A 48-year-old female developed a slowly progressive paraparesis due to a posterior epidural mass extended on several thoracic segments. Clinical and myelographic appearance was strongly in favour of metastasis, the most common tumors in the spinal epidural space. A laminectomy was carried out and disclosed an encapsulated epidural tumor which was loosely adherent to the dura matter. Histological features were those of an angiolipoma. A brief review of clinical and pathological characteristics of spinal lipomas is performed. Extradural spinal angiolipomas are unusual benign neoplasms. The results of surgical treatment in this location are mostly excellent.

A new method for exploring the lumbar spine by means of serial computed tomography sections is proposed, radio-anatomical appearances of the normal lumbar canal and its contents being described and illustrated.

The authors report two cases of "sinus pericranii" a rare vascular tumor of the skull, communicating with the meningeal veins or with the dural sinuses. The partial reduction in size of the tumor in the sitting position may aid the clinical diagnosis. The X ray diagnosis is highly enhanced by the direct injection of contrast medium into the tumor, leading to the visualization of the outflow in the miningeal veins, while external carotid angiography is normal. Step by step coagulation of the verious supply coming through the skull allows a non-bleeding surgical excision.

Thirty of 200 cases of lumbar radiculalgia of degenerative origin presented clinical, radiologic, saccographic, and computed tomography signs of a typical narrow lumbar canal, half of these patients having benefited from surgical mobilization of the neuromeninges. Performances of new generation scanners have enabled the development of computed tomography images typical of a narrow lumbar canal, and also to improved assessment of radiotomographic and saccographic signs, as a result of direct visibility of osteo-articular radicular constrainst factors in the transverse axial plane. Findings in stenotic dysplasia, the role of vertebral arthrosis, the particular cases of spondylolisthesis, and the entity determined by isolated stenosis of the nerve root canal (mainly stenosis of the lateral recess) are discussed.
