Spectral turbulence analysis of the signal-averaged electrocardiogram of the atrial activation as predictor of recurrence of idiopathic and persistent atrial fibrillation.

The frequency domain analysis of the P-wave signal-averaged ECG (P-SAECG) is able to identify patients at risk for lone atrial fibrillation (AF) after cardioversion to sinus rhythm. The terminal portion of the P-wave of right precordial leads on 12-lead ECG is associated with electrical abnormalities in the atria. The aim of this study was to assess the spectral turbulence analysis (STA) of the P-SAECG as a predictor of recurrence of idiopathic AF. STA was performed in 41 patients with 2 or more symptomatic episodes of idiopathic and persistent AF after successful electrical cardioversion and drug-free state (Group A), and in 25 control individuals during sinus rhythm (Group B), matched by age, gender, and P-wave duration. The orientation of the terminal portion (positive or negative) of the Z-lead was assessed as representing right precordial leads potentials. After 6 months follow-up, Group A was divided into two groups according to recurrence: G-A1--at least one recurrence (21 patients), and G-A2--no recurrence (20 patients). Fragmented electrical activity (FEA) was observed in 19 patients of G-A1 and in 2 of G-A2 (odds ratio = 85.5; p<<0.001). STA showed 90.5% sensitivity and 90.0% specificity for early recurrence of AF. After 12 months, no patients of G-A2 and 15 of G-A1 developed >3 episodes of persistent AF, being FEA observed in 13 (odds ratio = 14.6, p = 0.002). No episodes of AF were observed in Group B. Average time for recurrence of FEA positive patients (4.3 +/- 0.7 months) was significantly shorter than of G-A2 (7.4+/-0.7 months), and log-rank analysis revealed significant difference of event-free rate over time (p = 0.004). In a logistic regression model FEA, use of amiodarone and a positive terminal portion of the Z-lead of the P-SAECG were independent predictors of recurrence of idiopathic and persistent AF.

[Assessing autonomic function in hypertrophic cardiomyopathy].

OBJECTIVE: Assess the autonomic function in hypertrophic cardiomyopathy (HCM) through heart rate variability (HRV) and to correlate it to echocardiographic data. METHODS: Two groups were studied, and compared for gender, age and HR: A) Ten (10) patients reporting septal HCM (70% non-obstructive); B) Ten (10) healthy volunteers. HRV was analyzed along four successive stages: at rest, under controlled breathing, while bending, and controlled breathing associated to bending. Variables means were compared between groups and intra-groups in the different stages; in Group A, variables means were correlated to echocardiographic measurements (interventricular septum and left atrial diameter). RESULTS: No HRV difference was reported among groups in the first 3 stages. In the fourth stage vagal activity was shown to be higher in Group A [quadratic mean log between RR intervals (RMSSD) - 1.35+/-0.14 vs 1.17+/-0.16; p=0.019; high frequency component logarithm (LogHF)- 4.89+/-0.22 vs 4.62+/-0.26; p=0.032]. Along the stages, vagal measurements [rate of pairs of consecutive RR intervals whose difference is > or =50 ms (pNN50) and LogHF] also showed lower reduction in the third stage in Group A, while LogHF showed some increase in last stage (p=0.027), thus indicating marked parasympathetic activity in that group. Group A HRV analysis showed no difference among patients reporting larger hypertrophy or atrial diameter. CONCLUSION: 1) Parasympathetic prevalence was shown during autonomic stimulation in HCM patients; 2) No correlation was found between HRV and echocardiographic measurements under analysis.

Effects of ACE inhibition during fetal development on cardiac microvasculature in adult spontaneously hypertensive rats.

BACKGROUND: Early angiotensin-converting enzyme (ACE) inhibition is able to re-program spontaneously hypertensive rats (SHR) to express an attenuated form of disease in adulthood. METHODS: Three groups of animals (n=5 each) were studied: Wistar male rats, SHR males, and SHR males obtained from dams treated with enalapril maleate (15 mg/kg/day) during gestation. Animals were sacrificed 180 days after birth, and hearts were removed for stereological quantification. Volume [Vv] (myocytes, cardiac interstitium and intramyocardial vessels), length [Lv] (intramyocardial vessels), surface [Sv] densities (myocyte and intramyocardial vessels), and the mean cross-sectional area [a] (myocyte) were estimated. RESULTS: Blood pressure (BP) was lower in Wistar group, higher in SHR group, and intermediate in SHR-enalapril group (respectively: 122+/-8.4, 194+/-11.4, and 158+/-7.6 mm Hg, p<0.0001). Increased Vv (p=0.016), Lv (p<0.01), and Sv (p<0.01) of intramyocardial vessels were observed in SHR-enalapril group when compared to untreated SHR. A small but significant reduction was observed in a of myocytes (p=0.045). CONCLUSION: Prenatal ACE inhibition resulted in partial hypertension attenuation as well as left ventricular hypertrophy (LVH). The positive impact on the vascular compartment came along with little or no difference in myocytes and interstitium, suggesting the involvement of a direct mechanism.

Enalapril alters the formation of the collagen matrix in spontaneously hypertensive rats.

OBJECTIVE: To assess the effect of the inhibition of the angiotensin-converting enzyme on the collagen matrix (CM) of the heart of newborn spontaneously hypertensive rats (SHR) during embryonic development. METHODS: The study comprised the 2 following groups of SHR (n=5 each): treated group - rats conceived from SHR females treated with enalapril maleate (15 mg. kg-1.day-1) during gestation; and nontreated group - offspring of nontreated females. The newborns were euthanized within the first 24 hours after birth and their hearts were removed and processed for histological study. Three fields per animal were considered for computer-assisted digital analysis and determination of the volume densities (Vv) of the nuclei and CM. The images were segmented with the aid of Image Pro Plus 4.5.029 software (Media Cybernetics). RESULTS: No difference was observed between the treated and nontreated groups in regard to body mass, cardiac mass, and the relation between cardiac and body mass. A significant reduction in the Vv[matrix] and a concomitant increase in the Vv[nuclei] were observed in the treated group as compared with those in the nontreated group. CONCLUSION: The treatment with enalapril of hypertensive rats during pregnancy alters the collagen content and structure of the myocardium of newborns.

Analysis of the prevalence of ventricular late potentials in the late phase of myocardial infarction based on the site of infarction.

OBJECTIVE: The initial site of myocardial infarction (MI) may influence the prevalence of ventricular late potentials (VLP), high-frequency signals, due to the time course of ventricular activation. The prevalence of VLP in a period of more than 2 years after acute MI was assessed focusing on the initially injured wall. METHODS: The prevalence of VLP in a late phase after MI (median of 924 days) in anterior/antero-septal and inferior/infero-dorsal wall lesion was analyzed using signal-averaged electrocardiogram in time domain. The diagnostic performance of the filters employed for analysis on was tested at high-pass cut-off frequencies of 25 Hz, 40 Hz and 80 Hz. RESULTS: The duration of the ventricular activation and its terminal portion were larger in inferior than anterior infarction, at high-pass cut-off frequencies of 40 Hz and 80 Hz. In patients with ventricular tachycardia, these differences were more remarked. The prevalence of ventricular late potentials was three times greater in inferior than anterior infarction. CONCLUSION: Late after myocardial infarction, the prevalence and the duration of ventricular late potentials are greater in lesions of inferior/infero-dorsal than anterior/antero-septal wall confirming their temporal process, reflecting their high-frequency content.
