[The Evis-"Elektron" endoscopic video-information system].

A modern Russian-made endoscopic system, worked out by "Electron" Ltd. (Saint-Petersburg) and designed for diagnostics, therapeutic manipulation and surgical interventions by means of home- and foreign-made fiberscopes, is described in the article. The system comprises two functionally finished units, i.e. a video-set fixed to an instrumental support designed for flexible endoscopy and an automated working place for the doctor.

[Endoscopic xenon illuminators].

The 100 W and 180 W commissioned Russian xenon illuminators designed for rigid and flexible endoscopes are described. The key parameters and characteristics as well as the depiction of the overall view of the illuminators and their adapters are presented.

[Percutaneous laser denervation of the zygapophyseal joints in the pain facet syndrome].

Percutaneous laser denervation of archoappendicular joints in spinal pain was made in 15 patients from a study group. Percutaneous high-frequency denervation of archoappendicular joints was performed in a control group consisting of 15 patients with facet syndrome. A preoperative protocol for each patient included at least two diagnostic segmental blocks. A surgical intervention was morphologically substantiated during an experiment; the most optimum surgery protocol was chosen. Destruction was performed by 0.97-micron laser radiation for 4-6 sec at 5-10 W. The number of cases of a complete or partial pain regression, the duration of its impact, side effects and complications were determined. Of the 15 patients from the study group, 8 and 6 patients were observed to have a complete and significant pain regression (more than 50% of the baseline). No effect was achieved in one patient. The similar results were obtained in the control group. There were no postoperative infectious complications in the patients of both groups; transient pain dysesthesia was registered in one case. The findings have led to the conclusion that laser denervation of archoappendicular joints is effective in the pain facet syndrome.

[Cerebral cavernomas followed by epileptic seizures: clinical picture, diagnosis, and treatment].

The clinical features of epilepsy associated with cerebral cavernomas and the results of different types of its surgical treatment: cavernonectomy, extended cavernomectomy, and epileptic focus resection were analyzed. The clinical, radiological, and electrophysiological data were studied in 48 patients with epileptic seizures associated with cerebral cavernomas who had been admitted to the Unit of Neurosurgery, Saint Anna Hospital (Paris, France) in 1982-2001. According to the severity of epileptic manifestations, the patients were divided into 2 groups: 1) 21 patients with single and rare seizures and 2) 27 patients with drug-resistant epilepsy. All the patients received medical antiepileptic therapy. Twelve patients from Group 1 underwent cavernomectomy or "extended" cavernomectomy. In Group 2, 23 patients were operated on and 3 types of operations (cavernomectomy, "extended" cavernomectomy, and corticoectomy) were performed. Statistically significant differences were found in anatomic, clinical, and electrophysiological parameters. Criteria associated with the development of drug-resistant epilepsy were established. These included: the early onset of seizures, temporal cavernoma, and a combination of partial and generalized seizures in one patient. The paper shows it necessary to choose a surgical intervention in relation to the duration of seizures, their frequency, and a response to medical antiepileptic therapy. The advantages of surgical treatment over medical one were not found in single and rare seizures (Group 1), while in drug-resistant epilepsy, surgical treatment yielded much better epileptological results than did medical treatment. The volume of a surgical intervention depends on the sizes of an epileptic focus detected by an in-depth preoperative study and on the site of a cavernoma.

[Surgical treatment of multileveled diskogenic compression of the cervical spine and its roots].

The authors studied 54 patients with diskogenic compression of the cervical spine and nervous roots, who had undergone interventions including diskectomy, anterior cervical spondylodesis, and plate fixation. The purpose of the study was to provide rationale for choice of a surgical treatment for multileveled diskogenic compression of the cervical spine. Twenty-seven patients with multileveled lesion underwent diskectomy at 2-3 levels with and without vertebral body resection. Preference was given to the mini-invasive technique involving diskectomy, anterior cervical spondylodesis, by using osteoinductor blocks, and plate fixation. A control group consisted of 27 patients with one-leveled lesion who had undergone conventional surgical treatment. 0.5-3-year follow-ups showed positive results in 84% in the study group and in 89% in the control one. Fusion was successful in 89 and 96% in the study and control groups, respectively. The rate of complications and the level of successful professional and social rehabilitation were similar in both groups.

[Effect of diethylene-triaminopentamethylphosphonic acid complex on various hematologic and biochemical indicators in experimental animals].

A successive triple introduction of organophosphorus complexon DTPP (sodium-dicalcium salt of diethylene-triaminopentamethylphosphonic acid) in a dose of 500 mg/kg to albino male rats did not produce any marked effect on the composition of the peripheral blood. Some shortening of the initial period in blood coagulation process was noted. DTPP produced a moderate stimulation of the iron-containing enzymes (catalase, peroxidase, cytochroxidase) which led to an increased oxygen consumption. The blood serum and urine potassium, sodium and calcium content did not undergo any significant changes, while the magnesium concentration in the blood serum dropped by 37 pc and in the urine rose by as much as twice as compared to controls. This bears evidence to an elevated discharge of magnesium from the organism following the action of the DTPP complexon.

[The oxenoid model of the mechanism of activating molecular oxygen by cytochrome p450: the role of substrate structure].

The arene oxides formation energy for about 30 benzene derivatives were calculated in frames of MO LCAO method. The benzene derivatives are typical substrates of cytochrome P-450 catalyzed aromatic oxidation. The relationship between the formation energy of tetrahedral type arene oxides and the relative reactivity of substrates toward microsomal hydroxylation was found. The formation energy of arene oxides correlates also with toxicity of benzene derivatives. Some limitations of the oxenoid model are discussed.

[The binding of exogenous DNA pRK31acZ by rabbit spermatozoa, its transfer to oocytes and expression in preimplantation embryos].

It was shown that 35.3% ejaculated rabbit spermatozoa washed from seminal plasma are capable of interacting with heterogeneous DNA. The major part (85.2%) of bound DNA was located in the post-acrosomal part of the sperm head. After incubation with plasmid pRK31acZ the spermatozoa transferred it in the oocytes during in vivo fertilization, as shown by expression of reporter gene lacZ in 19.3% of preimplantation embryos. Additional treatment of the mobile spermatozoa with DMSO and heat shock raised the efficiency of exogenous DNA incorporation in the spermatozoa, as expressed in the percentage of embryos containing bacterial beta-galactosidase (61.7%). It is proposed to use the capture of heterogeneous DNA by the spermatozoa and its transfer in the oocytes for production of transgenic animals and in studies of regulation of gene expression at the early stages of embryogenesis.
