[Luxation of the prothesis after total hip arthroplasty].

In this report we evaluated the incidence of complications following the total hip arthroplasty. We have focused on the role of the surgical approach as thepatient cause of the luxation of the prosthesis. Retrospective analysis of 393 endoprosthetic surgeries performed in the Department of Orthopedics and Traumatology, Pomeranian Medical University, between April 2002 and December 2003 was conducted. Out of 9 patients with prosthesis luxation 6 patients were operated using the posterior approach. A comparative analysis of selected groups did not reveal any significant differences in clinical condition of the patient, the implantation technique or the type of the applied prosthesis. We suggest that the main cause of prosthesis luxation were mistakes that led to misaligment of the prosthetic elements, and the posterior approach was believed to be the the predisposing factor.

[Assessment of cytokines in the bone fracture union complications].

The opinion on the usefulness of selected genetic examinations in connective tissue diseases is reported. Particular attention was paid to healing of fractures especially in respect of pathological aspects of accretion processes. The reasons for a delay in or a lack of bone fracture union process have not been fully explained--in spite of several reports relating to this type of examination. There are a number of egzogenic and endogenic agents including cytokines which regulate these processes. Up to date views on the suspected influence of cytokines on genetic polymorphism are discussed in present paper. Genetic polymorphism of some cytokines may increase the risk of post-menopausal osteoporosis. It seems that genetic factors are likely to cause complications of fracture healing.

[The prevalence and character of neurologic deficit relating to clinical forms of cervical spondylosis].

The research was performed on 86 male patients between 41 and 83 years who were either hospitalised or treated in the out-patients clinic due to cervical spondylosis. The aim of the research was to evaluate the frequency and advance of neurologic, complications relating to the type and severity of lesions in cervical spine. The study showed significant relation between the degree of advance of cervical spondylosis and age, pain as well as prevalence and severity of neurologic deficit in the tested group. The authors have emphasized the need of precise evaluation and differentiation of presented forms of degenerative spine disease.

[The role of intramedullary nailing in polytraumatized patients].

The authors present the results and methods of long bone fracture treatment by means of intramedullary nailing in polytraumatized patients. The study involved 34 patients hospitalized at The Department of Orthopaedics and Traumatology of Pomeranian Medical University of Szczecin between 2001 and January 2003. The results were estimated both in early period and 12 months after surgery. One year treatment period gave us full recovery and mobility of operated limbs in 20 patients, in 9 there was inability to move resulting from coexisting brain injury, 3 of the whole group presented with bone infections and 5 died due to the extent of trauma.

[Neurological complication after total hip replacement].

The study was performed to evaluate neurological complication of 562 patients who had undergone primary total hip replacement. Lesions of sciatic, femoral and common peroneal nerve occurred in 18 patients (3.2%). Paresis of peroneal nerve was demonstrated in 15 patients. Within this group 7 patients had elongation of the extremity above 4 cm, and 8 patient had it in the range of 2-3 cm. Paresis of common peroneal nerve took place after direct compression of region of fibular head. Lesion of femoral nerve happened twice. In the group of 14 patients clinical symptoms of nerve paresis healed spontaneously in 6-12 month time. In the group of 4 patients (0.7%) constant lost of dorsal flexion of the foot was observed.

[Reconstruction of humeral head retroversion in the treatment of anterior shoulder instability].

Reduction of the physiologic retroversion of the humeral head can lead to shoulder luxation. After shoulder luxation it can also lead to shoulder instability. The aim of the paper was to check the level of humeral head retroversion in patients with anterior shoulder instability and to estimate the effectiveness of treatment of this instability with the derotational Weber osteotomy. The study included 38 patients, 34 males and 4 females, ranged 16-48 yr. (mean 31) treated in our Clinic. Every patient was done CT in order to estimate the level of humeral head retroversion. Patients with reduced humeral head retroversion qualified for operational treatment with Weber method derotational osteotomy. The outcomes were estimated according to the Rowe and Zarins score. 34 patients had excellent or good results. Pain was retreated, patients could come back to their normal activities, before the first luxation. Only 4 patients had another luxation in spite of our treatment. The control CT showed that the humeral head retroversion was 30 or more degrees. The arthroscopy was performed, the Bankart lesion was found and the patients were qualified for treatment this lesion. Weber osteotomy could be the satisfactory method of treatment of anterior shoulder instability in patients with physiologic reduction of humeral head retroversion.

[Early abductor muscle strength after total hip replacement performed by antero-lateral approach].

Between November 2000 and April 2001 31 patients with unilateral coxarthrosis were assessed in order to determine strength of the hip abductors and hip mobility 2 days prior surgery, 10 and 14 days post surgery. All patients underwent total hip replacement though an antero-lateral approach. It was found that maximal and average muscle strength decreased during the first 2 weeks post-op, with concomitant increase in time needed to achieve full muscle strength and hip mobility.

[Chronic, aortic aneurysm rupture as one reason for retrospinal back pain].

The authors present the issue of rare causes of retrospinal back pain. Particular attention has been given to abdominal aortic aneurysm--it's clinical picture, diagnostic problems and treatment modes. A retrospective evaluation of 65 operated patients was performed. Diagnostic problems in a 66-year-old patient are also presented: the final diagnosis was determined at the time of surgery, although a low back pain treatment regimen had been formerly applied to this patient. The authors emphasise the frequent occurrence of atypical clinical signs accompanying abdominal aortic aneurysm, frequent correlation with degenerative changes of the spine, and the importance of differential diagnostics in patients with atypical symptoms of low back pain.
