[A case report of testicular leiomyosarcoma].

This report describes a case of primary leiomyosarcoma of the testis in a 70-year-old man. Treatment consisted in orchidectomy by the inguinal route with no adjuvant treatment. The patient developed pulmonary metastasis 14 months after surgical treatment. Few reports have been published on primary leiomyosarcoma of the testis. Most reported cases have been in small series in association with germ cell tumors. Surgical treatment is the most effective therapeutic modality. The standard surgical technique is orchidectomy by the inguinal route. Retroperitoneal lymph node dissection is not always performed. The prognosis of isolated testicular sarcoma is better than that of sarcoma associated with germ cell components.

[Invasive thymoma extending into the superior vena cava and the right atrium: a case report and review of the literature].

If the neoplastic extension with the higher vena cava can be observed during the evolution of the invasive thymomes, the tumoral extension in the right auricle, remains on the other hand an exception. We report an invasive case of thymome to higher intracellar extension, right intraatrial and pleuropulmonary revealed by a syndrome undermines higher and a left pleurisy. The transthoracic echocardiography supplemented by the echocardiography transoesophageal pose the cardiac diagnosis of tumor, and it is the thoracic tomodensitometry which highlights a tumoral process mediastinal of malignant pace invading the left inominal venous trunk, the higher vena cava and extending in the right auricle. The diagnosis of certainty is carried by the endobronchial biopsy and the puncture tomodensitometric biopsy under control of the mediastinal mass after anatomopathological examination. Under chemotherapy, the evolution over 18 months is marked by the absence of cardiovascular complications in spite of the non regression of the tumoral mass. This observation stresses the importance of the realization of the echocardiography especially transoesophageal in front of all invasive thymome and the reliability of the puncture biopsy scanoguided like diagnoses technique not very invasive in the forms not extirpables. The forecast of this affection depends on the effectiveness of the processing. The surgery when it is possible, remains the principal therapeutic measurement which really proved reliable.

[Spontaneous perirenal hematomas. Report of 3 cases].

Spontaneous retroperitoneal hemorrhage is an uncommon affection, the diagnosis was recognized by sonography and CT scan, but the etiology remains unknown and exploration for diagnosis may become necessary. In the absence of an apparent etiology, patients with spontaneous renal bleeding should undergo radical nephrectomy, because of the extremely high incidence of small undetectable occult tumors. Three further cases were reported by the authors, who made a review of the literature.

[Aglomerular segmental hypoplasia of the kidney: report of a clinical case].

Aglomerular segmental hypoplasia of the kidney is a disease that is uncommon in the adult; the average age at appearance of symptoms is between ten and 15 years. A clear female predominance has been noted, and this disease is often misdiagnosed as chronic pyelonephritis. However, the radiological and pathological characteristics are different to the latter. The etiology of aglomerular segmental hypoplasia has not yet been determined. It is important to acquire further knowledge regarding this disease, as in certain cases this information could help to avoid unnecessary nephrectomy.

[Nodular fasciitis or pseudosarcoma].

Nodular fasciitis is a benign neoplastic and reactive proliferation of fibroblasts of soft tissues, which is often mistaken for a sarcoma because of its rapid growth, rich cellularity and mitotic activity. A case is reported that provides the opportunity to discuss the specific clinical and pathological features of nodular fasciitis.

[Undifferentiated sarcoma of the prostate: report of a case].

The authors report one case of indifferentiated sarcoma of the prostate revealed by phlebitis of the right inferior limb and lung's metastases in a man 41 years old. The tumor progress rapidly and infiltrate the bladder and the posterior urethra. The patient died five months later. They review the literature and study the clinical histological, therapeutic and evolutive aspects of this unusual tumor.

[Squamous cell carcinoma in an augmentation of the ilial bladder for tuberculosis].

The development of cancer on the ileal graft after augmentation ileocystoplasty benign bladder disease is a little known complication. The authors report a case of squamous cell carcinoma in the ileal bladder occurring 31 years after augmentation ileocystoplasty for tuberculous bladder, in a 60-year-old patient. The ileal bladder was resected and a new augmentation ileocystoplasty was performed. The postoperative course was uneventful. Histological examination of the operative specimen showed infiltration of all of the intestinal wall. The patient died one year after, in a context of peritoneal carcinomatosis.

[Granulomatous orchitis. A case report and review of the literature].

Idiopathic granulomatous orchitis is a rare testicle lesion of unknown etiology. We report one case of a specific granulomatous orchitis. Diagnosis and treatment problems are discussed. Clinically, it can be misdiagnosed as a malignant tumour or tuberculosis. The diagnosis is usually made on histological examination after orchiectomy. The aetiology of this disease is still debated. It is supposed to be associated with sperm extravasation causing self-immune reaction.

[Leiomyosarcoma of the vagina: a rare case].

Vaginal leiomyosarcoma is unfrequent. We report on a case in a 50-year-old multipara patient who had presented a posterior vaginal swelling since 6 months. The tumor was discovered at the occasion of pains and non hemorragic discharge. The histological pattern of the tumor was well-differentiated spindle cell sarcoma with pleiomorphic areas. The immunohistochemistry confirmed the smooth myogenic differentiation. The treatment consisted of posterior pelvic exenteration extended to the vagina. The patient is alive and free of disease at 20 months of fellow-up.

[Iatrogenic cervicofacial hydatid cysts. Apropos of a case].

The writers give an account of a cervicofacial hydatid dissemination case, provoked by iterative ponctions of hydatid cyst initially isolated in the left maxillary sinus. They state diagnostic and therapeutic difficulties sometimes found and they insist on the role that must be played by immunologic tests in diagnostic approach and treatment monitoring of this disease which is unfortunately current and serious.

[Spermatocytic seminoma. Apropos of a case and review of the literature].

We report a case of sperm cell seminoma caused by trauma. The data in the literature indicate the frequency is less than 5% of all seminomas. This case was exclusively located in the gonads and was a pure form. Orchidectomy with high ligature of the cord and adjuvant radiotherapy at the dose of 25 Gy centered on the para-aortic and subdiaphragmatic chains is adequate treatment. When inguinoscrotal surgery is performed, this zone must be irradiated with 25 Gy. Prognosis is satisfactory: 5-year survival is 100% with this protocol.

[Paratesticular rhabdomyosarcomas. Apropos of a case].

A case of paratesticular rhabdomyosarcoma in a 23 year-old patient is reported. Clinical signs are like any intrascrotal tumor. Scrotal ultrasound, CT scan and immunohistochemical markers are necessary for the diagnosis and stadification of this cancer. Orchidectomy followed by radiotherapy and/or chemotherapy is the usual treatment. Prognosis depends on the tumoral stage.

[Leiomyosarcoma of the bladder. Apropos of a case].

Bladder leiomyosarcoma is a rare neoplasm. Early diagnosis, cystectomy, radiation therapy and chemotherapy seem to improve prognosis. The authors report one case of a leiomyosarcoma of the bladder in a seventy year old woman. There has been no evidence of recurrence after partial cystectomy, with more than a two-year follow up. Clinical, histological, therapeutic and prognostic aspects of this unusual tumor are discussed.

[Small cell carcinoma of the prostate. Apropos of a case].

Small cell carcinomas of the prostate are rare. These tumors exhibit morphologic and functional neuro-endocrine characteristics. They are a highly aggressive. New therapeutic protocols are being developed in order to be effective against these tumors, particularly those unresponsive to hormonotherapy.
