We studied the quantity of Langerhans cells in 36 patients with cervical human papillomavirus (HPV) infection. Significantly fewer Langerhans cells (p < 0.05) were found in patients with compared to those without DNA tetraploidy. Similarly, patients positive for HPV 16/18 DNA by in situ hybridization or antipeptide IgA antibodies to HPV 18 tended to have fewer Langerhans cells than those negative for HPV 16/18 DNA or IgA antibodies. Our results suggest that depletion of Langerhans cells is associated with productive HPV 16/18 infection in the cervical epithelium.

A case of acute myocardial infarction (AMI) in late pregnancy is reported. The prognosis for pregnant women with AMI and the mode of delivery is discussed. A woman with recent myocardial infarction does not seem to benefit from parturition by Cesarean section but should rather have an assisted vaginal delivery with regional anesthesia.
Class-specific immune response in acute herpes simplex virus (HSV) infections to individual virus specified polypeptide antigens was analysed by immunoblotting. The HSV specified glycoproteins B (gB), C (gC), and D (gD) were detected. IgG-antibody response was shown to develop to various other virus specific polypeptides as well. The IgM- and IgA-antibody responses remained restricted to only a few HSV specific proteins, namely VP 13 (80 kD), VP 20 (52 kD), and VP 23 (40 kD) and to the low molecular weight polypeptides.

A total of 180 (0.21%) out of 85.177 deliveries were complicated by abruptio placentae (AP) during the period 1962-1981. Of these the obstetric records of 130 deliveries were retrospectively studied in order to elucidate risk factors for the occurrence of abruptio placentae as well as to find out factors influencing the outcome of the newborn. The control group consisted of 120 randomly chosen contemporary parturients. Preterm contractions during pregnancy seemed to be most significantly associated with the occurrence of abruptio placentae. Also mothers with gestational hypertension or pre-eclampsia, smokers and unmarried mothers seemed to run a more than two-fold risk of premature separation of the placenta, while twin pregnancy and high parity seemed to increase the risk only slightly. However, a history of abruptio placentae revealed an 11-fold risk of premature separation of placentae in subsequent pregnancy. The factors most significantly associated with favourable prognosis of the newborn were: duration of gestation, birth weight and the degree of separation of the placenta. However, degree of cervical dilatation, presentation, mode of delivery or the time interval between diagnosis of AP and delivery seemed to have only weak discriminative power between newborns who survived and those who were lost.
Altogether 117 deliveries (0.14%) out of 82 308 were complicated by umbilical cord prolapse (UCP) in the obstetrical department of Tampere University Central Hospital in the period 1962-81. The gross perinatal mortality rate in the present study was 16.2%, but during the last 5-year period the incidence of UCP (0.09%) as well as the perinatal mortality rate (5.9%) declined significantly. UCP seemed to be more prevalent during the first half of the year. The most prominent precipitating factors for the occurrence of UCP seemed to be abnormal presentation, multiple pregnancy, low birth weight, and multiparity. Amniotomy seemed not to increase the risk for UCP. Diminished perinatal mortality figures were closely associated with increased cesarean section rates in the management of deliveries complicated by UCP. Cesarean section is therefore recommended as the treatment of choice for deliveries complicated by UCP.
Thirty-one female patients were investigated 4.8 +/- 2.5 years after pubococcygeal repair for stress urinary incontinence (SUI). Eighteen patients had a successful operation with total cure or marked improvement and 13 had a failed operation. The results of the operation were further evaluated clinically by detailed patient history and urodynamically by urethrocystometry (UCM). The severity of the SUI symptoms was evaluated by recording the physical stress causing incontinence, restrictions of daily activities and social life and use of protective pads. The symptoms were graded by the SUI score ranging from 0 to 10. The bladder pressure rise necessary for urinary leakage during coughing (The SUI threshold) was measured by UCM. The mean SUI score was 2.2 +/- 1.0 and 6.9 +/- 4.8 after successful and failed operations, respectively and the SUI threshold was 85 cm H2O and 57.5 cm H2O after successful and failed operations, respectively. The SUI threshold had a significant negative correlation with the SUI score. It is suggested that the SUI threshold is a valuable addition to UCM determining objectively the results of incontinence surgery. It should be measured each time an UCM is performed.
In order to study the association between haemostatic-fibrinolytic balance during late pregnancy and retarded foetal growth, 22 mothers with suspected intrauterine growth retardation were studied by blood haematocrit and platelet count, plasma fibrinogen and serum fibrinogen degenerating products (FDP) in late pregnancy. Fifteen mothers with undisturbed foetal growth served as controls. No statistical differences could be found between these two groups of mothers in any of the studied blood parameters. It is thus concluded that a defect in the haemostatic-fibrinolytic balance seems not to play any very important role in the etiology of intrauterine growth retardation.
