ependyma on the surface. The tissues of the endodermal and mesodermal origin were also present in small quantities. It is evident from the literature that "neurogenic cysts" are much larger than "dermoid cysts", and they are reported only in preburtal girls. In the case presented the "neurogenic cyst" was not substantially larger than the "dermoid cyst" in the contralateral ovary. The the authors' knowledge, this is the first case of a "neurogenic cyst" of the ovary in the reproductive life reported so far.

[Lipoleiomyoma of the uterus].

A case of lipoleiomyoma uteri in a 44-year old women is presented. The tumor, 9 cm in diameter, was situated beneath the serous membrane, at the back wall. There were also smaller leiomyomas, but the tumour described differed from them in being pale-yellow and having a somewhat softer consistency.

[Endometriosis in an episiotomy scar].

A case of perineal endometriosis at the site of on episiotomy scar is reported. The first disorders in the scar appeared soon after the second delivery at the woman's age of 25. Surgical treatment was applied six years after the appearance of the first symptoms. Endometrioma recurred briefly after the excision. Clinical data suggested decidual autologous transplantation during manual uterine exploration.

[Primary insular carcinoid tumor in a benign cystic teratoma of the ovary (author's transl)].

A case of primary, insular, argentaffin carcinoid tumor (argentaffinoma) of the left ovary, arising in a bening cystic teratoma in a 60-year-old woman is reported. The tumor was not accompanied by any signs of the carcinoid syndrome despite a large volume, prominent acinar differentiation, and strong argentaffinity. Considering the morphologic carcinoid classification by Soga and Tazawa, the case had a mixed A + C pattern. A few Paneth-like cells and a few small calcifications in the stroma were present. Microscopic, ultrastructural, and histochemical findings were identical with those of carcinoids arising in midgut derivatives. There were small mucinous areas about 1.5 cm in diameter. Microscopic examination revealed pools of mucin with more of less disintegrated acini. This morphologic feature was not as yet reported in the insular carcinoid tumor. The patient feels well one and half year after hysterectomy and bilateral salpingo-oophorectomy, without any signs of metastatic disease.

[Intravenous leiomyomatosis of the uterus. (A case report)].

A case of the intravenous leiomyomatosis of the uterus in a 53-year-old woman is reported. The patient complained of abdominal enlargement, frequent urination, and a sensation of pressure on the rectum and urinary bladder. Her uterus contained 3 fibroids which ranged from 1.5 to 11 cm in diameter. The largest fibroid contained numerous areas of hydropic degeneration. In the vascular space of the myometrium a polypoid ramifying thrombus 4 x 1 cm was present, arising fom the capsular aspects of large fibroids. It is the smallest tumor reported so far. st could have been overlooked very easily. Only one similar case was found in the literature. The case presented is in agreement with the observation made by Harper i Scully that "intravenous leiomyomatosis may not always appear in obtrusive form" and "that a careful search should be made for it in cases of uterine leiomyoma". The authors analyse the relation between intravenous leiomyomatosis and metastasizing fibroleiomyoma of the uterus. The lesions reported so far suggest that they were well differentiated smooth muscle tumors with a different biologic potential and that they should not be taken as different stages of the same process. It is not suitable to use these terms as synonyms.
