[The comparative study on selective alpha1-blocker and behavioural therapy in the treatment of discoordinated voiding in children].

PURPOSE: To compare the efficiency of selective alpha1-blocker and behavioural therapy in the treatment of detrusor-sphincter discoordination in children. MATERIALS: Prospective, randomized study, approval of Ethics Committee, group of 60 children, age: 5 to 17, detrusor-sphincter discoordination. All children had a history of: recurrent UTI, irregular mictions with or without incontinence in "voiding diary". All have normal upper urinary tract in USG. Dicoordination was diagnosed by pathologic uroflowmetry curve and results with significant postvoiding residual urine in USG. Treatment group 1 (n=30): treatment with selective alpha1-blocker (doxazosin) in age related dosage. Group 2 (n=30): behavioral therapy and rehabilitation with conducting "voiding diary" with timed voiding. All children were also instructed on proper toilet posture, relaxation of the pelvic-floor muscles. After 6 weeks: changes in voiding patterns: No. of micturitions/day, average voided volume were estimated. Uroflowmetry was performed and voided volume, Av. flow rate, shape of the curve (1=normal, 2=flatened, irregular, 3=portions), residual urine (10% of Void.vol.) were calculated. Urinalysis and urin culture were obtained. RESULTS: Of 60 children 57 completed the study: Group 1 (n=30), Group2 (n=27). In Group 1 (alpha1-blocker): improvement in voiding patterns n=21, improvement in UF parameters n=20. In Group 2 (rehabilitation): improvement was estimated in 17 patients. Side effects were noted in 6 children from Group 1 (alpha1-blockers) (headache, hypotonia, vertigo, epistaxis), and non from the Group 2 (rehabilitation). CONCLUSIONS: Both treatment methods seemes to be effective in detrusor-sphincter discoordination. Selective alpha1-blockers alone are more effective than training and rehabilitation exercises.

[Wegener's granulomatosis--diagnostic problem].

UNLABELLED: Wegener's granulomatosis there is a small--and middle--vessels vasculitis. The pathomorphological diagnostic criteria is known as Wegener's triad: 1) necrotizing granlomatous inflammation of upper and/or lower respiratory tract, 2) systemic or focal necrotizing vasculitis involving arteries and vein, and 3) focal segmental necrotizing crescentic gromerulonephritis. According to the current theory of pathogenesis of Wegener's granulomatosis, Staphylococcus aureus is involved. The rise in ANCA level during vascular inflammation is very important for monitoring disease. The main localization of Wagener's granulomatosis is in lung, nasal sinusites, nose, kidneys and nasal pharynx. MATERIAL AND METHODS: The history of 60-years old female patient who was being treated in Department of Otolaryngology for chronic bilateral otitis media for many years is presented. During hospitalization the sinusites, rhinitis and renal failure additionally were found. The CT sinuses and head, histopatological and ANCA exam in patient were performed. RESULTS: Based on clinical, physical, additional exam Wegener's granulomatosis was diagnosed. CONCLUSIONS: 1) Surgical procedure is not indicated method of treatment of Wegener's granulomatosis, because it can increase the pathologic process. 2) The prognosis of Wegener's granulomatosis depends mainly on the ability to diagnose the early disease and the aplication of adequate treatment.

[Diagnosis and treatment of thyroid cancer in children in the multicenter analysis in Poland for PPGGL].

INTRODUCTION: Differentiated thyroid carcinoma (DTC) in children presents different biological behavior in comparison to adults. Authors presents preliminary results of multicenter analysis concerning incidence, diagnostics and treatment of DTC in children. MATERIAL AND METHODS: The study is a retrospective analysis of 107 pediatric patients from 14 academic centers based on the data from 2000 to 2005 obtained by questionnaire in hospitals involved in the treatment of DTC in children. RESULTS: Papillary thyroid cancer was diagnosed in 83 children, follicular thyroid cancer in 10 children and medullary thyroid cancer in 14 children. Incidence of DTC in children was estimated between 18 and 23 cases per year. The biggest group of patients consisted of children between 11 and 15 years of age, with girls to boys ratio 3.3 : 1. Clinically DTC in children presented most often as solitary thyroid nodule. Cervical lymphadenopathy was observed in 42% of patients. Intraoperative verification indicated metastatic nodes in 50% of children. Low stage DTC predominated (T1 in 36% and T2 in 26% of children). One step surgery was performed in 65% of children with DTC, two step surgery in 25% of patients. I131 therapy was undertaken in 80% of children. Lung metastases were indicated in post therapeutic studies in 14% of children with DTC. Prophylactic thyroidectomies were performed in 79% of children in the group of patients with MTC and RET gene mutations. CONCLUSIONS: The necessity of introduction of unified therapeutic standard in children with DTC in Poland is underlined.

