Medium-term results of trapezio-lunate external fixation for scaphoid fractures.

We treated 36 scaphoid fractures in 34 patients with trapezio-lunate external fixation. According to the Herbert and Fisher classification, there were 16 type B1, 14 type B2, four type B3 and two type B4 fractures. Trapezial and lunate pins broke in three patients and these were rated as failures. The remaining 33 fractures were followed-up for 3 years and all had excellent outcomes, without signs of arthritis, avascularity or instability.

Bilateral peroneal nerve palsy after simultaneous bilateral total knee arthroplasty. Report of a case with rheumatoid arthritis.

In this study, we report on a 46-year-old female patient with bilateral severe gonarthrosis due to rheumatoid arthritis. Simultaneous bilateral cemented total knee arthroplasty was performed. Bilateral common peroneal nerve palsy was observed on the second postoperative day. The electromyographic diagnosis was "bilateral axonotmesis". Complete clinical and electromyographic motor recoveries were seen on both sides within 6 months postoperatively. The sensorial deficit was still present on one side at 2 years postoperatively. Preoperative severe flexion contracture and epidural anesthesia were assumed as the risk factors for the development of the nerve palsy in this patient.

Prevention of heterotopic ossification after total hip replacement: a prospective comparison of indomethacin and salmon calcitonin in 60 patients.

We did a prospective consecutive study of prophylaxis for heterotopic ossification (HO) comparing indomethacin (100 mg/day) and salmon calcitonin (3 MRC-U/kg/day) for 14 days. Each group consisted of 30 patients. 19 patients in the indomethacin group and 2 in the calcitonin group developed HO. We conclude that use of calcitonin in the prophylaxis of HO after total hip replacement is more effective than indomethacin.

Arthroscopic-assisted percutaneous stabilization of patellar fractures.

The authors did percutaneous, arthroscopic-assisted osteosynthesis of patellar fractures associated with skin abrasions and lacerations in 11 patients. Skin problems did not delay the operation and the rehabilitation. The average followup was 2.8 years. All fractures healed without complications and good results were obtained in all patients using the criteria of Levack et al. This technique avoids the problems of patellar fractures with skin abrasions and may be adapted to other fractures of the patella.

Effect of aerobic and anaerobic metabolism on free radical generation swimmers.

PURPOSE: In this study, changes in antioxidant systems due to free radicals were investigated in short distance (100-m) and long-distance (800-m) swimmers, within whom the anaerobic and aerobic metabolisms dominate, respectively. METHODS: For this study, swimmers aged between 15 and -21 yr swam 800 m (N = 10) and 100 m (N = 9). Venous blood samples were taken before swimming, and at 1-, 20-, and 40-min intervals after swimming. Lactate, catalase (CAT), glutathione peroxidase (GPx), and reduced glutathione (GSH) levels were determined in the blood samples. RESULTS: The increase of lactate levels was statistically significant in the swimmers, both after the 100- and 800-m distances as compared with the preswimming levels (P < 0.001, P < 0.001). Catalase activity was increased in the first minute postswimming as compared with preswimming levels. Catalase activity then decreased at the 20- and 40-min intervals as compared with the 1-min postswimming interval, at both 100- and 800-m distances (P < 0.01, P < 0.001). GPx activity was also increased in the first minute after swimming as compared with preswimming levels. GPx activity then decreased at the 20- and 40-min intervals when compared with the 1-min postswimming level. This occurred in both 100- and 800-m swimmers (P < 0.001, P < 0.001). GSH activity was decreased in the first minute after swimming, compared with the preswimming levels. GSH activity then increased at the 20- and 40-min postswimming intervals, as compared with the first-minute level. Again, this occurred in both the 100- and 800-m swimmers (P < 0.001, P < 0.01). CONCLUSION: We concluded that both long-distance and particularly short-distance (100-m) swimming increased the activities of antioxidant defense enzymes.

Rotational profile of the lower extremity and foot progression angle: computerized tomographic examination of 50 male adults.

