Effect of impaired myocardial reperfusion on left ventricular remodeling in patients with anterior wall acute myocardial infarction treated with primary coronary intervention.

We assessed the effect of impaired myocardial blush after primary coronary intervention (PCI) on left ventricular remodeling in patients with ST-segment elevation myocardial infarction (STEMI). The study population consisted of 145 patients with first anterior STEMI that was treated successfully (Thrombolysis In Myocardial Infarction grade 3 flow) with PCI. Left ventricular remodeling was defined as an increase of > or =20% in end-diastolic volume based on repeated echocardiographic measurements in patients. The study population was divided into 2 groups according to the presence (myocardial blush grade [MBG] 2 to 3, n = 86) or absence (MBG 0 to 1, n = 59) of myocardial reperfusion. Left ventricular remodeling appeared in 21% of the entire study group. Poor myocardial blush after PCI was associated with an increased rate of remodeling compared with good myocardial reperfusion (32% vs 14%, hazard ratio 2.308, 95% confidence interval [CI] 1.21 to 4.39, p=0.014). Symptoms of heart failure were observed significantly more often in patients with MBG 0 to 1 (35.6% vs 18.6%, p = 0.032) than in patients with MBG 2 to 3. In multivariate analysis, only age (odds ratio 0.96, 95% CI 0.92 to 0.99, p = 0.02) and MBG 0 to 1 (odds ratio 3.15, 95% CI 1.35 to 7.31, p = 0.008) were associated with left ventricular dilation. In conclusion, impaired microvascular reperfusion is associated with left ventricular remodeling and development of congestive heart failure in patients with anterior STEMI that is treated with primary coronary angioplasty.

Effect of microvascular reperfusion on prognosis and left ventricular function in anterior wall myocardial infarction treated with primary angioplasty.

BACKGROUND: Myocardial reperfusion in acute myocardial infarction may fail despite successful recanalization of the infarct-related artery. The purpose of this study was to assess the impact of myocardial reperfusion on clinical outcome and left ventricular (LV) function. METHODS: The clinical significance of myocardial blush grade (MBG)-angiographic marker of myocardial reperfusion, in 104 patients (age 62+/-13 years) with first anterior myocardial infarction, successfully (TIMI 3) treated with primary angioplasty was analysed. Echocardiography was performed at baseline and after 6 months. Mortality and major cardiovascular event (MACE) rates were analysed 30 days and 1 year after acute myocardial infarction. Patients were divided into two groups according to presence (group 1, MBG 2-3, n=64) or absence of myocardial blush (group 2, MBG 0-1, n=40). RESULTS: One-year mortality was significantly higher in group 2 in comparison to group 1 (22.5% vs 6.25%, HR: 3.6, 95% CI: 1.187-10.9, p=0.0175). The rate of MACE was significantly lower in patients with MBG 2-3, both after 1 and 12 months (9.4% vs 30%, p=0.008 and 20.3% vs 60%, p<0.001, respectively). At baseline, both global and regional contractile function were significantly better in group 1 than in group 2 (ejection fraction (EF) 47.4+/-8.8% vs 43.3+/-7.9%, p=0.04 and wall motion score index (WMSI) 1.64+/-0.4 vs 1.87+/-0.3, p=0.001, respectively). Similarly, at 6 months follow-up, LV function was better in group 1 as compared with group 2 (EF 54.9+/-14.5% vs 46+/-13.2%, p=0.005 and WMSI 1.43+/-0.4 vs 1.76+/-0.46, p=0.001, respectively). CONCLUSIONS: Impaired microvascular reperfusion in patients with anterior myocardial infarction is associated with poor prognosis and worse early and late left ventricular function.

Effects of atypical and typical antipsychotic treatments on sexual function in patients with schizophrenia: 12-month results from the Intercontinental Schizophrenia Outpatient Health Outcomes (IC-SOHO) study.

PURPOSE: Sexual dysfunction in patients with schizophrenia can reduce quality of life and treatment compliance. This report will compare the effects of selected atypical and typical antipsychotics on sexual function in a large, international population of outpatients with schizophrenia who were treated over 1 year. SUBJECTS AND METHODS: Outpatients with schizophrenia, who initiated or changed antipsychotic treatment, and entered this 3-year, prospective, observational study were classified according to the monotherapy prescribed at baseline: olanzapine (N=2638), risperidone (N=860), quetiapine (N=142) or haloperidol (N=188). RESULTS: Based on patient perception, the odds of experiencing sexual dysfunction during 1 year of therapy was significantly lower for patients treated with olanzapine and quetiapine when compared to patients who received risperidone or haloperidol (all P< or =0.001). Females on olanzapine (14%) or quetiapine (8%) experienced a lower rate of menstrual irregularities, compared to females on risperidone (23%) or haloperidol (29%). Significant discordance was evident between patient reports and psychiatrist perception of sexual dysfunction, with psychiatrists underestimating sexual dysfunction (P< or =0.001). CONCLUSION: These findings indicate clinically relevant differences exist in the sexual side effect profiles of these selected antipsychotics. These factors should be considered when selecting the most appropriate treatment for outpatients with schizophrenia.