[Evaluation of intensity of angiogenesis in granulation tissue in chronic otitis media--preliminary report].

In chronic otitis media molecular pathogenetic mechanisms are still unknown, however, angiogenesis may play a role. The aim of the study was to determine the intensity of angiogenesis in granulation tissue in chronic otitis media of different clinical course. Twenty six granulation tissue specimens (twenty two--from chronic otitis media, four--from prolongated otitis externa) taken during surgery were analyzed. The angiogenesis (microvessel density) was measured in paraffin-embedded tissue by an immunohistochemical method, by staining for endothelial cells with a monoclonal antibody against CD 34. The presence of CD 34 was found in all specimens. The expression was more intense in tissue samples from the group with good clinical course (good healing and without recurrence) than those in the group with poor healing and recurrence (mean number of dots for mm2 589,2 vs 533,3, respectively) and from the group without bacterial infection by Pseudomonas aeruginosa than those with this infection (mean number of dots for mm2 645,5 vs 440,8, respectively), but differences were not significant. In conclusion it is suggested that angiogenesis may contribute to different clinical course of chronic otitis media.

Proton translocation in monomolecular Langmuir-Blodgett films including 2-naphthol and 1,4-anthraquinone derivatives.

The scope of the present work is the investigation of proton transport through monomolecular Langmuir-Blodgett (LB) films. The films were formed from amphiphilic molecules: 2-naphtholo-6-sulfonamide of dodecylamine (N) and 1,4-anthraquinono-2 sulfonamide of dodecylamine (A). The 2-naphthol derivative can act as a proton donor due to excited state proton transfer (ESPT) and the 1,4-anthraquinone group can play the role of proton acceptor because of protonation of the reduced form if it is present. Absorption and emission spectra of LB films containing N and A were registered and separated into component bands. Individual absorption and emission peaks observed were assigned to given forms of chromophores. The behavior of different component bands reflects the state of anthraquinone dependent on proton concentration. A correlation of rate and efficiency of ESPT, with changes of the spectra of A, may be expected to yield information concerning the transport of protons from N to A. The influence of the donor-acceptor distance, sample humidity, film arrangement and the presence of protonophores (Gramicidin A) on proton transfer is studied. Our results indicate that the proton can be transported through the film but its concentration vanishes at the distance greater than 30 A. The efficiency of proton transfer depends strongly on water content, film structure and the presence of ion channels.

The action of photosensitizers and serum in a bactericidal process. II. The effects of dyes: hypericin, eosin Y and saphranine O.

The aim of the present work was to recognize the reasons for differences in the photodynamic action of dyes against various bacterial strains. It is expected that a better understanding of this problem may help in design of new photosensitizers. The sensitivity of 6 various bacterial strains to the photodynamic action of 5 photosensitizers was determined. The hydrophobicity of cell surface and susceptibility of bacteria to the natural defense mechanism of human serum, were estimated. The differences in the photodynamic efficiency of dyes could be contributed to various affinities of cell membrane to dyes, to known details of membrane architecture as well as to different mechanisms of photosensitization.

[Rare tumours--are they really rare in the Polish children population?].

Clear statement that pediatric neoplasms are really rare is not easy. Thus the incidence of rare tumours in children has not been defined so far. The paper efforts to assess the topic of rare tumours of childhood in the Polish population. Following two categories are proposed: tumours typical for adults, but possible in children (neoplasms of epithelial origin--mainly carcinomas, melanomas, carcinoids) and paediatric tumours consisting less than 10% of cases in corresponding clinical groups according to the ICCC classification. Data on 317 patients aged 0-18 years treated in centres associated in the Polish Paediatric Group for Solid Tumours (PPGST) were analysed. Classical adult malignancies were registered in 130 patients: carcinomas in 90 (mean age 12.6 +/- 4.5 years), melanomas in 25 (mean age 9.4 +/- 4.9) and carcinoids in 9 (mean age 14.5 +/- 1.2 years). Non epithelial neoplasms were registered in 187 patients (mean age 10.4 +/- 5.5). That group included rare tumours of soft tissue, CNS, bones and other organs. Treatments of certain groups were specified by separate therapeutic protocols within PPGST. Rare malignancies of adult-type among children under 18 years of age in Poland comprised 1.5% of all pediatric neoplasms. The incidence of adult-type neoplasms increased with age until 14 years. In patients over 15 years of age the number of registered cases decreased. It may suggest a first peak of incidence in early adolescence or an underestimation of number of patients with carcinoma aged over 15 years. In the analyzed group, the mean age of patients with carcinomas and other epithelial and unspecified tumours significantly exceeded the age of children with rare neoplasms of non-epithelial origin (12.1 +/- 4.7 vs 10.4 +/- 5.5 years; p<0.05). A very young age at diagnosis of malignant melanomas (mean 9.4 years) and numerous cases of carcinomas affecting the digestive tract (n=24; 27% of all carcinomas), especially those located in colorectal region (n=10), seem surprising. The preliminary analysis of the collected data on rare neoplasms in Poland encourage to undertake a prospective study, meant to link the epidemiology and characteristics of rare epithelial tumours in childhood with diagnostic and therapeutic suggestions for these types that are not coordinated within Polish Paediatric Group of Solid Tumours.

