[PDT photodynamic therapy in orthopedic inflammatory conditions?].

The infections in orthopedic surgery and traumatology represents important medical problem. The results of treatment of motor-organs infections in high degree were improved by the introduction of antisepsis and asepsis in XIX century and the antibiotics' discovery in the beginning of XX century. However, widespread usage of antibiotics leaded to the rise of refractory tribes of bacteria on their activity, which caused higher percentage of fails in the therapy. Also the rapid civilization development, which flown on arising of a new invasive methods of operative trauma protection and stabilizations of fractures in motor-organs area increased the number of infectious complications during treatment. These facts gave a reason to look for a more effective therapeutic methods. It seems that photodynamic therapy gives us the new possibilities of infectious treatment, which avails oneself activity of therapeutic light laser with proper wave length on the human tissue with photosensitizer included. Indeed there are known until now splendid results of PDT in neoplasma treatment, however it seems, based on experimental investigations and publications in medical literature, that this method can be a chance of effectual and non-invasive treatment of the soft tissues and bones inflammations.

Modified percutaneous ethanol injection in the treatment of viscous cystic thyroid nodules.

Here we describe a prospective 2-year evaluation of the efficiency of a new procedure of percutaneous ethanol injection (PEI) treatment of viscous thyroid cystic nodules (VTCN). Three patients with VCTN were selected from a group of 22 patients with solitary thyroid cystic nodules. A modified two-stage ethanol injection procedure was planned for these patients. Stage 1 was performed through the injection of a small dose of ethanol into the nodule. The viscosity of the content was reduced within 2 weeks after the injection. In the second stage the nodule was decompressed and an established dose of sterile 95% ethanol was injected. Patients were followed up for 2 years. In all cases a reduction in nodule volume was found. In the following 18 months further reduction was observed, with an average of 91.7%. The effect was stable throughout the follow-up period. No complications were observed. Therefore, our new two-stage PEI procedure appears all efficient alternative approach for the treatment of VCTNs even if further investigations on a larger series are required.

[Leśniowski-Crohn's disease in the upper part of the gastrointestinal tract].

Leśniowski-Crohn's disease (L-CD) includes several clinical and pathological pictures which are manifestations of transmural inflammation. L-CD may affect any portion of the gastrointestinal tract (GI) and in the upper part of GI it has been described in 0.5%-13.0% of all cases of this disease. In the upper part of GI, in the course of L-CD, there appears a considerable variety of endoscopical and morphological pictures. Advances which have been achieved in endoscopic methods, along with very precise histopathological description, allow us to diagnose a greater number of incidences of L-CD in the upper part of GI, compared to previous years.

[Therapy of Lesniowski-Crohn disease of the upper part of the gastrointestinal tract].

The treatment of a form of Lesniowski-Crohn disease (L-CD) which occurs in the upper part of the gastrointestinal tract (GI) in principle is similar to that which is used if other parts of the GI tract are involved in L-CD, however with certain modifications, and these problems are discussed in this paper. The treatment is based on 5-aminosalicylic acid (5-ASA), glucocorticosteroids, antibiotics and immunosuppressive drugs. For patients in whom the conventional therapy doesn't give expected results, the advent of infliximab (biologic agents) has greatly improved treatment possibilities of L-CD. Lesniowski-Crohn disease is not curable either by drugs or by surgery. The goal of treatment in this disease remains the mitigation of symptoms and improvement of the quality of life and at the same time elimination or delaying of the appearance of complications and early and late side effects as a result of treatment. The introduction of biologic agents into the treatment arsenal for inflammatory bowel diseases is a revolutionary step.

Alternating extremely low frequency magnetic field increases turnover of dopamine and serotonin in rat frontal cortex.

