[Spontaneous dissection of the internal carotid artery].

A typical case history of spontaneous dissection of the internal carotid artery is presented. In young patients with cerebral ischemia, initial pain in the affected side of the head and neck, a possible provocative mechanism and a transient or even persistent Horner's syndrome are highly suggestive. The angiographic picture of carotid artery dissection is characteristic. Spontaneous resolution is common, and recurrence rare. Surgical treatment can therefore be reserved for individual cases with recurrent ischemia and caused by emboli originating in the dissected segment of the artery.

[Percutaneous transluminal angioplasty (PTA) of the arteries of the arm in brachial and cerebral ischemia].

35 patients with brachial and/or cerebral ischemia underwent 37 PTA's. Indications for PTA were stenosis (n = 31) or occlusion (n = 1) of the subclavian artery, stenosis of the brachiocephalic trunk (n = 2), stenosis of the axillary artery (n = 2) and stenosis of the brachial artery (n = 1). 23 patients demonstrated a subclavian steal phenomenon. However, the minority of patients (n = 10) presented with neurologic symptoms, which could be provoked by brachial exercise in only 3 patients. 7 of the 10 patients with cerebral ischaemia demonstrated additional significant stenosis of the extracranial arteries. The technical success rate for PTA was 89.2%. Two patients showed relapse of the stenosis. The clinical success rate regarding brachial ischaemia was 94.4% (follow-up: 6-37 months). Neurologic symptoms disappeared after treatment in 72.7%. Minor complications without clinical sequelae occurred in 8.6%.

[Inflammatory aneurysm of the infrarenal aorta].

Abdominal aneurysms with extensive periarortic fibrosis should be considered a variant of idiopathic retroperitoneal fibrosis. Rupture of these inflammatory aortic aneurysms seems to be less frequent than in arteriosclerotic aneurysms. Due to this experience preoperative corticosteroid treatment may be allowed, which appeared effective in patients with retroperitoneal fibrosis. 5 cases of inflammatory aneurysms are presented.

[Stress tolerance of pancreatic cancer patients].

The patient with pancreatic carcinoma is tested in 4 phases: during diagnosis the non-invasive procedures take precedence; as regards pre-operative treatment, primary biliary decompression seems to bring no advantages in the icteric patient; the indications for various treatment forms show a resectability rate of only 13%; the final test is the mortality rate of the treatment selected: in 200 duodenopancreatectomies it amounted to 2.5%.

[Insulin sensitivity and glucose utilization of patients after total duodenopancreatectomy (author's transl)].

Studies in 7 patients after total duodenopancreatectomy showed a raised insulin sensitivity and a prolonged effect of depot insulin. Insulinemia is not the only reason for the tendency to hypoglycemia shown by these patients, which seems rather to be due to the lack of pancreatic glucagon, which is not available to antagonize the inhibition of glucose release from the liver by insulin (and also in alcohol abuse).

[Spontaneous erosion hemorrhage in pancreatic cysts].

Spontaneous hemorrhage without earlier exploration in pseudocysts is a serious complication of chronic relapsing pancreatitis. In the past six years we have seen 6 patients with that disease in 91 cases of pancreatic cyst; 4 patients had distal pancreatectomy and partial excision of the cyst without postoperative complication; 2 patients died without having operative intervention.

[Pancreatic colon stenoses].

Benign colonic stenosis may be caused by pancreatitis. Colonic obstruction arising during the acute stage of pancreatitis is mostly found to be reversible. Persistant colonic stenosis after acute or relapsing pancreatitis may point to an adjacent abscess. Two patients with pancreatitic colonic stenosis illustrate those different courses of the disease.
