[Diagnosis and treatment of neck chemodectoma].

The work is based on the analysis of clinical data of 10 patients with chemodectoma of the neck, 8 of them being subjected to surgery. The diagnosis in 8 operated patients was confirmed histologically, 2 non-operated patients had angiographic manifestations. It is emphasized that carotid angiography plays an important role in establishing the diagnosis and differential diagnosis of benign and malignant chemodectoma. There were noted no mortality and postoperative complications. The oncological aspects of surgery for chemodectoma are also discussed.

[Ultrasonic endarterectomy].

The authors used a generator of ultrasound oscillations and a special wave conductor in endarterectomy in the clinic and in the experiment. They assess positively the described method of endarterectomy.

[Reconstructive operations in surgical treatment of vasorenal hypertension].

The experience of 313 reconstructive operations performed in 289 patients with renovascular hypertension is analysed. The importance of choosing the surgical procedure with due regard of the etiology and extent of the pathological process is emphasized. The most effective and frequently employed reconstructive procedures consisted in the author's original technique of transaortic endarterectomy (232 operations), resection of the renal artery with reimplantation of the ostium into the aorta (19 operations), renal artery resection with an end-to-end anastomosis (23 operations). In cases of bilateral lesions of the renal arteries two-stage procedures are recommended, spaced 3-4 months apart. Reconstructive interventions on the single renal artery are described.

[The perioperative diagnosis of cerebral ischemia during reconstructive operations on the carotid arteries (initial experience in using transcranial oximetry)].

Intraoperative monitoring of cerebral ischemia during surgery on the brachiocephalic arteries is carried out by several methods: transcranial dopplerography, electroencephalography, recording of the somatosensory evoked potentials, puncture measurement of intravascular pressure, etc. However, not all these methods are sufficiently informative, reliable, and safe. One of the methods meeting these requirements is noninvasive cerebral oximetry. The study was carried out by the Invos-3100 device (Somanetics, USA). The method of cerebral oximetry is based on optic spectroscopy using infrared light in the 650-1100 nm waveband. Monitoring was carried out in 34 patients with involvement of the brachiocephalic arteries. The results persuasively prove high informative value and safety of the method.

[Cerebral oximetry in the perioperative evaluation of the reserve of cerebral circulation in patients with brachiocephalic artery lesions].

The reactivity of cerebral vessels and the reserve of collateral compensation of cerebral flow were studied in 71 patients with perioperative carotid stenosis of different degrees. Cerebral oximetry was made at rest and during hypercapnic and compression loads. The method's efficiency was demonstrated for the preoperative evaluation of the cerebrovascular reserve in patients with atherosclerotic affection of vessels in the brachiocephalic stem. A comparison of the preoperative and intraoperative dynamics of rSO2 at compression points at the feasibility of using the cerebral oximetry in predicting the degree of reduced regional oxygenation of the brain in the basin of an intraoperatively pinched artery.

[The specific features of ocular blood flow and brachiocephalic arteries in patients with non-exudative senile macular degeneration].

A hundred and twenty patients (240 eyes) aged 40-65 years (mean age 56.9 +/- 2.1 years) who had non-exudative age-related (AMD) macular degeneration (AMD) were examined. Ultrasound studies, including Doppler color mapping, were conducted to study blood flow in ophthalmic vessels and brachiocephalic arteries. Hemodynamic parameters deteriorated in all patients with non-exudative AMD, which suggests that blood flow deficit in the ophthalmic arterial system, central retinal artery, and posterior ciliary arteries. 88.3% of patients with non-exudative AMD were diagnosed as having brachiocephalic arterial deformities of which bilateral C- and S-shaped pathological tortuosity of carotid and vertebral arteries were most common.

Diagnostic investigation and special treatment of chronic abdominal ischaemia.

Chronic abdominal ischaemia (C.A.I.)--abdominal angina--is a neglected gastrointestinal vascular condition, with chronic complaints and symptoms for the patient posing severe diagnostic and therapeutic problems. In the angiologic department of the Interunion Scientific Center of Surgery during the past 25 years, chronic intestinal ischaemia was diagnosed in 150 patients, a number representing 2.5% of the whole population with pathology form the aorta and its branches. Etiological factors of disease of the celiac axis, superior mesenteric artery and inferior mesenteric artery occlusion were the intravascular factors in 99 patients, atherosclerosis in 63 and nonspecific aorto-arteritis in 36. Extravascular factors: median arcuate ligament of the diaphragm, celiac plexus, periarterialfibrous, nerve fibers and tumor of pancreas were found in 51 patients. Of the 150 patients with C.A.I., 84 (56%) were operated and 87 operations were performed; of these 6 (4%) had a transluminal dilatation. A left thoraco-diaphragmatic approach to these vessels, via a lumbar incision through the last intercostal space in 62 patients was carried out. In those cases where the operation on the inferior mesenteric artery was combined with an operation on the abdominal aorta, the vessels were approached through a typical laparotomy (16 patients). In C.A.I. we perform 2 types of operations: partially reconstructive (decompressive) and reconstructive operations. Partially reconstructive (decompressive) operations were performed in case of compression of unpaired branches of the abdominal aorta in order to free the artery from extraluminal causes of pressure.(ABSTRACT TRUNCATED AT 250 WORDS)

[Long-term results of surgical treatment of patients with renovascular hypertension].

The paper analyzes the long-term results of surgical management of vasorenal hypertension in relation to the type of the reconstructive operation made. It provides data on the mortality rate of these patients in the late period and shows the ways of its reduction. The follow-up was from 5 to 25 years. The findings allow angiosurgeons to make the rational method for reconstructing the diseased renal artery clear.
