Significance of alterations in PBMC immunophenotype of children with chronic viral hepatitis C-- the role of dendritic cells.

There are differences in the clinical course of chronic viral hepatitis C between adults and children, but it is generally accepted that the disease has cell-mediated immune background. The aim of this study was to evaluate PBMC subsets in children with chronic hepatitis C before treatment in order to find some predictive factors, useful for patients management. Several PBMC subsets, in particular lymphoid and dendritic cell (DC) ones, were tested by flow cytometry in HCV(+) paediatric patients (n = 46) and in control children matched in terms of age and sex (n = 20). Data were subjected to extensive statistics. It was found that cells with cytotoxic potential such as CD8(+)CD28(-) T cells, NK and NKT cells as well as lineage(-)HLA-DR(+) DC were increased in per cent values, while CD4(+) T cells and CD4:CD8 ratio were decreased in hepatitis C group. In HCV(+) patients, CD4(+) T cells were inversely correlated with alanine aminotransferase (ALT) levels and with viraemia. DC subset of myeloid origin (CD11c(+)) assessed both in per cent values and as mean fluorescence intensity (MFI) of HLA-DR expression was shown to be downregulated in hepatitis patients, in spite of increased numbers. To conclude, PBMC subsets, and in particular DC, are affected by HCV chronic infection in children, reflected by the correlation with clinical parameters, such as ALT and viraemia.

Activity of antioxidant enzymes and concentration of lipid peroxidation products in selected tissues of mice of different ages, both healthy and melanoma-bearing.

The activity of antioxidant enzymes and the concentration of the lipid peroxidation product malondialdehyde (MDA) as indicator of oxidative damage were determined in selected tissues of healthy mice and transplanted B16 melanoma-bearing mice with increasing age. A total of 60 male mice were divided into 6 groups. Groups 1, 2 and 3 consisted of tumor-free, healthy mice aged 1, 9 and 16 months, respectively (average life span: 2 years). Groups 4, 5 and 6 consisted of mice of the same age as the healthy mice, but given intraperitoneally 10(6) cells of B16 melanoma for 2 weeks. An increase in the concentration of MDA was found in all the studied tissues (brain, liver, lungs, erythrocytes) and blood plasma of 16-month old healthy mice compared with the younger ones. The activity of superoxide dismutase (SOD) and catalase (CAT) was elevated in the brain and the activity of CAT and glutathione peroxidase (GPx) in the liver of aged healthy mice. The transplantation of melanoma caused an increase of the concentration of MDA and of the activity of all studied enzymes in all tissues. This elevation was most pronounced in the youngest mice group 4 and was higher than in the oldest healthy group 3. Thus, these early changes of the "(anti-)oxidative status" in the investigated tissues caused by the tumor development have similarities with age-associated alterations of healthy mice, especially in regard to MDA in all tissues or SOD and CAT in brain.

Heterogeneity of extraparenchymal primitive neuroectodermal tumors within the craniospinal axis.

Four cases of primitive neuroectodermal tumors (PNETs) with unusual localization (three intraspinal extramedullary and one pontocerebellar) are reviewed. Histologically, they were small round blue cell tumors with diverse patterns. Immunohistochemically, all tumors were positive for at least two neuronal markers, two cases were Mic-2 positive and one showed glial differentiation. The paraffin-embedded tumor specimens were examined by interphase FISH using dual-color probes specific for EWS, HER-2 and BCR loci. Molecular cytogenetic study revealed the presence of EWS rearrangement in two cases and the presence of i(17q) in one tumor. Three tumors exhibited 22 disomy and one was 22 polyploid. Extraparenchymal PNETs within craniospinal axis are heterogeneous from the clinical, histological, immunohistochemical and molecular point of view. These PNETs can be of a central or peripheral type. Multidisciplinary approach is of a basic importance in differential diagnosis of such cases.

Lipid peroxidation and antioxidant capacity in selected tissues of healthy black C57BL/6J mice and B16 melanoma-bearing mice.

