Massive rectus sheath hematoma.

Rectus sheath hematoma is an uncommon cause of acute abdomen and is known to mimic various common surgical conditions. Although the management is essentially conservative, occasionally a surgical exploration is necessary.

Pseudomonas orchitis in puberty.

Acute epididymo-orchitis is a common cause of 'acute scrotum' in adolescence and young adults, and the common causative pathogens are Chlamydia trachomatis and Neisseria gonorrhoeae. This is a rare case of acute epididymo-orchitis due to Pseudomonas aeruginosa in a pubertal boy with a history of 'ano-receptive' intercourse. On Medline search there are no reports of pseudomonas orchitis in this age group.

Conundrum of the cocoon: report of a case and review of the literature.

Idiopathic, sclerosing, encapsulating peritonitis, or abdominal cocoon, is a rare cause of bowel obstruction in the elderly. A 65-year-old male patient presented with acute bowel obstruction caused by torsion of the encapsulated small bowel. He was treated successfully with subtotal excision of the fibrocollagenous membrane and small bowel resection. Despite anecdotal reports of a preoperative diagnosis, in the majority of cases, sclerosing, encapsulating peritonitis is a fortuitous finding. A better awareness of this condition may facilitate preoperative diagnosis.
