[Hyperlipidemic pancreatitis during pregnancy--a case report].

We present a case of acute pancreatitis during pregnancy, associated with hyperlipidemia. The patient, 23 years old in 36 g. w., was hospitalized at the Clinic of Obstetrics and Gynecology with nausea, multiple vomitting, persistent abdominal pain and febrility. Because of the clinical apperance of acute pancreatitis Ceaserean Section was performed with subsequent revision of the abdominal cavity, necrectomy of the pancreas, laparostoma, lavage, drainage, nutritional yienostoma. Intubation with artificial pulmonary ventilation and high volume continuous veno-venous haemofiltration (HV CVVH) was performed because of the development of polyorgan insufficiency and ARDS. After the procedure the patient condition became stable. After four months of hospitalisation, she was discharged from hospital with stable vital signs.

[Mesenteric venous trombosis and pregnancy--a case report and a short review of the problem].

Mesenteric venous thrombosis is extremely rare surgical pathology during pregnancy and frequently is associated with hemoglobinopathies beta-thalassemia, congenital defects of the coagulation and antiphospholipide syndrome. It has nontypical clinical appearance, which hardens the timely diagnosis and the adequate surgical treatment. We present a case of a 22 year-old girl with hemozygote form of beta thalassemia, pregnant in ml II, with mesenteric venose thrombosis. The diagnosis was made on the 24th hour from the beginning of the disease. The patient was operated successfully by a resection of the necrotic changed part of the intestine. She noticed vaginal bleeding due to a missed abortion on the 22nd day after the operation.

[Perforation of duodenal ulcer in the puerperium--a case report].

The authors present a case of perforated duodenal ulcer in the puerperium at a patient who underwent Ceaserian section. Urgently 12 hours after the perforation relaparotomy was made. The exit of the illness depends on the precise and early diagnosis and subsequent surgical treatment.

[Acute pancreatitis and pregnancy--analysis of a 10 year period of time].

Acute pancreatitis during pregnancy is rare and complicates one in 1000 to 5000 pregnancies. The commonest reasons during pregnancy are: biliary disease, congenital or acquired hypertriglyceridemia and very rarely acute pancreatitis is associated with preeclampsia-eclampsia or HELLP syndrome. We presented in the research the clinical course of 14 women who developed acute pancreatitis and were treated at the Clinic of Thoraco-abdominal and Hepato-Biliary Surgery at the University Hospital Plovdiv for a tenth year of a period. The leading etiology was biliary disease. Eleven of 14 patient were successfully treated conservatively during pregnancy; 5 of them underwent surgery after pregnancy. Only one patient underwent operative treatment during pregnancy. Although acute pancreatitis is a rare disease during pregnancy, in order to improve the prognosis and to decrease the maternal and fetal morbidity and mortality, a timely diagnosis and an adequate conservative or surgical treatment is necessary.

[Therapeutic and microbiologic aspects of pleural empyema].

The pleural empyema is a severe disease which passes with respiratory insufficiency and general toxic symptoms. The early staged surgical treatment combined with total evacuation of the puss from pleural cavity is important for the successful outcome of the disease. We made retrospective study including 82 patients with pleural (61 with acute and 21 with chronic) empyema. We performed the following operative interventions: thoracocentesis with active aspiration--at 43, VATS--20, decortication and pleurectomy--in 16, thoracoplastic with muscle flap--3. Three patients died. The most common causes for the empyema were Gr (+) coccy. At most of the patients combined operative treatment and antibiotic therapy were the most effective.

[Videoassisted thoracoscopic treatment of retained fluid collection in the pleural cavity after thoracotomy].

INTRODUCTION: One of the common complications of the thoracotomies is the formation of incapsulated fluid collections in the pleural cavity. There are different invasive methods for their treatment. MATERIALS AND METHODS: The authors shared their experience with the videoassisted thoracoscopic treatment of 19 patients with pleural collections, treated in the clinic for a period of four years (2000-2003 year). RESULTS: VATS at 17 of the patients leads to healing. We performed rethoracotomy at two of the patients because there were no effect of VATS. CONCLUSION: VATS is an alternative of rethoracotomy in the operative treatment of postoperative fluid collections in the pleural cavity.

[Peritoneal cyst--a case report].

Peritoneal cysts are rare benign tumors of the abdominal and pelvic peritoneum. We present a case of 26 year old patient with a peritoneal cyst treated operatively.

[Renal complications in severe acute pancreatitis].

UNLABELLED: Except of the success in the diagnosis and treatment of acute pancreatitis, and at present this illness and its complications continue to be actual problem of urgent surgery. The object of our study is to establish the frequency of renal complications at patients with severe acute pancreatitis and to assess the most important risk factors of their development and mortality. MATERIAL AND METHODS: The object of our study is 198 patients with severe acute pancreatitis, 75 of whom have renal complications.