During the process of melanogenesis free radicals are generated. The aim of this study was to investigate the effects of melanogenesis in B16 melanoma on lipid peroxidation and antioxidant capacity in selected tissues of black C57BL/6J mice. The study was conducted on 24 mice: 12 healthy controls and 12 with a transplanted B16 melanoma. Two weeks after the melanoma transplant, when the average weight of the tumours was approximately 2.0 g, blood samples were taken from the orbital venous plexus. The mice were killed by dislocation of the spinal cord, and the brain, liver and lungs were removed for analysis. The level of thiobarbituric acid-reactive substances (TBARS) and of 1,1-diphenyl-2-picrylhydrazyl (DPPH) reactive substances were determined in full liver, lung and brain homogenates and in serum. The activity of superoxide dismutase (SOD) was determined only in homogenized tissue. The concentration of TBARS and the SOD activity were statistically significantly higher in all the studied tissues from mice with B16 melanoma than in tissues from healthy mice. The antioxidant capacity, however, was lower in the tissues of melanoma-bearing mice. The results obtained demonstrate an increase in oxidative stress in the tissues of mice bearing a transplanted B16 melanoma.

Activity of some lysosomal enzymes in serum and in tumors of patients with squamous cell lung carcinoma.

The aim of the study was an assessment of some lysosomal enzymes activity in serum and in tumors of patients with lung cancer histopathologically confirmed as squamous cell lung carcinoma. The first group constisted of 10 patients with stage II of the disease and the second group consisted of 11 patients with stage III of the disease. Lysosomal enzymes activities were assayed in serum before surgery and on the 10th day after surgery in serum and in tumors. Arylsuphatase, cathepsin D and acid phosphatase activities were higher in the patients serum than in that of the control group. The decrease of arylsulphatase and cathepsin D activities after surgery was statistically significant in both groups of patients, but the cathepsin D activity was still 3 times higher in patients than in those from the control group. The decrease of acid phosphatase activity after surgery was about 50% in both groups of patients and this decrease was statistically significant. The arylsulphatase and acid phosphatase activity in tumors was nearly 3 times higher in stage III patients than it was in stage II patients, but the cathepsin D activity was nearly the same in both patient groups. Higher lysosomal enzyme activity may be a useful factor in diagnosing and monitoring of lung cancer. However, further investigations are needed.

Activity of antioxidant enzymes and concentrations of thiobarbituric acid reactive substances (TBARS) in melanotic and amelanotic Bomirski melanoma tissues in the golden hamster (Mesocricetus auratus, Waterhouse).

The activity of superoxide dismutase (SOD) and glutathione peroxidase (GSHPx), as well as the concentration of thiobarbituric acid reactive substances (TBARS) in tissues of transplantable melanoma in the golden hamster were measured and compared. Ten inbred male hamsters were used for the experiment. They were divided into two groups and were given Bomirski melanoma cells subcutaneously. The first group was given melanotic (Ma) melanoma cells. The second group was given amelanotic (Ab) melanoma cells. Thirty days after the transplantation the hamsters were dissected and the tumor tissues were taken and homogenized. A statistically significantly higher activity of the measured antioxidant enzymes was found in homogenates of Ma tumor than in homogenates of the Ab tumor. Activity of SOD is 8% higher in melanotic melanoma, 24% higher in CAT, and 45% higher in GSHPx. Statistically significant differences between TBARS concentrations were not confirmed. The higher activity of antioxidant enzymes in the melanotic tumor is a result of increased generation of oxygen-derived free radicals. It is presumed that it is strictly connected with intensified production of quinone and semiquinone radicals in the process of melanogenesis.

Enhanced lipid peroxidation processes in patients after brain contusion.

Erythrocyte superoxide dismutase (SOD-1) activity and cerebrospinal fluid (CSF), blood plasma low-density lipoprotein (LDL) and erythrocyte thiobarbituric acid reactive substance (TBARS) concentrations were determined in 30 patients with brain contusion and in 37 control patients with low back pain due to noninflammatory degenerative lumbar disc disease. In comparison to controls, during 10-day follow-up patients with brain contusion had significantly increased erythrocyte SOD-1 activity and CSF, blood plasma (LDL), and erythrocyte TBARS concentrations. The highest CSF TBARS concentrations were observed in five patients who died 2, 7, or 8 days following head injury. A significant negative correlation was found between erythrocyte SOD-1 activity or TBARS concentrations, in the blood plasma LDL fraction and erythrocytes, and The Glasgow Coma Scale score. These results suggest that enhanced lipid peroxidation processes, which seem to correlate with the severity of head injury, accompany brain contusion.

[Significance of cathepsin B and D in physiologic and pathologic processes].

The lysosomal proteases play an important role in the cells nourishment, immunogenesis, development of the arteriosclerosis perturbations in blood vessel cells and in the pathogenesis of degeneration diseases, cancer diseases and in the great number of others. For the many cancer disease durations, an increased activity of the lysosomal enzymes both in blood serum and in tumor tissues was revealed. Over the past few years a particular attention have been paid to the fact that the high activity of some lysosomal enzymes i.e. cathepsin B and D is connected with the increased cancer invasiveness. The results of preliminary clinical research point to great diagnostic value of cathepsin B and D in breast cancer detection.

