[Comparative efficacy of the laparotomic and laparoscopic operative procedures in patients with an acute biliary pancreatitis].

Comparative estimation of the laparotomic and laparoscopic operative interventions efficacy was done in 75 patients with an acute biliary pancreatitis. In control group included 39 patients, operated using laparotomic access, in main group--36 patients, in which was done diagnostic and medical laparoscopic interventions. Application of laparoscopy have allowed to establish diagnosis, form of disease, presence of peritonitis and determinate medical tactics. In emergent order operated 8 (22.2%) patients with acute phlegmonous cholecystitis in combination with noninfected pancreatonecrosis. In 28 (77.8%) patients were performed medical laparoscopic interventions with endoscopic papillosphincterotomy.

[Diagnostic laparoscopy and treatment choice in patients with closed abdominal injuries].

The results of laparoscopic investigations, conducted in 94 injured persons with preliminary diagnosis of closed abdominal trauma were analyzed. Of them in 47 the damage of internal organs was excluded. In an emergent order there were operated on 47 patients. Splenectomy was performed in 28 patients (including 2--in whom simultaneously the hepatic and pancreatic lacerations were closed by sutures), cholecystectomy--in 3, cholecystostomy, drainage of bursa omentalis--in 1, resection of small intestine--in 2, resection of omentum--in 1, closure of the small intestine wounds with sutures--in 3, of the bladder with epicystostomy performance--in 2, of the small intestine mesentery--in 2, of the liver--in 1, drainage of bursa omentalis--in 1, opening of infrahepatic abscess--in 1, examination of retroperitoneal hematoma with drainage--in 2.

[Results of diagnostic laparoscopy in patients with acute pancreatitis].

Diagnostic laparoscopic intervention was performed in 339 patients with preliminary diagnosis of an acute pancreatitis (AP). In 164 (48.4%) patients other acute surgical disease was diagnosed and in 175 (51.6%)--an AP. After performance of laparoscopy 11 patients were operated on, in 164 (93.7%)--curative laparoscopic intervention was done. Diagnosis of AP was established in 15 (8.6%) patients during conduction of laparotomy, in 1 (0.6%)--during pathomorphologic investigation. Complications occurred in 4 (2.3%) patients.

[Cranio-abdominal injury].

Cranioabdominal trauma (CAT) is one of the most severe forms of combined injury, in which the mortality rises up to 80%. Four principle factors in diagnosis and treatment of CAT are noted. The absence or noncomplete volume of the aid provided in the before hospitalization period causes unsatisfactory result of treatment in 30% of injured persons. Instrumental methods of investigation play the main role in CAT diagnosis. The informativity of laparocentesis in the closed abdominal trauma constitutes 98%. Operative intervention is performed under the support of antishock intensive therapy. When an urgent operation conduction is necessary it is performed by two teams of surgeons.

[The use of laparoscopy in the diagnosis of a traumatic lesion of the spleen].

The results of laparoscopic diagnosis of splenic rupture in the closed abdominal trauma in 80 injured persons were analyzed. A splenic damage was excluded in 38 patients. In 12 of them hematoma, serohemorrhagic exudate, intestinal paresis were revealed. One patient with duodenal rupture was operated on, and in one more patient the idle laparotomy was conducted. Five patients died. According to the laparoscopic data a splenic rupture was diagnosed in 35 injured persons. Diagnostic failure was conceded in one patient, in whom a hepatic rupture was revealed while the laparoscopy conduction. A successful conservative treatment was conducted in three patients for subcapsular splenic hematoma. Relaparotomy conduction was needed in 3 patients. Three patients died: two of them--due to severe polytrauma and one--due to bronchopneumonia. One else patient died, which was hospitalized with severe polytrauma. In one patient bronchospasm have occurred.

[The indications for performing diagnostic and therapeutic laparoscopy in emergency surgery on the abdominal organs].

In 1352 patients with urgent diseases and traumatic injuries of the abdominal cavity organs laparoscopy was conducted. Diagnostic authenticity of method is 96.5%. Indications to remedial laparoscopy, conducted in 47.4% of patients, were rendered concrete and broadened. An acute inflammation was eliminated in 95.3% of patients, which were operated on according to the indications in a "cool period", 104 (7.5%) of patients died.

[Closed injuries of the liver].

The results of diagnosis and treatment of 42 patients with the closed hepatic injury are analysed. Instrumental diagnosis methods are the most informative ones in the early exposure. Laparoscopy application is preferable to reveal the blood in abdominal cavity, to determine the diagnosis, localization and degree of hepatic injury, the presence of continuing bleeding and the effused blood volume. The operation magnitude was determined individually. Three (7.1%) patients have died.

[The diagnosis and treatment of acute pancreatitis].

From 1986 to 1990 1,577 patients were treated for acute pancreatitis. Destructive forms were found in 585 (37.1%) patients. Clinical, laboratory, roentgenological, endoscopic, and radiation methods were employed in establishing the diagnosis. Laparoscopy was the most informative method which combined successfully diagnostic and therapeutic aspects. Operations were performed on 165 patients with complicated forms of acute pancreatitis. The remaining 1,412 patients received nonoperative treatment including laparoscopic drainage. Total mortality 2.29%, postoperative mortality 21.29%.
