[Water-electrolyte balance in patients during operation on the aorto-femoral segment using various types of anesthesia].

The indices of aqueous-electrolyte metabolism in patients, who underwent the reconstructive operations on the aorta, were studied. In patients with concomitant diabetes mellitus, a method of choice for anesthesia is an epidural anesthesia, with ischemic heart disease--combined epidural anesthesia. The most severe changes in electrolyte metabolism were noted after restoration of blood flow in blood loss of more than 10 ml/kg, under conditions of epidural anesthesia these shifts are less pronounced.

[Use of hypertonic sodium chloride solution for correction of arterial hypotension in patients with endogenous intoxication].

Reaction to infusion of 7.5% sodium chloride solution was studied in 81 patients with endogenous intoxication during development of critical arterial hypotension which could not be prevented or arrested by routine high-dose therapy. The reaction was considered adequate on condition that arterial pressure increased no higher than the safe level, subsequent maintenance of hemodynamics was effective, toxin binding with plasma albumin was increased, diuresis increased, and hypernatriemia was transitory in the presence of general improvement of patient's status. Such a reaction was observed in 83% patients. The efficiency of this method is determined by indications for low-dose therapy and probability of adequate reaction to infusion.

[The volume of circulating blood during surgery of the aorta performed under epidural anesthesia].

Various methods of circulating blood volume (CBV) determination have been compared. The calculation method based on anthropometric to hemodynamic parameter ratio was shown to be optimal during surgery on the aorta and major vessels. Epidural anesthesia in 15% CBV deficit in the preoperative period and anesthetic administration into the epidural space intraoperatively in 10% CBV deficit were contraindicated. Epidural anesthesia caused CBV reduction, with its maximum changes observed upon blood flow recovery in the vascular bed. 10-15 ml/kg, 15-20 ml/kg and 20-30 ml/kg blood loss was 2.5,3 and 3.5 times overcompensated, respectively.

[Effect of transurethral resection of the prostate on the state of patients with ischemic heart disease].

The water-salt equilibrium and the degree of endogenous intoxication (albumin fluorescence test) were examined in 60 patients with transurethral resection of the prostate (TURP), 40 of whom had ischemic heart disease (IHD). Body temperature, ECG, ST segment and echocardioscopy were daily monitored perioperatively in all patients. The results showed a lack of any pronounced changes in the water-salt equilibrium in TURP that lasted up to 1.5 hours and included big volumes of 5% glucose; they were also indicative of a lower postoperative binding albumin ability, which is normally most pronounced in patients with hyperthermia. As for the IHD patients, hyperthermia was found to be concurrent in them with the onset of ischemia of the myocardium and with its low contractive ability, which can be referred to as a significant factor in case of the above patients.

[Choice of lung ventilation at suspension laryngoscopy in patients with laryngeal neoplasms].

Three methods of artificial ventilation (AV) (volumetric, high-frequency injection, and high-frequency percutaneous transtracheal jet ventilations) were studied in 340 patients when laryngeal tumors were removed, by using a Stortz laryngoscope under multicomponent general anesthesia and local airways anesthesia. Examining the parameters of respiration and circulation in the use of three AV methods has indicated the advantages of the high-frequency percutaneous ventilation that ensures the prevention of aspiration complications and steady-state values of gas exchange and blood circulation, including in those with laryngeal stenosis and at a high risk for hemorrhage.

[The use of nitroglycerin in general anesthesia during vascular alloprosthesis].

Glycerol trinitrate (GTN) was used during anesthesia in angiosurgical patients for the correction of vascular tone in initial hypertension, normalization of cardiac output and retention of compensatory vascular reactions in critical conditions of the circulatory system during inclusion and exclusion of the aorta from circulation. The combination of GTN with calypsol and narcotic analgesics reduces the hypertensive effect of calypsol and tachysystolic effect of GTN.

[The combined efferent therapy of a blood-transfusion conflict in a puerpera].

Plasmapheresis, ultrafiltration of blood, and hemodialysis were effectively used in the treatment of a 20-year-old woman after cesarean section who was transfused 300 ml of wrong group blood. The resultant hemotransfusion conflict was characterized by shock, disseminated intravascular coagulation, hemolysis, acute pulmonary edema and renal failure. Plasmapheresis helped abolish the hemolysis. Pulmonary edema was eliminated by ultrafiltration of 2 liters of blood. Six sessions of hemodialysis administered within 10 days arrested renal insufficiency. The treatment administered may be considered as the method of choice in such patients.
