Restorative proctocolectomy: one surgeon's experience of a now routine procedure in a district general hospital.

Restorative proctocolectomy is now the operation of choice for the definitive management of ulcerative colitis and familial adenomatous polyposis coli. The aim of this review is to evaluate clinical and functional results of the first fifty patients treated with restorative proctocolectomy over a period of seven years. A total of 50 patients (28 males, 22 females) underwent restorative proctocolectomy over a period of seven years in our district general hospital. A retrospective analysis of hospital notes was undertaken and supplemented with a patient questionnaire. Forty-three patients had ulcerative colitis and seven had familial adenomatous polyposis coli. Two pouch designs were used, a four limbed "pear pouch" (94%) and a two limb "J" pouch (6%). The majority (98%) of patients had a stapled ileal pouch-anal anastomosis and a similar number had a defunctioning ileostomy. There was no mortality. Early complications (within 30 days of operation) occurred in 18(36%) patients. Late complications were seen in 22 (44%) patients. The median number of daily bowel movements was four (range 1-10). Pouchitis was seen in ten patients (20%). Restorative proctocolectomy is a safe procedure for patients with ulcerative colitis and familial adenomatous polyposis coli. Although total morbidity is appreciable, functional results are generally good and patient satisfaction is high.

Alpha spectrometry for measuring activity of glass-embedded 210PO isotope.

Measurements of exposure to radon are performed using numerous research methods which register either temporary or periodic radon concentrations. At the end of the 1990s reports on the possibility of defining past exposure to radon on the basis of measurement of the contents of 210Pb embedded in surface layer of glass were published. The registration of alpha particles emitted with glass was made using the spectrometric system. Use of the system for alpha spectroscopy leads finally to a significant lowering of the bottom threshold of detection and limits the influence of the negative background on the examined glass.

[Natural radioactivity of underground waters: surveying of alpha and beta global activity].

Two sets of measurements used in surveying of the alpha and beta global activity of drinking water drawn from 44 abyssal wells located in the Lódź cretaceous basin are discussed. The measurement method used involved the comparison between the activities of a model source and a prepared sample of water. Radioactive isotopes Am-241 were employed in alpha and K-40 in beta measurements. In order to detect radiation, a semiconducting radiation detector Si of high purity for registration of alpha molecules and a set of two G-M counters operating in the anticoincidence circuit for registration of beta radiation were used.

Satisfaction and trust in intimate relationships do lesbians and heterosexual women differ?

Although satisfaction and trust have received considerable attention in the literature on heterosexual relationships, little is known about lesbian relationships. Because females are socialized differently and acquire a different sexual script than males do, we predicted that lesbians would have similar goals regarding their relationships and, therefore, score higher on satisfaction and trust than heterosexual women. 50 lesbians and 50 heterosexual women were approached during lesbian and heterosexual social activities held at four colleges in the northeast and asked to complete a demographic survey and measures of satisfaction and trust. Findings partially supported our predictions.

Splanchnic and total body oxygen consumption in experimental fecal peritonitis in pigs: effects of dextran and iloprost.

Tissue oxygenation in the gastrointestinal tract and in the liver was studied in a porcine model where septic shock was induced by fecal peritonitis. The effects of different fluid regimes were compared. In one group (n = 8) a moderate amount of crystalloid fluids was given, in another (n = 7) crystalloids and colloids, and in a third group (n = 6) iloprost, a prostacyclin analogue, was administered intra-arterially (10 ng x kg-1 b.w. x min-1) in combination with the crystalline and colloid fluid regime. Septic shock induced by fecal peritonitis reduced cardiac index and oxygen supply to splanchnic organs. Iloprost improved the hepatic arterial blood flow, and tended to attenuate the reduction in liver oxygen delivery. Oxygen consumption (VO2) in the gastrointestinal tract and the liver was significantly increased in the group given crystalloids. These animals developed a hypovolemic/hypodynamic septic shock. Liver VO2 in these animals became flow dependent reflected by increasing hepatic venous lactate values and inversion of lactate turnover by the liver. In the two other groups gastrointestinal and liver VO2 remained constant during the observation period. Oxygen extraction over the liver increased when oxygen delivery decreased. The increased liver VO2 is suggested to be secondary to impaired microcirculation and accumulation of macrophages and leukocytes in the septic liver.

Hypoxanthine, xanthine, and urate in synovial fluid from patients with inflammatory arthritides.

As nucleotide catabolism increases during tissue injury the appearance of purine metabolites in inflamed synovial fluid might be of value in understanding the joint damage in inflammatory arthritides. In this study, therefore, synovial and plasma concentrations of hypoxanthine, xanthine, and urate in 16 patients with rheumatoid arthritis (three with psoriatic arthropathy) were analysed. It was found that their plasma concentrations of hypoxanthine were greater than those of a reference group of healthy subjects. The synovial fluid concentrations of hypoxanthine, xanthine, and urate were higher than corresponding concentrations in plasma. Positive correlations were found between the respective plasma and synovial fluid values of xanthine and urate. These findings indicate a local enhanced purine metabolism in inflamed joint tissue and diffusion of oxypurines from joint cavity to plasma. No relation was found between measured metabolites and disease duration, radiological joint findings, or synovial fluid cells. Except for a weak correlation between plasma urate and serum haptoglobin, measured purine metabolites were not related to laboratory measures of systemic inflammation.

[Contribution to surgical treatment of voice disorders].

The lack of positive health effect in the treatment of cordis vocalis insufficiency after having used the accessible methods, made the authors analyze the anatomic conditions of the organ of voice. Long-lasting observation brought to our mind an original idea the appliance of which gave us rapid and fruitful effect.