The aim of this study was to evaluate the influence of an extremely low frequency sinusoidal magnetic field (ELF MF) with frequency of 10 Hz and intensity of 1.8-3.8 mT on the levels of the biogenic amines dopamine (DA), 3,4-dihydroxyphenylacetic acid (DOPAC), homovanillic acid (HVA), 3-methoxytyramine (3-MT), 5-hydroxytryptamine (5-HT), 5-hydroxyindolacetic acid (5-HIAA), and noradrenaline (NA), as well as on DA and 5-HT turnover in corpus striatum and frontal cortex of adult male Wistar rats. We found that ELF MF exposure for 14 days, 1 h daily, did not influence the level of the examined biogenic amines and metabolites, but increased the rate of synthesis (turnover) of DA and 5-HT in rat frontal cortex as compared to control, sham exposed rats. On the basis of the present results and our previous findings, extremely low frequency magnetic field (ELF MF) exposure has been found to alter both turnover and receptor reactivity of monoaminergic systems, as well as some behaviors induced by these systems or their agonists and antagonists.

[Photodynamic diagnosis and therapy in gynecology--current knowledge].

Photodynamic diagnosis (PDD) is a new method based on the detection of different forms of fluorescence of tissues after previous administration of photosensitizers. The photosensitizer is gathered in the pathological tissue at much higher concentration than in the healthy tissue, thus the fluorescence differs. Localizing wrong fluorescence allows precise choosing of the spot to collect tissue for histopathological or cytological study. Photodynamic therapy (PDT) is a technique in which tissue is irradiated with light after the use of a photosensitizing drug that produces singlet oxygen, which has a cytotoxic effect. The authors describe new trends in photodynamic diagnosis and treatment of some vulvar epithelial diseases (VIN, lichen sclerosus, condylomata acuminata) and cervical intraepithelial neoplasia. They describe photodynamic method in their own studies: diagnosis and treatment of lichen sclerosus and diagnosis of uterine cervix cancer.

[Heat shock proteins in colorectal carcinoma].

Heat-shock proteins (HSP) are a group of proteins, whose production in induced as a intracellular response to different kinds of stresses. They are also included in the group of chaperones, which are involved in protein folding and repairing of denatured proteins in the cells. Because of the protective role of HSP on the cellular level, many studies are focused on the role of heat shock proteins in anti-carcinogenic effect. The present study is a review of the literature regarding the value of HSP in the carcinogenic process.

[The significance of probiotics in medicine--a new look].

The article presents the definition, activity mechanism and application of probiotics, as well as their side-effects. Probiotics are microorganisms living in humans which may favorably influence human health and prevent some diseases. The mechanism of their activity is not yet fully understood. There are numerous attempts to apply probiotics as supportive therapy, mainly in gastrointestinal tract diseases. Other possible applications of probiotics in medicine require further clinical studies, concerning the mechanism of activity, determination of indications for use and optimal therapeutic dose.

Autofluorescence endoscopy for detection of high-grade dysplasia in short-segment Barrett's esophagus.

BACKGROUND: The occurrence of precancerous lesions, such as high-grade dysplasia, in patients with short-segment Barrett's esophagus is controversial. This study assessed the efficacy of autofluorescence endoscopy for detection of high-grade dysplasia in short-segment Barrett's esophagus. METHODS: A total of 34 patients (28 men, 6 women; age range 40-77 years) with histopathologically proven short-segment Barrett's esophagus were studied. Autofluorescence endoscopy was performed by using monochromatized blue light (425-455 nm) filtered from a conventional xenon light source. A total of 136 and 109 biopsy specimens were taken from Barrett's mucosa under control, respectively, white light endoscopy and autofluorescence endoscopy. RESULTS: High-grade dysplasia was found in 9 (8.3%) autofluorescence-guided biopsy specimens, which was significantly greater than the number of white light endoscopy-guided biopsy specimens with this finding (one positive biopsy specimen, 0.7% of total biopsy specimens obtained). Autofluorescence endoscopy detected high-grade dysplasia in 7 patients, 6 more than were identified with white light endoscopy. In the one patient with high-grade dysplasia detected by white light endoscopy-guided biopsy specimens, autofluorescence-guided biopsy specimens revealed only low-grade dysplasia. CONCLUSION: Autofluorescence endoscopy in patients with short-segment Barrett's esophagus increases the detection rate of high-grade dysplasia.

Photodynamic therapy (PDT) using topically applied delta-aminolevulinic acid (ALA) for the treatment of oral leukoplakia.

