Favorable effect of hemodialysis on decreased serum antioxidant activity in hemodialysis patients demonstrated by electron spin resonance.

There is considerable evidence that uremic patients are in a highly peroxidative state. The purpose of this study was to investigate the serum antioxidant activity that may regulate, or represent, the redox state in vivo. Serum from pre- and posthemodialysis patients and from healthy control subjects was added to a system generating the hydroxyl radical, and then the signal intensities of reactive oxygen species were measured by electron spin resonance and spin-trapping technique. The electron spin resonance signals of the reaction mixture containing prehemodialysis sera were significantly stronger than those of the reaction mixture containing healthy sera (P < 0.001, n = 19), and there was no significant difference in the signals between the reaction mixture containing posthemodialysis and healthy sera. These findings demonstrated that serum antioxidant activity in hemodialysis patients is significantly decreased, and this pathological condition is improved by hemodialysis treatment.

Characterization of cis-acting elements of the gene for macrophage-stimulating protein from the human. The involvement of positive and negative regulatory elements.

To analyze the promotor region of the human macrophage-stimulating protein (MSP) gene, the 5'-flanking region of this gene was cloned. The major initiation site was determined at T located 49 base pairs upstream of the translation initiation site by primer extention with mRNA from HepG2 and Hep3B cells. There was no TATA sequence in this region. Transient transfection assay with 5'-deletion constructs showed that the transcription of this gene was regulated by positive and negative regulatory elements (PRE and NRE). The PRE (-34 to +2) was essential for the maximal transcription of this gene, and the NRE (-141 to -34) appeared to be responsible for the tissue-specific expression of the gene. The PRE contained the CCAAT sequence and a mutation from CCAAT to CTGAT resulted in a significant loss of the transcriptional activity. Electrophoretic mobility shift assay suggested that two different proteins bound to the PRE (MSP-PRE-binding protein-1 (MSP-PREB1) and 2). MSP-PREB1 and 2 were detected in various cell types, and the CCAAT sequence was involved in these bindings. These findings indicate that MSP-PREB1 and 2 are positive regulators. Further characterization also revealed that MSP-PREB2 was identical to CCAAT binding transcription factor, also known as NF-Y.

Longitudinal body wall muscles are electrically coupled across the segmental boundary in the third instar larva of Drosophila melanogaster.

Longitudinal body wall muscles in the third instar larva of the fruitfly, Drosophila melanogaster, were systematically examined for electrical and dye coupling. These muscle cells were found to be electrically coupled but rarely dye-coupled across the segmental boundary. The inter-segmental coupling coefficients between muscle #6s and muscle #7s across the segmental boundary were 0.33 +/- 0.09 (mean +/- S.D., n = 12) and 0.43 +/- 0.09 (n = 5), respectively, which are much larger than values previously reported in Drosophila but similar to those reported in the blowfly and hawkmoth. By contrast, the intra-segmental coupling coefficient between muscles #6 and #7 was smaller, 0.16 +/- 0.08 (n = 28). Other muscle cells which had apparent physical contacts with these longitudinal muscles were examined but were not electrically coupled to them. Nerve-evoked as well as miniature excitatory junctional potentials were found also electrotonically spread across the segmental boundary. The inter-segmental coupling between muscle #6s was not blocked by the gap junction inhibitors halothane or 1-octanol. Functional significance of this electrical coupling is apparently in coordination of larval body movements.

Decreased serum antioxidant activity of hemodialysis patients demonstrated by methylguanidine synthesis and microsomal lipid peroxidation.

This study aims to raise the possibility of methylguanidine, a peroxidative product of creatinine, as a measure of the peroxidative state. As a known standard, we measured the inhibitory effect of uremic serum on the NADPH-dependent microsomal lipid peroxidation. This is an established method for evaluating the peroxidative state and is compared to the effect of uremic serum on methylguanidine synthesis. The study shows decreased serum antioxidant activity in hemodialysis patients by both methods, though there is no correlation between them. These results support the use of methylguanidine as a peroxidative marker and suggest a difference in the reactive oxygen species involved in the reactions of methylguanidine synthesis and microsomal lipid peroxidation.

[Increased CA19-9 level in serum and bronchoalveolar lavage fluid from a patient with pulmonary tuberculosis].

A 66-year-old woman was admitted to the hospital because of general malaise and loss of appetite. Laboratory examination revealed an abnormally high level of CA19-9 in serum and a chest roentgenogram revealed soft reticular shadows in the right mid-lung field. An extensive examination revealed no malignant disease. Bronchoalveolar lavage fluid obtained from the right B5 contained an abnormally high level of CA19-9. Immunochemical staining for CA19-9 was positive in the epithelial cells of respiratory bronchioles. The level of CA19-9 decreased during anti-tuberculosis therapy. This case suggests that the CA19-9 level can reflect the activity of pulmonary tuberculosis.

[Bucillamin induced lung injury in rheumatoid arthritis].

