Reconstruction of the aortofemoral segment using a minilaparotomy access. Experience with 100 operations.

This paper analyzes the results of 100 aortofemoral reconstructions using a miniaccess (patient group I), performed between July 2002 and December 2004. Of these, 92 reconstructions were bilateral and 8 unilateral. The patients' age ranged within 29-83 years (mean 58.3+/-0.9 years). The access to the aorta was gained through a median minilaparotomy (MLT) measuring 5-12 cm in length. The proximal anastomosis was formed above or at the level of the inferior mesenterial artery. The intra- and postoperative data were compared with the results of 162 aortofemoral reconstructions performed using a standard laparotomy (StLT) group II. The times of operation and aortic clamping did not increase during MLT (the time of operation was 192.3+/-4.0 and 207.7+/-5.1 min., the time of aortic clamping 24.3+/-1,6 and 25.8+/-1.7 min in groups I and II respectively). The lowering of traumatic injury during MLT resulted in a decrease of the amount of myorelaxants (by 17.1%; P<0.001) and of the volume of intraoperative infusion (by 12.4%; p<0.05). The short-term postoperative period in group I patients ran a milder course. The painful syndrome was less pronounced and bowel function returned to normal earlier. The incidence of local vascular complications was not different (9.0% during MLT and 10.5% during StLT). In group I, the incidence of local non-vascular complications decreased (from 14.8 to 8.0%, p=0.15) due to the absence of eventrations. The incidence of systemic complications during MLT dropped from 21.0% to 11.0% (p=0.056). During StLT the postoperative lethality accounted for 3.1%, that during MLT for 1.0% (p=0.49). The postoperative hospital stay decreased by 27.8% (p<0.001).

[Long-term hemodynamic characterization of distal anastomoses after aortofemoral reconstruction].

Duplex scanning was used to characterize the qualitative and quantitative parameters of blood flow in portofemoral bypasses and distal anastomoses in the long-term (3- 144 months) period after operation. In 50% of cases with an end-to-side anastomosis, the examination reverled arterial segment occlusions caudal to the anastomosis. 12.5% of patients showed the maintenance of antegrade blood flow in the native arteries (parallel with the bypass). Retrograde blood flow through the anastomosis was recorded only in 32.5% of cases. As a rule, it was restricted by 1-2 branches of the external iliac artery, reaching the internal iliac artery only in 5% of cases. After one year the cumulative retrograde patency of the end-to-side anastomoses constitute 59.1+/-7.2% and after 5 years it is equal to 18.6+/-9.9%. If the superficial femoral artery was patent, the volume blood flow in the bypass was higher. The rise of the volume blood flow was also induced by the presence of the retrograde patency of the anastomosis which was apparently linked with the origination of an additional runoff pool. This advantage is especially completely realized when the condition of the distal perceptive bed is poor.

[The long-term prediction of atherosclerosis of the aortofemoral segment in patients who were not provided reconstructive operation].

The present work deals with the 10-year follow up of 225 patients presenting with Leriche's syndrome. The patients suffered from not less than stage IIb chronic arterial insufficiency of the lower limbs and did not undergo reconstruction. The study was made of the factors influencing disease prediction. The 5- and 10-year survival of patients without critical lower limb ischemia (CLLI) accounted for 55.0+/-7.9% and 20.5+/-8.4%. In patients with CLLI, it constituted 41.5+/-7.2% and 18.7+/-7.5%. After primary amputation only 5% of patients had survived toward the 5th follow up year. The presence of coronary artery disease worsened prediction. In patients without CLLI, the limb could be saved toward the 5th follow up year in 81.7+/-6.9% of cases, toward the 10th year in 78.3+/-6.9% of cases: in patients with CLLI, in 59.8+/-6.9% and 56.8+/-6.9% of cases respectively. The given indicator is significantly lower in patients with occlusion of the femoropopliteal segment. Adequate conservative therapy was carried out only in 30.5% of patients.

[Functional characteristic and molecular topology of voltage independent sodium channels].

The channel proteins so far known are transmembrane oligomers arranged in a manner that the polar residues are lining the central ion-conducting hydrophilic pore. In the last decade, electrophysiology and molecular biology studies revealed the principal similarity in the functional properties and membrane topology within a large family of sodium-conducting channels. Amiloride-sensitive channels are expressed in the apical membranes of renal epithelia. Moreover, in different mammalian cells non-voltage-gated sodium-selective channels have been recently found. According to molecular cloning of the respective DNAs and amino acid sequence analysis, epithelial channel subunits, degenerins and some other channel proteins display a significant homology in the regions forming two presumable transmembrane domains. This paper reviews some relevant data and current opinions of the superfamily of sodium-conducting cation channels.

