Sever-L'Episcopo tendon transfer in obstetric brachial plexus palsy.

Obstetric brachial plexus palsy produces functional and cosmetic impairment. The Sever-L'Episcopo procedure has been successfully used to improve external rotation, primarily in younger patients. Previous studies have shown steady improvement in motion and function for 1 year from the date of surgery, with little additional change beyond this period. This is a retrospective study of 7 patients who have had the procedure at the Shriners Hospital for Children in Tampa, Florida. Functional and cosmetic results have been uniformly excellent at an average follow-up of 1 year 3 months. No complications have occurred. In addition, the degree of improvement seen in our relatively older patient population has not been previously detailed in the literature.

Treatment of vascular injuries from penetrating and nonpenetrating trauma.

The treatment algorithm for vascular injury after trauma, the role of vascular studies, the role of immediate exploration, and other treatments are discussed. Case examples are provided and include direct vascular repair or a vein graft. The potential complications including compartment syndrome also is discussed. This article focuses on the trauma patient who has a vascular injury.

Concomitant skeletal and soft tissue injuries.

The diagnosis and treatment of those injuries most commonly seen in conjunction with distal radius fractures are reviewed in this article. Concomitant tendon, arterial, and nerve injuries often are associated with a high-energy mechanism of trauma. These injuries may complicate an already difficult fracture care plan, especially if the diagnosis of these injuries is delayed. Late tendon rupture and intercarpal injuries are discussed also.