Relationship between tissue reperfusion and postinfarction left ventricular remodelling in patients with anterior wall myocardial infarction treated with primary coronary angioplasty.

INTRODUCTION: Pathological left ventricular remodelling is considered the main cause of heart failure in patients after myocardial infarction. AIM: The purpose of this study was to evaluate correlations between the degree of coronary microvascular reperfusion assessed by means of the angiographic myocardial blush grade (MBG) scale and adverse left ventricular remodelling in patients with acute myocardial infarction treated with primary coronary angioplasty. METHODS: This study involved 92 consecutive patients, hospitalised because of their first anterior wall myocardial infarction, who underwent successful (TIMI-3 grade flow) primary coronary angioplasty. Angiographic myocardial reperfusion parameters (MBG, corrected TIMI Frame Count) were assessed. Three days and 6 months after the index PCI all patients underwent an echocardiographic examination and such parameters as end-diastolic volume (EDV), left ventricular ejection fraction (EF) and contractility index (WMSI) were calculated. RESULTS: The patients were divided into two groups: group 1 with impaired myocardial reperfusion (MBG 0-1) (n=32) and group 2 with adequate tissue reperfusion (MBG 2-3) (n=60). Negative left ventricular remodelling was observed more frequently in group 1 than in group 2 (28.1% vs 10%, p=0.029). More patients in group 1 presented heart failure symptoms (56.3% vs 25%, p=0.013). CONCLUSIONS: Failure of tissue reperfusion assessed by means of angiographic indices (MBG 0-1) in patients with myocardial infarction treated with primary coronary angioplasty is associated with a higher rate of adverse myocardial remodelling and heart failure at 6 months after myocardial infarction.

Chronic infarct-related artery occlusion is associated with a reduction in capillary density. Effects on infarct healing.

AIM: To assess the relationship between infarct-related artery (IRA) stenosis and capillary density and to assess its effect on scar formation in the human heart. MATERIALS AND METHODS: Morphometric evaluation was performed in 51 human hearts, as follows. Group I non-cardiac death (control), Group II post-Q-wave myocardial infarction (QMI) death and Group III patients who survived QMI and who underwent aneurysmectomy. Using morphometric parameters, the relationship between left ventricle (LV) mass, infarct size, IRA stenosis, cellular hypertrophy and changes in microcirculation were analyzed within the infarcted area and free LV wall. RESULTS: A significant reduction in capillary density within the infarcted area was noted in group II when compared to the control group (1525.6+/-378.5/mm(2) vs. 2968.7+/-457.3/mm(2); p<0.001). Reduction in capillary density was inversely related to infarct size (r=-0.616; p=0.006) and degree of IRA stenosis (r(S)=-0.512; p=0.03). The most significant reduction in capillary density was observed in patients with total IRA occlusion (1204.6+/-156.9/mm(2) vs. 1676.6+/-245.8/mm(2); p<0.001). Similarly, a reduction in capillary density of over 60% (1030.7+/-241.8/mm(2)) was observed within aneurysms resected surgically. CONCLUSIONS: The study demonstrated precise quantification of the capillary network in patients following QMI. The most significant reduction in capillary density was observed in patients with chronic total IRA occlusion.

[Value of ST-segment elevation resolution after primary coronary angioplasty in predicting early and late left ventricular function in patients with anterior acute myocardial infarction].

