Successful bronchial revascularization in experimental single lung transplantation.

It is the purpose of this paper to report our experience with bronchial artery revascularization in an experimental model of single lung transplantation in swine. Thirty-three large white pigs weighing 20-40 kg underwent left lung allotransplantation. In 24 animals, bronchial artery revascularization was attempted by anastomizing the aortic patch containing the bronchial artery orifice with the recipient descending aorta. Eight survivors were put to death on postoperative days 11-15; five animals were put to death or died on postoperative days 2-9; the other animals died intra-operatively or within a few hours. Preservation of left bronchial vascularization was achieved in all cases attempted, as documented by post-mortem injection of dye (methylene blue) or contrast medium. Five of the 8 animals surviving for 11-15 days showed diffuse graft hepatization, associated with diffuse vascular thrombosis. Whether this was caused by damage to the endothelium due to poor graft preservation or by rejection was unclear. In animals surviving for 11-15 days without gross lung pathology, the anastomosis and bronchial mucosa were completely normal; in contrast, bronchial ischaemic changes were found in nonrevascularized animals and in survivors with graft hepatization. Our experience confirms that re-anastomosis of the bronchial arteries can prevent bronchial healing problems in single lung transplantation. The pig is an ideal model for these experiments since the bronchial arteries have a constant common aortic origin, allowing easy identification and preservation of left bronchial vascularization.

[Experimental use of intraoperative pulse wave Doppler in the study of vascular microanastomosis].

Personal experience with pulse wave Doppler (PWD) at 20 MHz with real time signal spectral analysis in FFT (Fast Fourier Transform), for the valuation of the vascular microanastomosis patency degree is reported. Fifty-nine Sprague-Dawley rats have been studied after abdominal aorta dissection: in 19 (group A) an organic stenosis has been simulated varying from 5% to 95% by a 8/0 silk suture; in 40 rats (group B and C) we the abdominal aorta was stitched with interrupted suture after transversal section. In the group C (20 rats) a longitudinal arteriotomy on abdominal aorta within 10-15 min from anastomosis was performed to value under microscopic control the suture line. The PWD technique is able to recognize stenoses more than 10% of intraluminal diameter, showing a higher sensibility than traditional techniques.

[Experimental study of colonic anastomoses. Comparison of different suture methods in the rat].

A comparative experimental study was conducted on three different suture techniques for end-to-end colonic anastomoses. The development of stenosis, perivisceral adhesions and resistance to endoluminal pressure were assessed. The results show that the one layer all-coat interrupted stitch anastomosis displays less resistance to endoluminal pressure during the first seven days but is followed by fewer complications.

Temporary intraluminal bypass for superior vena cava reconstruction after cancer invasion.

We report a case of right upper lobe bronchogenic cancer widely infiltrating the superior vena cava (SVC) in which right pneumonectomy was performed with partial resection of the SVC wall. The SVC was reconstructed by means of a pericardial patch; during reconstruction a temporary intraluminal bypass was set up to obtain a proper venous return to the right atrium.

Selective bronchial intubation for one lung anaesthesia in thoracic surgery. A new method.

A new method for one lung anaesthesia in thoracic surgery is described. Separate lung ventilation is obtained with selective main bronchus intubation, by means of an appropriate cuffed tube inserted through a standard orotracheal tube. Ventilation is carried out separately through the bronchial tube on one side and the residual tracheal tube lumen on the other side. This method greatly simplifies the technique of bronchial intubation and offers many advantages over commercially available double-lumen tubes.

Silicone rubber membrane for extraction of high vapor tension toxic substances. Experiences with ammonia and ethanol.

The purpose of this study is to evaluate the permeability of a silicone rubber membrane to two toxic substances, ammonia and ethanol, under high vapor tension. A device was assembled for testing the diffusion of the substances through a silicone rubber membrane interposed between two aqueous phases. 9 experiences were carried out with ammonia at different temperatures: 3 at 11 degrees C, 3 at 24 degrees C and 3 at 37 degrees C. 8 experiences were carried out with ethanol at different temperatures: 3 at 11 degrees C, 3 at 24 degrees C and 2 at 37 degrees C. Mean transmembrane flow (mumol/m2 X h) for ammonia was 43.99 at 11 degrees C, 59.18 at 24 degrees C and 145.45 at 37 degrees C. Mean transmembrane flow (mmol/m2 X h) for ethanol was 6.48 at 11 degrees C, 12.54 at 24 degrees C and 22.51 at 37 degrees C. The results confirm that ammonia and ethanol in water solution can pass through a silicone rubber membrane. Better results could be expected with both aqueous phases in motion as in dialysis devices.