BACKGROUND: Photodynamic therapy (PDT) is a non-invasive method for topical and selective treatment of pre-malignant lesions of oral cavity. The aim of our study was to determine therapeutic response to PDT in patients with oral leukoplakia. METHODS: Twelve patients participated in our study. Lesions affected a variety of intraoral sites. The most common location was buccal, gingival and mandibular mucosa. Patients were treated with topically applied 10% delta-aminolevulinic acid (ALA) and light from an argon-pumped dye laser. Irradiation was performed in several (6-8) sessions using light at 635 nm wavelength, delivering a total dose of 100 J/cm2 per session. RESULTS: A complete response was obtained in 10 out of 12 treated patients. One recurrence was observed during 6 months. CONCLUSION: Photodynamic therapy appears to be a feasible alternative to conventional therapy of pre-malignant lesions of oral cavity.

[The application of cryogenic temperatures in medicine].

An increased interest in effects of low temperatures on human organisms has been observed during last years. Low temperatures have been applied in cryotherapy and cryosurgery. In our article we show the influence of low temperatures on different physiological mechanisms and the possibility of their application in the therapy.

[Prognostic difficulties in pseudomyxoma peritonei].

Pseudomyxoma peritonei is a rare disease leading to the accumulation of gelatinous masses in peritoneal cavity, usually secondary to neoplasmatic process of appendix or ovary. The authors describe a case report of 50 years old man with pseudomyxoma peritonei. Literature based discussion on prognostic difficulties and operative therapy perspectives in patients with this disease is presented.

[Clinical response to photodynamic therapy in premalignant lesions and advanced head and neck carcinomas].

The main advantage of PDT in laryngology seems to be its non-invasiveness and the possibility of using it despite of previous application of conventional methods. In the study application of PDT in two separated groups of patients, i.e. 8 patients previously treated by surgery and radiotherapy with local recurrence (SCC) or with advanced primary lesion and 12 patients with oral leucoplakia. In the first group delta-aminolevulenic acid (ALA) was administered orally, while in second one an ointment containing 10% ALA was applied locally. In the both groups prior to irradiation the tissue level of protoporphyrin IX was verified by adapted Xillix LIFE machine. All treated lesions were irradiated with argon-pumped dye laser. In leucoplakia group complete response was achieved in 10 out of 12 treated patients. In group of advanced cancers the only partial response was obtained--the diminution of cancer ulceration was observed. On the basis of our preliminary results the usefulness of PDT may be suggested in eradication of premalignant lesions of oral cavity and palliation of advanced cancers within pharynx and larynx.

[Heat-shock proteins in myocardial ischemia].

Heat-shock proteins (HSP) are a group of highly conserved proteins, which production is induced as a intracellular response to different kinds of stresses. They are also included in the group of chaperones, which are involved in protein folding and refolding in the cells, stabilising of macromolecular structure and repairing of denatured proteins or promoting their degradation when damages are too advanced. Because of protective role of HSP on the cellular level, a lot of studies is focused on the place of HSP in cardioprotection. The exact mechanism of this activity is still unknown. The experiments provided convincing proofs that cardiomyocytes which were subjected to hyperthermia or many other stress factors were reacting increased synthesis of HSP what guaranteed protection against further, stronger episodes of different stresses. The problem, which is still actual, is finding the possibility of taking advantage of cardioprotective properties of HSP in the treatment. Such a situation is due to lack of noninvasive methods of inducing HSP expression. The present study is a kind of review of literature taking up this problem, starting with protocols using hyperthermia, through the pharmacological therapy and ending with molecular techniques and genetic engineering.

[The influence of habitual cigarette smoking on level of endogenic histamine determined simultaneously in plasma, gastric juice and gastric mucosa in healthy individual].

UNLABELLED: The aim of this study was to examine the amount of endogenic histamine measured simultaneously in the plasma, gastric juice and gastric mucosa in habitual smokers and nonsmokers and to suggest possible implications of the obtained results. METHODS: 48 individuals were selected for this test. None of them suffered from any symptoms of ulcer disease. 28 were habitual cigarette smokers, 20 were nonsmokers who had never smoked. The tests were carried out standard conditions. Each patient underwent endoscopy. Gastric juice was taken by catheter via endoscopes, and biopsy specimens were taken for microscopic examination and establish histamine levels, which were also measured in plasma. Blood samples were taken before endoscopy. RESULTS: The levels of endogenic histamine in plasma and gastric juice were significantly higher in habitual smokers than in nonsmokers, especially in plasma. In the tissue of both groups no statistically significant differences were observed, but the histamine level was slightly higher in nonsmokers. CONCLUSION: The amount of histamine in plasma, as well in gastric juice, is may be connected with habitual cigarette smoking, and its level in tissue may also depend on smoking.

