The article contains experimental data on angiogenesis stimulated by plasmid containing the angiogenin gene. After the introduction of the gene construction, the number of capillars in the chorion-allantois membrane increases 2 to 3 times; in an ischemized limb of a rat it increases by 20 to 30%. Intramuscular administration of genetic engineering construction to patients with chronic lower limb ischemia improved the patients' condition, consisting in an increase in painless walking distance and ankle-brachial index, as well as in trophic defect healing and the betterment of muscular perfusion. Positive effects were noted after 2 to 4 weeks of treatment and remained during 6 to 24 months. There were no side-effects, except low grade fever during 1 to 2 days.
A new method for study of microcirculation potency and tissue viability in patients with critical lower limb ischemia is described. Microcirculation resistance was studied with dynamic scintigraphy which shows primary passage of radiopharmaceutical into microcirculatory system. Accumulation of radiopharmaceutical associated with necrotic and inflammatory zones was detected with static scintigraphy. Results of surgical treatment have been studied in 105 of 123 examinees. Based of these data the criteria of lower limb microcirculation efficacy and tissue viability have been elaborated. Significant value of these two criteria in assessment of efficacy of various types of revascularization and amputations is demonstrated.

Experience in the treatment of 386 patients with stage 2-3 of varicose disease is analyzed. There are five main variants of surgical treatment in these patients: combined phlebectomy, sclero-obliteration of the trunk, short stripping with ligation of insufficient perforant veins, short stripping with excision or destruction of varicose branches, short stripping and sclero-obliteration of the trunk. It is demonstrated that minimally invasive surgical procedures decrease the number of postoperative subcutaneous haematomas, ensure good cosmetic effect, allow to avoid an injury of n.saphenus and lymphorrhea. It is concluded that minimally invasive surgical procedures may be used for treatment of initial forms of varicose disease.
Between January 1998 and December 2003 thirty-one patients with extracranial common and internal carotid artery aneurysms underwent surgical treatment at the Department of Vascular Surgery, Russian Scientific Center of Surgery. Of these patients, 29 subjects were provided reconstructions on these arteries and two patients underwent ligature operations. In the short-term postoperative period, one (3.2%) patient died. The lethal outcome was induced by acute disorder of cerebral circulation (ADCC). Disorders of cerebral circulation (DOC) occurred in 4 (12.8%) patients. Of 2 (3.2%) cases of stroke, one eventuated in lethal outcome and 2 (6.4%) patients developed transient ischemic attacks (TIA). The lethality in the long-term postoperative period (the follow up lasted as long as 5 years) accounted for 6 (19.3%) cases. In patients who had undergone reconstructions, the long-term postoperative lethality reached 13.8% (4 cases) whereas the lethality following ligature operations constituted 100% (2 cases). So, the reconstructive operation is a highly effective and safe enough approach to the treatment of patients with extracranial carotid artery aneurysms. The short- and long-term results were beneficial.
Recently an ever growing importance has been acquired by the problem of the treatment of iatrogenic aneurysms of the peripheral arteries, first of all, this is linked with a steady growth of the number of endovascular interventions. The development of false post-puncture aneurysms is one of the most hazardous complications from such interventions. Along with traditional surgical treatment (reconstructive operations) much attention is paid to the use of little invasive treatment methods for these aneurysms, e.g. the compression modality under ultrasound guidance.
The paper describes management results of 56 patients with occlusive and stenotic atherosclerotic lesions of carotid arteries, complicated by different forms of ocular ischemia. Complex assessment (duplex scanning, color Doppler mapping, transcranial Doppler) before and a year after carotid endarterectomy revealed a significant improvement in objective parameters of ocular hemodynamics: registration of antegrade blood flow in ophthalmic artery, the rise of maximal systolic blood flow velocity and decrease of resistance index in ophthalmic, central retinal and posterior ciliary arteries.
Combined ablation of great saphenous vein (GSV) remains the most common way of lower limb variceal management. In recent years different surgical methods were proposed for the treatment of varices. Nevertheless, their invasiveness, poor esthetic outcome, prolonged postoperative rehabilitation and recurrences cail for the development of new minimally invasive and highly effective methods. In the last 5 years a new procedure of intraoperative trunk catheter scieroobliteration of GSV and its tributaries was introduced. From 1997 to 2002 total 493 surgical corrections of lower limb varices were fulfilled, among them 374 patients were operated for primary variceal vein dilatations including 188 standard combined venectomies (1st group) and 186 trunk obliterations (2nd group). Groups were similar in age, sex and lesion extension. Outcomes were followed up to 4 weeks - 5 years. Outcomes comparison between groups has demonstrated significantly lower incidence of postoperative subcutaneous hematomas in the 2nd group, absence of saphenous nerve injuries, better esthetics in the absence of additional incisions (remaining variceal tributaries were sclerosed postoperatively by puncture scieroobliteration). Besides postoperative rehabilitation period was double shorter in ihe 2nd group then in the 1st one. Recurrence rate in long-term period was similar in both groups. Thus intraoperative trunk scieroobliteration of GSV can be a valuable alternative for standard combined venectomy.
