[The parenteral administration of terridecase in the treatment of suppurative-inflammatory diseases of the hand].

Patients with purulent inflammatory diseases were treated by parenteral administration of terrydecaza (polyglucin-immobilised terrylythin). It was noted that injections of terrydecaza facilitated the penetration of antibiotics to the purulent focus. The terms of cleansing the purulent wounds and transition of the inflammatory process to the second stage were 2-3 days shorter. The microcirculation was improved by the daily thermographic monitoring method. The increased penetration through the biological membranes was confirmed by higher concentration of middle weight molecules in plasma of the venous blood flowing off from the purulent focus.

[The role of angiotrophoneurosis of the upper extremity in the course of complicated forms of paronychia].

The degree of the influence of angiotrophoneurosis on the development and outcome of purulent diseases of fingers and hand is analyzed on the basis of treatment of 394 patients with complicated forms of panaritium. It was established that angiotrophoneurosis is a predisposing factor to the development of complicated forms of panaritium and profound phlegmons of the hand. A scheme of using reovasotropic medicines is proposed for the compensation of trophic lesions of tissues of the upper limb distal segments.

[New approaches in the surgical treatment of pyo-necrotic forms of tendovaginitis].

New trends in surgical treatment of purulent tendovaginitis of the hand are described on the basis of clinical data of 295 patients with purulent tendovaginitis and its complications. A sparing operation technique was used as well as the fractional bathing of the purulent focus and regional antibacterial therapy at the postoperative period. This method of surgical treatment resulted in a shorter first stage of inflammation, less risk of complications (dermatogenic contracture, limited mobility in the joints of the fingers and toes, trophoneurotic disturbances) and less influence of injurious factors on the tendon in the process of treatment.

[Social aspects of the problem of suppurative surgical diseases].

The case records of 259 patients with purulent surgical diseases are analysed. The following social groups of patients were distinguished; students--10.4%, office workers--20.5%, factory and agricultural workers--35.9%, lonely pensioners, invalids, and homeless persons--33.2%. The last named group is least socially protected, and the diseases in these cases have a characteristic course and outcomes. In the groups of students and office workers the disease is predominantly marked by the formation of abscesses with rapid resolution after an operative intervention. In the socially unprotected group with somatic diseases, poor nutrition, and absence of care, the purulent process takes a phlegmanous course calling for the use of antibiotics and immunostimulators. The percentage of rehospitalization among these patients is high (up to 20%) due, in many respects, to social factors.

[The characteristics of the microflora in suppurative wounds in a large city].

Based upon their experience with treatment of 259 patients the authors have shown that purulent diseases are caused mainly by Staphylococcus aureus in combination with other microbes. The microflora of purulent wounds often was antibiotic-resistant. The duration of treatment is dependent on the character of the microflora.

[The treatment of acute paraproctitis in an emergency proctology department].

An experience with treatment of 3160 patients with acute paraproctitis under conditions of a specialized department is presented. Rational curative methods allow to achieve healing of 2280 patients (72.2%) with small amount of complications (22 cases). The duration of treatment was shortened to 11.2 days. No recurrences were noted.
