[The interferon therapy of patients with chronic viral hepatitis: the factors affecting the treatment results].

The paper presents clinicoimmunological characterization or therapeutic efficacy of alpha-interferons produced in Russia and criteria of their administration in patients with chronic viral hepatitis. A 6 to 12 months course (single dose 1 x 10(6)-3 x 10(6) IU improves the disease running, hepatic function and corrects immune status in 55.6% of patients with chronic active hepatitis eventuating in liver cirrhosis and in 57.9% of patients in chronic active hepatitis of moderate activity. Alpha-interferons are indicated in chronic active hepatitis and cirrhosis under high activity of cytolytic process, immunodeficiency, weak autoimmune process and protein shifts.

[The treatment of patients with chronic persistent hepatitis B with interferon preparations].

Alpha-interferon preparations were injected intramuscularly in a single weekly dose 1.10(6) - 1.5.10(6) IU in courses lasting from 1 to 6 months to 37 patients with chronic persistent hepatitis B (CPHB) who entered a randomized clinicoimmunological trials. Except for moderate hyperfermentemia and HBs-antigenemia CPHB patients responsive to 3-4-month courses, alpha-interferon treatment of CPHB had no advantages over conventional therapy of the disease.

[The interferon status and cellular immunity of pregnant women with viral hepatitis B].

The examination of 62 pregnant women with acute viral hepatitis B has established in them inhibition of interferon genesis and the presence of secondary T-cell immunodeficiency. The most pronounced and long-lasting was a fall of gamma-interferon. A simultaneous lowering of alpha-, gamma- and serum interferon was indicative of complicated pregnancy or premorbid condition. A persistent deficient production of gamma-interferon and T lymphocytes, imbalance of their subpopulations promoted long-term course of hepatitis and persistence of HBsAg.

[The use of reaferon in treating patients with protracted viral hepatitis].

The paper reports the results of a randomized clinicoimmunological trial of domestic recombinant alpha 2-interferon (reaferon) in 30 patients with protracted viral hepatitis (27 subjects had viral hepatitis B and 3 patients HBV + HDV-coinfection). The reaferon treatment started on the disease week 7-8 and lasted for 2 months (1 x 10(6) IU, once a week, i.m.). VHB patients benefited from reaferon treatment as evident from improved clinical and laboratory indices, cases of complete cure and the agent elimination. In a protracted course of HBV + HDV coinfection reaferon in the above doses failed to produce any effect.

[The clinical significance of immunodeficiency states in patients with viral hepatitis B].

Examination of 374 patients with acute viral hepatitis B made it possible to unravel certain mechanisms of the development of secondary immunodeficiency in the T immunity system: hypoxia and hypoxemia, activation of lipid peroxidation, suppression of antioxidant activity of blood serum, derangement of the synthesis of free radical forms of oxygen by mono- and polynuclear cells and ATR deficiency in them, disturbance of the interferonogenesis and synthesis of cyclic nucleotides, suppression of metabolic activity of immunocompetent cells. The character and intensity of immunodeficiency states underlie the formation of different clinical forms of viral hepatitis B.

[Reaferon and interferonogenes in the therapy of acute viral hepatitis B].

Monotherapy with reaferon (alpha 2-interferon) versus prodigiosan plus ibuprofen combination was investigated in acute viral hepatitis B. The drugs were found to stimulate cellular immunity and to induce a decrease of total serum IgE levels. The effect of prodigiosan combined with ibuprofen proved more pronounced.

[An assessment of the therapeutic efficacy of gene-engineered human alpha 2-interferon in patients with acute viral hepatitis B].

The clinical trials of alpha 2-interferon (alpha 2-IF) (reaferon) in 412 patients with acute viral hepatitis B (AVHB) permitted the establishment of the optimal therapeutic dose and the schedule for the drug use. The drug tolerance appeared satisfactory on the whole. alpha 2-IF was found to have a beneficial effect on the clinical course of VHB. The relationship was established between the therapeutic effectiveness of the drug and the clinical variants of the disease as well as the time of drug administration. The highest clinical effect (in 86.3%) was attained as a result of early (before the 7th day of jaundice) use of alpha 2-IF in patients with the acute-cyclic pattern of VHB. That effect consisted in the reduction of the treatment period (by 6-8 days), in a 2-fold reduction of the incidence of HBs-antigenemia, in rapid stimulation of the IF system, and in the initially low activity of natural killers. If the drug was administered later, the positive clinical effect was detectable only in 61% of the patients. In cholestatic VHB, alpha 2-IF did not produce any therapeutic effect.

[Immunomodulating properties of leukinferon].