Acetabular, femoral and tibial torsion of 50 normal adult male subjects were measured by computerized tomography and the relationship between these angles and foot-progression angle was examined. The mean acetabular anteversion was 15.6 degrees on the right and 15.8 degrees on the left, (range 3 degrees-30 degrees). The mean femoral torsion was 6.5 degrees on the right and 5.8 degrees on the left (range 14 degrees-28 degrees). The mean tibiofibular torsion was 30.9 degrees on the right and 29.1 degrees on the left (range 16 degrees-50 degrees). Although the normal range of torsional measurements of the lower extremity was very broad, subjects usually had out-toeing, with a mean foot-progression angle of 13.7 degrees on the right and 13.0 degrees on the left (range 6 degrees-21 degrees). No correlation was detected on the rotation between different levels of the lower limb. No difference was detected in the lower extremity rotational profile between right and left sides.

Interspinous distance in congenital dislocation of the hip.

We measured radiographically the interspinous distance in 50 normal children and in 50 children with congenital dislocation of the hip. The value increased proportionally with age in normal children, but not in those with congenital dislocation of the hip. Our results indicate that a bony parameter, rather than the age of the child, is suitable for the indication of lower age limit for Salter's innominate osteotomy.

Evaluation of simple excision in the treatment of symptomatic accessory navicular associated with flat feet.

The results of 17 patients who had been treated by simple excision, for symptomatic accessory navicular were reviewed 2 to 5 years postoperatively. Although all patients had good or excellent results by subjective criteria, careful examination revealed difficulty in performing the "single-heel rise test" in 8 patients who also had preexisting flat feet. These results suggest the necessity for an objective evaluation system, as well as a different treatment approach, for the association of accessory navicular and flat foot.

Acoustic field variability induced by time evolving internal wave fields

A space- and time-dependent internal wave model was developed for a shallow water area on the New Jersey continental shelf and combined with a propagation algorithm to perform numerical simulations of acoustic field variability. This data-constrained environmental model links the oceanographic field, dominated by internal waves, to the random sound speed distribution that drives acoustic field fluctuations in this region. Working with a suite of environmental measurements along a 42-km track, a parameter set was developed that characterized the influence of the internal wave field on sound speed perturbations in the water column. The acoustic propagation environment was reconstructed from this set in conjunction with bottom parameters extracted by use of acoustic inversion techniques. The resulting space- and time-varying sound speed field was synthesized from an internal wave field composed of both a spatially diffuse (linear) contribution and a spatially localized (nonlinear) component, the latter consisting of solitary waves propagating with the internal tide. Acoustic simulation results at 224 and 400 Hz were obtained from a solution to an elastic parabolic equation and are presented as examples of propagation through this evolving environment. Modal decomposition of the acoustic field received at a vertical line array was used to clarify the effects of both internal wave contributions to the complex structure of the received signals.

Approximate expressions for viscous attenuation in marine sediments: relating Biot's "critical" and "peak" frequencies.

Simple approximate relations are proposed for the viscous attenuation per cycle of the fast compressional and shear waves in the low-to-intermediate frequency range. Corresponding closed-form formulas are derived for frequencies at which maximum viscous attenuation per cycle occurs according to the Biot-Stoll theory of elastic wave propagation in marine sediments. In the new formulas, Biot's approximation [M. A. Biot, J. Acoust. Soc. Am. 34, 1254-1264 (1962)] for the frequency-dependent viscosity correction factor F(f) and the assumption of relatively low specific loss (Q(-1)<(0.2) [J. Geertsma and D. C. Smith, Geophysics 26(2), 169-181 (1962)] are used to provide an accurate representation of the fast compressional and shear wave attenuation from low frequencies through a transition region extending to two or three times Biot's critical frequency f(c). The approximate viscodynamic behavior of marine sediments for the fast compressional and shear waves shows similarities to that of a "homogeneous relaxation" process for an anelastic linear element [A. M. Freudenthal and H. Geiringer, Encyclopedia of Physics (Springer-Verlag. 1958), Vol. 6].

Effects of various irrigating solutions on articular cartilage. An experimental study in rabbits.