[Method of primary jejunogastroplasty after gastrectomy and subtotal resection of the distal stomach for cancer].

Post-gastrectomy and post-resection conditions are occurring more frequently due to the growing number of radical operations carried out for stomach cancer. The available procedures for restoring the natural continuity of the gastrointestinal tract are either too sophisticated or ineffective. A new technique of primary jejunogastroplasty following subtotal distal resection of the stomach is suggested. Application of newly-devised esophagoen-terostomy and enteroduodenostomy makes possible storage and passage of chyme into the duodenum and prevents refluxesophagitis and reflux-jejunitis from developing. A 3 month-3 year follow-up of 35 surgical patients showed that a number of grave post-gastrectomy complications could be avoided if the procedure had been used.

[Functional properties and cytoskeletal-dependent regulation of sodium channels in leukemia cell membranes].

The paper is devoted to membrane mechanisms of sodium influx from the extracellular medium to the cytoplasm in nonexcitable cells. With the use of patch clamp technique, the activity of non-voltage-gated ionic channels in plasma membrane of human leukemia K562 cells was examined. We have identified two types of Na-permeable channels characterized by unitary conductance of 12 pS and differing in their selectivity among monovalent cations. A relative permeability value PNa/PK was estimated for both types referred to as channels of high (HS, PNa/PK = 10) and low (LS, PNa/PK = 3) selectivity, resp. Both the channels were impermeable to bivalent cations (Ca2+, Ba2+), not blocked by tetradotoxin. Their sensitivity to amiloride was extremely low. Cytochalasin D treatment of cells resulted in a significant increase in the activity of LS Na-conducting channels. Application of exogenous gelsolin to the cytoplasmic surface of inside-out membrane patch at free Ca2+ level of 1 mkM induced a similar effect of sodium channel activation; the subsequent addition of actin reduced the channel activity up to the background level. Our results show that the cortical F-actin network plays an important role in regulating the novel family of sodium channels in nonexcitable cells. It could be assumed that the actin disassembly causes a rise in LS channel activity, whereas the actin assembly induces inactivation of the channels.

[Morphological changes in the thymus in experimental autoimmune hemolytic anemia].

Morphological changes in the mouse thymus in the experimental autoimmune hemolytic anemia were correlated with the results of the study of the immune status. The epithelial component of the thymus was more involved in the response to the experimental autoimmune hemolytic anemia during the whole experiment; this allows one to assume that just this component predetermines the character of this autoimmune process.

[Spinal anesthesia with lidocaine combined with moradol].

The efficacy of spinal anesthesia with lidocaine (1 mg/kg) combined with moradol (0.005 mg/kg) during urologic surgery has been compared with that of spinal anesthesia with lidocaine (1 mg/kg) combined with morphine (0.01 mg/kg). The adequacy of anesthesia was evaluated using common clinical and biological parameters. Analysis of the data obtained has shown that spinal anesthesia with lidocaine in combination with moradol is a simple and effective technique of regional anesthesia. A combination of local anesthetics with moradol ensures adequate anesthesia and pronounced prolonged postoperative analgesia and sedation. The absence of serious side effects makes it possible to substitute moradol for narcotic analgesics used in spinal anesthesia.

[The characteristics of the immune status of patients with chronic calculous cholecystitis].

Under study was the state of the immune status in 89 patients with chronic calculous cholecystitis and 30 donors. It was shown that the level of the immune parameters in patients with chronic calculous cholecystitis as compared with donors might be considered as the secondary immune deficiency. This category of patients represents a group of high risk in relation to postoperative infectious complications. At the postoperative period in patients with chronic calculous cholecystitis it is necessary to investigate the immune status for correction of its disturbances.

[The use of combined methods of magnetoelectrotherapy in treating polyneuropathies].

A comparative evaluation by such parameters as alleviation of pain syndrome, improvement of peripheral resistance and vegetotrophic processes, a decline in pareses and sensory disorders has been performed in 3 groups of patients: group 1 underwent benzohexonium electrophoresis, group 2 benzohexonium electrophoresis in the magnetic field produced by the unit "Polyus-I" followed by low-frequency electrotherapy with bipolar impulse current, group 3 benzohexonium electrophoresis in the magnetic field from the unit "ADMT-Magnipuls" followed by low-frequency electrotherapy with bipolar impulse current. The best clinical and physiological results were reported in group 3 patients.
