The biomechanics of point contact-dynamic compression plate and its effects on bone perfusion.

OBJECTIVE: To compare the mechanical properties of point contact-dynamic compression plate (PC-DCP) and its effects on cortical bone perfusion with that of dynamic compression plates (DCP) in goat tibiae. METHODS: Twenty pairs of matched fresh goat tibiae were used. A transverse fracture model was established. The fractures with a 3mm interspace between the fracture ends were subject to fixations with the DCPs and the PC-DCPs respectively, then the four-points bending tests and the torsion tests were conducted to compare the mechanical properties of the PC-DCP with that of DCP. Another 13 sexually mature goats underwent fixations with the DCPs and the PC-DCPs, respectively, at the mid-shafts of the intact bilateral tibiae. Ischemic zones were observed at four time points (1 day, 2, 6, and 12 weeks after operation) using disulphine blue staining technique. RESULTS: There were no significant differences in mechanical properties, such as bend- and torsion-resistance, between the DCPs and the PC-DCPs. One day, 2, and 6 weeks after operation, on the side of DCP fixation, outer cortical bone ischemia under the plate persisted, and this condition did not reverse until 12 weeks after operation. However, on the side of PC-DCP fixation, cortical bone ischemia occurred only in the periphery of the screw holes and at the contact sites of the PC NUTs 1 day after operation, and it disappeared at 2 weeks after operation. CONCLUSIONS: The PC-DCP has similar biomechanical properties of the DCP, but is less detrimental to local bone blood circulation than the conventional plates.

[Study on quality for Herba Polygoni Orientalis].

OBJECTIVE: To establish the qualitative and quantitative detective methods of Herba Polygoni Orientalis. METHOD: Isorientin, orientin, protocatechuic acid and gallic acid in Herba Polygoni Orientalis were identified by TLC. The contents of isorientin and orientin in Herba Polygoni Orientalis were determined by HPLC. RESULT: Isorientin, orientin, protocatechuic acid and gallic acid could be identified by TLC. Isorientin and orientin were well separated with Diamonsil C18 column and acetonitrile-0.1% phosphoric acid (18:82) as mobile phase. The linear range of isorientin was 0.075-0.90 microg. The average recovery of isorientin was 98.8% and RSD was 2.1%. The linear range of orientin was 0.041-0.49 microg. The average recovery of orientin was 98.8% and RSD was 2.1%. CONCLUSION: The methods can be used for qualitative identification and quantitation determination of Herba Polygoni Orientalis.

Primary total hip arthroplasty for acetabular fracture.

OBJECTIVE: To explore the operative indications and operative methods of primary total hip arthroplasty for acetabular fracture and to observe the clinical curative effect. METHODS: We retrospectively summarized and analyzed the traumatic conditions, fracture types, complications, operative time, operative techniques, and short term curative effect of 11 patients (10 males and 1 female, with a mean age of 42.4 years) with acetabular fracture who underwent primary total hip arthroplasty. RESULTS: The patients were followed up for 6-45 months (mean=28 months). Their average Harris score of postoperative hip joint was 78. CONCLUSION: Under strict mastery of indications, patients with acetabular fracture may undergo primary total hip arthroplasty, but stable acetabular components should be made.

[Determination of gallic acid in Melastoma dodecandrum by RP-HPLC].

OBJECTIVE: To establish a RP-HPLC method for the determination of gallic acid in Chinese herb Melastoma dodecandrum, and to evaluate the quality of the herb. METHOD: The RP-HPLC analysis was achieved by using a Polaris C18 column and tetrahydrofuran-methanol-0.2% phosphoric acid (0.5:0.5:99) as the mobile phase, with a flow rate of 1.0 mL x min(-1), and detected by UV at 274 nm. RESULT: The content of gallic acid was from 0.020% to 0.081%, in ten groups of M. dodecandrum collected from different locations. CONCLUSION: The method is a simple, convenient and rapid. It can be used for quality evaluation of M. dodecandrum.

[Remote fetal monitoring for gestational diabetes mellitus].

OBJECTIVE: To investigate the clinical value of remote fetal monitoring network in the antepartum management of gestational diabetes mellitus (GDM). METHODS: Non-stress test (NST) was preformed in 50 GDM cases by remote electronic fetal monitoring network (test group), and another 50 monitored by daily fetal movement counting and regular NST check-up in hospital served as the control group. Abnormal NST and perinatal outcome were compared between the two groups. RESULTS: The incidence of abnormal NST was significantly higher in the test group than in the control group (34.6% and 25.5%, P<0.05), as with Apgar scores of the neonates between the two groups (9.58+/-0.77 vs 8.70+/-1.23, P<0.01). The incidences of neonatal asphyxia (12.0%) and preterm birth (8.0%) in the test group were lower than those in the control group (24.0% and 22.0%, respectively, P<0.05). There was no significant difference in the rate of cesarean section between the two groups (P>0.05). CONCLUSION: Remote fetal monitoring network can be used to improve perinatal outcome of GDM, and offers a new option of self-monitoring means for pregnant women with GDM.

[Dynamic evolution of MMP-13, TIMP-1, type I and III collagen and their interaction in experimental liver fibrosis].