[A rare case of adenoid cystic carcinoma of the larynx].

One case of rare laryngeal tumor--adenoid cystic carcinoma in 58 year old woman was reported in this paper. Authors wanted to drew attention to few complaints and only slight dysphagia in comparison to local extensiveness of the neoplasm. Diagnostic difficulties were underline. Epidemiology, clinical course and therapy were demonstrated in laryngeal adenoid cystic carcinoma.

The application of electron microscopic morphometry as helpful method in the diagnosis of focal segmental glomerulosclerosis (FSGS) early phase. I. Morphometric electron microscopic studies of renal glomeruli in cases of focal segmental glomerulosclerosis (FSGS), minimal change disease (MCD) and mesangioproliferative glomerulonephritis (GNMES) in children.

Electron microscopy morphometric studies were carried out on material obtained from children with minimal change disease (MCD), mesangioproliferative glomerulonephritis (GNMES) and focal segmental glomerulosclerosis (FSGS). The results indicated that an increase in the volume of the matrix in mesangioproliferative glomerulonephritis was due to an increase in the number of cells. The amount of produced matrix in mesangioproliferative glomerulonephritis was proportional to the number of cells in mesangium (so there is no overproduction of matrix). The ratios of mesangial matrix volume to cell volume and matrix volume to the volume of the entire mesangial region in MCD and GNMES were similar and differed significantly from the ratios found in FSGS. The present findings indicate that morphometric studies may be useful in the early diagnosis of glomerular sclerosis. This is particularly significant in cases where light microscopic examination fails to reveal changes indicative of glomerular sclerosis.

The application of electron microscopic morphometry as a helpful method in the diagnosis of focal segmental glomerulosclerosis (FSGS) early phase. II. Clinical usefulness of electron microscopic morphometric studies in cases of minimal change disease (MCD) and mesangioproliferative glomerulonephritis (GNMES) with suspicion of progression into focal segmental glomerulosclerosis (FSGS).

Clinical and morphological analysis (including morphometric studies of electron microscopic material) was made in 15 children with MCD and 15 children with GNMES. In both groups, an early phase of FSGS was suspected on the basis of electron microscopic studies. Moreover, analysis included the results obtained in 13 children with the diagnosis of FSGS and in whom repeated biopsies were performed. In most of them, MCD or GNMES was diagnosed from the first biopsy. In most children in whom electron microscopic studies revealed an increase in matrix area in some mesangial regions, thereby suggesting an early stage of glomerular sclerosis, the results of morphometric studies resembled or were identical to the results obtained from a control group with the established diagnosis of FSGS. These findings indicate that morphometric studies of electron microscopic material are significant. The results, when compared with the clinical data, confirm the usefulness of such a diagnostic procedure.

[The role of Epstein-Barr virus (EBV) infection in carcinogenesis of palatine tonsil carcinoma (PTC)].

OBJECTIVES: Casual link of Epstein-Barr Virus (EBV) and endemically manifesting nasopharyngeal carcinoma has been already well documented. Our study, conducted in Karol Marcinkowski University of Medical Sciences in Poznań, aimed at examining whether EBV infection occurs also in patients with another tumor, palatine tonsil malignant neoplasms. METHODS: The studies were performed on 20 patients, aged 32 to 80 years, in whom tonsil malignencies was diagnosed by histological methods. The treatment of choice was surgery. Sera of the patients was tested for EBV DNA (Sharp Signal System; Digene), IgM-anti-EBV antibodies (Behring), IgG-anti-EA antibodies (ETI-EA-G; Dia Sorin) and IgG-anti-VCA antibodies (ETI-VCA-G; Dia Sorin). RESULTS: Serological exponents of reactivated EBV infection (serum IgG-anti-EA and IgG-anti-VCA antibodies) were observed in 12 patients. In parallel, ten of the patients demonstrated EBV DNA. In the remaining 8 patients, three manifested past EBV infection (positive IgG-anti-VCA) and five probed to be EBV negative. CONCLUSIONS: The results indicated, that tonsil malignancies may be associated with EBV infection.

Comparative studies of nuclear DNA content in benign and malignant thyroid lesions.