UNLABELLED: The aim of our study was to define a predictive value of ST-segment resolution after coronary reperfusion for early and late left ventricular (LV) function. We also studied its relation to angiographic marker of myocardial reperfusion, myocardial blush grade (MBG). MATERIALS AND METHODS: Study population consisted of 105 patients (75 men, 30 women, mean age 62+/-13 years) treated with primary PTCA for anterior myocardial infarction (AMI) within 12 hours from symptoms onset. ECG was performed before - and 30 min. after PTCA. We evaluated angiographic markers of recanalization (TIMI, corrected TIMI frame count) and myocardial reperfusion - MBG. Echocardiography was performed 3 days and 6 months after AMI. Patients were divided into 2 groups regarding reduction -50% in ST segment elevation after recanalization (rST): group I (n=37) with rST, and group 2 (n=68) without rST RESULTS: In group I we noticed higher CPK level (2890,2+/-2404,2 U/I vs. 3980,9+/-2812,2 U/I, p<0,01). MB grade 3 was significantly more often found in group 1 at 29 pts (78,4%) than in group 2 at 18 pts (23,5%), p<0,0001. MG grade 0 or 1 were more often in group 2 (57,3% vs. 2,7%, p<0,01). Ejection fraction (EF) was higher in group 1 after 3 days (48,9+/-9,5% vs. 44,2+/-14,3%, p<0,01), as well as after 6 months (56,4+/-14,4% vs. 49,5+/-14,3%, p=0,001). After 6 months end-diastolic volume (EDV) was higher in group 2 (128,1+/-46,1 ml vs. 110,2+/-49,7 ml, p<0,008). CONCLUSIONS: ST-resolution after successful PTCA is a simple and effective marker of myocardial reperfusion after recanalization which correlates with angiographic markers of tissue reperfusion. Early ST-segment resolution is connected with better early and late LV function. Patients without early ST-segment elevation resolution have a higher risk of LV remodelling.

Prognostic value of angiographic markers of myocardial reperfusion in patients treated with primary coronary angioplasty for anterior wall acute myocardial infarction.

BACKGROUND: Modern therapy of acute myocardial infarction (AMI) is aimed at rapid and persisting restoration of blood flow in an infarct-related artery (IRA). However, in some patients myocardial reperfusion is not achieved in spite of effective IRA recanalisation. Myocardial Blush Grade (MBG) is one of the angiographic markers useful for the detection of this phenomenon. AIM: To assess the prognostic value of MBG in patients with anterior AMI treated with primary angioplasty. METHODS: The study group consisted of 104 patients (74 males, 30 females, mean age 62+/-13 years) treated with primary angioplasty due to anterior ST-segment elevation AMI. MBG was assessed after the procedure. The mortality and major cardiovascular event (MACE) rates were analysed one and six months after AMI. RESULTS: Patients with preserved myocardial reperfusion following angioplasty (MBG 2-3, n=64 (61.5%)) had a trend towards lower one-month mortality and significantly reduced six-month mortality compared with 40 (38.5%) patients with an impaired (MBG 0-1) myocardial reperfusion (3% vs 12.5%, NS; and 6.25% vs 20%, p<0.05, respectively). The rate of MACE was significantly lower in patients with rather than without reperfusion both after one and six months of follow-up (9.4% vs 27.5%, p=0.027 and 12.5% vs 42.5%, p<0.001, respectively). Compared with patients with a high MBG score, patients with altered reperfusion more frequently had diabetes (30% vs 12.5%, p=0.04), hypertension (67.5% vs 45%, p=0.043), longer time from the onset of symptoms to balloon inflation (355.9+/-199 min vs 215.5+/-113 min, p<0.001) and lower left ventricular ejection fraction, measured 3 days after AMI (43.3%+/-8 vs 47.4%+/-9, p=0.02). CONCLUSIONS: MBG has a significant prognostic value in patients with anterior AMI treated with primary angioplasty. Diabetes, hypertension and long delay of treatment are associated with the impairment of myocardial reperfusion.

[Does the treatment delay influence reperfusion efficacy and short- and long-term left ventricular function in patients with anterior myocardial infarction treated with primary coronary angioplasty?].

UNLABELLED: The benefit of thrombolysis in acute myocardial infarction (AMI) is time dependent, but there is lack of evidence of such a close correlation in patients undergoing primary percutaneous transluminal coronary angioplasty (pPTCA). The aim of this study was to evaluate the importance of delay of treatment for myocardial reperfusion and early and late ventricular function in patients with anterior AMI treated with pPTCA. METHODS: 106 consecutive patients (76 men, 30 women, age 63 +/- 11 years) treated with pPTCA for anterior AMI was included in this study. We evaluated angiographic (myocardial blush grade (MBG), TIMI flow, corrected TIMI frame count) and electrocardiographic (resolution of ST segment elevation--nST) markers of myocardial reperfusion. Echocardiography was performed 3 and 180 days after pPTCA. End-diastolic volume (EDV), end-systolic volume (ESV) and ejection fraction were calculated. Patients were divided into three groups: group A (41 pts.) reperfused within 3 hours, group B (40 pts.) reperfused between 3-6 hours, group C (25 pts.) reperfused between 6-12 hours from symptoms onset. RESULTS: MBG 0-1 (lack of myocardial reperfusion) was significantly more common in group B and C, compared with group A (p = 0.002). The incidence of nST was higher in group A (p < 0.0001). The EF after 3 days (p = 0.03) and after 6 months (p = 0.04) was better in group A in comparison to group C. There was not significant difference in EDV after 3 days, but after 6 months EDV was significantly higher in group C (p = 0.02). CONCLUSIONS: The delay of treatment over 3 hours is associated with decreasing of myocardial reperfusion efficacy and deterioration of the early and late LV function.

