Effects of perfusion solutions on kidney ischemia-reperfusion injury in pigs.

During ischemia-reperfusion, free oxygen radicals which directly affect renal cells may cause delayed graft function. We investigated whether there was a difference regarding antioxidant enzyme content between use of Ringer's lactate (RL) versus University of Wisconsin (UW) perfusion solutions in kidney transplantation. Ischemia was achieved by clamping the renal pedicle for 20 minutes followed by perfusion with either solution for 20 minutes and reperfusion for another 20 minutes. A parenchymal biopsy was taken before and after the ischemia, perfusion, and reperfusion (IPR) process. The levels of superoxide dismutase (SOD), glutathione peroxidase (GPx), and malondialdehyde (MDA) were investigated in the biopsy specimens. We used paired t tests within groups and t tests for comparisons between groups. The results were expressed as mean values +/- SEM with P < .05 accepted as statistically significant. After IPR, SOD, GPx, and MDA were decreased in all groups: only GPx (P = .001) and MDA (P = .04) for the RL group and SOD (P = .001) and MDA (P = .05) for the UW group were statistically significant. In the control group, we did not observe any difference (P > .05). Comparisons between groups did not reveal differences (P > .05). In our study, no difference was observed between RL and UW regarding their effects on antioxidant enzymes following renal I/R injury in pigs. More investigations are needed to evaluate graft function in this setting.

[Operative treatment of developmental abnormalities of sexual organs in girls].

DESIGN: The aim of our study was a clinical analysis of operative treatment of developmental abnormalities of sexual organs. MATERIALS AND METHODS: The authors describe developmental abnormalities of sexual organs in girls aged 12-19 79(1.4%) girls were treated from among 5432 surgically treated patients in the course 6 years. Among these abnormalities were the following: agenesia of vagina, agenesia of cervical canal, absence of cervix of uterus, obstructed normal or rudimental vagina, occlusion of hymen. There were complex abnormalities too. Congenital abnormalities were subject to the reconstructive operative treatment. CONCLUSIONS: We recommend that developmental defects should be treated in specialist centres well equipped and experience of in operative procedures and postoperative monitoring.

Thoracic actinomycosis.

OBJECTIVE: Thoracic actinomycosis is a disease that is difficult to diagnose because its appearance varies from similarities with bronchogenic carcinoma to pneumonitis-like tuberculosis infections. Therefore the majority of patients undergo operations for diagnosis. We had seven patients with thoracic actinomycosis which were not diagnosed before operation. METHODS: Between July 1990 and November 1997, seven patients with thoracic actinomycosis were diagnosed after thoracotomy in our centre. They all had non-specific symptoms and radiographic findings, so we failed to diagnose the disease during preoperative examinations. Therefore they all underwent thoracotomy for diagnosis. Four patients were operated because of suspicion of malignancy, two patients were operated because of pulmonary infiltration and abscess formation and one patient because of undiagnosed pulmonary infiltration. Lobectomy was performed in five of them because of destroyed lobes, and wedge resection was performed in two patients. In two patients the disease was diagnosed by fresh smears obtained from specimens in the course of operation and confirmed histopathologically and in the others histopathologically in the postoperative period. RESULTS: Three major complications, acute renal failure, empyema and persistent air leakage developed in three cases after the operations. Bronchopleural fistula was found in only one of them and the patient died because of uncontrolled infection and sepsis on the 26th postoperative day. All of the other six cases are still alive. We did not observe any other problem in their long-term follow-up. All patients regularly took 20 million units/day of intravenous crystalline penicillin G when they were in the hospital. After that, antibiotic treatment was completed up to 2 months with procaine penicillin. CONCLUSIONS: Sometimes diagnosis of the actinomycosis of the lung is very difficult although it is an infection. In that case thoracotomy is needed for the diagnosis and sometimes for the treatment. In some cases because of the irreversible parenchymal change resective surgery may be needed. Actinomyces israelii infections should be suspected of in chronic infiltrative, nodular, cavitary process and tumour-like mass lesions besides other most probable causes. After diagnosed, it is treated using penicillin chemotherapy at least for 2 months.

Isolated liver metastases from neuroendocrine tumors: does resection prolong survival?