Experience of surgical treatment of 325 patients with critical lower limb ischemia due to affection of infrainguinal vessels are analyzed. Comparative analysis of efficacy of direct, indirect and combined revascularizations was carried out, quality of life of patients in long-term period after surgery was evaluated.
The CELO recombinant avian adenovirus carrying the gene coding the human angiogenine (ANG) synthesis was obtained. Expression of the angiogenine gene was shown in the LMH cell culture after infection with the CELO-ANG virus. The ability of CELO recombinant adenoviruses to carry out the delivery and expression of alien genes in muscle cells was demonstrated in experiments with laboratory animals (Wistar line rats). The induced neovascularization in rat muscles after the animals were administered the CELO-ANG viruses was shown.
The article deals with analysing examination and surgical treatment of 80 patients with predominant lesions to the distal arterial bed of the lower extremities, who were operated on at the RRSC from 1996 to 2003 with various degree on the background of chronic obliterating diseases of lower limbs arteries. All the patients were subdivided into two groups according to the outcomes of the epidural block: Group I -- with positive outcomes consisted of 36 (45 %) patients, and Group II -- with a negative outcome -- included 44 (55 %) patients. The patients from the both groups underwent the following operations: lumbar sympathectomy combined with direct and indirect methods of revascularisation, as well as lumbar sympathectomy as an independent therapeutic method. To solve the problem concerning feasibility of either done, or concomitant lumbar sympathectomy, we devised assessment of microcirculation state (alterations in volumetric blood flow indices in epidural block in relation to the indices at rest, as well as in the immediate or remote postoperative period) and peripheral haemocirculation (dynamics of the onkle brachial index values on the background of epidural block test, nitroglycerine test in relation to the ARI at rest both in the short-, and long-term postoperative period) by data of scintigraphy and dopplerography of the lower limbs.
The paper is concerned with a study into the effect of halidor on the results of conservative (17cases) and surgical (22 cases) treatment of patients suffering from chronic lower limb ischemia. The surgical treatment included revascularization interventions. The study groups were compared to the respective control groups of patients who did not receive halidor. As compared to the control group, the group undergoing surgical treatment demonstrated a significant increase in the ankle/brachial index after parenteral infusion of halidor administered in courses (p<0.05). Also, in the group undergoing conservative treatment, there was a significant increase in the distance of painless walking.

Presented herein is an experience gained with 1356 reconstructions for vasorenal hypertension, performed at the RSCS, RAMS, over the period 1961 to 2001. The authors defined the criteria for the diagnosis of the given disease and the indications for surgical treatment. Also, the others demonstrate specific features of surgical treatment of patients presenting with different variants of renal artery lesion, provide the short- and long-term results of the main types of reconstructions accomplished for vasorenal hypertension, and emphasize the importance of the use of roentgenendovascular techniques. Based on the experience gained the conclusion is made about the advantages of surgical management versus conservative treatment methods.
Incidence rate of arterial aneurysms is 3.4-4.6% of that of vascular diseases and has a tendency towards further increase. In National Research Center of Surgery, RAMS, 410 patients with arterial aneurysms of various localization were treated. The majority of the patients were men--221 (65.4%). Surgical procedures were performed in 338 (82.4%) patients. Reconstruction surgeries were performed in 273 (80.8%) patients, ligature surgeries--in 65 (19.2%) patients. In early postoperative period good results (recovery and improvement) were seen more often (p < 0.05) after reconstruction surgeries (89.8% patients) than after ligature ones (47.7% patients). Long-term results (follow-up > 10 years) were also better after reconstruction than after ligature surgeries.
The paper presents a new approach to management of lower limb critical ischemia which implements recent advances in molecular biology and genetic engineering technologies. A new original compound incorporating angiogenin gene was developed to activate neoangiogenesis processes after injection into living tissues. Experimental data evidence a potential efficacy of new method for complex management of critical ischemia.
Presented herein is a clinical report of the surgical treatment of a female patient with left internal carotid aneurysm. Patient M, aged 58 years was hospitalized to the Department of Vascular Surgery, RSCS RAMS, with complaints of a pulsatile formation in the area of the neck on the left, persistent headaches, frequently occurring dizziness, and multiple episodes of loss of consciousness. The patient has been ailing for 20 years. Examination discovered a rounded pulsatile formation about 3 cm in diameter in the left lateral area of the neck in the internal carotid projection. Duplex scanning revealed aneurysm of the left internal carotid artery measuring 2,3 cm x 3.8 cm. Computerized tomography confirmed aneurysm of the left internal carotid artery. On December 10, 2002 the patient underwent resection of aneurysm of the left internal carotid artery followed by establishment of an end-to-end anastomosis. The immediate postoperative period was uneventful. The patient was discharged home in a satisfactory condition.
