Malignant atrophic papulosis in an infant.

We report a 7-month-old girl with malignant atrophic papulosis (Degos' disease). She also showed spontaneous aggregation of platelets. A good clinical response was obtained by treatment with aspirin and dipyridamole.

Expression of GM-CSF receptors in male germ cells and their role in signaling for increased glucose and vitamin C transport.

We studied the expression and function of the granulocyte-macrophage colony stimulating factor (GM-CSF) receptor in male germ cells. RT-PCR showed expression of mRNAs encoding the alpha- and beta-subunits of the GM-CSF receptor in human testis, and the presence of the alpha- and beta-proteins was confirmed by immunoblotting with anti-alpha and anti-beta-antibodies. Immunolocalization studies showed the level of expression of GM-CSF alpha- and beta-subunits in the germ line in the testis and in ejaculated spermatozoa. Receptor binding studies using radiolabeled GM-CSF revealed that bull spermatozoa have about 105 high-affinity sites with a K(d) of 222 pM and approximately 1100 low-affinity sites with a K(d) of 10 nM. GM-CSF signaled, in a time- and dose-dependent manner, for an increased uptake of glucose and vitamin C.

Ultrasound biomicroscopy in silicone oil-filled eyes.

PURPOSE: To establish reproducible ultrasound biomicroscopy (UBM) imaging patterns characteristic of the presence and behavior of silicone oil and of peripheral vitreoretinal proliferation in eyes that have undergone vitreoretinal surgery with silicone oil injection. METHODS: The study design was a case series. Ultrasound biomicroscopy was performed on 34 eyes of 34 patients (age range, 20-68 years). For an image to be considered a UBM pattern, it was required to be consistently reproducible in the same eye and to be observed in more than one eye. RESULTS: Four patterns are described: silicone oil particles, ghost images, "painting" or surface impregnation of iris and anterior chamber angle, and peripheral proliferative vitreoretinopathy. One type of ghost image, the secondary reflection of the cornea, paired with disappearance of the aqueous-silicone oil interface corresponded to massive silicone oil displacement into the anterior chamber. Gross proliferation produced funnel and tentlike images, whereas minute, networklike images seemed to represent early changes. There was a close correspondence between these UBM patterns and previous ophthalmologic clinical findings. CONCLUSION: Easily recognizable, distinct UBM patterns are characteristic of conditions found in silicone oil-filled eyes. Ultrasound biomicroscopy seems to be a useful tool for assessing changes in silicone oil-filled eyes.

Identification and partial characterization of excretory/secretory products with proteolytic activity in Giardia intestinalis.

This report examines the presence of proteolytic activity detected in media collected from in vitro cultures of Giardia intestinalis, and the partial characterization by gelatin-substrate polyacrylamide gel electrophoresis and inhibition studies. Gelatin-substrate polyacrylamide gel electrophoresis revealed 6 bands with proteolytic activity, with estimated molecular weights of 36, 59, 63, 72, 103, and 175 kDa. These bands were not present in the control medium. On the other hand, G. intestinalis trophozoite lysates showed proteolytic bands at 16, 20, 66, 82, 108, and 120 kDa, thus indicating that intracellular proteases could be different from the excretory/secretory (E/S) products. Based on inhibition studies, 2 bands of 59 and 63 kDa were inhibited by iodoacetic acid, indicating the presence of cysteine proteases. Partial inhibition of a band of 36 kDa was found with EDTA, a metal-chelating agent, suggesting the possible presence of metalloproteases. The presence of aspartic and serine proteases were not detected under the assay conditions used. As G. intestinalis E/S may be involved in differentiation mechanisms of the parasite and also be responsible for the mucosal alterations that occur in giardiasis, the characterization of these proteases may facilitate their evaluation as targets in the therapy of the disease.

Cholesteatoma in a child with congenital ichthyosiform erythroderma.

A 7-year-old girl had non-bullous congenital ichthyosiform erythroderma (NBCIE) in treatment with retinoids. She also suffered continuous desquamation of the external ears, recurrent external otitis, chronic adenoiditis, tubaric dysfunction and intermittent episodes of otitis media and developed a cholesteatoma in her left ear. Chronic scaling disorders, such as NBCIE and other ichthyosis, can be entities at risk for the development of cholesteatoma in childhood.

Determination of methylmercury and inorganic mercury in whole blood by headspace, cryofocusing gas chromatography and atomic absorption spectrometry.

A method for the determination of different mercury species in whole blood is described. Inorganic mercury (InHg) was determined in 2 ml of standard solutions or blood samples using head space (HS) injection coupled to atomic absorption spectrometry (AAS) after treatment with concentrated sulfuric and tin(II) chloride as a reductant agent in a closed HS vial. After stirring, the InHg was converted to elementary mercury and carried with a nitrogen flow through a quartz cell heated at 200 degrees C and the absorbance signal was evaluated by AAS. For the determination of methylmercury (MeHg), 2 ml of a standard solution or a blood sample were treated with 10 mg of iodoacetic acid and 0.4 ml of concentrated H2SO4. Then, the MeHg species were HS-injected into a gas chromatograph (GC), separated on a semicapillary column (AT-1000) with a flow of helium, then carried to the quartz cell heated at 1000 degrees C and detected by AAS. The high content of salts in blood samples, where sodium chloride is the major component (0.14 mol l-1), affected the gas-liquid distribution coefficient of both mercury species in the HS vial. A linear calibration graph was obtained in the ranges 1-20 and 1-125 micrograms Hg l-1 added as InHg and MeHg, respectively. The detection limits for InHg and MeHg were 0.6 and 0.2 microgram Hg l-1, respectively. The relative standard deviations for eleven independent measurements were 5% for both mercury species. Recovery values ranging from 98 to 106% for InHg and from 95 to 105% for MeHg and from 93 to 95% for ethylmercury (EtHg) were obtained. The accuracy of the proposed method was also established by the analysis of certified whole blood samples for InHg and MeHg. No difference between the sum of these two species determined by our procedure and the recommended total mercury concentrations in the certified samples was observed. Results for the determination of MeHg and InHg in 30 controls and 30 dentists are presented to illustrate the practical utility of the proposed method.

