PET/CT using 2-deoxy-2-[18F]fluoro-D-glucose for the evaluation of suspected infected vascular graft.

An infected vascular graft was identified using a combined positron emission tomography (PET) and computerized tomography (CT) system. The fusion of 2-deoxy-2-[18F]fluoro-D-glucose (FDG) PET and CT images, acquired in a single session, allowed for the precise localization of the abnormal FDG uptake to the vascular graft and led to the correct diagnosis of prosthetic infection. This hybrid modality, which provides precise registration of metabolic and structural imaging data, may enhance the potential use of FDG in the diagnosis and management of infected vascular grafts.

Anomalous origin of right coronary artery: diagnosis and dynamic evaluation with multidetector computed tomography.

16 slice multidetector CT findings of an anomalous right coronary artery originating from the left sinus of Valsalva are presented. Multidetector CT depicted the malignant coronary anomaly in great anatomic detail as well as enabled dynamic evaluation through the cardiac cycle, documenting a substantial reduction in arterial diameter during peak systole. This case illustrates the full capabilities of multidetector cardiac CT in the evaluation of coronary artery pathology.

A new pitfall on abdominal PET/CT: a retained surgical sponge.

Positron emission tomography (PET)/computed tomography (CT) and multidetector CT findings of an abdominal retained surgical sponge (RSS) are presented. A PET/CT study performed for evaluation of a suspected abdominal tumor demonstrated the inconclusive finding of a hypometabolic area surrounded by increased 2-[fluorine-18] fluoro-2-deoxy-D-glucose uptake. Follow-up contrast-enhanced multidetector CT suggested the correct diagnosis of an RSS. This is the first known report of the PET/CT appearance of an RSS.

Anomalous origin of a posterior descending artery from the right pulmonary artery: report of a rare case diagnosed by multidetector computed tomography angiography.

The case of a mildly symptomatic 43-year-old man with evidence of reversible inferior wall ischemia by radioisotope stress testing is described. Invasive catheterization showed abnormal connections between the coronary arteries and the pulmonary circulation. Multidetector computed tomography elucidated the previously undescribed finding of a posterior descending artery originating from the right pulmonary artery.

Pitfalls of the lactating breast on computed tomography.

Breasts are usually only incidentally included in computed tomography (CT) scans of the abdomen and chest in spite of the fact that pathologic findings are not rare. Because postpartum women are normally healthy, they rarely require examination; therefore, the presentation of lactating breasts on a CT scan is poorly known. The most common postpartum breast feature is engorgement, which is characterized by painful swelling associated with a sudden increase in milk volume, lymphatic and vascular congestion, and interstitial edema. Its appearance on a CT scan does not seem to have been described in the literature to date. The CT picture of a lactating breast shows the enlarged gland with bilateral cord- and mass-like hyperattenuated tissue. After abrupt cessation of breast-feeding, the edema component supervenes and breast tissue may become hypoattenuated. In such cases, further ultrasound evaluation is needed. We present 3 cases of postpartum breast engorgement as seen on CT.

Septal bounce in constrictive pericarditis. Diagnosis and dynamic evaluation with multidetector CT.

Sixteen-slice multidetector CT findings of a case of constrictive pericarditis in a pediatric patient are presented. Multidetector CT depicted a variety of diagnostic findings including dynamic evaluation of interventricular septal motion through the cardiac cycle, documenting a diastolic septal bounce. This case illustrates the full capabilities of multidetector cardiac CT in the evaluation of pericardial pathology.

Renal cell carcinoma invading the right ovarian vein: multidetector computed tomography and ultrasound Doppler findings.

Multidetector computed tomography (MDCT) and Doppler ultrasound findings of a renal cell carcinoma invading the right ovarian vein are presented. An MDCT study performed for evaluation of macroscopic hematuria showed a heterogeneously enhancing malignant thrombus in the right ovarian vein that was in continuity with inferior vena cava and right renal vein thrombi of identical characteristics. Further investigation with Doppler ultrasound confirmed these findings and showed arterial blood flow of low resistance within the ovarian vein and inferior vena cava thrombi. To the authors' knowledge, this is the first report of MDCT and Doppler ultrasound findings of a renal cell carcinoma invading the right ovarian vein.

Comprehensive multidetector computed tomography assessment of severe cardiac contusion in a pediatric patient: correlation with echocardiography.

Multidetector computed tomography (MDCT) cardiac findings in an unconscious teenager after blunt chest trauma are presented. Multidetector computed tomography enabled accurate comprehensive evaluation of the coronary arteries, myocardial perfusion, and left ventricular function. This case illustrates the full capabilities of MDCT in the evaluation of cardiac contusion in a noncooperative pediatric patient.

Electromagnetic navigation during flexible bronchoscopy.

BACKGROUND: Flexible bronchoscopy is routinely utilized in the diagnosis and treatment of various lung diseases. Nondiagnostic bronchoscopy leads to more invasive interventions, such as transthoracic needle aspiration, mediastinoscopy or even thoracotomy. Electromagnetic navigation is a novel technology that facilitates approaching peripheral lung lesions, which are difficult to sample by conventional means. The navigation system involves creating an electromagnetic field around the chest and localizing an endoscopic tool using a microsensor overlaid upon previously acquired CT images. OBJECTIVES: To determine the practicality, accuracy and safety of real-time electromagnetic navigation, coupled with previously acquired 3D CT images, in locating artificially created peripheral lung lesions in a swine model. METHODS: Peripheral lung lesions were created in four swine models by insertion of a metal tube (1 x 10 mm) via a transthoracic approach. An electromagnetic field was created by placing the animal on an electromagnetic location board. A position sensor incorporated into the distal tip of a dedicated tool was used to navigate to the various target lesions. Information gathered in real time during bronchoscopy was presented on a monitor simultaneously by displaying previously acquired CT images. Upon reaching the target lesion, biopsies were performed and the functionality and safety of the superDimension/Bronchus System was observed and documented. RESULTS: The registration accuracy expressed by the fiducial target registration error, expressing both the registration quality and the stability of fiducial (registration) points, was 4.5 mm on average. No adverse effects, such as pneumothorax or internal bleeding, were encountered in any of the animals in this study. CONCLUSIONS: Real-time electromagnetic positioning technology coupled with previously acquired CT images is an accurate technology added to standard bronchoscopy to assist in reaching peripheral lung lesions and performing biopsies.