[Neurilemmoma of the neck--case report].

Neurilemmoma is a mild tumor growing from the Schwann's cells of the external part of the nerve trunk. Authors, in their works describe a case of a 32-year-old man with a neck tumor on its right side. The tumor was identified as the neurilemmoma on the grounds of a postoperative histopathological examination. Thin-needle biopsy that was made twice during the history of the patient that lasted two years did not reveal the character of the tumor.

[The influence of Aronia Melanocarpa Elliot and acetylcysteine on selected biochemical parameters of experimental animals with chronic lead acetate poisoning].

Lead mostly shows that it accumulates in bones in an insoluble phosphate form. Its toxicity grows with the deficiency of calcium, calciferol, irons and coppers in food. The examination was carried out on 40 female and male Wistar rats weighing 200-250 g. They lived in the animal quarters with a stable temperature and humidity. They were fed with standard fodder (Murigan) and water ad libitum. In the work carried out the effect of anthocyanins of Aronia Melanocarpa Elliot and acetylcysteine on the selected biochemical parameters of experimental animals with chronic lead acetate poisoning were examined. After administration anthocyanins statistically important substantially decreased the concentration of the products of the unsaturated fatty acids oxidation in urine and examined organs. Those anthocyanins also decreased excretion of delta-aminolevulinic acid and 8-hydroxy-2-deoxyguanosine in urine.

Contrasting requirements for ubiquitylation during Fc receptor-mediated endocytosis and phagocytosis.

Fc receptors on leukocytes mediate internalization of antibody-containing complexes. Soluble immune complexes are taken up by endocytosis, while large antibody-opsonized particles are internalized by phagocytosis. We investigated the role of ubiquitylation in internalization of the human FcgammaRIIA receptor by endocytosis and phagocytosis. A fusion of FcgammaRIIA to green fluorescent protein (GFP) was expressed in ts20 cells, which bear a temperature-sensitive mutation in the E1 ubiquitin-activating enzyme. Uptake of soluble IgG complexes mediated by FcgammaRIIA-GFP was blocked by incubation at the restrictive temperature, indicating that endocytosis requires ubiquitylation. In contrast, phagocytosis and phagosomal maturation were largely unaffected when ubiquitylation was impaired. FcgammaRIIA-GFP was ubiquitylated in response to receptor cross-linking. Elimination of the lysine residues present in the cytoplasmic domain of FcgammaRIIA impaired endocytosis, but not phagocytosis. The proteasomal inhibitor clasto-lactacystin beta-lactone strongly inhibited endocytosis, but did not affect phagocytosis. These studies demonstrate a role for ubiquitylation in the endocytosis of immune receptors, and reveal fundamental differences in the mechanisms underlying internalization of a single receptor depending on the size or multiplicity of the ligand complex.

Modulation of the cytosolic and phagosomal pH by the NADPH oxidase.

Proton (equivalents) are primary participants in the control and potency of the NADPH oxidase. Both the cytosolic and intraphagosomal pH are influenced during oxidase activation, and maintenance of the optimal environment requires the coordinated action of a series of sophisticated, highly regulated H(+) transporters, including the Na(+)/H(+) exchange, vacuolar H(+)-ATPase, and H(+)-conductive pathway(s). In addition, protons that are produced during some of the NADPH oxidase reactions then are substrates for the dismutation of superoxide, which precedes production of additional bactericidal agents. In this review, pH homeostasis is shown in conjunction with the NADPH oxidase to present an integrated picture of leukocyte physiology during the phagocytic response.

[The effect of anthocyanins from Aronii melanocarpa and acetylcysteine on selected after-effects of lead acetate poisoning].

In recent years we have widely examined the results of protracted exposures on lead. Lead mostly shows that it accumulates in bones in an insoluble phosphate form. In much of the experimental research we observed that lead can cause reactive forms of oxygen and oxidative stress. The examination was carried out on 40 female and male Wistar rats weighing 200-250 g. They lived in the animal quarters with a stable temperature and humidity. They were fed with standard fodder (Murigan) and water ad libitum. In the work carried out the effect of anthocyanins of Aronia Melanocarpa Elliot and acetylcysteine on the selected parameters of oxidative stress of experimental animals with chronic lead acetate poisoning were examined. After administration anthocyanins substantially decreased the concentration of unsaturated fatty acid peroxidation and carbonyls in blood serum. At the same time we observed a significant decrease of 8-hydroxy-2'-deoxyguanosine in urine. Antocyanins from Aronia Melanocarpa appear to be a good substance for the diminishing of oxidative stress, these results caused with long-term administration of lead acetate.