Thirteen cases with rheumatoid arthritis who experienced lung injury during the treatment with bucillamine (Bc), about whom the questionnaires were answered by the physicians and whose X-ray films could be rechecked, were studied. Nine cases out of the 13 showed patchy mottled infiltrates in the bilateral center sparing the periphery, and the other 4 showed diffuse infiltrates. In these 9, serum gamma-globulin level decreased when lung injury appeared. The gamma-globulin level before the start of Bc administration, the level when lung injury appeared, the gamma-globulin decrease, and its ratio to the level before Bc were 1790 +/- 661 mg/dl, 1297 +/- 666 mg/dl, 459 +/- 320 mg/dl and 29.1 +/- 18.0%, respectively. In 5 out of the 9, gamma-globulin level reincreased when they recovered from the injury: 2 out of the 5 showed the reincrease even after steroid therapy. The data were obtained only from 2 out of the 4 with diffuse pattern in X-ray, and the decrease and the decrease ratio were 200 mg/dl (5.8%) in 1, and 49 mg/dl (3.6%) in the other. The characteristics of Bc-induced lung injury might be mottled infiltrates in the center appearing concurrently with serum immunoglobulin decrease.

[Diffuse alveolar septal amyloidosis associated with multiple myeloma (IgA lambda type)].

A 75-year-old man with multiple myeloma presented with fever and respiratory symptoms. A chest roentgenogram showed interstitial shadows and transbronchial lung biopsy revealed diffuse infiltration of amyloid in the alveolar walls. Analysis of bronchoalveolar lavage fluid showed the presence of an IgA-lambda paraprotein, and the IgA/albumin ratio was similar to that in serum. However, plasma cells could not be detected in the cells obtained by lavage. We know of no previous case in Japan in which bronchoalveolar lavage fluid was examined in a case of diffuse alveolar septal amyloidosis.

[Five cases of myelodysplastic syndrome treated with cytarabine ocfosfate].

Five cases of myelodysplastic syndrome were treated with cytarabine ocfosfate. Ocfosfate (100-200 mg) was orally administered for 14 days with 14 days' interval. Etoposide was combined in one case, and nartograstim was added in two cases. Two of the five cases achieved remission, one case having complete remission, and another case showing a good response. Their remission durations were 141 and 112 days, respectively. Further improvement of therapy is needed to achieve a higher remission rate and a longer remission duration.

[A population based study on factors related to bone mineral density in Wakayama Prefecture].

For evaluation of the community health program, 1543 inhabitants aged 40-79 years were recruited as cohort members in Miyama Village, Wakayama Prefecture. In this cohort, bone mineral density (BMD) of 400 subjects, 50 males and 50 females each randomly selected from each of four age strata, was measured at the lumbar spine and femur neck by dual energy X-ray absorptiometry. Information on exposure to possible risk factors was obtained from a self-administered questionnaire. The results were as follows; BMD at the lumbar spine and femur neck in women aged 70-79 who maintained an exercise habit (> or = one hour/week) was significantly higher than women with no exercise habit. The BMD at the femur neck in men aged 60-79 with alcohol drinking habit was significantly higher than those with no drinking habit. Significant correlation was not seen between milk intake or smoking and the BMD. BMD at the femur neck in women aged 60-69 with multiple child births (> or = 4) were significantly higher than those with fewer child births. The BMD in women aged 50-59 with earlier onset of menarche was higher than those with late menarche. Significant reduction in BMD of postmenopausal women was observed compared to premenopausal women analyzed after matching for age. The result of analysis of quantification method of the first type showed that age and menopause had a marked effect on BMD. Concerning lifestyles as other factors, a positive correlation was observed between BMD and drinking for men and exercise for women. These factors appear to be useful indicators of risk for osteoporosis.

[A case report of germinoma with syncytiotrophoblastic giant cells (STGC) in the basal ganglia].

We report a case of germinoma with syncytiotrophoblastic giant cells (STGC) in the basal ganglia which was encountered at the Department of Neurosurgery, Hiratsuka City Hospital, Kanagawa, Japan. A 9-year-old boy was admitted to our hospital on September 13th, 1993 because of left hemiparesis and precocious puberty. Endocrinological examination revealed a high serum level of human chorionic gonadotropin (HCG) (290 mIU/ml) and beta-subunit human chorionic gonadotropin (HCG-beta) (1.3 ng/ml). Plain CT scan showed an irregularly-defined mass lesion in the right basal ganglia, which had several low density spots and was inhomogeneously enhanced on contrast CT scan. The tumor developed from the right putamen to the thalamus and showed a tendency towards involvement of the hypothalamic region on MR imaging. We performed a right front-temporal craniotomy on 29th September, 1993, and a piece of the tumor was removed. It was histologically diagnosed to be germinoma with STGC. We treated the patient with carboplatin and etoposide (CBDCA 200 mg/m2day1, VP-16 60 mg/m2day1-5) chemotherapy and radiotherapy (local 30 Gy) after surgery. Postoperative course was good without recurrent signs. He is now in good health and going to elementary school without neurological deficit. There are very few reports of germinoma with STGC arising in the basal ganglia. We provide a new treatment of germinoma with STGC, chemotherapy and radiotherapy after surgery.

