[Comparative evaluation of various methods of treatment of patients with Sjögren's disease and syndrome at the hospital-polyclinic stages].

The paper is concerned with the results of a follow-up and treatment of 97 patients with Sjogren's disease and syndrome. When disease was at its height they were treated in a hospital, when the activity of a process was on a decrease or absent at all--in the consultation center of an outpatient clinic. Control was exercised in an open study. Glucocorticosteroid therapy was indicated at the height of disease at early stages and much less effective at late stages or in the absence of disease activity. At early stages this type of therapy resulted in substantial improvement of lacrimation and salivation and a positive time course of clinical symptoms. Chloroquine contributed to disease stabilization but did not prevent progression in some of the patients. Administration of local substitution therapy only was not indicated because during a follow-up period the majority of these patients developed xerotic symptoms.

[State of microcirculation and the system of hemostasis in Sjögren's disease, Sjögren's syndrome and chronic parenchymatous parotitis].

Microcirculation and hemocoagulation were examined in 88 patients with Sjögren's disease and Sjögren's syndrome (SD and SS) in 20 patients with chronic parenchymatous parotitis (CPP) without a "dry syndrome", and in 20 healthy donors by conjunctival angioscopy, capillaroscopy of the nail matrix, and histological study of the salivary glands. Hemostasis was explored with the use of 37 tests. SD and SS patients manifested significantly more pronounced microcirculatory disorders versus donors (P less than 0.001) and CPP patients (P less than 0.001). In patients with hypergammaglobulinemic purpura, microcirculatory disorders were found to be enhanced (P less than 0.05). An indirect relationship between these disorders and the degree of lacrimation lowering was established. Intravascular disturbances increased as the process activity rose and diminished under the effect of prednisolone therapy. It is assumed that microcirculatory and hemocoagulation disorders play an important role in the pathogenesis of SD and SS.

[Complex medical rehabilitation of patients with rheumatic heart disease at the sanatorium stage using radon baths from a local source and homologous immunoglobulin].

The paper discusses the efficacy of rehabilitation of 108 patients with rheumatic fever with the use of radon baths, of them in 36 in conjunction with homologous immunoglobulin. The patients were treated in a local balneological hospital. Balneotherapy has been shown to have a beneficial effect on the humoral and cellular immune response, and on factors of the body non-specific resistance in patients with unstable clinicolaboratory characteristics after the treatment for protracted rheumocarditis with minimal activity and latent course. The number of relapses and intercurrent infections were found to be decreased, and the working ability to be improved. The most pronounced effect was recorded in a group of patients who received radon baths and homologous immunoglobulin.
