[Standardization of coagulologic technique for antithrombin III estimation and its clinical application].

A standard assay procedure for estimating antithrombin III activity was derived from the Abildgaard technique as follows: 1) to decrease errors at the preanalytical stage, by using preliminarily defibrinated lyophilized plasma-calibrator and 2) to increase measurement accuracy, by using the concentration of a fibrinogen preparation and simplifying the analytical procedure. The estimation of plasma antithrombin activity in patients with pulmonary thromboembolism or sepsis points to the changes in antithrombin III activity.

[Study of storage stability of the commercial series of thromboplastin preparations].

The stability of commercial series of the thromboplastin preparation (obtained from rabbit brain) with calcium (TKS, Researach-and-Production Enterprise "Renam" MICH 1.1-1.2), of TKS "Tromborel" and of "Immunoplastin" (Technoclon) was comparatively studied. TKS "Renam" preserved its activity and sensitivity to factor VII in the liophilizated state for as long as 6 months. The preparations of "Renam", TKS, preserved the stable activity in the reconstructed liquid state, under the conditions of continuous storage, at 37 degrees C for 2 days, at 18-20 degrees C--for 2-3 days, and at 2-8 degrees C--for 5-7 days, which was accompanied by a stable high sensitivity to factor VII. The stability of the "Renam", TKS, was found to be similar to that of imported preparations.

[Disorder in the efficiency of oxygen release to the tissues from preserved human erythrocytes and the rate of its regeneration during their perfusion of the isolated heart].

The main hemodynamic and oxygen supply parameters of the isolated rat heart (128 experiments) were measured during its perfusion with human donor red cells differing by storage duration and kind of hemopreservatives. The degree of the reduction in efficacy of oxygen release to myocardial tissue and the speed of the release restoration depended both on the red cell storage time and the preservative composition. The authors advocate a new preservative citroglucophosphate with adenine as it proved superior to all the preservatives studied.

[Effectiveness of oxygen delivery to tissues during complete replacement of blood by bovine polyhemoglobin].

The study has been performed to test the hypothesis on feasibility of omitting pyridoxylation in production of bovine polyhemoglobin as compared to polyhemoglobin from human red cells. The model of a complete replacement of rabbit blood for polyglucin (Het approximately 2%) was used to compare efficacy characteristics for O2 delivery to tissues upon introduction of bovine polyhemoglobin against rabbit red cells. Immediately after the replacement, O2 release efficacy and that of circulation were identical, whereas 30 and 60 min after it, developing decompensation of hemodynamics indicated inability of bovine heterohemoglobin, unlike own red cells, to increase A-B difference by oxygen, probably due to metform rise.

[Gastric leiomyomas].

Leiomyomas are rare tumors found as a rule in the stomach (1-2% of all gastric tumor cases) and occasionally in the intestine. Unless they grow large or become ulcerated gastric leiomyomas run a silent course. Otherwise there is epigastric pain, hemorrhage or impaired gastric evacuation. When complicated, the disease presents diagnostic difficulties and is treated surgically. Gastrotomy with dissection of the tumor or local resection of the gastric wall proved a reliable therapeutic means. Gastric resection is indicated in ill-defined tumors or in suspected malignancy.

[Characteristics of the dynamic outpatient observation of workers at an industrial enterprise].

A computer-assisted staged system of a follow-up of factory workers with CHD and diabetes mellitus was developed. It included automated screening by questionnaire, ECG and biochemical screening, physical examination, investigation of the basic carbohydrate-lipid indices, more detailed examination in out- or inpatients settings using up-to-date diagnostic methods (bicycle ergometry, monitoring after Holter, etc.), identification of groups for a follow-up, and therapeutic and sanitary measures. A total of 27750 persons were investigated. The most important group was that including persons with risk factors: vegetovascular dystonia developing into CHD (6%) or essential hypertension (20%). Sanitary measures by unified methods caused a decrease in temporary disability with the economic effect of 197,000 rubles.

[Certain mechanisms of the effect of EDTA on thrombocyte aggregation].

Effect of EDTA on the ADP-induced platelet aggregation and intraplatelet regulatory mechanisms were examined. EDTA (and EGTA) was shown to inhibit platelet aggregation and to potentiate the similar effect of theophylline and dibutyryl cAMP. Inhibition of platelet aggregation was seen during incubation of EDTA in conjunction with dibutyryl cAMP at concentrations that did not change the platelet aggregation profile provided the agents cited were incubated alone. Verapamil and tetracain potentiated the inhibitory effect on aggregation of EDTA, dibutyryl cAMP and EDTA plus dibutyryl cAMP.

[Dependence of blood platelet aggregation capacity on agents affecting the intrathrombocyte Ca2+ content].