[Diagnosis and treatment of perforated pulmonary echinococcosis].

UNLABELLED: Pulmonar hydatidosis is widespread disease. It is presented with different complications one of which is the perforation of the cyst into the pleural cavity. The aim of our research is to share our experience in diagnosis and operative treatment of the complications with perforated pulmonary echinococcus. MATERIALS AND METHODS: In the study the we have examined 278 patients with pulmonary echinococcus, 14 of which have perforation of the cyst into the pleural cavity.

[Proximal resection of carcinoma of the gastric cardia].

Carcinoma of the gaster remains one of the major problems of the operative oncology, because of its high freguency, late diagnosis and low rate of five years survival. Although there are a lot of researches in this field, there is still a controversy about the size of the operative treatment of carcinoma of the gastric cardia. Some of the surgeons advocate gastrectomy as the only radical operation. Others prefer proximal resection as a method, saving a part of gaster, and in that way its important physiological functions for the organism.

[Respiratory complications at acute pancreatitis].

Acute pancreatitis is a disease which is associated with development of many local and systematic complications. From the previous an important place is given to the respiratory ones. They are the most common cause for the death of the patients in the first days of the illness.

[The effect of surgical treatment of benign thyroid disease on serum anti-thyroid antibodies].

The mechanisms of development of benign thyroid disease are still not entirely clarified. Immune disorders are possibly engaged in the thyroid pathological conditions. The aim of this prospective study was to follow up the dynamics of serum levels of anti-thyroid antibodies in patients with benign thyroidal disease as well as the local lymphocytic reaction. The autoantibodies to thyroglobulin and thyroid microsomes were detected prior to the surgery and on the 12th month after it using microhemaglutination test. Thyroid resection was performed depending on changes in patients. Lymphocytic infiltration of the thyroid gland from surgical histological specimens was found in various degree in the patients. Serum titres of anti-thyroid antibodies tended to fall in the majority of the patients a year after the surgical treatment. Anti-thyroglobulin antibodies were found in 60% of the patients prior to the surgery and anti-microsomal--in 31.4%. A year after surgery they were detected in 4.3% and 1.4% of the patients respectively. The described changes show that the dynamics of anti-thyroid antibodies may correlate with the effect of surgical treatment and eventual prognosis of relapse.

[About the surgical treatment of oesophageal cancer].

Oesophageal cancer is among the most malignant tumors with bad prognosis. Surgery offers the best chance for treatment. Subjects of our retrospective study are 56 patients with oesophageal cancer, treated at the Clinic of thoracic and abdominal surgery, University Hospital-Plovdiv, for 9 years period. Palliative surgery was performed at 18 of the patients, and resection of the oesophagus at 38 of them. Neoesophagus from the stomach we made in 30 of the patients, and in 9--from large intestine. In radically treated complications were observed in 14 (36.8%) cases. Six (15.8%) patients died.

[Diagnosis and treatment of retrosternal goiter].

Retrosternal and intrathoracal goiters are actual surgical problem. This is because of the severe clinical symptoms, complications and some difficulties at the operative treatment. For the period of 1991-2002 at the Clinic of Thoraco-Abdominal Surgery at Medical University Plovdiv are operated 1131 patients with thyroid disease. Of them 38 were operated with cervical incision, 3 with median sternotomy, 2 with thoracotomy. There were complication at 7 patients. Only one patient died from pulmonary embolism.

[Operative treatment of scarring esophageal stricture].

We share our experience about the operative treatment of 8 patients with strictures of esophagus in this summary. The new esophagus at 5 of them was made of colon, and at 3 of them of stomach. Only one patient died.

[Bowel injuries after blunt abdominal traumas].

Authors present 28 cases of bowel injuries after blunt abdominal trauma. Patients were from 14 to 71 years of age, 18 (64%) men and 10 (36%) women. The most common cause for the trauma was the car accident--in 18 cases. In the rest 10 the trauma was result of household activities. The small bowel was injured in 16 patients, the large bowel in 9 and in 3--the duodenum. In 15 cases the trauma was only abdominal and in the rest 13 it was accompanied by other traumas. Three of the 28 patients reported herein died.

[Surgical treatment of chronic nonspecific empyemas].

Approximately 4% to 20% of all acute empyema turn into chronic empyema despite the up-to-date complex treatment. The authors report their results of the surgical treatment of 38 patients with chronic nontuberculous empyema within a 5 years period. The age of the patients range from 18 to 72 years and they include 26 men and 12 women. Twenty eight were treated by thoracotomy and decortication, 7 underwent video-assisted thoracoscopy and decortication, and 3 thoracoplasty. In one of the patients a latissimus dorsi muscle flap was used.
