[The influence of ceruloplasmin on the rat neocortical ultrastructure in circulatory hypoxia].

Diffuse cerebral ischemia and circulatory hypoxia were modeled by carotid arteries occlusion in 40 white Wistar male rats. The changes of microcirculation bed and neurons of the sensomotor cortical area in hypoxia, preventive and therapeutic regimes of ceruloplasmin injections were compared to the neocortex state of intact rats. The changes of the neocortical ultrastructure established by electron microscopy were characteristic of hypoxic lesions described earlier in the literature. As a result of prolonged administration of ceruloplasmin there was a significant decrease of sensomotor neocortical hypostasis and improvement of microcirculation that suggested its neuroprotective properties.

[Efficacy of combined kineso- and psychotherapy in the treatment of patients with migraine].

The study aimed at determining the efficacy of combined kineso- and psychotherapy in the treatment of patients with migraine in between-attacks periods. Forty-one patients with migraine (3 males, 38 females, mean age 38.2 +/- 1.5 years) were examined. The patients were randomly divided into 2 age-matched groups. No significant between-group differences in the intensity and duration of migraine attacks, autonomic and psychological status and level of tolerability to physical loading were found. The patients of group 1 (23) recurred individual kineso- and psychotherapy, and patients of group 2 (18) received a course of standard physical therapy and explanatory psychotherapy. After the treatment course, a significant decrease in intensity of headaches of tension, increased bicycle exercise tolerance, equalizing of the ratio between activities of ergo- and trophotropic systems, sympathic and parasympathic tones evaluated by mathematical analysis of heart rhythm variability, a decrease of the level of emotional tension measured by psycho diagnostic testing were observed in both groups. In the patients of group 1, global estimation of therapeutic effect (2.7 +/- 0.2) was significantly higher (p < 0.003), comparing to those of group 2 (1.8 +/- 0.2). Two months after treatment finishing, there was a reduction of migraine attacks frequency in both groups without significant between-group differences in regression extent of this index. The results obtained indicate efficacy of combined physical therapy and psychocorrection in the treatment of migraine patients. Moreover, individual approach to kineso- and psychotherapy improves the immediate results of the treatment.

[Clinical and neurophysiological features and outcome prognosis criteria of Guillain-Barre syndrome in children].

Clinical and electroneuromyographical (ENMG) study of 24 children with Guillain-Barre syndrome (GBS) was conducted. Using ENMG, axonal and demyelinating GBS variants differing by course and rehabilitation time were distinguished. To predict a development of bulbar and respiratory disorders and to evaluate their severity and duration, diaphragm and adductive nerves conduction was investigated. Factors influencing motor function rehabilitation rate in GBS were the following: mean distal M-response amplitude during peroneal nerve stimulation, proximal to distal amplitudes ratio, M-response amplitude for femoral nerves. Three types of ENMG parameter rehabilitation in children with GBS (linear, stepped and undular) which characterize different rates of rehabilitation process are described.

[Neurophysiological criteria for the diagnosis and prognosis of facial nerve neuropathy in children].

Electromyography bindings of blink reflex, visual and acoustic evoked potentials have been studied in 35 children with facial nerve neuropathy (FNN), 20 children with FNN being manifested for the first time and 15 children being affected with recurrent type. Blink reflex analysis in FNN has revealed trifacial and controlateral facial nerve pathology in 10-20% cases. Factors essentional for rehabilitation and development of complications were the following: (1) duration of conduction block along trigeminis-facial nerve arch, (2) conduction block association, according to the blink reflex, with the absence of M-response in direct facial nerve stimulation, (3) a coefficient of M-response amplitude ratio on affected and intact sides, (4) P100 latency disturbance and its wave like rehabilitation during 4-6 months. Recurrent FNN was characterized by: (1) R1 latency increase and ipsilateral R2 in stimulation on intact side, (2) P100 latency fluctuations without returning to normal state, (3) bilateral increase of III peak latency and I-III intervals.

[The role of emotional pressure in the development of the early forms of chronic cerebrovascular insufficiency].

