Enhanced release of IL-6 and IL-8 into tears in various anterior segment eye diseases.

OBJECTIVE: To determine the levels of interleukin 6 (IL-6) and interleukin 8 (IL-8/CXCL-8) in tears collected from the eyes of normal individuals and of patients with different irritative eye diseases, in order to acquire information on the immunological changes occurring during the early postoperative period following various forms of eye surgery, including penetrating keratoplasty (PKP). METHODS: IL-6 and IL-8 levels were measured with the aid of human ultrasensitive ELISA kits in the non-stimulated tears of patients in the early postoperative period following PKP or cataract operation, and of patients with acute bacterial conjunctivitis or with a corneal foreign body. The IL-6 and IL-8 concentrations, the total amounts released in a given time and the rates of their release were calculated. RESULTS: A significant increase in IL-6 release was observed in all patient groups compared with the normal controls (p < or = 0.003). The IL-8 release levels were significantly higher in the tears of all patient groups (p < or = 0.03), except for the cataract operation group, where the IL-8 release was not significantly higher (p = 0.053) than in the control samples. No significant differences in IL-6 or IL-8 release were observed when the various patient groups were compared with each other. CONCLUSION: The release of IL-6 and IL-8 into the tears is enhanced in various anterior segment eye diseases, and this may be used as an indicator of various inflammatory reactions in the early postoperative period.

Microcornea associated with myopia.

BACKGROUND: The presentation of microcornea associated with axial myopia by means of modern examination techniques. METHODS: Clinical investigations. RESULTS: The horizontal diameter of the cornea was 8.00 mm on both sides. The average refractive power of the cornea was 39.27D/38.48D on the right/left sides by corneal topography. The central corneal thickness was 568 micro m/559 micro m on the right and left sides, respectively. The depth of the anterior chamber was 1.18 mm/1.14 mm and the origin of the irises was steep, as demonstrated by ultrasound biomicroscopy (UBM). The axial length was 26.42 mm/25.63 mm on the right/left sides. Endothelial morphology disclosed degeneration on both sides. CONCLUSIONS: The present case demonstrates that the clinical signs of microcornea are flat corneal surface, normal thickness, and degenerated endothelium. This disorder associated with axial myopia is an extremely rare ophthalmologic condition.

Pseudophakic accommodation and pseudoaccommodation under physiological conditions measured with partial coherence interferometry.

PURPOSE: To distinguish pseudophakic accommodation from pseudoaccommodation by measuring the physiologically and pharmacologically induced anterior chamber depth (ACD) shifts. SETTING: Department of Ophthalmology, University of Debrecen, Debrecen, Hungary. METHODS: This study comprised 100 pseudophakic eyes of 79 patients. Forty patients (Group 1) received the AcrySof MA60AC intraocular lens (IOL) (Alcon Laboratories), 50 patients (Group 2) received the SA60AT IOL (Alcon Laboratories), and 10 patients (Group 3) received the apodized diffractive SA60D3 ReSTOR IOL. Visual function was evaluated a mean of 10.2 months +/- 9.2 (SD) postoperatively, and the total pseudoaccommodative amplitude was determined with a defocusing technique. To distinguish pseudophakic accommodation from pseudoaccommodation, ACD measurements were performed using partial coherence interferometry during distance fixation and physiologic accommodation after pharmacologic relaxation of the ciliary muscle. RESULTS: Best corrected distance and near visual acuities were similar in the 3 groups (P = .75 and P = .08, respectively). Distance corrected near visual acuity was significantly better in Group 3 (P < .001), with all eyes achieving J1 or better. Three percent in Group 1 and 8% in Group 2 achieved J1 or better. Subjective accommodation was similar in Groups 1 and 2 (-0.82 +/- 0.18 diopter [D] and -1.00 +/- 0.35 D, respectively; P = .3). Group 3 had an accommodation curve with 2 peaks. Intraocular lens movement differences between the groups were not significant (physiologic stimulus: P = .07; cyclopentolate: P = .46), and significant ACD shifts from baseline were not detected (physiologic stimulus: P = .14; cyclopentolate: P = .10). CONCLUSIONS: Pseudoaccommodative amplitude of the investigated monofocal IOLs was independent of IOL movement. Anterior shift did not affect good near visual acuity with the AcrySof ReSTOR IOL.

Combined local treatment and chemotherapy in the management of bilateral retinoblastomas in Hungary.

This retrospective study supports that brachytherapy combined with cytostatic therapy may effectively contribute to tumor control. The second eye of 13 patients with bilateral retinoblastoma was treated with plaque brachytherapy after the enucleation of the first eye. Eleven patients received systemic cytostatic therapy. The mean follow up was 60 (+/-42 SD) months. Twelve patients are alive and free of tumor, 8 of them with acceptable visual acuity. After cytostatic therapy, late nephropathy was observed in 2 patients. In conclusion, brachytherapy combined with cytostatic therapy will be an option in the management of bilateral retinoblastoma after the enucleation of the first eye.

Apoptotic response of uveal melanoma cells upon treatment with chelidonine, sanguinarine and chelerythrine.

