Sarcoma at the site of a single hip screw. A case report.

We report a case of malignant fibrous histiocytoma of the hip which occurred 30 years after the insertion of a single chrome-cobalt alloy screw for a slipped femoral epiphysis. The possible aetiological association between malignant tumours and metallic implants is discussed.

Prevention of pressure sores in patients with fractures of the femoral neck.

A critical appraisal was made of the incidence of pressure sores occurring in the sacral region of patients undergoing treatment for fractures of the neck of the femur in an acute orthopaedic unit. A prospective controlled study was then carried out to investigate the effectiveness of the 'Reston' flotation pad in the prevention of such sores in a similar group of patients.

A biomechanical study using cadaveric toes to test the stability of fixation techniques employed in arthrodesis of the first metatarsophalangeal joint.

Arthrodesis of the first metatarsophalangeal joint of 15 pairs of cadaveric big toes were performed. The arthrodesis site was shaped with either a dome and socket or planar surface contact between the metatarsal head and proximal phalanx. A template was used to standardize the position of arthrodesis in 20 degrees of valgus and 20 degrees dorsiflexion. Various fixation devices were used to hold this position in corresponding pairs of specimens, and these fixation devices were tested to determine their relative effectiveness to prevent displacement on loading. Force-deflection curves were obtained for the various techniques tested, and by comparing these curves, the stability of the metatarsophalangeal arthrodesis site was found to be maintained most effectively using the cancellous screw fixation device with planar metatarsophalangeal surfaces.

Complications with antibiotics used prophylactically in joint replacement surgery: a case report of cephradine-induced pseudomembranous colitis.

A woman underwent joint replacement surgery with prophylactic antibiotic cover, and subsequently died as a result of pseudomembranous colitis. We have noted a high incidence of antibiotic diarrhoea with cephradine, and suggest that if administration of prophylactic antibiotics is restricted to 3 days or less, the incidence of diarrhoea would diminish, and the therapeutic value of the antibiotics would be maintained.

Avulsion fracture involving the body of the patella.

A report on an unusual transverse fracture through the body of the patella in a 13 year old high jumper is presented. The clinic features and treatment of the more usual avulsion injury of the patella is discussed.