The paper presents a comparative analysis of the results of biochemical, neuropsychological and neurophysiological examination of initial chronic cerebrovascular insufficiency in patients without significant psycho-emotional tension (the 1-st group--37 individuals) and with significant emotional tension (the 2-nd group--44 persons). Statistically significant differences between the groups concerned both cerebral hemodynamic parameters and EEG data. On the basis of the results obtained several possible pathogenetic variations of chronic cerebral circulatory insufficiency are suggested on the domination of either primary damages of the structures of the brain and its vessels or the secondary disorder of energetic metabolism and cerebral blood circulation. That, in turn, may be conditioned both by supertension of cerebral systems in chronic stress, and by somatic disturbances. More precise definition of the main pathogenetic factors of initial chronic cerebrovascular insufficiency may be useful for improving efficiency of rehabilitation in these patients.

[Diabetic polyneuropathy in children and adolescents].

Clinical electroneuromyography (ENMG) was carried out in 63 children aged 2-17 years with polyneuropathy (PNP). Correlations were studied between the results of the laboratory studies and ENMG evidence, duration, severity of diabetes mellitus (DM). Diabetic PNP was characterised as demyelinating motor-sensory or sensory PNP. ENMG signs of axonopathy and of a sensory block of the conductivity through the nerves of lower extremities at early stages of DM were estimated as prognostically unfavourable ones in terms of trophic complications development. Duration of DM had some influence on the nervous conductivity, while axonal function was more stable. The factor that determined the severity of PNP course was the age of DM onset. It resulted in the necessity of more accurate control because of the risk of PNP development in children with DM onset at the age of 12-13 years. At early stages of DM conductivity through peripheral nerves correlated with the level of glycosylated hemoglobin and with the frequency of hypoglycemic states.

[The psychological characteristics of patients with neurological manifestations of lumbar osteochondrosis].

Both clinical-neurological and psychological examinations of 42 patients with neurological manifestations of lumbar osteochondrosis with chronic or chronic recurring course were carried out. The main features of personality in most cases were anxiety, hypochondria, passivity and demonstrativity. The clinical signs of nonspecific psycho-emotional tension was found in 70% of cases. Its level correlated negatively with the effectiveness of the treatment. It was established that the negative influence of personality on the course of disease and on the effectiveness of its therapy was connected with the predisposition of such individuals to unfavourable type of reaction toward disease and to development of psychic adaptation disorders.

[The clinico-electroneuromyographic characteristics of acute inflammatory polyneuropathies in children].

Clinical electroneuromyographic examination (ENME) was carried out in 59 children at the age of 1-12 years with acute demyelinating polyneuropathies (PNP). The patients were divided into two groups, namely: 24 children with Guillain-Barré [correction of Giyenne-Barret] syndrome (the first) and 35 ones with diphtheritic PNP (the second group). The main clinical symptoms were investigated in each group. As a result of ENME investigation the axonal and demyelinating types of PNP's beginning were identified. They were distinguished both by severity of course and by rate of restoration. Diphtheritic PNP was characterized by demyelinating type of pathology rise with addition of axonal component in severe cases. The conduction through the additional nerves was investigated to prognose the development of bulbar disorders in PNP.

[The clinico-psychological aspects in the decompensation of the initial forms of a cerebral vascular lesion].

Clinico-neurological, instrumental and psychological methods were used to examine 35 patients at the of 55-70 years with early stages of cerebrovascular insufficiency. It was found out that subjective signs of cerebral vascular disorders were likely to be due to nonspecific cerebral dysfunction and associated with psychical dysadaptation. The degree of memory and attention disorders negatively correlated with intensity of mental activity. The results of the work may be used in prophylaxis of decompensation of cerebrovascular pathology.

[Decimeter radiothermometry in neurologic manifestations of lumbar osteochondrosis].

Using radiothermometry, a new noninvasive method based on recording the thermoradiation of tissues, the authors studied the in-depth temperature of tissues in 106 patients with lumbar osteochondrosis. A radiothermometer adjusted to a wave length of 30 cm was utilized. The accuracy of measurements was +/- 0.1 degree C. The findings showed an elevation of the in-depth temperature of lumbar tissues by 0.3 degree C as compared with the temperature of the mid-thoracic portion of the vertebral column. The elevation was maximal at the site of the involved intervertebral disc imaging. In the tissue of the leg on the side of pain the in depth temperature was 0.4-0.6 degrees C lower. The authors consider that temperature elevation in the lumbar tissue of the vertebral column is a manifestation of the attendant reactive inflammatory process while a decrease in the temperature of tissue on the side of pain is due to the neurodystrophic process with thermal disturbances. Decimetric radiothemometry may be used for the preoperative localization of intervertebral disk hernias.
