Saline infusion sonohysterography in endometrial cancer: assessment of malignant cells dissemination risk.

BACKGROUND: To assess the risk of malignant cells dissemination in patients with endometrial cancer undergoing saline infusion sonohysterography (SIS). METHODS: Fourteen consecutive patients (mean age: 58.1 years, range: 41-74 years) diagnosed as having a clinical stage I endometrial carcinoma and scheduled for surgical staging were entered in this study. SIS was performed at the time of laparotomy just when the abdomen was opened and prior to the start of the surgical procedure. 10-20 mL of saline was infused. All fluid spilled from the fimbrial ends of the fallopian tubes was collected and analyzed for the presence or absence of malignant endometrial cells. Samplings were also taken from ascites, if found, or cytological washings, prior to and after SIS. RESULTS: SIS could be satisfactorily performed in all patients. There was no spillage from either tube in nine patients. Mean fluid volume obtained from the other five patients was 4.4 mL. Cytologic analysis revealed the presence of malignant cells in the spilled fluid in one case (7.1%, one out of fourteen). CONCLUSIONS: Our data show that a small but real risk of malignant cell dissemination exists in patients with endometrial carcinoma who undergo SIS.

Evidence for polymorphism in glisentide.

The polymorphism of glisentide has been investigated. Three polymorphs (I, II, III) have been prepared by recrystallization from different solvents and other polymorphic form (IV) was obtained by heating polymorph III at 100 degrees C. In addition, two 1:1 stoichiometric solvates containing carbon tetrachloride and dioxane have been crystallized and finally, an amorphous solid has been obtained. It has been observed that the polarity of the recrystallisation solvent and its ability to form hydrogen bonds have a great influence on the polymorphism of glisentide. The different solid forms of glisentide have been characterized using X-ray diffraction analysis, differential scanning calorimetry (DSC), thermogravimetric analysis (TGA), IR spectroscopy and optical microscopy. The recrystallization of polymorph I in melted form II and also the transition of form III-IV have been detected by DSC and X-ray diffraction analysis.

Transvaginal color Doppler ultrasonography and CA-125 in suspicious adnexal masses.

OBJECTIVE: To compare diagnostic performance of color Doppler ultrasound and CA-125 in suspicious adnexal masses on B-mode sonography. MATERIALS AND METHODS: Data on 94 patients (mean age: 47.4 years, range: 17-79 years. Fifty-two (55.3%) premenopausal and 42 (44.7%) postmenopausal women) managed in our institution because of a suspicious adnexal mass were reviewed. All patients were evaluated by transvaginal color Doppler ultrasonography (CD) and serum CA-125 level determination prior to surgery. Definitive histopathological diagnosis was obtained in each case. Sonographic morphology evaluation was suspicious in all cases. CD was considered as suspicious when flow was detected and the lowest RI found was < or = 0.45. CA-125 cut-off was > or = 35 UI/ml. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated for each method and compared. ROC analysis was performed for RI and CA-125. Areas under curve (AUC) were calculated and compared. RESULTS: Fifty-six (59.6%) tumors were found to be malignant and 38 (40.4%) benign. Sensitivity, specificity, PPV and NPV for CD were 87.5% (95% CIs: 75.3-94.4), 84.2% (95% CIs: 68.7-94), 89.1% (95% CIs: 77.7-95.9) and 82.1% (95% CIs: 66.5-92.5), respectively. Sensitivity, specificity, PPV and NPV for CA-125 were 83.9% (95% CIs: 71.7-92.4), 68.4% (95% CIs: 51.3-82.5), 79.7% (95% CIs: 66.2-89) and 74.3% (95% CIs: 56.7-87.5), respectively. Sensitivity, PPV and NPV were not statistically different. CD had higher specificity (P = 0.01). AUC curve for Doppler (0.75) was significantly higher than for CA-125 (0.61) (P = 0.0002). CONCLUSIONS: Our results indicate that color Doppler ultrasound has a better diagnostic performance as compared with CA-125, being significantly more specific.

Influence of polyethylene glycol 4000 on the polymorphic forms of diflunisal.