The benzophenanthridine alkaloids sanguinarine, chelerythrine and chelidonine were reported previously to provoke cell death in a variety of tumor cells suggesting their potential application as anticancer agents. Here we tested their effects on a primary human uveal melanoma cell line, OCM-1. Flow cytometric analysis of annexin V binding/PI exclusion and DNA fragmentation disclosed that all these alkaloids could induce apoptosis in OCM-1 cells. Moreover, necrotic cell death was also observed upon alkaloid treatment. As it was also evidenced by light microscopic inspection of cellular morphology, chelidonine primarily caused apoptosis, while sanguinarine and chelerythrine were effective via a so-termed bimodal cell death (apoptosis and primary necrosis). The relative efficiencies of the two modes depended on the applied dose. This study is the first implication for the possible use of these alkaloids in the therapy of uveal melanomas, for which no really efficient therapeutic regimen is available so far.

[Radiotherapy of intraocular tumors with Ruthenium-106-containing, beta-emitting ophthalmic applicators. Experiences with treatments performed between 1986 and 1999 in Hungary].

AIM: Till October 15, 2004, 312 patients were irradiated with Ruthenium-106-containing ophthalmic applicators at the Department of Ophthalmology, Medical Faculty, University of Debrecen. In this paper we report on experience gained with the irradiation of 187 patients, treated between 1986 and 1999, whose follow-up time interval was at least 5 years. PATIENTS AND METHODS: The distribution according to diagnosis of the patients irradiated between 1986 and 1999 were: choroidal melanoma 148, retinoblastoma 15, retinal angioma 9, carcinoma metastasis 4, subretinal neovascularization 11. The instruments purchased from the German Bebig firm were sutured under surgical circumstances onto the sclera, and were removed from the eye with another operation. The applied dose calculated to the inner surface of the tumors was 100 Gy in the case of choroidal melanomas, and 50 Gy in the case of retinoblastomas. RESULTS: From the patients irradiated because of malignant intraocular tumors 154 (95.7%) were evaluated. From the evaluated patients 148 (96.1%) are alive. Among the living patients 4 (2.7%) is suffering from distant metastasis. From the irradiated eyes 148 (96.1%) could be saved. Six eyes had to be enucleated, 1 eye because of phthisis bulbi, 2 because of hemorrhagic glaucoma, and 3 because of uncontrollable tumor growth. DISCUSSION: With the help of radioactive isotope-containing applicators the intraocular malignant tumors can be destroyed, so that the eyes can be saved with useful vision, without risking the patients' life.

Membrane topography of HLA I, HLA II, and ICAM-1 is affected by IFN-gamma in lipid rafts of uveal melanomas.

The lateral distribution and colocalization of HLA I, HLA-DR, and ICAM-1 proteins was studied for the first time in the plasma membrane of two human uveal melanoma cell lines, OCM-1 and OCM-3. Our fluorescence resonance energy transfer and confocal laser scanning microscopic experiments revealed that these molecules are mostly confined to the same membrane regions, where they form similar protein patterns (homo- and hetero-associates) to those found previously on other cell types of lymphoid as well as colorectal carcinoma origin. Confocal microscopic colocalization experiments with GM(1) gangliosides and the GPI-anchored CD59 molecules showed enrichment of HLA I, HLA-DR, and ICAM-1 molecules in specific membrane domains (lipid rafts) excluding the transferrin receptor. IFN-gamma remarkably increased the expression levels of these molecules and rearranged their association patterns, which can affect the adoptive immune response of effector cells.

Neurofibromatosis type 1 associated with central nervous system lymphoma.

PURPOSE: To report the occurrence of central nervous system (CNS) lymphoma in a patients with neurofibromatosis type 1 (NF1). DESIGN: Interventional case report. METHODS: A 47-year-old male with a parieto-occipital lobe lesion was referred for ocular evaluation. Skin, brain, and vitreous biopsies were performed. RESULTS: The discovery of Lisch nodules of the iris prompted a physical examination that revealed cutaneous stigmata of NF1. Biopsy of the CNS lesion revealed diffuse large B-cell lymphoma. Biopsy of papules on the back and abdomen were compatible with neurofibroma. No malignant cells were found in the vitreous specimen. A clinical diagnosis of NF1 was made. CONCLUSION: This is the first report of CNS lymphoma in a patient with NF1. Additional reports may shed more light on the possible association between the two conditions.

Multiple sclerosis-associated uveitis.

BACKGROUND/AIMS: To describe the clinical characteristics and course of 16 patients with uveitis associated with multiple sclerosis (MS). METHODS: The records of 1254 patients with uveitis were reviewed. Sixteen of these patients had MS. The history, review of systems, ocular findings, and clinical test results of each of these 16 subjects were analyzed. The mean follow-up time was 38 months. RESULTS: Most patients with MS-associated uveitis were white females between 20 and 50 years of age. The diagnosis of MS preceded the onset of uveitis in 56%, followed it in 25%, and was made concurrently in 19% of the cases. In 94%, the uveitis was bilateral. Pars planitis was the most frequent form of uveitis in our study population (81%); concomitant anterior chamber inflammation was common and was granulomatous in nature 56% of the time. Forty-one percent of the eyes with MS-associated uveitis had 20/30 or better initial visual acuity. Among these treated patients, significant loss of visual acuity was uncommon. CONCLUSIONS: MS-associated uveitis should be suspected in white female patients with bilateral uveitis, especially if pars planitis is present. These patients often retain useful vision for many years if treated.