Informativeness of polymorphic markers for prenatal diagnosis of recessive dystrophic epidermolysis bullosa in Spanish families at risk.

DNA polymorphic markers are useful for the prenatal diagnosis of generalized recessive dystrophic epidermolysis bullosa (GRDEB) in families with at least one child affected with the disease. It is our policy to investigate families at risk using DNA polymorphic markers before a new pregnancy is intended, to inform about the real chances of offering an accurate prenatal diagnosis based on such DNA markers. We have analysed 10 Spanish families with at least one child affected with GRDEB with three different markers linked to the type VII collagen gene: the intragenic PvuII RFLP, and two markers mapped close to the COL7A1 gene, an MspI polymorphism belonging to the anonymous marker D3S2, and the microsatellite D3S1100. The PvuII marker was partially informative in two of 10 families, and was fully informative in three additional families; The MspI marker was partially informative in two of 10 families, and was fully informative in three additional families; it was not informative in five families. The D3S1100 microsatellite was partially informative in two out of 10 families, and fully informative in the other eight families. Combination of all three markers was shown to be partially informative in one family and fully informative in the remaining nine families. Using this combination of markers, we have inferred an accurate linkage-based prenatal diagnosis of GRDEB in four pregnancies. Surprisingly, the intragenic marker PvuII had a very limited usefulness, despite the results of previous studies; the microsatellite D3S1100 offered the best results for an eventual prenatal diagnosis of GRDEB. However, families at risk should be informed that extragenic markers may fail due to the possibility of recombination, estimated as 5% for D3S1100.

[Pseudotumor forms of demyelinating diseases. Report of three cases and review of the literature].

INTRODUCTION AND CLINICAL CASES: We present three patients with unusual clinical findings studied in our hospital, after a period of follow-up of at least two years from the time of diagnosis of their demyelinating condition. We discuss the clinical onset, CT and magnetic resonance findings, neuropathological studies and posterior clinical course. Anatomopathological studies were done in two of the cases, since the diagnoses were not clear and the other paraclinical investigations did not clarify things. DISCUSSION: The presence of large space-occupying lesions or ring-like outlines in myelinating disorders may make it difficult to make a differential diagnosis from other conditions such as neoplasias and abscesses. This may lead to an erroneous initial diagnosis and even to the use of unsuitable, aggressive treatments. CONCLUSIONS: In young patients in whom radiological imaging suggests space-occupying lesions (single or multiple) one should consider the possibility of a primary demyelinating disease of the central nervous system with the appearance of a pseudo-tumour. In certain cases, stereotaxic biopsy should be considered if the diagnosis remains in doubt, rather than begin unsuitable treatment. The fundamental reason for the presentation of our cases is to emphasize that these pseudo-tumoral forms of demyelinating diseases should be considered in diagnosis.

Absolute linkage of celiac disease and dermatitis herpetiformis to HLA-DQ.

This report shows the absolute genetic linkage of celiac disease (CD) to the HLA-DQ region, and supports the fact that dermatitis herpetiformis (DH) follows the same pattern of HLA-mediated susceptibility in extensive series of Caucasian Spanish patients. Ninety-five percent of CD (201 of 212) and 100% of DH (55) patients could produce DQ alpha 1*0501-DQ beta 1*02 heterodimers. Negative CD patients for this combination were mostly DR4-DQ8 (DQA1*03-DQB1*0302) (9 OF 11), along with a restricted number of complementary chromosomes. Comparison of observed and expected DQA1-DQB1 genotype distributions (Hardy-Weinberg equilibrium) showed that the excess of patients with DQB1*02 in double doses would be the consequence for which this allele should be complemented by DQA1*0501. Homozygosity for DQA1*0501 would restrain susceptibility to CD and DH.

Costello syndrome.

Costello syndrome is an uncommon multisystemic condition with highly characteristic cutaneous manifestations. We describe here the case of a patient with Costello syndrome, and review previous reports of this entity.

Caseating cutaneous granulomas in a child with common variable immunodeficiency.

Cutaneous, noninfectious, granulomatous lesions have been reported occasionally in different types of immunodeficiencies, including common variable immunodeficiency (CVD). We present a child with CVID and cutaneous granulomas with a strikingly prominent caseating necrosis. We think that such granulomatous lesions constitute a distinctive manifestation of immunodeficiency, and may reflect a altered immune response. Corticosteroids have been of benefit to our patient, as well as in similar cases.

Stiff skin syndrome: a case report and review of the literature.

Stiff skin syndrome (SSS) is a rare disorder characterized by stony-hard skin, limited joint mobility and mild hypertrichosis. We present a severe case in a 4-year-old boy. In addition to the clinical and histological features of SSS, our patient also had muscle and bone involvement along with pyloric stenosis, gastro-esophageal reflux, inguinal hernias and atopic dermatitis. We highlight the complexity of this case and the diagnostic approach to patients with this disorder. We also review and summarize the 24 cases previously reported in the literature.

Keratosis lichenoides chronica in a child.

A young boy developed childhood keratosis lichenoides chronica. The eruption cleared spontaneously after 13 years. This is a rare condition, regarded by some as a variant of lichen planus. The great preponderance of cases occur in adults, whereas the disease in childhood is uncommon.
