Laparoscopic radical nephrectomy: comparison of clinical Stage T1 and T2 renal tumors.

OBJECTIVES: To compare the outcomes after laparoscopic radical nephrectomy (LRN) at our institution to treat Stage T1 and T2 renal tumors. LRN for Stage T1 renal tumors (less than 7 cm) has become the standard of care at many institutions. The feasibility of performing more complex LRNs on higher stage tumors is continually evolving. METHODS: A retrospective review was performed of a prospective database of patients undergoing LRN at the University of Chicago from October 2002 to January 2006. The data on 141 unilateral LRNs, 98 for clinical Stage T1 tumors and 43 for clinical Stage T2 tumors, were analyzed. The demographic, operative, and postoperative data were compared between the two groups. RESULTS: The demographic data between the two groups was comparable. Operatively, patients with larger tumors had significantly greater blood loss, a longer operative time, and a longer surgical incision. Open conversions (1% versus 12%, P = 0.013) and intraoperative complications (5% versus 19%, P = 0.006) were more likely in patients with clinical Stage T2 tumors. Most intraoperative complications were hemorrhage requiring transfusion. However, the postoperative complication rates (25% versus 21%, P = 0.646) and hospital stay (2.0 versus 2.4 days, P = 0.134) were similar between the two groups. CONCLUSIONS: In experienced centers, clinical Stage T2 renal tumors can be managed efficiently with laparoscopy. LRN for larger tumors demonstrated equivalent perioperative safety compared with LRN for smaller tumors.

Intravesical botulinum type A toxin injection in patients with overactive bladder: Trigone versus trigone-sparing injection.

OBJECTIVE: Botulinum toxin type A (BTX-A) has been successfully used in the treatment of patients with overactive bladder (OAB) symptoms refractory to anticholinergic therapy, with most studies performing trigone-sparing detrusor injections. Increasing evidence suggest that sensory nerve dysfunction contributes to the pathophysiology of OAB and, for this reason, targeting the afferent innervation of the bladder trigone during injection may provide clinical benefit. MATERIALS AND METHODS: We conducted a pilot study to assess the benefit of trigonal-inclusion during BTX-A injection. A total of 40 patients with OAB refractory to anticholinergic treatment received trigone or trigone-sparing injection of BTX-A. Patients were evaluated using UDI-6 and IIQ-7 questionnaires prior to the BTX-A applications, at 3 weeks and 6 months after treatment. RESULTS: At 3-week follow-up, 15/24 (63%) and 10/16 (63%) patients showed improvement of symptoms in the trigone versus trigone-sparing groups, respectively. Combined 3-week UDI-6 and IIQ-7 scores improved from 37.3 prior to treatment to 27.4 (p < 0.05) and 35.5 to 27.2 (p < 0.05) in the trigone and trigone-sparing groups, respectively. Six-month follow-up demonstrated continued but diminished levels of symptom improvement when compared to pre-treatment and 3-week symptom scores. Improvement in symptom scores between the trigone and trigone-sparing groups was not significant. CONCLUSION: No significant difference in symptom score or treatment response was noted between the trigone and trigone-sparing groups. Further study using pre- and post-operative urodynamic study is needed to evaluate possible benefit to trigonal injection in a select sensory urgency cohort, and to assess for urodynamic improvement following trigonal injection.

Intra-operative prostate examination: predictive value and effect on margin status.

OBJECTIVE: To evaluate the ability of intra-operative prostate examination (IOPE) to predict extraprostatic extension (EPE) and its effect on margin status in the region of the neurovascular bundle (NVB) when combined with wide excision. PATIENTS AND METHODS: We retrospectively reviewed 403 patients with clinical stage T1c prostate adenocarcinoma undergoing radical retropubic prostatectomy (RRP). All patients had IOPE during RRP, and those with palpable abnormalities in the region of the NVB underwent wide excision. Pathological outcomes were analysed. RESULTS: Of 403 patients, 49 (12%) had a palpable abnormality in the region of the NVB. After wide excision, 18 (37%) of these 49 patients were found to have EPE at the site of the palpable abnormality; with wide excision of the NVB, only one of these 18 patients (6%) had a corresponding positive surgical margin (PSM). In 354 patients with a normal IOPE and who underwent bilateral NVB preservation, 30 were found to have EPE in the region of the NVB. The PSM rate in this subset was 23% (seven of 30). The positive predictive value of IOPE for detecting EPE was 37%. CONCLUSION: IOPE detects abnormalities in 12% of patients with preoperative stage T1c prostate cancer. Although the predictive value of this test is limited, IOPE may decrease PSMs in a subset of patients with EPE in the region of the NVB. The present study reaffirms the value of IOPE for assessing the risk of extraprostatic disease, and for guiding surgical management.

Luminal bacterial flora determines physiological expression of intestinal epithelial cytoprotective heat shock proteins 25 and 72.