Tolerance and immunomodulating properties of leukinferon for injections were studied in 13 healthy volunteers. The drug was administered by inhalation and intramuscularly in a single dose of 10,000 MU twice daily for 3 days. Intramuscular injections of the drug were accompanied by a number of side effects: pyrogenicity, weakness, headache, that ceased 8-12 hrs after the drug was discontinued. Despite these side effects, the drug was satisfactorily tolerated. Neither viscera nor systemic disorders nor allergic reactions were detected. T-lymphocytes proved to be the most sensitive to the drug. Leukinferon was found to activate the interferon system. The effect was the most marked in respect of IRL-gamma.

[Caused of intensified lipid peroxidation in the blood of patients with viral hepatitis B].

The tissue oxygen concentration, the serum antioxidant system state and the serum malondialdehyde (MDA) concentration were studied in patients with hepatitis B. The good correlations were studied in patients with hepatitis B. The good correlations between MDA concentration in patients serum and the oxygen concentration in tissues (R-0.79), and the cytoplasmic enzymes activity (R-0.75 for lactate dehydrogenase; R-0.75 for alanine transferase) were found. On the other hand, it was shown an antioxidant activity decrease of ceruloplasmin-transferrin system in patients serum. It is proposed, that the tissue hypoxia and the decrease of the serum antioxidant activity are the general factors leading to the MDA accumulation in the serum of patients with hepatitis B.

[The use of hyperbaric oxygenation in treating viral hepatitis B and the reaction of the blood leukocytes].

A total of 75 patients with virus hepatitis B of medium gravity were examined for the effect of HBO on the clinical course of the disease and blood leukocyte reaction. This reaction was tested on the basis of a complex of rosette-forming and cytochemical tests. HBO was found to produce a favourable effect if used at the early stages of the treatment (the first week of hospitalization). The effect consisted in significant reduction of the rate of exacerbations and residual phenomena. HBO provoked a decrease in the content of T and B lymphocytes by the 10th session of the treatment. A close relationship was revealed between HBO efficacy and the initial level of functional metabolic activity of leukocytes. The use of HBO was always accompanied by the rise of that activity during the treatment. The use of HBO at later times of the treatment (weeks 4-5) did not produce any well-defined clinical effect. Thus, HBO (8-10 sessions, pressure 1.5 absolute atmosphere, exposure 45 min) may be indicated as a prophylactic measure in respect of an unfavourable disease course with regard to the patients' selection on the basis of a complex of cytochemical tests.

[A monooxygenase enzymatic system and cortisol in the serum of pregnant women with viral hepatitis].

Specific features of the mono-oxygenase enzyme system functioning and blood serum cortisol level were studied in viral hepatitis patients in the second half of pregnancy. The more pronounced disorders of this system occur in a severe course of hepatitis. The level of cortisol and antipyrine clearance were in correlation which was changed depending on the form and severity of the disease course.

[Hemosorption in infectious pathology].

Hemosorption was used for the treatment of 152 patients with purulent meningitis, sepsis complicated by shock, virus hepatitis and leptospirosis. A favorable effect of hemosorption in infections was based on highly effective correction of microcirculatory disorders as a result of the removal of high molecular substances of microbic and endogenous origin.

[Blood lymphocyte and neutrophil function in patients with viral non-A, non-B hepatitis and a fecal-oral mechanism of the transmission of infection].

A decrease in the capacity of lymphocytes for rosette formation, accompanied by a rise in the leukocyte count, has been found in patients with the severe form of hepatitis non A, non B. Neutrophils in these patients have shown a tendency towards an increased rosette formation. All altered characteristics normalized by the period of convalescence. As noted in this investigation, a considerable increase in the leukocyte count, along with an increase in the number of active rosette-forming neutrophils and a decrease in the capacity of leukocytes for rosette formation, is an unfavorable sign predicting the possibility of acute hepatic encephalopathy.

[Reactogenicity, toxicity and tolerance of reaferon in healthy volunteers].

In the clinical trial of reaferon, introduced intramuscularly and intravenously in different doses (2 X 10(6), 6 X 10(6) and 9 X 10(6) I. U. daily) for 3 days, various side effects were observed. Their intensity depended on the dose of the preparation and the route of administration. The most regular reaction was a rise in temperature, accompanied by slight weakness, headache, loss of appetite, slight dizziness, euphoria. All side effects were transient. No toxic or allergic reactions involving the organs and systems of the body were observed. Low doses of the preparation (2 X 10(6) I. U. daily) promoted an increase in the amount of lymphocytes in the peripheral blood, stimulated cellular immunity factors and contributed to the optimal functioning of the interferon systems. High doses (6 X 10(6) to 9 X 10(6) I. U. daily) had a pronounced immunosuppressing effect. On the whole, reaferon proved to be fairly tolerable.