The effect of six different solutions (normal saline, ringer's lactate, chlorhexidine, povidone iodine, ceftriaxone, chloramphenicol) on articular cartilage was investigated in an in vivo rabbit model study. The right knees were aspirated and injected with one of these solutions for five days and, three days later, the patellae of the rabbits were excised and investigated histologically. Left knees were used as controls. There was no difference between the groups and the controls with respect to structure, cell density, and nuclei-to-lacunae ratio. These results suggest that, these solutions have no noxious effects on articular cartilage when used as irrigating fluids in orthopaedic practice.

Structural properties of the fibular band in congenital total absence of the fibula.

Nine children with unilateral total congenital absence of the fibula were examined by magnetic resonance imaging (MRI), and histological study of the fibular band was performed in four of them. Both examinations revealed two types of fibular band. In type I, it was identical to fibula on MRI appearance, and histological studies revealed hyaline cartilage in the area of the lateral malleolus. In this type partial excision of the band allowed the foot to be corrected. In type II, the band directly attached to the calcaneus without any evidence of hyaline cartilage, and the feet could be corrected only by total excision. We suggest partial excision of the band in type I and total excision in type II as the initial treatment of congenital total absence of the fibula.

The relationship of heel pad elasticity and plantar heel pain.

Loss of heel pad elasticity has been suggested as one of the possible explanations of plantar heel pain. This hypothesis is evaluated by this blinded observer prospective study, using an age and weight matched control population. Hindfoot lateral radiographs of 73 patients with plantar heel pain, 29 of whom had bilateral involvement (102 feet), and who ranged in age from 20 to 60 years, were studied and compared with 120 healthy subjects (240 feet). Heel pad thickness and heel pad compressibility index were not significantly different in patients with plantar heel pain than in healthy subjects matched for age and weight. In patients with unilateral heel pain, heel pad thickness and heel pad compressibility index on the painful side were not significantly different from the opposite painless side. The contribution of the heel pad elasticity measured as a visual compressibility index for plantar heel pain is a matter of debate.

Oxidant status increased during fracture healing in rats.

We evaluated oxidant status during bone healing in 50 rats. In 40 rats, the right tibia was fractured and fixed intramedullarly (study leg) and the left tibia was pinned but not fractured (control leg). Rats were killed on days 1, 3, 7, 14, 28 and malondialdehyde (MDA) levels were determined in tibial bone tissue. The MDA levels of study and control legs were compared with basal MDA levels in bone in 10 rats. There was no apparent difference between the basal level and control legs, but the study legs showed a statistically significant increase in MDA levels on days 7 and 14. We conclude that no oxidative stress injury occurs during the ischemic period of fracture healing, but it may be significant during inflammation and the formation of callus.

Percutaneous autologous bone marrow grafting for simple bone cysts.

Twelve patients with simple bone cysts treated by percutaneous autologous bone marrow grafting were studied prospectively to evaluate the radiographic healing of the cysts, complications, and clinical outcome. The procedure included percutaneous aspiration of the cyst's fluid and the injection of the autologous bone marrow into the cyst cavity, which was aspirated from the posterior iliac crest. The mean volume of the lesions was 51.2 cubic centimeters (27 to 74 cc). The mean follow-up time was 23 months (range: 18 to 38 months). Complete healing occurred in two of the patients. Three cysts had residual cystic defect in which two required curettage and bone chip grafts. Six cases recurred. One patient failed to show any signs healing. We concluded that factors such as the size, multi-loculation, and completeness of the filling of the cyst with bone marrow grafting might influence the postoperative results of the treatment of simple bone cysts with bone marrow injection. We found more favorable result with unilobular, small cysts. Therefore, we believe that this method of treatment should be applied to carefully selected patients.

Oxygen-free radicals impair fracture healing in rats.

We studied the effect of oxygen-free radicals on fracture healing. 30 male rats were divided into 2 groups: 15 rats were given saline 5 mL/kg i.p. (control group) and 15 were given zymosan 100 mg/kg i.p. to induce oxygen-free radicals through stimulation of NADPH oxidase in polymorphonuclear leucocytes. 1 hour later, the right forelimbs of the rats were broken by light manual compression. These treatments were given once a day until the fifth post-fracture day. All rats were killed on day 22, and histological sections of the radius and ulna were examined without knowledge of the treatment given. The administration of zymosan impaired the fracture healing and therefore oxygen-free radicals appear to play an important role in fracture healing.
