Level of conus medullaris in term and preterm neonates.

AIMS: To compare the levels of conus medullaris in preterm and term neonates; to show the time of ascent to normal; and to evaluate the babies with low conus medullaris levels for tethered cord syndrome. METHODS: Levels were assessed using ultrasonography in 41 preterm and 64 term neonates. RESULTS: In the preterm group the conus medullaris level in one infant (2.4%) was below L4. In three infants (7.2%) it was between L2 and L3 and in 37 infants (90.4%) it was above L2. In the term group it was below L4 in one baby (1.6%), between L2 and L3 in four (6.3%), and above L2 in 57 babies (92.1%). The difference in the conus medullaris levels between term and preterm neonates and genders was not significant. Two patients, one with a conus medullaris level at L4-L5, and the other at L2-L3, had Down's syndrome. CONCLUSION: The ascent of conus medullaris seems to occur early in life. It is important to follow up patients with conus medullaris levels at or below the 4th lumbar vertebra for the development of tethered cord syndrome.

Protection by Iloprost (stable analogue of prostacyclin) of endothelial damage due to chronic vasospasm in dogs: an electron microscope study.

The resolution of cerebral vasospasm and protection of endothelial damage by Iloprost was evaluated with multicisternal injections. Sixteen adult mongrel dogs (18-20 kg) were assigned to one of three experimental groups. All animals received a total amount of 15 ml fresh unheparinized arterial blood via three injections into the cisterna magna. Selective vertebral angiography was performed on day 0 and subsequently blood injections were performed on the 2nd and 3rd days after the first injection. On the 7th day angiography was reperformed to determine the chronic vasospasm. The first group (5 dogs) was the control group and received intrathecal saline which was equal to the amount of saline in which Iloprost was diluted. The second group (5 dogs) did not receive any treatment until the 7th day. The third group (6 dogs) received Iloprost intrathecally (total 10 micrograms kg-1). In the first two groups angiographic vasospasm was prominent. For the second group intraarterial Iloprost was given on the 7th day in order to evaluate its acute effect. However there was no evidence of resolution of vasospasm. In the third group, resolution of vasospasm was verified on angiograms. Electron microscope studies of basilar arteries of the first two groups revealed degenerative changes of the endothelial cells which were separated from each other and the elastic lamina was irregularly arranged. In the intrathecal Iloprost-treated group there was little thickening in the elastic lamina and the endothelial cells were almost normal in structure. These results can be considered as the evidence of the prophylactic effect of Iloprost given by the intrathecal route in the prevention of chronic cerebral vasospasm.

CT-guided percutaneous selective cordotomy.

In this article the technique of CT-Guided Selective Cordotomy is described. The advantages of CT guidance in percutaneous cordotomy are the measurement of spinal cord diameters at the lesion site for each individual patient, direct visualization of target electrode relations, demonstration of spinal cord displacement during the procedure and localization of the electrode system in a specific part of the spinothalamic tract. Local destruction of the spinothalamic tract leads to selective cordotomy. Since 1987, CT-Guided Cordotomy has been applied to 54 cases of intractable cancer pain. In 33 of the 54 cases, the cordotomy was selective enough to be successful with a local denervation of the area where the pain was dominant. Except for one temporary hemiparesis and one temporary ataxia, no complications or side effects were observed.

The effect of Iloprost on chronic cerebral vasospasm.

1. Experimental chronic cerebral vasospasm induced by subarachnoid haemorrhage (SAH) in rabbits was studied in order to evaluate the efficacy of repeated i.v. infusion of Iloprost (ILO). 2. The mean diameter of the basilar artery of intact animals was calculated as 737.5 +/- 52.8 microns while this value was reduced to 237.5 +/- 22.96 microns after SAH. 3. In the ILO treated group the mean diameter of the basilar artery was significantly increased and found to be 593.75 +/- 64.0 microns. 4. No change was observed in intracranial pressure (ICP) except a slight decrease in mean arterial pressure when relatively higher doses of ILO were used. 5. These results were taken as evidence of the high therapeutic value and low side effects of ILO in the treatment of persisting cerebral vasospasm due to SAH.

Medulloblastomas.

81 patients with biopsy proven medulloblastoma with a mean age of 11.4 years were reviewed. Total removal of the tumour was accomplished in 18 cases (22.2%), subtotal removal in 55 cases (67.9%) and biopsy alone in 8 cases (9.9%). 34 patients (27.5%) had a recurrence of the tumour and 8 patients underwent a second posterior fossa exploration at the time of recurrence. In patients with total resection, five-year survival was 27.7%. With partial resection, five-year survival decreased to 14.5%. In patients with radiotherapy chemotherapy, 21.4% survived 5 years after treatment but this rate decreased to 18.4% in patients treated with radiotherapy only. The surgical mortality rate was 12.3%.

The effect of intrathecal sodium nitroprusside on severe chronic vasospasm.

The efficacy of sodium nitroprusside in resolving cerebral vasospasm was evaluated with multicisternal injections. Twelve animals received fresh, unheparinized arterial blood via three injections (15 ml total) into the cisterna magna. Selective vertebral arteriography was performed on Day 0, and blood injections were performed on the second and third days after the first injection. On the seventh day selective arteriography was performed to evaluate the diameter of the basilar artery. In the sodium nitroprusside group, intrathecal injections of the drug were started on Day 4 and continued for two days (25 micrograms/kg/day). The diameter of the basilar artery was reduced 72.98 +/- 11.07% in control experiments. For the animals treated with intrathecal sodium nitroprusside, the mean diameter of the basilar artery was reduced 29.25 +/- 4.54%. The effect of intrathecal sodium nitroprusside on intracranial pressure (ICP), blood pressure (BP) and electrocardiogram (ECG) was also evaluated in 14 animals. There were no prominent changes in ICP, BP, or ECG when sodium nitroprusside was given intrathecally, but BP decreased and ICP and heart rate increased with intravenous doses of sodium nitroprusside. These results support the hypothesis that sodium nitroprusside administered intrathecally is an effective treatment for cerebral vasospasm.

An intramedullary dermoid cyst abscess due to Brucella abortus biotype 3 at T11-L2 spinal levels.

A 17-year-old boy was admitted to hospital with a history of backache, weakness of the lower extremities and fever. While the diagnosis of brucellosis was made his clinical course worsened. Acute medulla spinalis mass effect and signs of meningitis predominated. It was shown that he had a dermoid cyst infected with Brucella abortus biotype 3 which was the same strain growing in his blood cultures.

The role of denture cleansers on the whitening of acrylic resins.

Dentists and denture wearers have reported that the regular use of a denture cleanser causes whitening of dentures. The whitening effect of denture cleansers on acrylic resin dentures was investigated using four different types of agents on six different types of acrylic resin discs. The color values of the samples were measured using a reflectometer. The greatest whitening effect was seen on autopolymerizing acrylic resins for one of the denture cleansers with all the resins tested.