OBJECTIVE: To obtain a detailed pattern of the dynamic evolution and interactions among MMP-13, TIMP-1, type I and III collagen during experimental liver fibrosis. METHODS: Wistar rats were randomly allocated into a normal group, and a model group. To induce liver fibrosis, rats were intraperitoneally injected with dimethylnitrosamine (DMN) three consecutive times in the first week, then two consecutive times per week, totally for 6 weeks. In the normal control group, rats were treated with saline by the same means. Animals were sacrificed 1, 4, 10, 17, 28, 42, 56 days after starting DMN injections. Conventional histological examinations were performed after hematoxylin and eosin, and Masson stain. Fibrosis stages were classified into 0 to 4. Hydroxyproline contents were determined after liver tissues were hydrolyzed in HCl at 160 degrees C for 2 h and then measured with spectrometry at 560 nm wavelength. mRNA levels of MMP-13, TIMP-1, type I and III collagen were determined by semi-quantitive RT-PCR. RESULTS: In the model group, hepatic type I pro-collagen mRNA expression started to increase on the 10th day after DMN administration (t = 2.85, P < 0.05), type III started to increase on the 28th day (t = 4.16, P< 0.01), and TIMP-1 mRNA expression started to increase on the 4th day (t = 2.60, P < 0.05). They all remained much higher than in the normal group throughout the remaining study period. Hepatic MMP-13 mRNA expression started to increase on the 17th day after DMN administration and remained at a higher level than in the normal group until he 28th day (t = 4.08, P < 0.01), then gradually returned to normal level at the end of the study period. CONCLUSION: Although hepatic MMP-13 expression transiently increased during liver fibrosis, enhanced expression of TIMP-1 from the early periods of liver fibrosis inhibited the collagen degrading ability of MMP-13, therefore, over-expressed collagen accumulated in the liver. Thus, it is hypothesized that TIMPs play a pivotal role in liver fibrosis.

[Maternal plasma leptin levels and their relationship to insulin and glucose in pregnant women with gestational diabetes mellitus and gestational impaired glucose tolerance].

OBJECTIVE: To investigate the changes in leptin levels and the relationship between the substance and insulin and glucose in pregnant women with gestational diabetes mellitus (GDM) and gestational impaired glucose tolerance (GIGT). METHODS: Radio immunoassay was used to measure the leptin levels at fasting and 3 h after oral glucose load (50 g) in 36 patients with GDM and GIGT, and also in 24 normal pregnant women. In each pregnant women, fasting serum insulin levels and glycosylated haemoglobin values were measured by electrochemiluminescent immunoassays and pressure liquid chromatography (LPLC) respectively, glucose levels 1 h after the administration of glucose by glucose oxidase method. RESULTS: The serum leptin levels in pregnant women with GDM and GIGT and normal pregnant women were (14.9 +/- 4.3) micro g/L and (10.0 +/- 1.8) micro g/L, respectively, suggesting the serum leptin levels were significantly higher in pregnant women with GDM and GIGT than in normal controls. The levels of fasting serum insulin, glycosylated haemoglobin values and plasma glucose levels obtained 1h after oral administration of 50 g glucose were (12.9 +/- 4.3) mU/L, (6.1 +/- 1.1)%, (11.0 +/- 1.4) mmol/L, respectively, whereas the corresponding values in normal controls were (8.6 +/- 3.2) mU/L, (4.5 +/- 1.0)%, (7.8 +/- 1.2) mmol/L. A positive correlation was found between maternal serum leptin levels and fasting serum insulin levels in pregnant women with GDM and GIGT (r = 0.835, P < 0.01). A positive correlation was also found between maternal leptin concentrations and glycosylated haemoglobin values, as well as between leptin levels and plasma glucose levels obtained 1 h after the administration of 50g glucose in women with GDM and GIGT (r = 0.758, P < 0.01; r = 0.561, P < 0.01). CONCLUSIONS: Leptin levels are elevated in pregnant women with GDM and GIGT compared with healthy pregnant women, a positive and significant correlation was found between the maternal leptin levels and fasting insulin levels, as well as between the maternal leptin levels and plasma glucose levels obtained 1 h after the administration of 50 g glucose in women with GDM and GIGT.

[Qualitative and quantitative studies on qianliebeixi capsules].

OBJECTIVE: To establish a method of quality control on qianliebeixi capsules. METHOD: To clarify semen vaccarine by TLC, to determinate the contents of quertin and kaempferol in prepations, by HPLC. Chromatographic conditions: C18 column with METH-0.2%H3PO4(47:53) as mobile phase; the temperature of column was 40 degrees C, and quercetin was detected at 370 nm with flowing rate 1 ml/min. RESULT: The spots of semen vaccarine were clear in TLC, no interference in negative contrast. The average recovery rate of quertin was 98.9% and the RSD was 2.2%; the average recovery rate in kaempferol was 100.4% and the RSD was 1.7%. CONCLUSION: The resolution of spots were perfect in TLC, the spots were easy to distinguish, The method is simple, rapid as well as reproducible and can control the quality of qianliebeixi capsules effectively.

[A digital tongue image analysis instrument for Traditional Chinese Medicine].

A digital tongue image analysis instrument (TIAI) based on computer for Traditional Chinese Medicine is introduced. Some of the key techniques, including the image acquiring condition and method, the evaluation of tongue characters, and the management of tongue image data, are discussed as well. TIAI can provide a non-invasive, quantitative, and objective analysis approach and enhance the theory level and practical ability of the Traditional Chinese tongue diagnosis.