Chlorogenic acid as a natural scavenger for hypochlorous acid.

The effect of chlorogenic acid (CGA) on the killing of Escherichia coli by hypochlorous acid (HOCI) was examined. CGA prevented E. coli from bactericidal action of HOCI in a concentration-dependent manner. HOCI reacted rapidly with CGA. By comparison with the one- and two-electron oxidation products produced by peroxidase and phenolase reactions, respectively, the main product was identified as o-quinone of CGA via the formation of o-semiquinone of CGA as an intermediate The quinone form of CGA reacted further with HOCI to yield unstable product(s).

A new approach to the control of central deafferentation pain--spinal intrathecal baclofen.

We investigated the short-term effects of an intrathecal bolus injection of baclofen on central pain due to stroke or spinal cord injury. Pain relief was obtained in 64% of the patients. The effects developed 1-2 hours after the injection and continued for 10-24 hours. Both spinal segmental and supraspinal mechanisms may be involved in the production of baclofen-analgesia.

Evaluation of lead exposure in workers at a lead-acid battery factory in Korea: with focus on activity of erythrocyte pyrimidine 5'-nucleotidase (P5N).

OBJECTIVE: To evaluate lead exposure among lead-acid battery workers in Korea, to evaluate in more detail the erythrocyte pyrimidine 5'-nucleotidase (P5N) test for lead exposure, and to evaluate the abnormal accumulation of erythrocyte pyrimidine nucleotides in the battery workers. METHODS: Activity of P5N and other biological variables were examined in 66 exposed workers in a lead-acid battery factory and in 26 non-exposed workers in Korea. RESULTS: At the factory the time-weighted average of 13 (72%) of 18 air samples for lead exceeded 0.05 (range 0.012-0.468) mg/m3. Blood lead concentration (PbB) in 39 of the 66 exposed workers was above 40 micrograms/dl, and the mean (SD) PbB in the exposed group was 45.7 (15.7) micrograms/dl. Compared with the nonexposed group, free erythrocyte protoporphyrin in the exposed group was significantly increased, whereas erythrocyte P5N activity and activity of erythrocyte delta-aminolevulinic acid dehydratase (ALAD) were significantly inhibited. Erythrocyte P5N activity had valid correlation biologically with PbB and with other biological variables, such as ALAD activity. In 28 exposed workers, the concentration of erythrocyte pyrimidine nucleotides (uridine 5'-diphosphate-glucose and cytidine 5'-triphosphate) correlated inversely with P5N activity and positively with PbB. CONCLUSIONS: These findings show that the depression of erythrocyte P5N activity by lead exposure results in the accumulation of erythrocyte pyrimidine nucleotides. In general, the standard analysis of PbB performed in laboratories around the world remains the most useful index of recent exposure. The results indicate that the erythrocyte P5N activity test provides supporting evidence of lead exposure and shows the effect of lead on nucleotide metabolism.

Effects of various doses of intracoronary diltiazem on coronary resistance vessels in humans.

We investigated the vasodilative effects of various doses of intracoronary diltiazem on coronary resistance vessels. Nine patients with normal angiograms were studied. A coronary Doppler guide wire was inserted into the left anterior descending coronary artery. Diltiazem, in doses of 0.1 mg, 0.5 mg, 1.0 mg, 2.0 mg and 4.0 mg, was injected into the left coronary artery at 10 min intervals and coronary blood flow velocity was measured. The coronary artery was pretreated with intracoronary nitroglycerin to avoid changes in the cross-sectional area. As a measure of the change in coronary vascular resistance, a coronary vascular resistance index (CVRI) was calculated as mean aortic pressure/coronary blood flow velocity. An injection of diltiazem into the left coronary artery caused a significant dose-dependent decrease in CVRI. A dose of 2.0 mg was the lowest dose on the plateau of the dose-response curve. In conclusion, the injection of diltiazem into the left coronary artery causes a dose-dependent dilatation of coronary resistance vessels, and the maximal effect can be produced with a dose of 2.0 mg.

[The first case of Sjögren's syndrome with anti-Wa antibody].

A 42-year-old woman presented in 1992 with Raynaud's phenomenon, polyarthralgia and morning stiffness and next year developed xerostomia and xerophthalmia. Investigations revealed 20 mm/hr of ESR, positive test for RAPA (1 : 320), ANF (1 : 1280), anti-SS-A antibody and anti-Wa antibody, and positive Schirmer tear test. A biopsied specimen of minor salivary gland of lip showed lymphocytic infiltration around the ducts and fibrotic changes and a sialogram demonstrated diffuse dilatations of the peripheral ducts. Resulting from the data shown above, the diagnosis of Sjögren's syndrome was made. Anti-Wa antibody, which recognizes a 48 kD tRNA associated protein was identified in serum from a patient with systemic sclerosis by Yamagata in 1985. So far the antibody has been specific for systemic sclerosis, because all of six patients with anti-Wa antibody previously reported were diagnosed as having systemic sclerosis. However, we here described the first case of Sjögren's syndrome with anti-Wa antibody.
