C-peptide concentration in pancreatic juice obtained during endoscopic pancreatography in patients with and without insulinoma.

ERCP was performed in three patients with insulinoma. One had a large malignant tumor, while the remaining two had small tumours. In two of these patients pancreatic juice was collected for C-peptide determination. Pancreatography was performed and pancreatic juice was obtained in seven other subjects comprising: five control subjects and two patients in whom insulinoma was suspected because of symptoms suggestive of hypoglycaemia. Pancreatography was normal in all subjects except the patient with a large insulinoma in whom an obstruction of the main pancreatic duct was found. The maximal C-peptide concentrations in pancreatic juice of patients with insulinoma were found to be several-fold higher than in the control subjects and in one of the patients in whom insulinoma was suspected but unproven. The remaining patient with suspected insulinoma had a maximal C-peptide concentration comparable with those found in patients with proven insulinoma. Thus remarkable differences in maximal C-peptide concentrations obtained in patients with and without insulinoma were found. However, the clinical significance of the findings needs further evaluation. The value of ERP in patients with suspected insulinoma may be twofold: an obstruction of the main pancreatic duct may indicate a large, hardly resectable tumour; in patients in whom the duct is unaffected the relation between the tumour as visualized by angiography, and the duct, is of value for the surgeon when planning the operation.

Lateral fistulae and cysts of the neck. Heredity and diagnosis.

A family with congenital fistulae from the second cleft or pouch over four generations is described. In order to obtain some idea of how often anomalies are found in relatives of patients with this type of malformation a material embracing 114 patients has been examined. The investigation demonstrates that patients with a complete lateral fistula of the neck from the second cleft or pouch showed a heredity of 35%, whilst patients with lateral neck cysts did not show any recognizable familial tendency. Views on differential diagnosis are presented, including the value of fine-needle biopsy, where in 92% of the cases a diagnosis of branchial cysts of the neck was obtained.

Influence of indomethacin on glucose-induced insulin response in normal man--role of prostaglandins in the rapid insulin release?

The possibility of a role of endogenous prostaglandins in glucose-induced insulin release was investigated in normal human subjects with short-term indomethacin treatment. This treatment was found profoundly to inhibit the early insulin response to a 1 h glucose infusion test. In contrast, the later insulin response was enhanced, probably mainly as a result of the higher degree of hyperglycemia induced in experiments with the drug. Together with in vitro data on the relationship between prostaglandins and cyclic AMP in the pancreatic islets, the present finding is considered to point at the possibility of an involvement of prostaglandins in the normal regulation of the rapid glucose-induced insulin release.

Influence of severe potassium depletion and subsequent repletion with potassium on muscle electrolytes, metabolites and amino acids in man.

1. Two women with severe hypokalaemic alkalosis were investigated by means of muscle biopsy before and at the end of 2 and 3 weeks respectively of intense therapy with potassium chloride. 2. The muscle biopsy material was analysed for water, electrolytes, adenine nucleotides, phosphocreatine, free creatine, pyruvate, lactate, glycogen and free amino acids. The extra- and intra-cellular distribution of water, electrolytes and amino acids was calculated by the chloride method. 3. Both patients showed a marked loss of intracellular potassium and an increase in intracellular sodium concentration. The muscle magnesium content was also slightly decreased. After repletion with potassium chloride, muscle sodium and potassium became normal. 4. The contents of creatine phosphate, ATP, ADP, AMP, lactate and pyruvate were within normal limits, but the phosphocreatine/total creatine ratio was reduced. After repletion, a small change in the apparent creatine-phosphokinase equilibrium had occurred, suggesting a minor increase in intracellular pH. 5. The concentrations of the basic amino acids, lysine, arginine and ornithine were increased far above normal. The intracellular accumulation of arginine was much higher than the increase in lysine concentration and histidine concentration was normal. This differs from findings in potassium-depleted rats, where the intracellular lysine concentration is much higher than arginine concentration and histidine is high as well. After potassium repletion the intracellular concentration of ornithine, lysine and arginine became normal in one case and decreased considerable in the other. An increased intracellular concentration of glutamate and glutamine was also observed after potassium repletion.

Palsy of the recurrent nerve following mediastinoscopy.

As demonstrated in earlier material, the frequency of palsy of the recurrent nerve following mediastinoscopy varies from 0.18 to 0.53 percent (17,000 patients). In 67 patients operated via mediastinoscopy, where pre- and postoperation indirect laryngoscopy was carried out, four postoperative palsies (6 percent) were discovered. Of these, 3 were found among the 24 patients with malignant infiltration of the mediastinum. The value of pre- and postoperation examination of the larynx is stressed.