Heat shock proteins (HSP) 25 and 72 are expressed normally by surface colonocytes but not by small intestinal enterocytes. We hypothesized that luminal commensal microflora maintain the observed colonocyte HSP expression. The ability of the small intestine to respond to bacteria and their products and modulate HSPs has not been determined. The effects of luminal bacterial flora in surgically created midjejunal self-filling (SFL) vs. self-emptying (SEL) small-bowel blind loops on epithelial HSP expression were studied. HSP25 and HSP72 expression were assessed by immunoblot and immunohistochemistry. SFL were chronically colonized, whereas SEL contained levels of bacteria normal for the proximal small intestine. SFL creation significantly increased HSP25 and HSP72 expression relative to corresponding sections from SEL. Metronidazole treatment, which primarily affects anaerobic bacteria as well as a diet lacking fermentable fiber, significantly decreased SFL HSP expression. Small bowel incubation with butyrate ex vivo induced a sustained and significant upregulation of HSP25 and altered HSP72 expression, confirming the role of short-chain fatty acids. To determine whether HSPs induction altered responses to an injury, effects of the oxidant, monochloramine, on epithelial resistance and short-circuit current (I(sc)) responses to carbachol and glucose were compared. Increased SFL HSP expression was associated with protection against oxidant-induced decreases in transmural resistance and I(sc) responses to glucose, but not secretory responses to carbachol. In conclusion, luminal microflora and their metabolic byproducts direct expression of HSPs in gut epithelial cells, an effect that contributes to preservation of epithelial cell viability under conditions of stress.

Use of botulinum-A toxin for the treatment of refractory overactive bladder symptoms: an initial experience.

OBJECTIVES: To define the role of botulinum toxin type A (botulinum-A) intradetrusor injections in the treatment of patients with symptoms of bladder overactivity in whom previous anticholinergic therapy has failed. METHODS: Thirty-five patients (29 women and 6 men) with frequency, urgency, and/or urge incontinence received 300 U of botulinum-A toxin injected transurethrally at 30 sites within the bladder. Patients were evaluated at 3 weeks and 6 months after treatment by completion of the short forms of the Incontinence Impact Questionnaire (IIQ-7) and the Urogenital Distress Inventory (UDI-6), as well as questions assessing global response to the treatment. RESULTS: After 3 weeks, the mean IIQ-7 score decreased from 19.4 to 13.9 (P = 0.0006) and the mean UDI-6 score decreased from 16.8 to 12.8 (P = 0.0003). Overall, 21 (60%) of 35 patients reported slight to complete improvement of voiding symptoms after 3 weeks. Among the initial responders followed up for 6 months, the mean IIQ-7 score improved from 20.6 to 15.1 (P = 0.008) and the mean UDI-6 score improved from 16.9 to 13.5 (P = 0.008). Mild hematuria, pelvic pain, and dysuria were seen in 7 patients, lasting for 3 days or less after the procedure. CONCLUSIONS: Botulinum-A toxin injections may provide improvement in symptoms associated with bladder overactivity in a subset of patients. Improvement may be seen for at least 6 months after treatment. The procedure was well tolerated with minimal side effects. Additional study to define the role of injections in a broad group of patients with irritative voiding symptoms is warranted.

Region-specific adaptation of apical Na/H exchangers after extensive proximal small bowel resection.

After massive small bowel resection (MSBR), the remnant small intestine adapts to restore Na absorptive function. The possibility that this occurs through increases in cellular Na absorptive capacity was examined by assessing the regional effects of 50% proximal MSBR on the function and expression of the apical membrane Na/H exchangers (NHEs) NHE2 and NHE3. Morphometric analysis confirmed adaptive changes consistent with villus hypertrophy, particularly distal to the anastomosis. Villus epithelium prepared by light mucosal scrapings from 2-wk-postresected and -posttransected control rats exhibited comparable brush-border hydrolase activities, total cell protein per DNA, and villin expression but increased basolateral Na-K-ATPase activity. Parallel increases of two- to threefold in protein and mRNA abundance of NHE2 and NHE3 were observed only in ileal regions distal to the anastomosis of resected rats. Basolateral NHE1 expression was unchanged. After 80% resection, increases in NHE2 and NHE3 became evident in proximal colon. We conclude that increased enterocyte expression and function of apical membrane NHEs in regions distal to the anastomosis play a role in the adaptive process after MSBR. The increased luminal Na load to distal bowel regions after proximal resection may stimulate increases in apical membrane NHE gene transcription and protein expression.

Metabolic acidosis in rats increases intestinal NHE2 and NHE3 expression and function.

Chronic metabolic acidosis increases intestinal Na absorption, although through undefined mechanisms. Whether this occurs through enhanced expression and/or function of the brush-border Na+/H+ exchangers (NHE)2 and NHE3 is unknown. Metabolic acidosis was induced in rats by feeding ammonium chloride through their drinking water. Intestinal NHE activities were measured using brush-border 22Na+ uptake. Western and Northern blots measured changes in protein and mRNA expression, respectively. Acidosis occurred within 2 days of ammonium chloride feedings but increased after 6 days. NHE2 and NHE3 activities, protein expression, and mRNA levels increased in acidotic rats compared with controls. In contrast, basolateral NHE1 expression was not affected. Brush-border alkaline phosphatase showed no effect of metabolic acidosis on cellular differentiation. This study demonstrated a direct effect of metabolic acidosis on NHE2 and NHE3 activity, expression, and gene transcription. Metabolic acidosis is one of the few circumstances shown to affect NHE2 function and expression, thus providing insights into the role of NHE2 on intestinal physiology.

Computed tomography appearance of the Lapra-Ty and Weck hem-o-lok clips in patients who recently underwent laparoscopic urologic surgery.

The nonmetal suture securing Lapra-Ty clip (LTC) and vessel ligating Weck hem-o-lock clip (WKHL) are useful tools for the laparoscopic surgeon. We report the postoperative computed tomography (CT) scan appearance of these clips. The CT imaging of patients who underwent urologic surgery demonstrate the WKHL and LTC to be radiopaque. Urologists and radiologists should be aware of the radiographic appearance of the LTC and WKHL given the ease with which they could be confused with urinary lithiasis when applied near the urinary system.