The present study was carried out to investigate the physico-chemical characteristics of diflunisal-PEG 4000 solid dispersions prepared by melting, solvent and melting-solvent methods. The solvents chosen were chloroform, methanol and ethanol-water due to the fact that the drug presents different polymorphic forms in these solvents. The characterization of solid dispersions was performed by X-ray powder diffraction because this technique has the advantage over other identification methods that it can detect both drug and ligand simultaneously. The X-ray diffraction patterns of the diflunisal-PEG systems suggested that the drug/polymer ratio and the solvent nature play an important role in the crystallization of the drug. In this regard, diflunisal crystallizes in form I at high concentrations of the drug (drug/polymer 2:1) in the solidified melt dispersions, however, polymorph III is mainly obtained as the polymer content increases (1:1 and 2:3). Likewise, in solid systems obtained by the solvent and melting solvent methods the drug solidifies in form III in ethanol/water and methanol while polymorph IV crystallizes in chloroform. Finally, DSC thermograms and hot-stage microscopy data of solid dispersions prepared by the melting method have allowed to draw the diflunisal-PEG 4000 solid-liquid phase diagram.

[Breast cancer: value of various examinations in the diagnosis of the primary lesion].

Breast physical examination which is enormous help in clinical diagnosis of cancer of the breast is not useful in the diagnosis of early lesions. We analized in this study different complementary examinations we do for the diagnosis of the cancer of the breast. Mammography, though maintaining a 5-7% false negative results is the examination of choice of the breast. Not with standing its simplicity, its contribution with respect to breast and neoplastic biology makes it a useful investigation. The diagnosis obtained through mammography in non palpable tumors permit the use of conservative therapeutic techniques at the same time allowing improvement in curation and survival rates when we deal preferently with Stage I cancer. It is still necessary in our means an information-sensibilization of the female population about breast autoexamination and the need for early specialized medical consultation. Any abnormal breast sign should alert us that more than 2.5 months delay would suppose a difference from a T1 to a T2 tumor.

[Follow-up of breast cancer: review of 750 cases after 5 years].

Recurrence of breast cancer can present in very small tumors even 20 years after initial treatment. Periodic revision of all the operated population during a long time will be necessary to detect all the recurrences. For this the cost-benefit relation of follow-up in breast cancer is a controversial topic. We present our results in 750 cases of breast cancer operated from 1980 and submitted to a follow-up protocol during five years. We analyzed the value of follow-up for the discovery of metastases, local recurrence after conservative treatment and after mastectomy. Finally we considered the usefulness of early diagnosis of familial breast cancer and cancer in the contralateral breast. Metastases was discovered in asymptomatic patients in 68%, which could improve the survival. Recurrence after mastectomy was seen in 1.3% of the patient and a half of these after treatment presented survival superior to 3 years. Follow-up favours early diagnosis and could have influence on survival.

[Surgical options in the treatment of polycystic ovary syndrome].

Since it was defined, SOP has been related to surgery. Before clomifero was introduced, ovarian wedge resection was the only way to stimulate ovulation and gestation in women with this syndrome. Surgical techniques were derelict in favour of less aggressive medical treatments. However, surgery has reduced its side effects due to the incorporation of laparoscopic techniques and other advances. Because of this fact, surgery is being reintroduced in the treatment of polycystic ovarian disease.

[Imaging techniques for axillary lymph node staging in breast cancer].

The axillary lymph node status is the most important prognostic factor in breast cancer, and the axillary dissection as the gold standar for staging. It requires radical surgery, which is accompanied by importants postoperaive problems. Axillary lymph nodes can be imaged with a wide variety of available diagnostic radiological test (ultrasonography, mammography, computed tomography and magnetic resonance imaging). In these anatomic imaging, the limph nodes whit metastatic disease appear dense, enlarged or spiculated. Difficulties arise, not in visualization of the axillary lymph nodes, but in reliably separating normal from those involved with metastatic disease. Radionucleide studies and positron emisión tomography provide biochemical information, but are limited by resolution constrains.