Anthocyanins--an adjunct to cardiovascular therapy?

BACKGROUND: Anthocyanins are one of the most important water-soluble plant pigments. They belong to flavonoids and are derivatives of 2-phenylo-benzo-gamma pyren. They have antioxidant and anti-inflammatory properties and, therefore, may be potentially used to combat oxidative stress, frequently present in cardiovascular diseases. AIM: To assess the effects of anthocyanins from chokeberry (Aronia Melanocarpa) on some parameters of oxidation-reduction balance in an animal model. METHODS: Of 20 male Wistar rats, 10 received for 3 months pure water, and the other rats 10-100 mg/l of anthocyanins from Aronia melanocarpa. Afterwards, blood samples were collected for assessment of the (1) content of substances reacting with thiobarbitural--TBARS, (2) antioxidant status and glutathione peroxidase activity, (3) concentration of sulphydryl groups, and (4) nitrite concentration. RESULTS: Anthocyanins significantly reduced the content of TBARS and thiol protein groups and non-significantly increased glutathione peroxidase activity, total content of antioxidants and nitrite concentration. CONCLUSIONS: Anthocyanins from chokeberry decrease lipid peroxidation which may be potentially used to combat oxidative stress.

Determinants of the phagosomal pH in neutrophils.

Phagosomes formed by neutrophils are much less acidic than those of other phagocytic cells. The defective acidification seen in neutrophils has been attributed to consumption of protons during the dismutation of superoxide, because a large, sustained acidification is unmasked when the cells are treated with inhibitors of the NADPH oxidase. Consumption of protons transported into the phagosome by dismutation would tightly couple the activities of the NADPH oxidase and the vacuolar type H(+)-pump (or V-ATPase). We tested the existence of the predicted coupling using microfluorimetry and digital imaging and found that the rate of superoxide generation was independent of the activity of the H(+)-pump. Moreover, we failed to detect the alkalinization predicted to develop through dismutation when the pump was inhibited. Instead, two other mechanisms were found to contribute to the inability of neutrophil phagosomes to acidify. First, the insertion of V-ATPases into the phagosomal membrane was found to be reduced when the oxidase is active. Second, the passive proton (equivalent) permeability of the phagosomal membrane increased when the oxidase was activated. The increased permeability cannot be entirely attributed to the conductive H(+) channels associated with the oxidase, since it is not eliminated by Zn(2+). We conclude that the NADPH oxidase controls the phagosomal pH by multiple mechanisms that include reduced proton delivery to the lumen, increased luminal proton consumption, and enhanced backflux (leak) into the cytosol.

[Malignant vascular tumours in children -- report from the Polish Pediatric Rare Tumors Study].

UNLABELLED: Malignant vascular tumours represent a rare group of neoplasms, usually treated according to protocols for soft tissue sarcomas. THE AIM OF THE STUDY: To assess the clinical characteristics, disease course and outcome in Polish children with malignant vascular neoplasms. MATERIAL AND METHODS: The Polish Paediatric Rare Tumours Study retrospectively analysed multicentre data on 32 children with vascular tumours, registered between 1992 and 2002. On the basis of the histological type of the neoplasm these patients have been divided into three groups: group I -- 10 patients with angiosarcoma (ASA), group II -- 7 children with haemangioendothelioma (HE) and group III- 15 patients with haemangiopericytoma (HP), of both infantile (7 children) and adult-types (8 patients). RESULTS: Group I: 5 patients presented with local, 3 with systemic and 2 with regional disease. Primary complete resection (PRC) was performed in 7 patients, not resulting in local control in any of them. Radiotherapy (RTX) was administered in 5 patients, chemotherapy (CHT) in all. 7 patients relapsed, 3 never entered clinical remission. 9 of the 10 patients of group I, died of disease progression. Group II: PRC was performed in 5 patients and led to local control in 2. Adjuvant RTX was used in 2 and CHT in 4 patients. 2 children relapsed and died of the disease. Infantile HP: PRC was performed in 3 children, remaining 4 patients received adjuvant CHT All children entered complete remission and are disease-free. Adult-type HP: PRC was performed in 5 patients, resulting in local control in 4. Five children were given adjuvant RTX and six CHT Three patients relapsed and died of disease progression. CONCLUSIONS: The effectiveness of primary complete resection in all groups was doubtful. The high rate of metastatic relapses suggests that the currently given systemic therapy is not satisfactory. The only tumour with excellent prognosis was infantile type HP (all patients are alive and free of disease). Adequate treatment for children with angiosarcoma remains still unknown -- 9 of 10 patients died of disease progression. Prognosis in patients with haemangioendothelioma is intermediate, however the role of immunotherapy should be further investigated.