[Application of variable magnetic fields in medicine--15 years experience].

The results of 15-year own experimental and clinical research on application of variable magnetic fields in medicine were presented. In experimental studies analgesic effect (related to endogenous opioid system and nitrogen oxide activity) and regenerative effect of variable magnetic fields with therapeutical parameters was observed. The influence of this fields on enzymatic and hormonal activity, free oxygen radicals, carbohydrates, protein and lipid metabolism, dielectric and rheological properties of blood as well as behavioural reactions and activity of central dopamine receptor in experimental animals was proved. In clinical studies high therapeutic efficacy of magnetotherapy and magnetostimulation in the treatment of osteoarthrosis, abnormal ossification, osteoporosis, nasosinusitis, multiple sclerosis, Parkinson's disease, spastic paresis, diabetic polyneuropathy and retinopathy, vegetative neurosis, peptic ulcers, colon irritable and trophic ulcers was confirmed.

Influence of alternating extremely low frequency ELF magnetic field on structure and function of pancreas in rats.

The aim of this study was to estimate the influence on ultrastructure and function of endocrine and excretoric part of pancreas in rats of extremely low frequency alternating magnetic fields with parameters used in therapy in humans. The animals from the two experimental groups were exposed to a rectangular magnetic field waveform at a frequency of 10 Hz and induction of 1.8-3.8 mT--(group P) or a sinusoidal magnetic field at a frequency of 40 Hz and induction of 1.3-2.7 mT--(group S), respectively. The control rats were subjected to sham exposure. The cycle of 1, 3, 6, 9, and 14 daily exposure sessions lasting 30 min was made in all groups. Some of rats after finishing the cycle of 14 exposures were left in the same conditions except for the magnetic field for 3 or 10 days. In both groups of rats exposed to magnetic field, a distinct tendency to decrease glucose concentration, compared to control group, was observed during the exposure cycle. Serum glucose became normal after the end of exposure sessions. The concentrations of insulin in both groups of rats exposed to magnetic field were significantly higher, compared to the controls, during the exposure cycle. After the end of exposure cycle the concentration of insulin in group S became normal. In contrast, in group P the concentration of insulin decreased significantly on the last day of exposure, with a subsequent increase in the following days. The activity of alpha-amylase and lipase in the serum of experimental and control rats was not affected. In both groups of exposed rats, reversible changes of ultrastructure of the pancreatic islets, including expansion of the Golgi apparatus, extension of rough endoplasmatic reticulum, mitochondrial swelling, expansion of beta-granules and increase in number of empty vesicles in beta cells, occurred during the exposure. In acinar cells of exposed animals, a slight extension of rough endoplasmatic reticulum and mitochondrial swelling as transitory changes were observed. The structural and functional changes in pancreas are probably adaptative ones.

Concentration of chosen oxycholesterols in plasma of pregnant women with pregnancy-induced hypertension.

Solid-phase extraction (SPE) was applied for isolation of oxycholesterols from plasma lipid extract from pregnant women with hypertension and from a control group. Separation of oxycholesterols fraction was performed in an SD II horizontal chamber (Chromdes, Poland) using silica gel and octadecyl RPC18 silica gel TLC plates (Merck and Machery Nagel). Visualization was carried out under UV light after Liebermann-Burchard reaction specific for cholesterol and its derivatives. The oxycholesterols (5-cholestene-3beta-ol-7-one, sum of 5-cholestene-3beta, 7beta-diol and 5-cholestene-3beta, 7alpha-diol and sum of 5alpha,6alpha-epoxycholestan-3beta-ol and 5beta, 6beta-epoxycholestan-3beta-ol) were quantified by chromatograms scanning in reflectance and fluorescence mode using a CS 9301 densitometer (Shimadzu). The total concentration of the investigated oxycholesterols in the plasma of pregnant women was up to 5000 ng/mL and was statistically significantly higher in women with pregnancy induced hypertension (PIH).