Currently available data suggest that DNA aneuploidy is associated with aggressive behavior of and unfavorable prognosis in several malignant human tumors as compared with diploid malignancies. However, the diagnostic and prognostic importance of flow cytometric DNA measurements in the case of thyroid neoplasms remains controversial. Therefore, the aim of our study was to evaluate utility of DNA index (DI) and proliferative index (PI) in distinguishing benign from malignant thyroid lesions taking into account the possible influence of intra-tumor heterogeneity and tissue preparation mode on DNA flow-cytometry measurements. A retrospective study was performed on 71 paraffin-embedded specimens from 57 patients with benign and malignant thyroid pathologies: 13 colloid goitres, 12 parenchymatous goitres, 19 adenomas and 13 carcinomas. In 14 of 57 cases two separate specimens taken from different areas of the same lesion were analysed and DNA parameters were compared. Additionally, flow cytometry DNA analysis was parallelly performed on 3 adjacent but differently processed tissue sections (fresh, formalin-fixed and paraffin-embedded) taken from each of 26 surgically excised thyroid lesions. DNA content was also analysed in both fresh and formalin-fixed twin specimens of normal pig thyroid glands (N = 6). We demonstrated that all tumors diagnosed as thyroid carcinomas were associated with abnormal nuclear DNA content although aneuploidy was not found specific to malignant thyroid tumors. Aneuploid samples of benign thyroid lesions exhibited higher proliferative activity, expressed as mean PI values, than diploid ones. In carcinomas the mean PI values were significantly higher than in benign lesions, independently whether they concerned aneuploid or diploid tissues. Considering intra-tumor heterogeneity, the flow cytometric DNA parameters can be assumed as reproducible despite differences in the mode of tissue fixation and preparation for analysis.

[Accuracy of ultrasound-guided fine-needle aspiration biopsy in diagnosis of changes in thyroid nodular lesions].

The aim of this study was to compare the results of US-FNAB with definitive histological examination of thyroid nodular lesions. 590 patients who underwent surgery were reviewed (473 females, 117 males, ranging in age from 9 to 81 years, average 36 years). Histological evaluation of cytologically diagnosed benign nodules revealed nodular goiter in 407 cases (91.5%), Hashimoto's thyroiditis in 2 (0.4%), follicular adenoma in 31 (7%), papillary carcinoma in 2 (0.4%) and follicular carcinoma in 3 (0.7%). In the cytological group of follicular nodule (n = 71) histological diagnoses included: nodular goiter in 11 cases (15.5%), follicular adenoma in 36 (50.7%), papillary carcinoma in 2 (2.8%), follicular carcinoma in 20 (28.2%). The diagnosis of papillary carcinoma (n = 65) was confirmed histologically in 59 cases (90.8%), in the remaining 6 cases Hashimoto's thyroiditis and medullary carcinoma were diagnosed. In the cases diagnosed cytologically as medullary carcinoma (n = 8) histological diagnoses included: medullary carcinoma in 7 cases (87.5%). The cytological diagnosis of anaplastic carcinoma (n = 1) was confirmed histologically. These results support the value of US-FNAB in the diagnostics of thyroid neoplasms. US-FNAB performance was as follows: sensitivity 78%, specificity 97%, accuracy 92%, 2.3% of false positive and 6.1% of false negative results.

Influence of diethylenetriamine (DETA) and sodium nitroprusside (NaNP) on sister chromatid exchange frequency and cell kinetics in cultured human lymphocytes.

Diethylenetriamine (DETA) and Sodium Nitroprusside (NaNP), the exogenous NO-generating compounds, were tested for their genotoxicity in human lymphocytes in vitro using the sister chromatid exchange (SCE) technique. Both compounds were found to be inactive in inducing SCE in concentrations from 0.3 to 30 pM. However both compounds displayed an inhibiting effect on cell kinetics.

[Diagnostic difficulties in nasopharyngeal tumors].

Diagnostic possibilities in lethal midline granuloma (Wegener's granulomatosis, angiocentric lymphoma) based on the literature were presented. Results of diagnostic examinations in a case of 34 years old man admitted to the Dept. of Otolaryngology in Poznań were described.

[Metastasis of papillary thyroid carcinoma resembling the cervical lateral cyst].

Cystic neck mass, enlarging for many years, as the first symptom of occult thyroid carcinoma was presented. On the basis of clinical and ultrasonographical examination the initial diagnosis was made: bronchiogenic neck cyst. The metastases of papillary carcinoma of thyroid gland were recognized in histological examination. The patient was admitted for surgical treatment.