[Prevalence of depression in neurological outpatients. DEPEND study].

For the assessing the incidence of mood disturbances among the neurological out-patients 3287 of them were examined by 111 neurologists during their routine practice. Early diagnosis, the type of mood disturbances and the depth of depression were estimated by the use of Beck's Depression Inventory, the questionnaire based on The Mini-International Neuropsychiatric Interview and Hamilton Depression Rating Scale, as well. Around half of the patients (50.47%) were suspected on depression, as an early diagnosis. In suspected and diagnosed depressive patients the symptoms as anxiety, low activity precordial pain, headaches, dry mouth, constipation, sleep and appetite troubles were significantly (p < 0.01) more frequent than in euthymic subjects. Among all studied patients the episode of depression were found as a final diagnose in 17.2%, recurrent depressive disorders--in 17.6% and dysthymia--in 2.8% of subjects. In finally diagnosed depressive patients the chronic neurological problems were significantly (p = 0.013) more frequent, as the cause of the visit, than in the euthymic ones. The low mood was equally frequent among the patients with Parkinson's disease, multiple sclerosis and cerebrovascular disorders, as well.

[Seven hospitalisations - diagnostic difficulties in a male patient with a 20-year history of chest pain].

A case of a 45-year-old male patient with hypertrophic cardiomyopathy (HCM) is presented. The patient suffered from chest pain since his childhood. He was hospitalised several times but accurate diagnosis was not established for nearly 20 years. The ECG changes were interpreted as myocardial ischaemia. The proper diagnosis was based on echocardiographic and catheterisation data. Nowadays, echocardiography is the method of choice in the detection of HCM. HCM is an important cause of chest pain, especially in young patients.

Risperidone treatment of schizophrenia: improvement in psychopathology and neuropsychological tests.

The aim of the study was to assess the effects of short-term risperidone treatment on schizophrenic symptoms and on neuropsychological frontal tests, and to find an association between the improvements in these two kinds of domains. Twenty-two schizophrenic patients treated with risperidone (2-6 mg/day) for 4 weeks were studied. Treatment with risperidone resulted in a significant decrease in the intensity of schizophrenic symptoms and in an improvement in all neuropsychological tests applied. A robust correlation was obtained between the amelioration of negative symptoms and improvements in many neuropsychological tests, specifically with amelioration in the Wisconsin Card Sorting Test (WCST), perseverative errors. Some correlation was also found with the improvement of positive symptoms, specifically with amelioration in the WCST, completed categories. The results may suggest a common neurobiological substrate of negative and cognitive symptoms, reflected in prefrontal cortex pathology and in therapeutic response to atypical antipsychotics.

[The effect of sertraline on cognitive functions in patients with obsessive-compulsive disorder].

The aim of this study was to assess the effect of sertraline on psychopathological symptoms and cognitive functions and in patients with obsessive compulsive disorder (OCD). The investigated group consisted of 25 patients with OCD (12 male, 13 female) aged 17-47 (mean 29 +/- 9) years, duration of illness was 1-15 (mean 4 +/- 2) years. After treatment with sertraline, a significant improvement in OCD symptoms measured by YBOCS and in neuropsychological "frontal" tests were observed. Little correlation was found between the effect of sertraline on OCD symptoms and on cognitive dysfunctions. This may suggest that these two effects may be connected with different pharmacological properties of the drug. The effect of sertraline on OCD symptoms, similarly like other drugs inhibiting serotonin transporter (clomipramine, other SSRI) is associated with its influence on serotonergic system. On the other hand, the effect of sertraline on dopaminergic neurotransmission may be related to its favorable action on cognitive functions connected with the activity of frontal lobe.

[Safety and efficacy of olanzapine versus perphenazine in patients with schizophrenia: results of multicenter, 18-week, double-blind clinical trial].