BACKGROUND: Neuroendocrine tumors commonly metastasize to the liver. Although surgical resection is considered a treatment option for patients with localized metastases confined to the liver, the longterm survival benefit of liver resection has not been clearly demonstrated. We examined the survival of patients undergoing liver resection for this disease. STUDY DESIGN: Between 1984 and 1995, we evaluated 38 patients with liver-only metastases from neuroendocrine tumors, including 21 carcinoid, 13 islet cell, and 4 atypical neuroendocrine neoplasms. Data from a combined prospective and retrospective database and a tumor registry were analyzed. Of these patients, 15 underwent complete resection of all known disease. The remaining 23 patients, who also had disease confined to the liver, had comparable tumor burden but were believed to be unresectable. The longterm survival rates of these two groups were compared. RESULTS: Patients who underwent liver resection did not differ from those who were unresectable with regard to age, pathology, primary tumor site, serum alkaline phosphatase levels, or percentage of the liver involved. All resections were complete, leaving no residual disease, and consisted of lobectomy (n = 3), segmentectomy (n = 1), and wedge resections (n = 11). There were no operative deaths. Patients who underwent hepatic resection had a significantly longer survival than unresected patients. Although median survival had not been reached in resected patients, the median survival in the unresectable group was 27 months. Patients who underwent liver resection had a higher 5-year actuarial survival (73% versus 29%). CONCLUSIONS: Hepatic resection in selected patients with isolated liver metastases from neuroendocrine tumors may prolong survival. This conclusion was reached by comparing our resected group with an unresectable group with similar tumor burden.

[Behavior of SCC antigen in girls with a high risk of developing cervical carcinoma].

The work presents the results of studies on level of SCC antigen in 28 patients of 14 girls, sexual clinical active with cervical erosion type II, among which in 10 cases HPV virus were founded. The second group consists of 14 women from normal environments, treated for CIN. The antigen SCC was determined in patients blood serum by immunoenzymatic method. The conducted studies revealed statistically significant differences. Levels of SCC antigen were lower in the first group, compared to second group.

[Ultrasonography, laparoscopy, neoplastic markers--new possibilities of conservative operative treatment for ovarian neoplasms in girls and young women].

The knowledge of ovarian physiopathology and the employing of modern diagnostic methods (ultrasonography, laparoscopy) allow to avoid to operate patients with functional cysts. Girls and young women should be evaluated individually for ovarian surgical procedures. The operative treatment should be so conservative as possible employing all diagnostic and monitoring methods including specific markers assays.

[Behavior of SCC marker level in cervix erosion, cervix dysplasia and cervical pre-invasive carcinoma].

The work presents the results of studies on the behaviour of SCC antigen in 52 patients aged 21-70 years, with pathologic changes involving the uterine cervix and classified to the groups of erosion, dysplasia and carcinoma in different stage of advancement. The SCC antigen was determined in patients blood sera by radioimmunologic method, on the basis of Abbott Firm testes. An increase in concentration was observed exceed the norm (2 ng/ml) in cases of CIN and CIS. The rise of marker depended on the stage of the neoplastic process advancement in the uterine cervix.

[Important suggestions for achieving optimal conditions for success in operations for tubal patency restoration].

The most important factors are presented which cause that surgical treatment of tubal obliteration is not giving fully satisfactory results. The selection of patients for tubal reconstruction and the choice of operation term are discussed. The modern diagnostic methods in tubal obliteration are described with particular reference to USG and laparoscopy. The most important suggestions for the surgeon in preoperative, intraoperative and postoperative management are made, discussing the problems of macroscopic and microscopic surgery. Particular attention is called to prevention of postoperative obliterative syndrome (intraperitoneal infusion of dextran and local and systemic glycocortico-therapy). Pregnancy developing after surgical treatment of tubal obliteration requires careful monitoring and should be regarded as high risk pregnancy.

[Significance of laparoscopy in decisions on the preventive therapy of various ovarian tumors in girls].

Of 125 laparoscopies, carried out in the Clinic of Surgical Gynaecology in the years 1972 to 1974, 12 referred to girls aged from 15 to 18 years (9,6%). As a consequence of the mentioned diagnostic-therapeutic procedure 6 girls (50%) avoided laparotomy. Laparoscopy is an excellent technique to establish diagnosis in cases of ovarian tumours especially in the developmental age.