The effect of agents changing absorption and redistribution of Ca2+ in platelets on platelet aggregation was studied. It was shown that inhibition of intercellular Ca2+ absorption by verapamil and tetracain block of intracellular Ca2+ redistribution decrease platelet aggregation. A joint action of verapamil and tetracain increase the inhibition of aggregation. EDTA decreases platelet aggregation affecting platelet regulatory mechanisms associated with the action of cAMP and probably directed at a decrease in the platelet cytoplasmic Ca2+ content.

[Effect of the new Soviet thrombolytic preparation streptodecase on the fibrinolysis system].

The usefulness of the thrombolytic therapy in diseases of the cardio-vascular system, complicated or caused by thromboses is beyond doubt. However, the existing medicinal forms of fibrinolytic enzymes possess a number of disadvantages which limit their use. To enhance the efficacy of thrombolytic therapy a streptokinase, named streptodekase, has been created and immobilized on a water-soluble matrix. Streptodekase was tested on the haemocoagulation system in 7 patients with thrombosis of the femoral artery or the branches of the pulmonary artery. It was seen that streptodekase causes a considerable and rapid increase of fibrinolysis without unwanted disorders in the blood coagulation and other side effects. The convenience in use and simplicity (the therapeutic dose of 3,000,000 units is administered in a single injection, intravenously and immediately), the safety and high specific activity permit one to think that the drug may be widely used.

Effect of sympathectomy on platelet aggregation and blood coagulation in rats.

It is shown that the ability of blood platelets to aggregate in partially and completely sympathectomized rats is significantly lower than in intact animals. The blood clotting system of sympathectomized rats is hyperactive. The sympathectomy-provoked changes may be due to the increased content of adrenaline in the blood.

[Effect of bioresolving microspheric preparations of immobilized fibrinolysin on the fibrinolysis system].

The effect of microspheric preparations of immobilized fibrinolysin on the fibrinolysis system has been studied in dog experiments. It was shown that the fibrinolytic activity of whole blood increases in the region where the preparation is applied due to the presence of the introduced enzyme and to the increase in the activity of natural plasminogen activator. The application of immobilized fibrinolysin causes predominantly an increase in the activity of the plasminogen activator rather than in the fibrinolytic activity itself. The data obtained suggest the therapeutic importance of the method of immobilized thrombolytic preparation application under investigation.

[Tissue fibrinolytic activity of different sections of the heart in sympathectomized animals].

In the heart of intact rats aged 6 and 10 weeks the activity of nonenzymatic fibrinolysis was found to be maximum in the auricular tissue. In partly sympathectomized (destruction of 75% of neurons in the ganglia) 10-week-old rats fibrinolytic activity was sharply reduced in the auricular tissue, and in the atrial and ventricular myocardium. The content of the plasminogen activator in all parts of the heart decreased to zero value. In stress situations 50% of the rats die from thrombi in the atrial cavity. In partly sympathectomized 10-week-old rats (characterized by restoration of peripheral sympathetic innervation) the studied parameteres were normalized. The fibrinolytic potential of the heart tissue was markedly decreased in totally sympathectomized rats (destruction of more than 99.5% of the neurons); under the effect of stress 80% of these animals died.

[Effect of desympathization on thrombocyte aggregation capacity and the blood clotting potential of rats].

It was shown that platelet aggregation in partially (with stellate ganglia containing 25% neurons of normal amount) and completely (0,5% neurons) sympathectomized rats was significantly lower than in intact animals. Concurrently the blood coagulation system of sympathectomized rats was hyperactive. The reasons for sympathectomy-induced changes seems likely to be elevated adrenalin blood concentration in such rats.

[Blood kinin system in acute myocardial infarct treated with thrombolytic preparations].

The treatment of patients with acute myocardial infarction by thrombolytic agents affects several indices characterizing the pathological process. The effect of thrombolytic therapy was studied with reference to the activation of the blood kinin system. The data on a activating effect of myocardial infarction upon the state of the kinin sys tem were proved. When thrombolytic agents (streptoliase, streptase and fibrinolysin) and heparin are given, the kinin system is activated still further which clearly correlates with an elevated fibrinolytic activity of the blood. Under fibrinolytic therapy the most sensitive component of the kinin system proves to be kallikreinogen. The possible effect of the activation of the blood kinin system by thrombolytics upon the state of myocardial infarction patients is discussed.

[The effect of plasmin on kallikreinogen].

The effect of purified plasmin preparation on kallikreinogen isolated from rat blood plasma was studied by chromatography on DEAE-Sephadex. Plasmin was shown to affect kallikreinogen to form an active enzyme--kallikrein. The dependence of the activation degree on the plasmin activity and incubation time was studied. Evidence was found for the absence of Hagemann factor in the reaction medium, which excludes the possibility of indirect action of plasmin on kallikreinogen through the Hagemann factor fragments formed.