Urokinase-type plasminogen activator to prevent haze after photorefractive keratectomy, and pregnancy as a risk factor for haze in rabbits.

PURPOSE: To observe the effect of urokinase-type plasminogen activator (uPA) on the development of subepithelial haze after photorefractive keratectomy (PRK) and to assess pregnancy as a risk factor for haze. METHODS: In 30 rabbits, both eyes of 27 were subjected to PRK and both eyes of 3 served as the nonsurgical control. In the first part of the experiment, for 7 days after PRK, three rabbits (one was pregnant) received aprotinin in one eye and no aprotinin in the contralateral eye. uPA activity was measured by a spectrophotometric method from tear samples in these eyes. In the second part, for 5 days after PRK, 24 rabbits (8 were pregnant) were treated with uPA in one eye and no uPA in the contralateral eye. Haze grading was performed according to the system of Hanna. RESULTS: In the first experiment, the aprotinin-treated eyes and the aprotinin-untreated eye of the pregnant rabbit showed development of haze. In the second, there were clear corneas in 24 of 24 uPA-treated eyes and in 15 of 24 uPA-untreated eyes. Post-PRK haze formed in 9 of 24 uPA-untreated eyes (7 of the 9 haze observations in pregnant rabbits). Within the uPA-untreated group, haze formed in corneas of 7 of 8 pregnant versus 2 of 16 nonpregnant rabbits. There was a strong association between the uPA treatment and clear corneas (P = 0.003) and between pregnancy and haze (P = 0.002). CONCLUSIONS: The present results suggest that pregnancy is a risk factor for post-PRK haze in untreated rabbit eyes and that uPA is effective in preventing the occurrence of haze.

Plasminogen activator activity and inhibition in rabbit tears after photorefractive keratectomy.

Plasminogen activator is a normal component of tear fluid that plays a role in corneal wound healing processes. This work examines whether inhibitor-induced low levels of plasminogen activator activity (PAA) during corneal re-epithelialization after excimer laser photorefractive keratectomy (PRK) correlates with the eventual occurrence of haze in rabbit eyes. Tear samples were collected with glass capillaries from 16 eyes of eight New Zealand rabbits, using i.m. injection of pilocarpine hydrochloride for stimulation. Tears were collected before and after PRK surgery, and then daily for 5 days, and every fourth day thereafter for 3 months. Both eyes underwent PRK treatment. One eye of each rabbit was treated as a control while the contralateral eye was treated with aprotinin, a serine protease inhibitor, over the first 7 days. PAA in the tear samples was measured by a spectrophotometric method using human plasminogen and chromogenic peptide substrate S-2251. For the eight control eyes after PRK, the PAA values were significantly lower (day 1) and higher (days 2 and 3) than the equilibrium PAA (p<0.001). The corneas remained clear in each of these control eyes. For the eight contralateral aprotinin-treated eyes after PRK, the PAA values on days 1-7 were significantly lower than the equilibrium PAA (p<0.001). All eight of these aprotinin-treated eyes developed corneal haze after 2 months. There was no significant difference (p=0.06) between control and aprotinin-treated eyes for the equilibrium PAA after 19 days. We conclude that a corneal wound healing abnormality (haze) develops in rabbit eyes after PRK when PAA levels are reduced using aprotinin for a week following PRK.

[Two cases of Terrien's marginal degeneration treated with peripheral full thickness keratectomy, and followed-up by computer-assisted corneal topography].

BACKGROUND: In 1970 Alberth and Süveges published a method, peripheral full thickness keratectomy, for the treatment of Terrien's marginal degeneration of the cornea. They hypothesized that this type of surgery acts by excising the thinned and extended corneal parts and by restoring normal corneal surface and curvature. METHOD: The right eyes of two patients with Terrien's disease were operated with full thickness keratectomy at our Department, and were followed-up by computer-assisted computer topography. RESULTS: The corneal surface became more regular, astigmatism smaller and fully correctable, visual acuity improved, and the results did not change between 1 and 3 years after surgery in either of the two cases. CONCLUSION: Peripheral full thickness keratectomy, in our experience, proved effective in reducing astigmatism, in improving visual acuity, and in stopping the progression of the disease. Corneal topography can be used in the diagnosis of Terrien's disease, as well as in the postoperative follow-up of patients, giving more information than simple keratometry.

Healon5 tamponade of corneal perforation during transplantation surgery.

In "a chaud" keratoplasty in cases of corneal perforation, the main difficulty is the trephination of the recipient cornea in a soft eye. A temporary tamponade of the perforation can be provided by filling the anterior chamber with sodium hyaluronate 2.3% (Healon5). The cornea can then be grasped with vacuum and trephinated.
