[Pulmonary tissue welding--the method of nonresectional intervention for spontaneous pneumothorax].

New method of the viable tissues welding was applied for spontaneous pneumothorax treatment. There was presented first experience of pulmonary tissue welding without its resection. Since January, 2005 there were performed 17 operations of restricted thoracotomy in randomly selected patients with spontaneous pneumothorax. During the operation there were revealed solitary disseminated bullas in 4 (23.5%) observations, intrapulmonary and superficial--in 8 (47%), thin-wall bullas conglomerate--in 5 (29.5%). In 6 patients up to the operation performance the air passage was preserved. In all the patients the parts of emphysematously changed pulmonary tissue were processed using bipolar set up of original construction. In all the patients there were achieved the bullas electroablation and pulmonary tissue closure without usage of suturing devices or manual suturing. The pneumothorax recurrence, bleeding, durable passage of air, infection complications were not observed. The authors consider the method of emphysematously changed tissue welding a secure one in treatment of spontaneous pneumothorax, omitting pulmonary tissue resection.

Stimulation of reparative osteogenesis after xenotransplantation of human prenatal mesenchymal stem cells and chondroblasts.

Regeneration of the bone tissue after unilateral xenogeneic transplantation of a suspension of human mesenchymal cells and chondroblasts was studied in rats with experimental damage to both femurs. The state of animals was satisfactory and non-depressed in the early and late postoperation period. No local pathological reactions and complications were seen. Administration of mesenchymal stem cells and chondroblasts into the area of bone defect accelerated regeneration on days 10 and 30 (compared to the control group). Implantation of mesenchymal stem cells more significantly stimulated reparative osteogenesis compared to treatment with chondroblasts. This was mainly associated with increased ratio of mature lamellar bone tissue. Unilateral transplantation of the cell suspension stimulated regeneration in the contralateral limb due to accelerated maturation of the bone tissue. The bone tissue formed after transplantation of mesenchymal stem cells and chondroblasts was integrated into bone organ and underwent complete remodeling. Xenotransplantation of prenatal mesenchymal stem cells and chondroblasts without immunosuppression was not followed by the development of early and delayed complications or local reactions of graft rejection.

[Lung contusions: diagnostics, treatment stages].

The authors studied a group of patients with simple thorax injury, contusion of lungs and injury of thorax frame (1225 patients), who had been refused to be hospitalized to specialized poly-trauma departments due to some reasons. 34% of these patients had fractures of ribs, thorax and rib arcs and 15% of them had two-sided traumas. The group of the patients with simple thorax injury and lung contusion, but without injuries of thorax frame, consisted of 63% of the patients, who addressed for an aid at first hours after accidents. Clinical X-ray pictures have been not clearly expressive. 37% of patients, who had asked for medical aid in 3-5 days after accidents, had trauma complications. As a result, they had long-term outpatient treatment of pneumonia and pleurisy. In order to provide necessary aid to patients and carry out prophylactics of delayed complications it is recommended to hospitalize these people to therapeutic and pulmonary departments.

[Differential diagnosis of venous aneurysm of femoral hernia].

Eight hundred and twelve patients with varicose disease were examined, 18 (2.2%) of them had aneurysms in the region of sapheno-femoral anastomosis. Venous aneurysm was diagnosed before surgery in all 18 patients. All of them underwent surgery with positive result. Diagnosis of aneurysm of great saphena vein in inguinal region is not difficult in most cases. Difficulties usually occur in patients with overweight. In 5 patients with obesity aneurysm was initially diagnosed as femoral hernia. Duplex scanning is the best non-invasive method of diagnosis of venous aneurysm in obese patients.

[The evaluation of the external respiratory function in patients in late follow-up period after pneumonectomy].

The external respiration function (ERF) indexes were studied in 215 patients in 25-40 years after performance of pneumonectomy. In 55.3% patients ventilation indexes have exceeded the due values by 50% for two lungs, in 8.4%--significant disorders of the ERF indexes were noted in connection with an early and late complications occurrence and with presence of specific and metaphthisic changes in a single lung as well.

[Use of electrostimulation for treatment of critical lower limb ischemia].

The analysis of efficiency of bone electrostimulation in the treatment of lower limb arteries occlusions was carried out. The method was used in a combination with lumbar sympathectomy and revascularising osteotrepanation in 52 patients with chronic critical limb ischaemia, in whom reconstructive operation was impossible. Short-term good results were achieved in 69.2% of the patients. The method allows to accelerate compensation of circulation in ischaemic limb.

[Origination of mediastinal hernia in late follow up period after performance of pulmonectomy].

In 25-40 years after pneumonectomy conduction the mediastinal hernia (MH) occurred in 62.4% of 210 patients. Principal clinico-radiological signs of MH were vicarious hypertrophy and true pulmonary emphysema. Vicarious hypertrophy of single lung is forming, as a rule, in patients, operated on in childhood, is characterized by favourable clinical course and constitutes the MH compensated stage.

[Diagnosis of a closed injury of the diaphragm in the acute phase].

The diagnosis experience of the closed diaphragmatic injury (CDI) while an acute period in 42 injured persons was analysed. The roentgenological examination methods, in the injured persons with combined trauma in particular, were adduced. The informativity of roentgenological signs, including those examined in dynamics, was analysed. The indications and contraindications of the contrast gut examination, pneumoperitoneum, thoracoscopy for the CDI suspicion are substantiated. The methods of the security enhancement during the diagnostical pneumoperitoneum conduction are proposed.

[The efficacy of leukinferon in suppurative-septic diseases].

In the complex of treatment of purulent-septic diseases leukinferon at a dose of 300 IU/kg was used according to the following scheme: 3 injections with the 48 h interval between injections. The effect of leukinferon mainly on the function of neutrophil granulocytes and subpopulation of T-lymphocytes was established. Together with clinico-laboratory data indicative of the effectiveness of treatment, normalization of interferon status and decrease in the level of a circulating factor of tumour necrosis in seriously ill patients was observed.

[Identification, isolation and physico-chemical properties of neurospecific alpha2-globulin].

In the immunization process of rabbits with the protein fraction of the water-salt extract from human brain partially and concentrated at 500-600 times was received antiserum revealed brain specific alpha 2-globulin that is not identical to the known cytoplasmatic brain specific antigens. This antigen has got electrophoretic mobility of alpha 2-globulins, molecular weight 90 +/- 10 kD and isoelectric point 4.1-5.4. Develop the procedure for purification of this antigen on the basis of the combination ion change, affinity, hydrophobic chromatography gel-filtration and isochromatofocusing.

[Peace-time thoraco-abdominal injuries].

The article discusses the diagnosis of thoracoabdominal wound (TAW) in 190 patients and the results of their operative treatment, information given in a questionnaire by 70 patients and out-patient examination of 19 of them in the late-term period. The clinical abdominal symptomatology is encountered 7 times more frequently in TAW than in and in combination with the X-ray signs allowing the diagnosis to be made in 21.58% of cases with TAW. Contrast study of the wound canal, thoracoscopy, and diagnostic pneumoperitoneum make it possible to establish or reject the diagnosis of TAW in all patients examined. Thoracotomy in the 7th intercostal space or at a lower level with transdiaphragmatic laparotomy allows for correction of the injuries in the chest and abdomen in 92.30% of patients. The mortality rate was 2.63%.