AIM: The primary objective of the study was to evaluate the severity of extrapyramidal symptoms during treatment with olanzapine (10-20 mg) versus perphenazine (8-40 mg) using the Simpson Angus Scale (SAS). The secondary objective was to assess the safety profile and clinical efficacy of the investigated drugs. MATERIAL AND METHOD: A total of 95 patients with schizophrenia who met the criteria for DSM-IV were randomized to a double-blind, 18 week prospective comparative trail conducted in Poland. The tolerance of treatment was assessed with the use of scales: BAS, SAS and UKU. The efficacy of treatment was evaluated with BPRS, PANSS and CGOI scales. RESULTS: For olanzapine patients, the severity of extrapyramidal symptoms improved after 3 first weeks of treatment, and significantly decreased from the baseline to endpoint. Perphenazine patients showed an increase of extrapyramidal symptoms. The difference of the SAS scores change was statistically significant between olanzapine and perphenazine groups. Akathisia symptoms decreased significantly in the olanzapine group during the treatment period, whereas symptoms of akathisia increased in the perphenazine group. Statistically significant differences of mean change of BAS total score from baseline to endpoint were noted between treatment groups Treatment--emergent adverse events occurred more frequently in patients receiving perphenazine (46%), than in patients receiving olanzapine (17%). The proportion of patients complying with improvement criteria for CGI scale score was statistically greater in the olanzapine group (72.7%) than in the perphenazine group (47.9%). Results of this study showed that the tolerance profile in patients taking olanzapine is superior to perphenazine. CONCLUSIONS: Olanzapine was better tolerated than perphenazine. After olanzapine treatment more subjects fulfilled the criterion of improvement and schizophrenic symptoms were less severe than in patients treated with perphenazine.

[Prevalence of depressive disorders among perimenopausal women seeking gynecological services].

AIM: The aim of the study was to evaluate the prevalence of depressive disorders in perimenopausal women reporting to gynecologists and to assess a possibility of screening and diagnosing depression in gynecological practice. METHOD: The study entailed 2262 female patients of the age of 45-55 attending 120 gynecologists in Poland. Patients were first asked to fill in the Beck's Depression Inventory and next examined by their gynecologists to verify the presence of the basic symptoms of depressive episode according to ICD-10 diagnostic criteria. Patients who obtained 12 points or more in BDI total score were referred for a psychiatric consultation, including the MINI questionnaire. The study was monitored by 'Servier Polska'. RESULTS: 32.5% of women aged 45-55 years visiting gynecologists scored 12 points or more on the BDI scale. Patients with high BDI scores reported long-lasting somatic and emotional symptoms associated with menopause more frequently than patients with low BDI scores. Women with high BDI scores also more frequently experienced gynecological surgery in the past, more frequently attended gynecological clinics, and reported more peri- and postmatal mood disorders. 57% of patients did not visit psychiatrists. Depressive disorders were diagnosed in 76% of patients who came for the psychiatric evaluation. CONCLUSIONS: The study suggests that more than 19% of perimenopausal women attending their gynecologists suffer from depressive disorders. Gynecologists in Poland can diagnose depression accurately and start pharmacological treatment of the disorder in half of the patients.

[Prospective observational study of outpatients with schizophrenia in Poland: preliminary 6 month efficacy and tolerability results].

AIM: To compare the effectiveness and tolerability of olanzapine and other antipsychotic treatments (6 months after enrollment) in Polish patients with schizophrenia participating in the IC-SOHO (the Intercontinental Schizophrenia Outpatient Health Outcomes) study. METHOD: IC-SOHO is a 3-year, global, prospective, observational study examining health outcomes in outpatients with schizophrenia undergoing treatment with antipsychotics. Patients were offered enrollment by a psychiatrist if they initiated or changed antipsychotic medication for the treatment of schizophrenia. The study was designed to provide two patient cohorts of approximately equal size: patients who initiated or switched to olanzapine, and patients who initiated or switched to non-olanzapine antipsychotic therapy. Mean changes in positive, negative, cognitive, depressive and overall symptoms from baseline to 6 months, as measured using the CGI scale, were assessed. The incidence of extrapyramidal symptoms and sexual dysfunctions and weight changes were compared. RESULTS: Out of 7648 patients participating in the IC-SOHO study, 626 patients were enrolled in Poland. After 3 and 6 months of treatment, significantly greater improvement in overall symptoms measured using the CGI scale was observed in patients with olanzapine than in those with other antipsychotics. Response rate at 6 months was significantly higher in the olanzapine group than in the non-olanzapine group. Incidence of extrapyramidal symptoms and sexual dysfunctions was lower in olanzapine group. CONCLUSIONS: After 6 months, olanzapine-treated patients showed greater improvement of clinical status compared with other antipsychotic treatment. Olanzapine treatment was more beneficial for patients; comparing to other antipsychotic treatment less extrapyramidal symptoms and sexual dysfunctions were observed.