[Analytical expression of acid-base equilibrium in human blood. I. Editing of a subroutine program].

A subroutine for evaluating the acid-base status in human blood is described. It employs as inputs seven parameters: pH-pO2-pCO2-Hb-Ht-the subject's temperature T and the plasmaprotein content PP and is based on a series of mathematical interrelationships existing between these various blood constituents. As output it returns the base excess (BE) of the whole blood and a number of pertinent data relating to the acid-base status and to the gas transport mechanisms in erythrocytes and plasma. An important advantage of this subroutine on other major procedures is that it can be accommodated into the memory space of a medium seized electronic calculator.

[Analytical expression of acid-base equilibrium in human blood. II. Verified application of a subroutine program].

A program listing of a subroutine for conversion of seven human physiological parameters: pH-pO2-pCO2-Hb-Ht-the subject's temperature T and the plasmaprotein content PP into whole blood base excess is detailed for use with a medium seized electronic calculator coupled with a data printer. The operating instructions are also given. The subroutine is self-contained and can easily accept a correction of the empirical factors which are in animals different from human. Its use was tested in a series of 36 clinical samples of blood and the results were compared wih those obtained on the same samples with three different nomograms. No statistically significant difference was found between the four methods.

[Possible causal factors in postoperative immunodepression].

The relationships between postoperative immunodepression and duration of general anesthesia, volume of blood transfusions, cortico-steroid administrations and amount of surgical trauma have ben studied, in cancer patients undergoing radical operations. Postoperative depression of cellular immunity, assayed by the lymphocyte blastogenic response to phytohemagglutinin, was significantly correlated with the amount of surgical trauma and did not correlate with blood transfusions and corticosteroid administrations, nor with the duration of anesthesia.

[Sequential evaluation of lymphocytic blastogenesis in cancer patients after surgical treatment].

Sequential evaluation of lymphocyte blastogenic response (LBR) to PHA was performed in 10 melanoma patients and in 10 gastric cancer patients undergoing radical operations. Preoperative determinations showed a significant depression of LBR in both patient groups as compared to healthy controls. In patients operated for melanoma the average duration of anesthesia was 101 minutes and in patients who underwent gastric resection it was 192 minutes. In both patient groups a further significant depression of LBR was observed in the early postoperative period; however the LBR returned to preoperative levels more promptly in patients who underwent melanoma excision than in those who underwent gastric resection.

[Simple ulcer of the colon. Considerations on a case].

A case of "simple ulcer" of the colon is reported. The lesion is uncommon and not well known. As far as pathogenesis is concerned, the Authors emphasize the role of peptic, neurogenic, circulatory, bacterial, toxic and mechanical factors. The following clinical picture was observed: sudden and acute abdominat pain due to colonic perforation with pneumoperitoneum. As already described by literature preoperative correct diagnosis was not possible; this is usually due to the fact that the ulcer tends to perforate before other symptoms arise. During surgery suture of the ulcer, temporary colostomy and Mikulicz's drainage were performed; after 4 months colostomy was closed.

Postoperative immunologic monitoring of cancer patients undergoing radical surgery.

Immunologic parameters of cellular immunity in vitro and circulating immunoglobulin levels were evaluated before and after radical surgery in twenty-five patients with localized primary solid tumors. Peripheral blood lymphocyte counts, T-lymphocyte counts and lymphocyte blastogenesis after optimal stimulation with phytohemagglutinin were found significantly depressed at the time of diagnosis. Sequential determinations of immunologic parameters during the postoperative period showed a slight fall of lymphocyte counts and T-lymphocyte counts; subsequently these cell counts progressively returned to preoperative values. The blastogenic response of lymphocytes sharply decreased following surgery and then slowly returned to preoperative levels in one month. No significant alterations of circulating immunoglobulins were detected at diagnosis or during the postoperative period. These findings indicate that patients with localized tumors present impairment of some parameters of cellular immunity preoperatively and that postoperative immunodepression lasts about one month.

Histopathological studies on dog lymphoid structures during malnutrition and total parenteral nutrition.

Lymphoid tissues of six malnourished dogs were histologically evaluated and compared to the same lymphoid structures of dogs nutritionally repleted by means of total parenteral nutrition (TPN). The results show that thymus, spleen, lymph nodes and Peyer's patches are markedly atrophic and severely cell depleted in malnourished dogs, and provide evidence that subsequent TPN therapy stimulates and supports lymphoid cell prolipheration, leading to cell repopulation of previously depleted lymphoid structures.
